Job Characteristics and Status of Community Occupational Therapist : Focus on OTs in Public Health Centers (지역사회 작업치료사의 업무 특성 및 실태 조사 : 보건소 근무 작업치료사를 중심으로)

  • Objective : This study was conducted to identify occupational therapists working in public health centers, the characteristics and actual conditions of occupational therapists in the community, and use them as basic data on occupational therapists in the community as of 2020. Methods : 77 questionnaires were replied by e-mail from OTs work at nationwide health public centers. Job characteristics and status were analysed by descriptive statistics and check correlation between job satisfaction and other factors. Results : Most survey respondents were female(77.9%) and 20-30(96.1%).. Some occupational therapists worked for dementia related team(72.7%) and others worked for like visiting care, health care, and rehabilitation center etc. Rate of experiences of public health center was 1-2 years(67.5%), the most common type of contract was flexible part-time worker(61%) and work intensity(94.8%) and satisfaction of work was very high(85.7%). The highest difficulty of their job was budget administrative work(26.7%) and of non-work difficulty was inequality under contracts(27.2%). They usually participated at dementia shelter, visiting OT, group OT. Difficulty of their job was high in budget administration, dementia shelters, and visiting work treatments. Goals of treatment were high in improvement of cognitive ability and, family support. Frequency of treatment was high in improvement of cognitive therapy, family support, and evaluation. Occupational therapy targets for health centers were dementia, the general elderly, and adult brain lesions, including those for ordinary people, psychiatric disorders and children. It was found that the primary occupations for evaluation were nurses (35.7%) and occupational therapists (33.7%), and that MMSE-DS, SGDS, and SMCQ were used a lot. Conclusion : This study could identify the job characteristics and status of community OTs. We hope that this result could be basic data for building expertise and role for community OTs in changing situations like community cares.

The Service of Community-Based Rehabilitation in Stroke Patient: A Case Study (뇌졸중 환자의 지역사회중심재활서비스: 사례연구)

  • Objective : The purpose of this case study was to proposed a introduction and result of the disabled with stroke in community using services of Community-Based Rehabilitation. Methods : The subject was a 63-years old female patient with stroke who first visit to Public health center in July. She was evaluated on activity daily living action level by using the Modified Barthel Index(MBI) and depression level by using Beck Depression Inventory(BDI). The intervention that was suggested by WHO were Community-Based Rehabilitation(CBR) and general occupational therapy. We were applied from April, 27th 2011, to November 30th, 2011 by occupational therapist. Results : The patient's total MBI score increased from 35 to 55, it means that activity daily living performance capacity was improved. And BDI score decreased from 26 to 14, it means that depressed symptoms were in great decline. She could stand until 5-10 minutes at first, but later stand over 30minutes now. It means her muscle strength and endurance level was increased. The patient's mood condition also was stabilized and then she often showed her smiling face during the therapy. And her protector feels the client's change too. Conclusion : We could know that function level and quality of life of the patient with stroke who return to the community was improved through service of community-based rehabilitation.

Delivery of Therapist's Intervention to the Education of Ayres Sensory Integration$^{(R)}$ (ASI$^{(R)}$) (Ayres Sensory Integration (ASI$^{(R)}$) 중재 교육에 따른 치료사의 치료 수행도 변화)

  • Objective : This study was to perform the education of the ASI$^{(R)}$ intervention for six occupational therapists and to know the delivery of ASI$^{(R)}$ core principle through a self-assessment, a peer-assessment, an expert-assessment. Methods : The study performed from November 2013 to June 2014 for six occupational therapists without completion of the education of ASI$^{(R)}$ intervention. The participants were educated about the ASI$^{(R)}$ intervention during 8 weeks and took and assessed films before and after education. The assessment was the self-assessment, the peer-assessment, the expert-assessment and the data of assessment was analyzed by Mann-Whitney and ICC. Results : The result of process factors before and after education according to methods of assessment, the self-assessment was significant in 'self-regulation,' 'collaboration,' 'ensures success,' 'play,' 'alliance,' and 'total item'. The peer-assessment was significant in all item exception 'safety'. The expert-assessment was significant in all items exception 'sensory opportunities'. The results of self-assessment and expert-assessment before and after the education of ASI$^{(R)}$ intervention were significant in 'safety'. Conclusion : The results of this study provide to need the education of ASI$^{(R)}$ intervention for accuracy sensory integrative intervention. The occupational therapists need to check the style of intervention.

