Inhaled iloprost for the treatment of patient with Fontan circulation

  • Decreased exercise capacity after Fontan surgery is relatively common and the failure of the Fontan state gradually increases with age. However, there is no further treatment for patients with Fontan circulation. Pulmonary vasodilation therapy is an effective method to solve this problem because pulmonary vascular resistance is a major factor of the Fontan problem. Inhaled iloprost is a chemically stable prostacyclin analogue and a potent pulmonary vasodilator. We experienced two cases of Fontan patients treated with inhaled iloprost for 12 weeks. The first patient was an 18-year-old female with pulmonary atresia with an intact ventricular septum, and the second patient was a 22-year-old male with a double outlet right ventricle. Fifteen years have passed since both patients received Fontan surgery. While the pulmonary pressure was not decreased significantly, improved exercise capacity and cardiac output were observed without any major side effects in both patients. The iloprost inhalation therapy was well tolerated and effective for the symptomatic treatment of Fontan patients.

Clinical Evaluation of Lung Cancer in Patients Younger Than 40 Years (약년자 [40세 미만] 폐암 수술증례의 임상적인 검토)

  • The lung cancer in patients under 40 years old is rare and reported to be more rapidly fatal than in older persons. We reviewed the records of 18 cases who younger than 40 years with histologically proven lung cancer between 1959 and 1992 at department of Thoracic Surgery in National Medical Center. There were 12 male and 6 female patients. Two male and all female patients had never smoked. The 17 cases had respiratory symptoms for a mean duration of 3 months. The squamous carcinoma was found in 7 cases[38.9%], small cell carcinoma in 4 cases[22.2%], mixed cell carcinoma in 4 cases[22.2%], adenocarcinoma in 2 cases[11.1%] and bronchoalveolar cell carcinoma in 1 case. Among the 6 resected tumors, one case was stage I, two cases were stage II and three cases were stage IIIa. The operation was considered complete and curative in 6 patients and noncurative in 6 pations because of an exploratory thoracotomy. We conclude that lung cancer in young persons is virulent and that diagnosis is frequently delayed.

A study on Korean medicine methodology for active postpartum care by examining San-Hu-Byung cases (산후병 치험례로 살펴본 적극적 산후관리를 위한 한의학적 방법론 연구)

  • Objects : The purpose of this study is to evaluate Korean medicine treatment as an active postpartum care for women with San-Hu-Byung. Methods : We examined 35 patients(39 cases) with San-Hu-Byung who had hospitalized at the M Korean medicine hospital in Gyeonggi Province between January 1st, 2006 and March 31th, 2015. Based on medical records and questionnaires, we analyzed the general characteristics, main symptoms, duration of hospitalization, treatment satisfaction, etc. We also reviewed and analyzed the results using multifaceted questions. Results : 1. The patients were divided into four age groups; 30-34 years old(46%), 35-39 years old(33%), 27-29 years old(13%), more than 40 years old(5%) and less than 26 years old(3%) in the order. 2. Patients appealed musculoskeletal symptoms(44.05%), reproductive and breast symptoms(21.43%), circulatory symptoms(10.12%), neuropsychiatric symptoms(8.33%), digestive symptoms(7.14%), the other symptoms(5.95%), urinary symptoms(1.79%) and respiratory symptoms(1.19%) in the order. 3. Duration of hospitalization were; within 2 months(33.33%), more than 3 months(25.64%), within 1 month(17.95%), within 3 weeks(12.82%), within 3 months(5.13%) in the order. 4. The improvement of musculoskeletal symptoms including back pain was measured by VAS. After 6 weeks, the pain fell to less than half. After 12 weeks, the pain fell to less than 1. 5. Average satisfaction with Korean medicine treatment is a 4.51 out of 5. Conclusions : As an active postpartum care, Korean medical treatment had significant effect on San-Hu-Byung and patients found the treatment satisfying.

Quality of Life in Older Adults with Cochlear Implantation: Can It Be Equal to That of Healthy Older Adults?

