  • Objective : Five most generally adopting basic oriental prescriptions (yookmijihwangtang, ojeoksan, sipjeondaebotang, samultang, bojoongikgitang) samples were collected from 10 oriental hospitals nationwide and the transferring ratio to aqueous phase (TRAP) were calculated. Method : The samples are grounded evenly and the decocted at $120^{\circ}C$ for 12 min on hot plate and then filtered. The concentrations of As, Cd, Co, Cr and Pb in before and after decoction were measure by ICP/MS and the TRAPs were calculated. Results : The concentrations of As, Cr and Pb in the ground samples before decoction were all below criteria level, and TRAP values were also very low (<10%). For Cd, although some ground samples before decoction showed relatively high concentration level compare to the criteria level, because of their low TRAP (~3%), the actual concentrations in the decoction were quite low compare the criteria level. The Co's concentrations in the ground samples before decoction were all relatively low level. However, the TRAP of Co was astonishingly high value of around 40% average. Conclusion : The toxic heavy metal concentrations in the decoction were very low compare to the criteria level. The TRAPs are less than <10% except Co. The toxic heavy metals in the prescriptions are mainly remained in the residue and the only very small fraction may transferred to the decoctions. Therefore the toxicity of the oriental herbal medicine about the toxic heavy metal may be overstressed. The Co's TRAP turned out to be quite high value (40% average). Since the criteria level wes not established yet, the safety of decoctions according to Co was not fully confirmed.

In distribution according to types of accident, Rear-end collision accident numbered the most (34.3%), followed by Frontal collision (21.2%) and Lateral collision (14.6%). 3. In distribution according to duration of treatment, most (54.7%) patients discharged within 1 week, followed by 1 week to 2 weeks(37.2%) 4. In distribution according to mean elevation of symptoms, Neck pain was the most (75.9%) symptom, followed by Low back pain (67.2%), Shoulder pain (46.0%) and Headache (37.2%). 5. In distribution according to treatment results, symptom improvement was the most (62.8%), followed by excellent (19.7%), mild improvement (14.6%). 6. In distribution according to duration of treatment, Most frequently prescribed herbal decoction was Tongdosan (45.7%), followed by Dangguijakyaksangagam (29.6%), and most frequently prescribed extract powder was Yunkyopaedocksan and Ojeoksan (12.7%). • Background : Traditional herbal medicine is used extensively among the Korean populations, and other Asian countries employ similar therapies as well. In recent years, extensive focus was laid on adulteration of the herbal medicine with liver damage. The use of herbal preparations as remedies for various medical conditions has continuously increased in Korea. Large proportions of Korean patients use herbal medicinal products, folk remedies, and health food. However, studies on the safety of herbal products are conducted on a less than sufficient basis even in the countries like Korea where herbal medicine is being used extensively. Some of the reports on the safety of herbs were done by the doctors of western medicine but lack of knowledge and misclassification led to misunderstandings. Objectives : This study aims to verify the evidences on safety of frequently used 4 herbal medications (Sipjeondaebo-tang, Bojungikgi-tang, Ohjeok-San, Yukmaijihwang-tang) in the lab animal model. Methods : Sprague-Dawley rats was treated by 4 herbal medications during 31 days. After 1 month, we checked body weight, liver weight, and serum enzyme associated with liver function. Results : There is no significant difference in body weight and liver weight after 1 month of administrations. In all experimental groups, no abnormal findings was observed in histotogical research and lab liver Functions test(AST, ALT etc). Conclusions : These four herbal medications, frequently used in oriental medicine clinics and hospitals, are safe from hepatoxic events in the lab animal model.

  • Objectives: To develop guidelines for concomitant use of four herbal medicines (Gamisoyosan, Banhasasimtang, Ojeoksan, and Bojungikgitang) with escitalopram. Methods: A guideline development team was assembled and relevant prior research was systematically reviewed to gather evidence. The potential for drug interactions was evaluated by analyzing changes in pharmacokinetic parameters. Safety was assessed through the analysis of adverse drug reactions associated with combined use. Recommendations and concomitant administration guidelines were formulated through a consensus process. Results: No significant drug interactions were identified between the four herbal medicines and escitalopram, indicating no need for dosage adjustment of escitalopram. However, it is recommended to monitor potential adverse reactions during concurrent use. Conclusions: This study provides recommendations for combined use of four herbal medicines covered by domestic insurance combined with escitalopram. Further research on interactions between antidepressants and herbal medicine is necessary to refine and enhance concomitant administration guidelines.
