척수손상환자의 고령화에 따른 2차합병증 발생과 만성질환발병에 관한 연구 (A Study of Occurrence of Secondary Complications and Chronic Diseases due to Aging of Spinal Cord Injury)

  • 본 연구는 척수손상이 고령이 됨에 따라 만성질환과 2차 합병증에 미치는 요인을 알아봄으로써 효율적으로 관리와 예방에 관한 기초적인 자료를 제공하고자 한다. 대상자는 2013년 4월부터 2018년 4월까지 S병원에 입원한 척수손상 200명 대상으로 하였다. 의무기록 조사를 통해 만성질환인 고혈압, 당뇨, 간염, 결핵과 2차 합병증으로 폐렴, 기립성저혈압, 욕창, 부종, 자율신경반사항진, 심부정맥 혈전증, 이소성골화증, 중추성통증, 경직(상지/하지), 근 골격계 통증, 변비, 구축, 우울증, 골절, 고칼슘혈증, 불면증, 신장결석, 신경인성 방광 및 장, 비요로감염, 비만, 골다공증, 폐혈증, 수면무호흡, 요로감염의 발생유무를 조사하였다. 연구 결과 50세 미만보다 50세 이상 척수손상이 만성질환 및 2차합병증에 영향을 미치는 요인들로 나타났다. 50세 이상의 만성질환 유병 오즈비는 고혈압 11.8배, 당뇨 6.7배이며, 2차합병증 유병 오즈비는 골다공증 7.5배, 폐렴 5.2배, 신경인성통증 0.4배로 나타났다. 고령척수손상의 만성질환 및 2차합병증의 지속적인 관리와 서비스의 필요성을 시사하고, 향후 대상자의 확대와 다양한 특성을 포함한 연구가 이루어줘야 할 것이다.

Relationship Between Sleep Duration and Suicidal Ideation Among Farmers: Safety for Agricultural Injuries of Farmers Cohort Study of Jeju, Korea

  • Background: This study was conducted to identify the association between sleep duration and suicidal ideation among farmers in Korea. Methods: We used Safety for Agricultural Injuries of Farmers cohort data collected from September 2015 to June 2018, which was an agricultural survey on the health and behaviors of adult farmers in Jeju island, Korea. Results: A total of 964 participants were included in the analysis, and 3.7% of them were identified with having suicidal ideation. The frequencies of average daily sleep duration of <6 h (short sleep), 6-8 h (normal sleep), and >8 h (long sleep) were 24.4%, 70%, and 5.6%, respectively. Multivariate analyses revealed that short sleep duration was significantly associated with suicidal ideation compared with normal sleep duration (odds ratio = 2.49, 95% confidence interval = 1.07-5.77). Conclusion: Our findings suggest that short sleep duration in farmers result in higher suicidal ideation. Because individuals who have suicidal ideation often commit suicide, careful monitoring is required to prevent suicide in farmers with short sleep duration.

  • 본 연구는 국내 농업인의 천식 및 만성 폐쇄성 폐질환을 파악하기 위해 「농업인의 업무상 질병 및 손상조사」를 조사하였다. 관련된 국내연구자료가 미비하여 국외 농업인의 천식 및 만성 폐쇄성 폐질환 연구결과를 찾아보았고, 그 결과. 국내 농업인들의 천식 및 만성 폐쇄성 폐질환 유병률은 국외 연구 결과들보다 낮게 나타났다. 실제 국내 농업인의 호흡기 질환 유병률은 더 높을 수 있으므로, 추가적인 연구가 필요하고, 호흡기 질환 예방대책이 마련되어야 한다.

직업성 피부질환에 대한 현황 파악 및 관리 대책 수립을 위한 연구 (A Study on the Establishment of Management Methods about Occupational Dermatoses)