The Effect of Education on Motor Skill Assessment Tool for Occupational Therapists (작업치료사를 위한 운동성 평가도구의 교육효과)

  • Objective : The purpose of this study is to investigate the effect of intensive one-day training on Movement Assessment Battery for Children-2 screening developmental coordination disorder. Methods : We provided intensive one-day training which combined lecture and practice on MABC-2 assessment tools for 33 trainees. Interrater reliability was calculated by using Intraclass correlation coefficient (ICC) with the raw score of results scored simultaneously by experts and trainees of MABC-2. Results : The 33 female trainees with an average of five years and three months of experience and less experience using motor skill assessment tools scored raw score while watching the performance scenes of normal children. The results of the ICC between experts and training participants in MABC-2 obtained a high confidence 0.95 and above (p<.001). Conclusion : Clinicians who participated in one-day training seems to use MABC-2 effectively. Through this, one-day training seems to be effective in acquiring new motor skill assessment tools.

A Study on the Effectiveness of Rehabilitation by Virtual Reality Program: Systematic Review (가상현실 프로그램을 사용한 재활치료의 효과성 연구: 체계적 고찰)

  • The purpose of this study is to provide a basic data about the virtual reality rehabilitation therapy to the occupational therapist in the domestic clinical field by systematically analyzing the types and effects of the rehabilitation therapy using the virtual reality program. Research articles were retrieved from January 2007 to December 2017 using the Research Information Sharing Service (RISS), Google Scholar, and Pubmed database. The main search terms were 'Virtual Reality AND Rehabilitation' and 'Virtual reality AND Effect'. Finally, 10 foreign studies were selected as the subjects of this study. Selected studies were organized using PICO (Patient, Intervention, Comparison, Outcome) format. The results were as follows: The subjects were divided into 4 types, stroke, brain tumor, Parkinson 's disease, and kidney disease. The tools used for arbitration were three IREX (30 %), two X-box Kinect (20 %), two Nintendo Wii (20 %), one [Existing tool + VR] (10 %), one [Mobile application + VR] (10 %). As a result of the intervention, improvement of function was observed in all 10 studies from overseas, and physical function was 24 times (66 %), mental function 6 times (17 %), cognitive function 5 times (14 %), Activity of daily living 1 time (10 %). this study could be used as a basic resource to enhance the professionalism and quality of rehabilitation services and expand the scope by organizing virtual reality-based rehabilitation and its effects.

Application of Lee Silverman Voice Treatment-BIG(LSVT-BIG) Intervention to Improve Motor Functions and Quality of Life in People With Parkinson Disease (파킨슨병 환자에게 Lee Silverman Voice Treatment-BIG(LSVT-BIG) 프로그램의 적용이 운동기능과 삶의 질에 미치는 효과)

  • Objective: The purpose of this study is to compare the effect of Lee Silverman Voice Treatment-BIG(LSVT-BIG) intervention which consisted of standardized exercise programs and occupation-based activities for people with Parkinson Disease(PD) on motor functions and quality of life Methods: This study applied a one group pretest-posttest design. The experiment was divided into two parts: pre intervention and post intervention period. Before and after LSVT-BIG intervention, Unified Parkinson's Disease Rating Scale(UPDRS), Time up and go(TUG), Parkinson's Disease Questionnaire-39(PDQ-39) were used to measure the participants' motor functions and quality of life. Based on the LSVT-BIG protocol, three participants received 16 one-hour sessions over 4 weeks by a certified occupational therapist. The results were analyzed by using SPSS. Results: There were improvements in UPDRS and TUG. Additionally, PDQ-39 scores decreased in all participants, which means that their quality of life was improved. Conclusions: The study demonstrated positive effects of LSVT-BIG intervention on motor functions and quality of life of patients with PD.