  • Background and Objectives: This study aimed to evaluate the audiologic results after cochlear implantation (CI) in older patients and the degree of improvement in their quality of life (QoL). Subjects and Methods: Patients over 65 years old who underwent CI at implant center in Bozyaka Training and Research Hospital were included in this study (n=54; 34 males and 20 females). The control group was patient over 65 years old with normal hearing (n=54; 34 males and 20 females). We administered three questionnaires [World Health Organization Quality of Life-BREF (WHOQOL-BREF), World Health Organization Quality of Life-OLD (WHOQOL-OLD)], and Geriatric Depression Scale (GDS) to evaluate the QoL, CIrelated effects on activities of daily life, and social activities in all the subjects. Moreover, correlations between speech recognition and the QoL scores were evaluated. The duration of implant use and comorbidities were also examined as potential factors affecting QoL. Results: The patients had remarkable improvements (the mean score of postoperative speech perception 75.7%) in speech perception after CI. The scores for the WHOQOL-OLD and WHOQOL-BREF questionnaire responses were similar in both the study and control groups, except those for a two subdomains (social relations and social participation). The patients with longer-term CI had higher scores than those with short-term CI use. In general, the changes in GDS scores were not significant (p<0.05). Conclusions: The treatment of hearing loss with CI conferred significant improvement in patient's QoL (p<0.01). The evaluation of QoL can provide multidimensional insights into a geriatric patient's progress and, therefore, should be considered by audiologists.

REPORT OF 2 CASES OF ARTHROPLASTY IN TEMPOROMANDIBULAR ANKYLOSIS PATIENTS (하악관절강직증처치를 위한 관절성형술 2예 보고)

  • The authors have observed and treated the two patients with true bony ankylosis of temporomandibular joint in 16 years old school boy and 24 years old korean male. The ankylosed patients were treated by condylectomy and arthroplasty such as resin condylar head graft having been made into acrylic resin. 1. In 16 years old school boy, temporoandibular ankylosis of left side was surgical_y approached by condylectomy including coronoid process and posterior border of ramus and resin condylar head graft by means of interosseous wiring. 2. In 24 years old korean male, bilateral temporomandibular ankylosis was treted by bilateral condylectomies and resin condylar head grafts. 3. The mouth opening has been continued as 3-5cm since 13 months and 8 months before and facial appearances were very excellent.

Nutritional Status of Hospitalized Geriatric Patients Using by the Mini Nutritional Assessment (MNA를 이용한 노인 환자의 영양 상태 판정)

  • The prevalence of undernutrition in hospital populations is known to be high. The presence of malnutrition is associated with depression, infections, sarcopaenia, falls, fractures, reduced autonomy and increased mortality. This study specifically examined the prevalence of malnutrition in patients aged 65 or older at the time of admission as determined by the Mini Nutritional Assessment (MNA) which has been a frequently used nutritional risk screening tools in detecting undernutrition in old people. This study was done for one hundred eight hospitalized geriatric patients in Seoul National University Bundang Hospital, Seoul, Korea. On admission baseline history, anthropometrics measurements, laboratory data and nutritional status by MNA were assessed. Length of hospital stay was obtained by reviewing medical charts. We used one-way analysis of variance to compare the differences in variables. Spearman's rank correlation coefficients were calculated for associations between MNA and variables. On admission, $22.3\%$ of patients were malnourished and $40.7\%$ were at risk of malnutrition according to the MNA. Percent of ideal body weight, anthropometrics data, albumin, and hemoglobin were lower in the malnourished patients (p<0.05). The malnourished patients stayed in the hospital 7.3 days longer, as compared with well nourished patients (p<0.05). Percent of ideal body weight, albumin, hemoglobin and total cholesterol were correlated inversely with nutritional status according to MNA (p<0.05). MNA can be used for nutritional assessment in Korean old people, because MNA significantly correlated with other nutritional assessment parameters, such as, anthropometric and laboratory data in hospitalized geriatric patients. The high prevalence of malnutrition in the elderly was observed and the presence of malnutrition on admission predicted a significant increase in the length of hospital stay in this study. Therefore further studies are needed to determine whether nutritional interventions in old people with low MNA scores can improve clinical outcomes during the hospital course.

