Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare form arising within the jaws. PIOSCC is not related to the oral mucosa, presumably developing from remnants of the odontogenic epithelium. Because odontogenic cyst epithelium often transforms malignantly into PIOSCC, it could be misdiagnosed as odontogenic cyst based on a relatively ill-defined radiolucent lesion. Therefore, definite diagnosis is established from histological examination of biopsy samples taken during cyst enucleation in many cases. The present study is reported with a case of patient complaining of discomfort on his mandible. He was diagnosed as a putative dentigerous cyst and underwent a cyst enucleation treatment. After definite diagnosis as PIOSCC was established based on histologic findings, partial mandible resection and mandible reconstruction were performed. Up to the present, 10 months follow up of the patient showed satisfactory healing without recurrence and abnormal findings; thereby, we are reporting this case with literature review.
Objectives: Since World Health Organization reclassified the odontogenic keratocyst as a keratocystic odontogenic tumor(KCOT) in 2005, management of KCOT remains controversial. The purpose of the this study is to present a current concept of the treatment of the KCOT. Methods: Recent articles were focused on the method of the surgical therapy and the recurrence rate of KCOT. Results: The treatment modality is divided into conservative treatment-such as simple enucleation with or without curettage or marsupialization-and aggressive treatment including peripheral ostectomy, chemical curettage with Carnoy's solution, and resection. Conclusions: Keratocystic odontogenic tumors have higher recurrence rates than other odontogenic lesions; thus, the selected type of treatment is very important.
Calcifying odontogenic cyst(COC) is comparatively rare in occurrence. COC represents about 1% of jaw cysts, and although it may occur in soft tissue, it is most commonly found within bone. Both the intraosseous and extraosseous forms occur with about equal frequency in the maxilla and mandible, mainly in the incisor and canine areas The most notable features of this pathologic entity are histopathological and include a cyst lining demonstrating characteristic "ghost" epithelial cells with a propensity to calcify and the occasional association of this finding with certain odontogenic tumors including the odontoma and the ameloblastoma. In this case, COC was associated with anterior wall of the maxillary sinus which appeared in the anterior maxilla of 64-year-old woman, was reported. We report that the clinical experience of COC with review of literatures.
선양 치성 종양(adenomatoid odontogenic tumor)은 악골에 드물게 발생하는 치성 기원의 종양이다. 이전에는 법랑모세포종(ameloblastoma)의 한 종류로 여겨졌으나 양성이며 재발되는 경우는 극히 드물다. 상악 견치에 호발하며 대개는 무증상이지만 크기가 천천히 커져 무통성 종창을 일으킬 수 있다. 방사선학적으로 함치성 낭종(dentigerous cyst) 및 단방성 법랑모세포종(unicystic ameloblastoma)과 매우 유사하다. 일반적으로 보존적인 소파술(curettage) 및 적출술(enucleation)을 통해 치료한다. 본 증례는 하악 우측 측절치 미맹출을 주소로 내원한 10세 여자 환아에서 방사선 사진 검사를 통해 치아의 매복 및 병소를 확인하고 적출술을 시행하였다. 치아에는 브라켓을 부착하여 교정적 정출을 유도하였다. 조직학적 검사를 통해 선양 치성 종양으로 확진되었으며 양호한 치료 결과를 보였기에 보고하는 바이다.

