Total Anomalous Pulmonary Venous Connection in Adult -A Case Report- (성인 총폐정맥 환류이상 -1례 보고-)

  • We experienced one case of TAPVC patient who was 39 years old man with mild dyspnea on exertion and easy fatigability. He was the oldest patient among TAPVC which was reported in Korea. Preoperative echocardiogram, cardiac catheterization and angiogram revealed supracardiac type TAPVC drained through left innominate vein. An anastomosis between common pulmonary venous trunk and left atrium, pericardial patch closure of ASD and ligation of left vertical vein were performed with extracorporeal circulation. The postoperative course was uneventful and discharged with excellent general condition and has been well during 8 months follow-up.

Intrapulmonary Teratoma - A Case Report - (폐기형종;1례 보고)

  • The intrapulmonary teratoma is an extremely tumor. This paper reports a case of int-rapulmonary teratoma located in the right upper and middle lobes of the lung in a 16-years-old male patient. The initial symptoms were right chest pain and coughing. Chest X-ray revealed huge soft tissue mass density in the right lower lung field. Right upper and middle lobectomy with resection of invaded pericardium was done. The gross and microscopic findings of res-ected specimen revealed characteristic findings of the intrapulmonary teratoma. The patient was recovered uneventually. We would like to describe this case of rare tumor with the review of literatures.

Cricopharyngeal myotomy for cervical dysphasia: report of a case (윤상인두 절개로 치험한 경부연하곤증)

  • We studied and managed successfully a case of cervical dysphagia, 72 years old male patient. Cervical dysphagia is a relatively uncommon disease and encounters most commonly after the age of sixty in equal sex distribution. The final diagnosis can be made by esophagography and esophagomanometry. We performed cricopharyngeal myotomy of Belsey method with excellent result. After operation patient showed no more dysphagia, normal esophagogram and pressure drop in esophagomanometry.

Surgical Treatment of Coronary Artery Aneurysm - A case report - (관상동맥류의 외과적치료: 1례보고)

  • We experienced one case of the coronary artery aneurysm. The patient was 67 years old female, who complained of dyspnea on exercise[NYHA functional class III] for two years. Coronary angiogram demonstrated a localized saccular aneurysm on left main coronary artery, just above bifurcation of left anterior descending coronary artery and circumflex artery. On the operative field, the aneurysm was 2cmx2cmx 1.5cm in size with 0.5cm in neck. Resection and aneurysmorrhaphy was done. The patient was recovered and discharged uneventfully. During 8 months follow-up period, she lives in good physical activity and absence of chest pain.

  • We experienced 2 years and 5 months old male patient with partial anomalous pulmonary venous return of the left lung into the coronary sinus without atrial septal defect. After incising the atrial septum and the wall between the left atrium and the coronary sinus, we made the roof of the coronary sinus and closed the artificial atrial septal defect, with using patch, then we could change the direction of the blood flow from the coronary sinus into the left atrium. The patient was discharged on the 13th postoperative day after uneventful postoperative course.

  • Pulmonary edema and respiratory failure is uncommon but fatal complications associated with major operations of the lung, especially after pneumonectomy. The extracorporeal membrane oxygenation[ECMO] is quite often used in infants with severe respiratory failure and congenital heart disease which is well documented in the literature. In adults, the results of ECMO is comparatively poor to those found in neonates. We have experienced a case of ECMO applied on a 48 year old male who had respiratory failure after pneumonectomy, and the patient was successfully weaned from bypass. Unfortunately however, the patient expired on postoperative 15 day due to multiorgan failure.

  • Squamous carcinomas of the thyroid gland are extremely rare, and its clinical course is very aggressive. It has poor prognosis, similar to that of anaplastic carcinoma. These tumors are radioresistant and often rapidly fatal. It is considered to originate from the follicular epithelium at present. Recently, authors had experienced 63-years old female patient, proved to be primary squamous cell carcinoma of the thyroid. We report this patient with a review of a literature.

  • The Authors have observed a case of hypopituitary patient in 15 year-old Korean child for 10 months. Since he was discharged, he was taken the hormonal therapy with corticosteroid and T₃. As the result of it, his body weight increased by 1.2kg and height grew by 3cm. However it is not clear whether this growth is due to the child's natural growth or due to the above hormonal therapy.

  • Cerebellar atrophy was found that a patient was taking oral phenytoin for 3 years. 53 years old female patient with General tonic clonic(GTC) type seizure was prescribed phenytoin. In the process, she developed ataxic gate, dysarthria. Brain magnetic resonance imaging(MRI) finding was revealed differential diagnosis cerebellar atrophy. She was prescribed epileptol instead of phenytoin. But leukopenia, thrombocytopenia occurred. As a result, phenytoin restarted. Development of medical state decreased abuse of anticonvulsants. Considering various convulsive disorders, we must give attention to using anticonvulsants.