Research on the Effect and Satisfaction between Independenced and Parallelism Treatment of Acupuncture and Physical of Lumbago Patients (요통환자에 대한 침치료와 물리치료의 단독 및 병행치료의 효과와 만족도 조사)

  • Objectives : Research on the effect and satisfaction between Acupuncture Treatment, Physical Treatment and contemporary Treatments of Acupuncture and Physical of lumbago patients Methods : We separated lumbago patients into three groups. A group with only Acupuncture Treatment and a group with only Physical Treatment and a group with contemporary treatments of Acupuncture and Physical. Questionnaire survey and score of Visual Analog Scale(VAS) and Oswestry Disability Index(ODI) were examined for comparison. Results & Conclusion : 1. Of the 54 patients Acupuncture Teatment group had 17 patients with similar ratio of gender and the average age was 39.5 years old. For the Physical Treatment group the ratio of male was 75% of the 8 patients and had the youngest average age which was 29.1 years old and the group with contemporary Treatments the ratio of female was 62% of 29 patients and had the oldest average age which was 46.7 years old. 2. The Acupuncture Treatment group had the shortest treatment period because of many acute stage caused by lumber sprain and the improvement score was the best. The group of contemporary treatments had the longest history day and treatment period by serious disease like HIVD and spinal stenosis but the improvement score of VAS and ODI was good similar to the Acupuncture Treatment group. For the Physical Treatment group there was many patients with degenerative change but not severe pain. The VAS, ODI and the improvement score was the lowest before and after the treatment.

Correlation Between Stress, Coping Patterns and Physical Symptom of Cancer Patient's Caregiver (입원한 암환자 가족원의 스트레스, 대처방법 및 신체증상간의 상관관계)

  • The purpose of this study was to identify correlation of stress, coping patterns and physical symptoms in cancer patient's caregiver. The stress was measured by VAS(Visual Analogue Scale). The coping methods were measured using the modified Ways of Coping Questionnaire by Yang (1998) and the actual physical symptoms were investigated. The phases of patient illness consisted of 1st (initial) stage, and 2nd (recurred) stage and 3rd (terminal) stage based on literature (Lewandowski & Jones, 1988). The data were collected by a survey conducted from March to July, 2000 and which included 196 cancer patients' caregivers from two hospitals in Seoul. The data were analyzed using paired t-test, unpaired t-test, ANOVA, Scheffe test and Pearson correlation coefficient. The results were as follows: 1. The average of caregivers' stress scores was 62.5. Problem-focused coping methods were significantly used more than emotion-focused coping methods by the cancer patients' caregiver. The mean number of caregivers' physical symptom was 1.03. 2. There were significantly high level of stress in women, those who were more than 60 years old, those who had a low education level, those who had no job, those who are patients' wives' and those who are terminal patients' caregiver. There were significantly low levels of coping in women, those who were more than 60 years old, those who had low education levels, those who had no job and those who are patients' wives. There were significantly higher number of physical symptoms in women, and those who have no job. 3. Caregivers' stress was significantly correlated to problem-focused coping methods (r=-.21, p=.006), and physical symptom (r=-.28, p=.0001). In conclusion, attempts to develop nursing interventions for cancer patients' caregiver in women, those who are more than 60 years old, with a low education level, have no job, and are cancer patients' wives could have an improvement on positive coping methods and provide relaxation from stress in the patients' experience.

Isolation of Candida albicans from Denture Patients and in Vitro Activities of Amphotericin B and Miconzole (의치환자에서 Candida albicans분리와 amphotericin B 및 miconazole에 시험관내 감수성)

  • Candida albicans is now well recognized among the denture stomatitis patients. The broth macrodilution test is the most widely used technique for antifungal susceptibility testing. The purpose of this study was to determine the C. albicans carrier rate of the denture patients in Iksan, chonbuk. To determine the C. albicans carrier rate of denture patients, culture were made from 227 sample taken in Iksan, Chonbuk during July 1997 to August 1997. Also activities of amphotericin B and miconzole against isolates of denture patients of C. albicans were tested by broth macrodilution test using RPMI medium 1640. The results were as follows : First C. ablicans was isolated from 6.6% of denture patients samples and the frequency of isolation fo C. albicans was highest(50%) in the age group of 71-year-old to 80-year-old denture patients. Second, against C. albicans, the MIC range of amphotericn B was $0.06{\sim}0.25{\mu}g/ml$. MIC50 and MIC90 were $0.13{\mu}g/ml$ and $0.25{\mu}g/ml$, respectively. Third, the MIC range of miconazole was $10-{\ge}20{\mu}g/ml$ and MIC50 and MIC90 were $20{\mu}g/ml$ and ${\ge}20{\mu}g/ml$, respectively. It was concluded from this study that C. albicans acrriages from healthy denture individuals only over 60-year-old ages were isolated, they remain susceptible to amphotericin B and not rarely resistant to miconzole.