  • Occupational dermatosis is one of the most prevalent occupational disorders. However, the extent of the occupational dermatoses including incidences and prevalencies of each disease entity, and etiologic materials are not yet welt stated in Korea. Authors reviewed the literatures on the statistic data and reports on the occupational dermatoses, and surveyed on the occupational dermatoses in two factories, and surveyed the physicians responsible to the occupational dermatoses with formed questionnaire. The results are as follows; 1. Among medical journals published since 1964, there were 31 articles on the occupational dermatoses. Of 31 articles, 18 were case reports and all others were review articles. Of 18 case reports, 9 were epidemiologic survey. The Workers' Periodic Health Examinations revealed that prevalence of the occupational dermatoses was highest(4.36 per 10,000 workers) in 1974, but number of the cases reported were decreased sharply since 1978 with some tendency to increase since 1981. There were 2,240 reported cases of occupational dermatoses between 1966 and 1992, which is 1.90% of all the reported occupational diseases. Skin infection and injuries due to chemicals were most frequent and there were 6 cases of skin cancer. 2. In an epidemiological survey on the dermatoses among 995 workers in a metal product manufacturing factory and 225 workers in a coal chemical factory, there were 794 with dermatomycosis, 296 workers with acne, 130 workers with scar, 123 workers with deformity of toe nails. Scars, photosensitivity dermatitis, deformity of finger and toe nails, and acne were more prevalent in the metal product manufacturing factory(p<0.05). In the metal prouct manufacturing factory, workers treating organic solvents and oils had more dermatoses than those without treating the materials(p<0.05). On the skin patch performed on 16 workers in the metal product manufacturing factory, there were 8 cases of irritation dermatitis and 5 cases of contact dermatitis. Prevalence of contact dermatitis in the metal product manufacturing factory was 1.3%. 3. On the questionnaire survey, 34 dermatologists, 29 doctors of preventive medicine, and 22 family physician replied. The proportion of occupational etiology among all dermatoses assumed by the physicians were below 9%, and the most important occupational dermatosis in Korea was contact dermatitis. Main etiologic materials related to the occupational dermatosis were organic solvent, acid and alkali, and metals. The reason for the scarcity of report of occupational dermatoses were difficulty in diagnosis and physician's ignorance of the occupational etiology. They replied that to prevent the occupational dermatoses in the workplace, the use of protective devices was most important, and development of diagnostic criteria on the occupational dermatoses is urgent. Above results shows us that there is many workers with occupational dermatoses, but they are mostly unreported. Measures to prevent and manage the occupational dermatoses are not satisfactory at present. Hence, authors suggest measures for the precise diagnosis, report and prevention of the occupational dermatoses. a. Dermatologist, preventive physician, and industrial hygienist should work as a team to examine the high risk group and establish the preventive measures. b. Disease entities, diagnostic criteria of occupational dermatoses should be listed, criteria for the compensation and job fitting at recruitment should be established, and manual for the proper treatment and effective prevention of each occupational dermatosis should be developed. c. Patch test antigens against each occupational category should be developed and it should be available to any physicians responsible. d. To facilitate the diagnosis of occupational dermatoses by the doctors responsible for the Workersr Periodic Health Examination, development of standardized questionnaire, education on the techniques of the patch test, and cooperation with the dermatologist in diagnosis of occupational dermatoses is essential.

Occupational Dermatoses by Type of Work in Greece

  • Background: To elucidate the relationship between seven occupational dermatoses (ODs) and 20 types of work in Greece. Methods: This was a prevalence epidemiologic study of certain ODs among 4,000 workers employed in 20 types of enterprise, in 104 companies, in 2006-2012, using data from company medical records, questionnaires, occupational medical, and special examinations. The ${\chi}^2$ test was applied to reveal statistically significant relationships between types of enterprises and occurrence of ODs. Results: A high percentage (39.9%) of employees included in the study population suffered from ODs. The highest prevalence rates were noted among hairdressers (of contact dermatitis: 30%), cooks (of contact dermatitis: 29.5%), bitumen workers (of acne: 23.5%), car industry workers (of mechanical injury: 15%), construction workers (of contact urticaria: 29.5%), industrial cleaning workers (of chemical burns: 13%), and farmers (of malignant tumors: 5.5%).We observed several statistical significant correlations between ODs (acute and chronic contact dermatitis, urticaria, mechanical injury, acne, burns, skin cancer) and certain types of enterprises. There was no statistically significant correlation between gender and prevalence of ODs, except for dermatoses caused by mechanical injuries afflicting mainly men [${\chi}^2(1)=13.40$, p < 0.001] and for chronic contact dermatitis [${\chi}^2(1)=5.53$, p=0.019] afflicting mainly women. Conclusion: Prevalence of ODs is high in Greece, contrary to all official reports by the Greek National Institute of Health. There is a need to introduce a nationwide voluntary surveillance system for reporting ODs and to enhance skin protection measures at work.

The Gender Difference in the Occupational Hazards and Injuries of Cleaning Workers and Janitors

  • Objective: The purpose of this study is to analyze the accident characteristics according to the gender of the injured workers in building cleaning and to reflect them in the Industrial Accident Prevention Policy. Background: An analysis of industrial accidents is an essential process for establishing systematic industrial accident prevention measures. In order to establish industrial accident prevention measures for workers effectively, it is necessary to analyze accident characteristics by job type for workers who do the same work. Method: In this study, we analyzed the accident characteristics of 1,645 janitors who were approved of work-related injuries in 2015. We also analyzed the characteristics according to gender by dividing them into worker-related factors and accident-related factors. Results: The accidents caused to the janitors showed different characteristics according to gender, age, work experience, agency of accident, and distribution of original cause materials. In other words, 70.2% occurred to workers over 60 years old and 56.2% occurred to unskilled workers with less than a year of work experience. In the case of accident pattern, 79.1% occurred in tripping (slip) hazards, and 68.2% of accidents occurred on the floor (including the ground) and the stairs, indicating that the accident occurred most frequently during cleaning work on the floor or stairs. Conclusion and Application: The results of the study on the accident characteristics of the janitors can be used as basic data for systematic preventive measures against accidents occurring to the elderly female workers in the service industry.

