A 67-year-old woman with severe congestive heart failure (New York Heart Association, NYHA class IV) was set to receive general anesthesia for cardiac surgery. For several months, she had been in a constant sitting position from which the slightest change evoked dyspnea. A patient in such a condition is rarely considered a candidate for general anesthesia, because such patients are never eligible for any type of surgery other than that used to fix the heart problem itself. We report this case to explain how anesthesia was induced with the patient sitting in a crouching position and discuss other methods of induction that can probably be used in similar situations.

Purpose: This paper is aim to report the effects of Kamisoyosan-gamibang (加味逍遙散加味方) on the back pain patient who was diagmosed gan-ul(肝鬱) Methods: A Woman, 48 years old, who had back pain, was enrolled in this study. She received herbal medicine for several months. During the treatments, we requested her to visit hospital every two weeks. Results: After the treatments, back pain was decreased. Conclusion: This clinical case indicate that Kamisoyosan-gamibang is effective in treatment of menopausal disorder patient who had back pain. More clinical data and studies are requested for the treatment of menopausal disorder.

A 21-year-old female patient complaining of hemiparesis was diagnosed with right middle cerebral artery infarction. No risk factor was found, despite an extensive young-age stroke work-up, except her history of marijuana use. The patient had smoked marijuana for treating depression for more than five years. Magnetic resonance angiography showed multifocal intra- and extracranial stenoses, suggesting cannabinoid-induced vasculopathy. Since the use of illicit drugs has increased nationwide, physicians should consider it as a possible cause of a stroke due to an unknown etiology.

The dermoid cyst is the one of common space occupying orbital lesion. This lesion is regarded as a non-invasive tumor, but infrequently causes destruction of adjacent bony structure and displacement of adjacent tissue. We experienced a characteristic ovoid orbital dermoid cyst that occupied in the frontal sinus and causes displacement of the eyeball with well-defined lining. This 55-year-old male presented a mass in left orbit, which rapidly increased in size for past 12 months, and patient could not open left eyelid. We removed this tumor totally then reconstructed the orbital roof and frontal sinus with an iliac bone graft and polyethylene sheet(Medpor Newnan, USA). This patient was followed up for 12 months and patient obtained satisfactory result without any complication suck as recurrence or infection.

Objective: To describe the effects of Korean medical treatment on a 51-year-old male patient with polyneuropathy and chief complaints of lower limb paralysis and dysarthria. Methods: The Toronto Clinical Neuropathy Scoring System (TCNSS) was used to evaluate the function of the lower limb and the symptoms of the patient were observed daily. We applied herbal medication and acupuncture daily, depending on the patient’s symptoms. Results: After the treatment, the TCNSS score was improved. Conclusions: In this case, Korean medical therapy was effective in improving lower limb function.

Pulmonary thromboembolism originated mostly from the venous thrombus, especially deep vein thrombosis in the lower extremities, which migrated upward and lodged the pulmonary vasculatures, Massive pulmonary thromboembolism usually leads to in \ulcornerhospital mortality if the patient was not treated properly. Under the cardiopulmonary bypass, a 49-year old man was treated successfully by emergent pulmonary embolectomy of pulmonary thromboembolism, which originated from the deep vein thrombosis in the right leg. Ligation or filtering device insertion of the inferior vena cava was not performed. The patient’s postoperative course was uneventful and discharged on postoperative 15th day He continued to receive oral anticoagulation with aspirin and persantin, which had been started on the third postoperative day. And he was well till recent days through the outpatient follow-up. The clinical courses of this patient are described, and massive pulmonary embolism and its management are discussed.

The hemorragic episode is the major problem of mechanical prosthesis, because of the necessity for anticoagulant therapy to prevent complication of thromboembolism. Double valve replacement was performed to 42 year old male due to aortic stenoinsufficiency and mitral stenosis. For anticoagulant therapy, the patient has been given wafarin under the control of prothrombin time 0.5-2 times of normal) in the our hospital. The patient was injuried the right pelvic area by waves in the beach 4 years after double valve replacement. Pelvic MRI scan showed huge hematoma in the right pelvic cavity. The patient was operated removal of intrapelvic hematoma.

Objective : The objective of this study is to report the effectiveness of conservative treatment on simple pelvic fracture with ostomy. Methods : A female patient 65 years old with ostomy, who were diagnosed as simple pelvic fracture by X-ray, were treated with conservative treatment by herbal medication with acupuncture and physical therapy. We evaluated the effectiveness by checking X-ray, Digital Infrared Thermal Imaging(D.I.T.I.). Results : The symptoms of the patient got improved and X-ray showed the fracture were healing. Conclusions : Conservative korean medical treatment can be effectively used for a patient with simple pelvic fracture and ostomy.

Clinical symptoms of acute stroke include loss of consciousness, aphasia, dysphagia, hemiplegia, without urination or defecation, headache, dizziness, chest discomfort, etc. As methods of oriental medical treatment at acute stroke state, acupuncture, fumigating, emetic, sternutatory therapy etc. were known. We treated a 69-year-old female patient who was unconscious after acute stroke with acupuncture and herbal medicine. Also, we chose Croton seed (Crotonis Fructus) Tansy (Artemisiae Argi Folium)-hwan for treatment and fumed it on the patient's nose once a day. After 7 days of treatment with fumigating therapy, we observed improvement in consciousness on the Glasgow coma scale, and other symptoms (aphasia, dysphagia, hemiplegia, without urination or defecation)

A 31-year-old man presented with acute onset of paraplegia. The patient's history was significant for thyroid carcinoma that had been treated 2 years earlier by thyroidectomy. A magnetic resonance imaging scan showed an enhancing intramedullary lesion at T7-8. Patient underwent surgical treatment and a tumor with hematoma was resected via posterior midline myelotomy. Postoperatively, the patient's motor weakness was improved to grade 3. The lesion showed typical histologic features consistent with papillary thyroid carcinoma. Early diagnosis and microsurgical resection can result in improvement in neurological deficits and quality of life of patients with an ISCM.
