A Clinical Report of a Psychosomatic patient who has treated by Giungoroen-therapy based on Song Psychotherapy (노래 심리 치료를 활용한 지언고론요법(至言高論療法)을 적용하여 호전된 심신증(心身證)환자 1예(例) 보고)

  • Objectives : This study was performed to report the effect of Giungoroen-therapy based on song psychotherapy in the treatment resistant patient with psychosomatic disease. Methods : A 43 year-old female patient suffered from headache, facial numbness, right upper and lower limb's numbness, ptosis, chest discomfort, dysarthria without identifiable physical cause. We diagnosed her as psychosomatic disease, and tried to apply Giungoroen-therapy with Korean traditional medical treatment. But she was resistant to Giungoroen-therapy. So we applied Giungoroen-therapy based on song psychotherapy. Results : After the Giungoroen-therapy based on song psychotherapy, the patient revealed her unrecognized hidden issues and got some insights about herself. Then the symptoms, such as headache, facial numbness, right upper and lower limb's numbness, ptosis, chest discomfort and dysarthria were reduced considerably. Conclusions : These results suggest that Giungoroen-therapy based on song psychotherapy can be effective on improvement of resistant psychosomatic disease.

Delayed Splenic Rupture Following Minor Trauma in a Patient with Underlying Liver Cirrhosis (간경화증 환자에서 경도 외상 후 발생한 지연 비장 파열)

  • The spleen is the most frequently injured organ following blunt abdominal trauma. However, delayed splenic rupture is rare. As the technical improvement of computed tomography has proceeded, the diagnosis of splenic injury has become easier than before. However, the diagnosis of delayed splenic rupture could be challenging if the trauma is minor and remote. We present a case of delayed splenic rupture in a patient with underlying liver cirrhosis. A 42-year-old male visited our emergency department with pain in the lower left chest following minor blunt trauma. Initial physical exam and abdominal sonography revealed only liver cirrhosis without traumatic injury. On the sixth day after trauma, he complained of abdominal pain and diarrhea after eating snacks. The patient was misdiagnosed as having acute gastroenteritis until he presented with symptoms of shock. Abdominal sonography and computed tomography revealed the splenic rupture. The patient underwent a splenectomy and then underwent a second operation due to postoperative bleeding 20 hours after the first operation. The patient was discharged uneventfully 30 days after trauma. In the present case, the thrombocytopenia and splenomegaly due to liver cirrhosis are suspected of being risk factors for the development of delayed splenic rupture. The physician should keep in mind the possibility of delayed splenic rupture following blunt abdominal or chest trauma.

  • Herpes zoster (HZ) is an acute infection of the unilateral sensory dermatome caused by varicella-zoster virus (VZV) and is characterized by vesicular eruption and unilateral pain along the involved dermatome. Although the pathogenesis of HZ is incompletely understood, it is thought that when cell-mediated immunity falls below a critical level, dormant VZV within cells of the sensory ganglia are allowed to replicate and infect the host with the resultant clinical presentation of HZ. It has been associated with immunosuppressed states, such as advanced age, leukemia, lymphoma, chemotherapy and/or radiation treatment. We present a case of a 62-year-old female patient with malignant lymphoma suffering herpes zoster ophthalmicus who did not respond to conventional treatment, and in whom the application of various nerve blocks and patient-controlled analgesia produced moderate pain relief. The patient died twenty days later due to cardiopulmonary failure.

Early-diagnosed silent sinus syndrome and cone-beam computed tomography in a pediatric patient: a case report

  • Chronic maxillary atelectasis (CMA) is a progressive alteration in the volume of the maxillary sinuses that may result in facial asymmetry. CMA in asymptomatic patients is known as silent sinus syndrome (SSS) and is a rare entity, especially in pediatric patients. This study reports a case of SSS in a pediatric patient who received an early diagnosis through cone-beam computed tomography (CBCT). An asymptomatic 12-year-old female patient in orthodontic treatment presented with opacification of the left maxillary sinus on a panoramic radiograph. Clinically, the patient had discrete hypoglobus and enophthalmos. CBCT and nasal video-endoscopy revealed ostiomeatal obstruction with bone deformity, leading to diagnosis of SSS. Endonasal endoscopic maxillary sinusotomy was performed. Two years later, the patient remained asymptomatic, and a second CBCT exam confirmed a stable condition. This case highlights the role of optimal radiographic interpretation for early diagnosis of maxillofacial alterations in pediatric patients.