Squamous cell carcinoma (SCC) is the most common malignant tumor in the oral cavity, and it accounts for about 90% of all oral cancers. Several risk factors for oral SCC have been identified; however, SCC associated with odontogenic keratocysts have rarely been reported. The present study describes the case of a 36-year-old man with SCC of the right ramus of the mandible, which was initially diagnosed as a benign odontogenic cyst. He underwent enucleation at another hospital followed by segmental mandibulectomy and fibular free flap reconstruction at our institution. In this case, we introduce a patient with oral cancer associated with odontogenic cyst on the mandible and report a satisfactory outcome with wide resection and immediate free flap reconstruction.
A clinical study of selected patients with cyst which were managed in the Department of oral and maxillofacial sugery from March 1994 to February 1998 was done. An following results were obtained 1. Male were involved more than females by cyst in a ratio of 1.95:1 and most of the cases occurred in the secondary decades 2. The primary site of radicular cysts were the anterior incisor area, dentigerous cysts were anterior incisor area and canine area, incisive canal cysts were maxillary anterior area, odontogenic keratocyst were the mandibular ramus area. 3. The common symptoms were swelling(65 cases), pain(12 cases) and the mean duration of syptomatic period was 10 days. 4. The rates of histopathologic classification were radicular cyst(58%), dentigerous cyst(22%), incisive canal cyst(9%), odontogenic keratocyst(11%). 5. Average of Alk. phosphatase was 235(IU/L) at pre-twenty age and 102(IU/L) at post-twenty age. 6. In treatment modalities, enucleation was most common, odontogenic keratocyst was treated by enucleation and curettage for prevention of recurrence.
치성각화성낭종(Odontogenic keratocyst, OKC)은 치체(dental lamina)로부터 발생하는데, 모든 악골 낭의 $5{\sim}17$% 정도를 차지하며 낭의 내부는 이장상피로부터 유래한 점액성이나 치즈양 물질인 케라틴(keratin)으로 채워져 있다. 치성낭 중 가장 높은 재발률을 보인다는 것이 이 낭의 중요한 특징이다. 간혹 기저층에서 인접 결체조직 벽으로 돌기가 증식하기도 하며 결체조직 벽 내에 존재하는 치성 상피조직 섬의 증식이 위성 소낭을 발생시키기도 한다. 이들 소낭들이 치성각화성낭종의 높은 재발률의 원인으로 여겨진다. 치성각화성낭종은 다양한 연령층에서 발견되나, 20대와 30대에서 호발한다. 남성에서 다소 호발하며 하악, 특히 제3대구 치와 하악지 부위에서 자주 발생한다. 대개 피질골의 팽융에 의해 처음 발견되며 매복치와 관련되어 있고 일반적으로는 증상이 없지만 동통과 감염이 나타날 수도 있다. 흡인시 걸쭉한 노란 치즈양 물질인 케라틴이 관찰되며 특히 재발이 잘 된다. 치성 각화낭의 중요한 특징 중의 하나는 병소가 피질골의 팽창을 많이 유발시키지 않으면서 악골의 내면을 따라 성장한다 는 것이다. 치성각화성낭종화낭은 인접 치아를 변위시키고 흡수시킬 수 있으나 함치성낭보다는 정도가 심하지 않다. 하악관을 하방으로 변위시키기도 하며 상악 병소의 경우는 상악동을 침범해서 상악동 전체를 차지하기도 한다. 본 증례는 7세 10개월 된 여아의 하악 우측 제2유구치 부위에 발생한 치성각화성낭종에 대한 증례로써 전신마취하 낭종적 출술을 시행하였다. 낭종에 포함된 제1,2유구치, 유견치 및 제1소구치 치배를 발거하였으며, 현재 공간유지장치를 장착후 주기적으로 내원중이다.

Objectives: The purpose of this retrospective study is to find the differentiating characteristics of cystic and cystic-appearing lesions that involve the impacted mandibular third molar by analyzing panoramic radiographs and computed tomography images, and to aid the preoperative diagnosis. Materials and Methods: Eighty-one patients who had a mandibular cystic or cystic-appearing lesion that involved impacted mandibular third molar and underwent cyst enucleation were included in the study. The preoperative panoramic radiograph and computed tomography findings were analyzed in accordance to the histopathologic type. Results: Most of the cystic lesions containing the mandibular third molar were diagnosed as a dentigerous cyst (77.8%). The occurrence of mesio-distal displacement of the third molar was more frequent in the odontogenic keratocyst (71.4%) and in the ameloblastoma (85.7%) than in the dentigerous cyst (19.1%). Downward displacement was primarily observed in each group. Odontogenic keratocyst and ameloblastoma showed more aggressive growth pattern with higher rate of bony discontinuity and cortical bone expansion than in dentigerous cyst. Conclusion: When evaluating mandibular cystic lesions involving the impacted mandibular third molar, dentigerous cyst should first be suspected. However, when the third molar displacement and cortical bone absorption are observed, then odontogenic keratocyst or ameloblastoma should be considered.

Introduction: Bone defects in the jaw are frequently observed after odontogenic cyst enucleation. The success of bone healing appears to be related to the size of the bone defect, the anatomical location, the patient's age and other parameters. The use of bone grafting material is dependent on the operator's preference. No evidence-based definite treatment protocol has been established. This study evaluated the effect of a bone graft into the defect after odontogenic cyst enucleation. Materials and Methods: A total of 55 patients, who had been treated for an odontogenic cyst with cyst enucleation from 2000 to 2009 at the department of Oral and Maxillofacial Surgery, Samsung Medical Center, were included in this study. Patients who were followed-up for more than 1 year were included. Two groups were defined according to the bone graft (with or without a bone graft) after cyst enucleation. The differences in the healing periods and patterns of bone healing were compared clinically and radiologically. The postoperative 1 year radiographs were analyzed for bone healing and density. Statistical analysis was performed using a Pearson chi square test and Wilcoxon rank-sum test. Results: More infection signs were observed in the bone graft group than in the other group, but there was no statistically significant difference. Radiographically, there was also no significant difference in the size of the radiolucent lesions between the two groups. Conclusion: There was no significant difference in healing between the groups with a bone graft and without bone graft after cyst enucleation.
The calcifying odontogenic cyst is rare lesion having features of both cyst and neoplasm. Clinically it frequently occurs in young adult and more frequent in the anterior area of the jaw. Radiographically, this lesion has several radiopaque foci within unilocular or multilocular radiolucency. The authors experienced cystic and neoplastic types of calcifying odontogenic cysts in the anterior area of the mandible in a 16-year-old male and 21-year-old female patients who suffered from pain, swelling and teeth displacemeent on the affected area. And we discussed the clinical, radiological and histopathological features with a brief review of the literatures.