Application of Cognitive Enhancement Protocol Based on Information & Communication Technology Program to Improve Cognitive Level of Older Adults Residents in Small-Sized City Community: A Pilot Study (중소도시 지역사회 거주 노인의 치매예방을 위한 Information & Communication Technology 프로그램 기반 인지향상 프로토콜 적용: 파일럿(Pilot) 연구)

  • Objective : This study, as a preliminary study, applied an Information & Communication Technology (ICT) home-based program to elderly people aged 65 years or older to confirm the effect of the cognitive enhancement program and to find the possibility of remote rehabilitation. Methods : This study from August to October 2022, three subjects were selected and the intervention was conducted for about 2 months. This intervention was conducted using Korean version of Mini-Mental State Examination, Korean version of Montreal Cognitive Assessment (MoCA-K), Computer Cognitive Senior Assessment System, and the Center for Epidemiologic Studies Depression scale to evaluate cognitive improvement before and after the program. The therapist remotely set the level of cognitive training according to the subject's level through weekly feedback. Results : After the intervention, all subjects showed improved scores in most items of the MoCA-K conducted before and after the intervention. In addition, among the items of Cotras-pro, upper cognition, language ability, attention, visual perception, and memory were improved. Conclusion : Cognitive rehabilitation training using an ICT home-based program not only prevented dementia but also made it habitual. Through this study, it was confirmed that remote rehabilitation for the elderly could be possible.

Dialog System based on Speech Recognition for the Elderly with Dementia (음성인식에 기초한 치매환자 노인을 위한 대화시스템)

  • This study aims at developing dialog system to improve the quality of life of the elderly with a dementia. The proposed system mainly consists of three modules including speech recognition, automatic search of the time-sorted dialog database, and agreeable responses with the recorded voices of caregivers. For the first step, the dialog that dementia patients often utter at a nursing home is first investigated. Next, the system is organized to recognize the utterances in order to meet their requests or demands. The system is then responded with recorded voices of professional caregivers. For evaluation of the system, the comparison study was carried out when the system was introduced or not, respectively. The occupational therapists then evaluated a male subjects reaction to the system by photographing his behaviors. The evaluation results showed that the dialog system was more responsive in catering to the needs of dementia patient than professional caregivers. Moreover, the proposed system led the patient to talk more than caregivers did in mutual communication.

Communication Aid System For Dementia Patients (치매환자를 위한 대화 보조 시스템)

  • The goat of the present research is to improve the quality of life of both the elderly patients with dementia and their caregivers. For this Purpose, we developed a communication aid system that is consisted of three modules such as speech recognition engine, graphical agent. and database classified by a nursing schedule. The system was evaluated in an actual environment of nursing facility by introducing the system to an older mail patient with dementia. The comparison study was then carried out with and without system, respectively. The occupational therapists then evaluated subject"s reaction to the system by photographing his behaviors. The evaluation results revealed that the proposed system was more responsive in catering to needs of subject than professional caregivers. Moreover we could see that the frequency of causing the utterances of subject increased by introducing the system.

Comparison of the Immediate Effects of Kinesio Taping on the Dynamic Balance of Stable Ankle and Functional Ankle Instability among Young Adults in Their Twenties: a preliminary study

  • Purpose : Kinesio taping applied to the ankle varies, and if the overall ankle is taped as much as possible, several effects, including balance, can be expected, but clinically the cost reduction for intervention is very important. Therefore, this study attempted to find out the optimal way to the effect and cost of kinesio taping on ankle dynamic balance. Methods : The subject of this study was 24 university students in their 20s (male: 13, female: 11), who received sufficient explanation of the purpose and method of the study. The Cumberland ankle instability tool (CAIT) questionnaire was used for the degree of ankle instability of the study subjects. If the subject's CAIT score was 28 points or more, it was classified as a stable ankle, and if the score was 24 points or less, it was classified as functional ankle instability (FAI). In this study, Biodex Balance System® measurement equipment was used to calculate the dynamic balance of study participants. The application of kinesio taping was performed by one physical therapist to attach in the same way, and a method of wrapping the ankle joint was applied in the eight-shaped bandage. Results : The results are as follows in before and after taping of the stable ankle and FAI group. There was no significant difference in the overall, anterior-posterior, and medial-lateral stability index. The comparison is as follows between groups for the differences (post-pre value) in before and after the application of kinesio taping. There were no significant differences between groups in all the overall, anterior-posterior, and medial-lateral stability index. Conclusion : In this study, no significant difference in kinesio taping was found in the dynamic balance of stable ankle and FAI (overall, anterior-posterior, and medial-lateral). It is necessary to continue to study ways to find the maximum effect while minimally attaching them to the application method of ankle kinesio taping.