A Case Report about Diarrhea of Soeumin Caused by Tube Feeding Treated with Korean Medicinal Treatment (경관 영양으로 유발된 소음인 설사에 관한 한방 처치 1례)

  • Objectives: This study aimed to investigate the effect of Soeumin-Seonghyangjeongki-san on diarrhea with tube feeding causing mental deterioration induced by subdural hemorrhage (SDH) in an 83-year-old male patient. Methods: The patient was treated with Soeumin-Seonghyangjeongki-san. His symptoms were assessed using the Bristol stool form index, the King's Stool Chart, and records of stool state. The treatment was executed for one week from September 10, 2018 to September 16, 2018. The patient was observed during that same period. Results: After being treated with Soeumin-Seonghyangjeongki-san for seven days, the patient's diarrhea symptoms were evaluated using the Bristol stool form index, the King's Stool Chart, and records of stool state. After the treatment, the results of all three evaluation methods showed that the patient's condition had improved. Conclusions: The results suggest that Soeumin-Seonghyangjeongki-san can be a valuable option for treating diarrhea with tube feeding causing mental deterioration induced by SDH.

  • Objectives : The purpose of this study is to evaluate the effect of oriental medical treatment on a sinusitis patient Methods : We treated a 56-years old man patient with sinusitis. His symptoms are Ozena, Postnasal drip, Cough, sputum, nasal obstruction, rhinorrhea and Headache. The patient was treated with herb-med(Kami-Hyunggyeyungyotang,加味荊芥連翹湯), acupuncture, aroma oil therapy. During about a month, the patient was treated only with oriental medical treatment and without any of western medicine including antibiotics. We confirmed the progress of treatment with subjective symptoms, physical examination and radiographic inspection(PNS image). Results & Conclusions : After one month, Combined application of the oriental treatment including Kami-Hyunggyeyungyotang showed remarkable effects on subjective symptoms, physical examination and radiographic inspection(PNS image). This study show that Kami-Hyunggyeyungyotang effects on sinusitis.

  • Introduction: The aim of this study was to report the effect of Korean medicine treatments on an elderly patient with acute hematochezia and anemia Case presentation: An 81-year-old female with chronic multifocal middle cerebral artery infarction had symptoms of acute hematochezia and anemia. Method: The patient was treated with Korean medicine therapy. Results: Treatment with Korean medicine, including herbal medicine, significantly improved the patient's symptoms. After treatment, her hematochezia was improved and the results of a complete blood count (CBC) also improved. No side effects were observed during treatment. Conclusion: Korean medicine treatment can be considered an effective treatment for acute hematochezia and anemia in an elderly patient.

  • The ideal scalp reconstruction involves closure of the defect with similar hair-bearing local tissue in a single step. Various reconstructions can be used including primary closure, secondary healing, skin grafts, local flaps, and microvascular tissue transfer. A 53-year-old female patient suffered glioblastoma, which had recurred for the second time. The neuro-surgeons performed radial debridement and an additional resection of the tumor, followed by reconstruction using a serratus anterior muscle flap with a split-thickness skin graft. Unfortunately, the flap became completely useless and a bilateral rotation flap was used to cover the defect. Two month later, seroma with infection was found due to recurrence of the tumor. Additional surgery was performed using multiple perforator based island flap. The patient was discharged two weeks after surgery without any complications, but two months later, the patient died. Radical surgical resection of tumor is the most important curative option, followed by functional and aesthetic reconstruction. We describe a patient with a highly malignant tumor that required multiple resections and subsequent reconstruction. Repeated recurrences of the tumor led to the failure of reconstruction and our strategy inevitably changed, from reconstruction to palliative treatment involving fast and stable wound closure for the patient's comfort.

Glossectomy in the severe maxillofacial vascular malformation with jaw deformity: a rare case report

  • In the field of oral-maxillofacial surgery, vascular malformations present in various forms. Abnormalities in the size of the tongue by vascular malformations can cause mandibular prognathism and skeletal deformity. The risk in surgical treatment for patients with vascular malformation is high, due to bleeding from vascular lesions. We report a rare case of macroglossia that was treated by partial glossectomy, resulting in an improvement in the swallowing and mastication functions in the patient. A 25-year-old male patient with severe open-bite and mandibular prognathism presented to our department for the management of macroglossia. The patient had a difficulty in food intake because of the large tongue. Orthognathic surgery was not indicated because the patient had severe jaw bone destruction and alveolar bone resorption. Therefore, the patient underwent partial glossectomy under general anesthesia. There was severe hemorrhaging during the surgery, but the bleeding was controlled by local procedures.

A Case Study of a Patient of Chronic renal failure (허로(虛勞)로 변증한 만성신부전 환자의 십전대보탕(十全大補湯) 투여 호전례)

  • Objectives : To evaluate the efficacy of herbal medicine on CRF, clinical study on one case of 48 year old male patient of CRF was performed. The patient complained of general weakness and anorexia, dizziness and both lower limb edema. Methods : According to the Differentiation of Symptoms(變證), the patient was classified as weakness(虛勞) prescribed Sipjeondaebo-tang(十全大補湯) as well as acupuncture and moxibustion. treatment. Change of BUN, Creatinine, Hgb, Albumin, VAS of weakness and urine volume was compared before and after treatment for 3 months. Results : After such treatments, the level of BUN and creatinine was decreased and the volume of urine was increased. Conclusion : Herbal medicine Sipjeondaebo-tang with acupuncture and moxibustion treatment would be efficient to the patient of CRF.