황산 손상 환자의 임상적 고찰 (A Clinical Analysis of Patient Exposure to Sulfuric Acid Injured)

  • Purpose: This study was conducted to identify the characteristics associated with sulfuric acid injury in the emergency department. Methods: Data were collected retrospectively from January 2007 to December 2015 on all sulfuric acid injuries presenting to the emergency department in Gu-mi Soonchunhyung University Hospital. Patients injured by sulfuric acid were recorded over a nine year study period and collected data included demographics, injury mechanism, injured body part, hospital care and final diagnosis. Results: A total of 26 cases were identified. Most patients were male (88.5%) and the face was the most commonly injured body part. The most common mechanism of injury was splashing injury. A total of 16 (61.5%) patients were identified as having lesions worse than second degree burns. Conclusion: Sulfuric acid can cause severe and fatal skin burn. When working with sulfuric acid, acid proof protect clothing, goggles and glove should be worn. Furthermore, safety education and workplace environment improvement are necessary to reduce sulfuric acid injury.

Ontology-based Safety Risk Interactions Analysis for Supporting Pre-task Planning

  • The construction industry remains serious accidents, injuries, and fatalities due to it's unique, dynamic, and temporary nature. On workplace sites, Safety pre-task planning is one of the efforts to minimize injuries and help construction personnel to identify potential hazards. However, the working conditions are complicated. Many activities, including tasks or job steps, are executing at the same time and place. It may lead to an increase in the risks from simultaneous tasks. This paper contributes to addressing this issue by introducing a safety risk interaction analyzing framework. To accomplish this objective, accident reports of the Occupational Safety and Health Administration (OSHA) are investigated. The pairs of task incompatibility, which have time-space conflicts and lead to incidents, are found. Ontology technology is applied to build the risk database, in which the information is acquired, structuralized. The proposed system is expected to improve pre-task planning efficiency and relieve the burdens encountered by safety managers. A user scenario is also discussed to demonstrate how the ontology supports pre-task planning in practice.

노·사주도 건강증진활동 비용지원사업의 산재예방 효과 (Impact of Subsidies for Labor and Management-directed Health Promotion Activities on Industrial Accidents Prevention)

  • Purpose: This study was conducted to compare the rates of industrial accidents in workplaces that adopted the 2011 Workplace Health Promotion Subsidy Scheme before and after the implementation. Methods: The study analyzed the raw data of industrial accidents of 304 workplaces which received subsidies for health promotion activities in 2011. The raw data covered the period from February 2010 to July 2012, based on the dates of industrial accidents. Results: Workplaces subsidized for health promotion activities reported fewer occurrences in staff injuries and illnesses than before the subsidization, as the total number of industrial accident victims dropped from 0.35 to 0.24. The rate of industrial accidents also dropped from 0.49 to 0.35, with the number of working days decreased from 35,433 to 23.867, about 33%. Conclusion: The study showed that financial support for health-promoting activities contributed to the decease in industrial accidents, which is an important indication advocating the need for corporate and government investment on workers health promotion programs. Furthermore, this study is also significant as it is the first research conducted in Korea to examine the impact of a workers health promotion project using a direct indicator, the rate of industrial accidents.

의원방문 근로자들의 업무상 사고.부상 실태에 관한 연구 (A Study on Industrial Safety Accidents Treated at A Primary Care Clinic)

  • This study was surveyed to assess the status of safety accidents occurred in work-places and prepare the fundamental data and prevent the safety accidents. The authors reviewed and analysed the charts of accident cases treated at a primary care clinic in A city from January 1991 to December 2006. The data were classified according to the USA Standards Institute and International Labour Organization method. We analyzed the data using SPSS program. The results were as follows : 1. The total cases of accidents were 455 for 8 years. 2. Accidents were mostly common in the workers who are in thirties and forties age(84.4%). 3. As season variation, spring and summer were common than others, but there was no statistical significance on season, month and weekday. 4. The most frequent injured part of the body were hand and finger, which was 36.0% among total cases. 5. According to the accidents type, cases of caught in, under or between were most frequently observed as 53.9% of the total cases. 6. The most common source of injuries was power machine(50.5%). 7. According to the unsafe acts, cases of carelessness and unsafe information were most frequently observed as 71.2% of the total cases. 8. Admission rate(5.5%) and official report rate(2.2%) were very low rate. As above results, the authors recommend to prepare the systemic control programs on environmental and human factors of safety accidents such as improving the working conditions, working facilities, working methods and safety education, and control of working time for working day.
