Healing of Aneurysm after Treatment Using Flow Diverter Stent : Histopathological Study in Experimental Canine Carotid Side Wall Aneurysm

  • Objective : Despite widespread use of flow diverters (FDs) to treat aneurysms, the exact healing mechanism associated with FDs remains poorly understood. We aim to describe the healing process of aneurysms treated using FDs by demonstrating the histopathologic progression in a canine aneurysm model. Methods : Twenty-one side wall aneurysms were created in common carotid artery of eight dogs and treated with two different FDs. Angiographic follow-ups were done immediately after placement of the device, 4 weeks and 12 weeks. At last follow-up, the aneurysm and the device-implanted parent artery were harvested. Results : Histopathologic findings of aneurysms at 4 weeks follow-up showed intra-aneurysm thrombus formation in laminating fashion, and neointimal thickening at the mid-segment of aneurysm. However, there are inhomogenous findings in aneurysms treated with the same type of FD showing same angiographic outcomes. At 12 weeks, aneurysms of complete and near-complete occlusion revealed markedly shrunken aneurysm filled with organized connective tissues with thin neointima. Aneurysms of incomplete occlusion at 12 weeks showed small amount of organized thrombus around fringe neck and large empty space with thick neointmal formation. Neointimal thickness and diameter stenosis was not significantly different between the groups of FD specification and follow-up period. Conclusion : Intra-aneurysmal thrombus formation and organization seem to be an important factor for the complete occlusion of aneurysms treated using the FD. Neointimal formation could occur along the struts of the FD independently of intra-aneurysmal thrombus formation. However, neointimal formation could not solely lead to complete aneurysm healing.

THE EFFECT OF OCCLUSAL SPLINT THERAPY ON CONDYLAR POSITIONAL CHANGES IN MALOCCLUSION PATIENTS (부정교합환자에서 교합안정장치의 사용이 하악과두의 위치변화에 미치는 영향에 관한 연구)

  • There are evidences that occlusal splint therapy is critical to diagnose hidden akeleto-occlusal disharmonies in malocclusion patients and capable of enhancing stability after orthodontic treatment. In addition, evidences have implicated occlusal splint therapy in condylar positional changes during TMJ disorder treatment. In view of these evidences, this study was performed to investigate the effect of occlusal splint therapy on condylar positional changes in malocclusion patients and the possible clinical application of the occlusal splint as an additional orthodontic tool. For this study, 8 Angle's Class I malocclusion patients, who had centric occlusion-centric relation discrepancy within 1.0 mm and had no clinical symptoms of TMJ disorder, were selected as control group. And 22 malocclusion patients who had centric occlusion-centric relation discrepancy over 1.0 mm were selected and subdivided as Class I Malocclusion group, Class II div. 1 malocclusion group, Class II div. 2 malocclusion group, Open bite group, and Mandibular asymmetry group. For each subject the occlusal splint with mutually protected type of occlusal scheme was applied for 3 months. Condylar positions in centric relation and centric occlusion were measured using Panadent articulators and Panadent condylar position indicator (CPI) before and after occlusal splint therapy. On the basis of this study, the following conclusions might be drawn: 1, In control group, Class II div. 2 malocclusion group, and mandibular assymetry group, there were no significant differences in condylar positions before and after occlusal splint therapy. 2. In Class I malocclusion group, condyles were moved $0.27{\pm}0.45mm$ forward (p < 0.05) and $0.98{\pm}0.25mm$ upward (p < 0.01) after occlusal splint therapy. 3. In Class I malocclusion group, condyles were moved $0.24{\pm}0.21mm$ backward (p < 0.05) and $1.01{\pm}0.33mm$ upward (p < 0.01) after occlusal splint therapy. 4. In open bite group, condyles were moved $1.24{\pm}0.30mm$ upward (p < 0.01) after occlusal splint therapy. 5. In both control and experimental groups, there were no significant differences in lateral condylar positions before and after occlusal splint therapy.

A RADIOGRAPHIC STUDY OF MANDIBULAR CONDYLE SHAPE AND POSITION: A COMPARISON OF TRANSCRANIAL RADIOGRAMS AND INDIVIDUALIZED CORRECTED TOMOGRAMS (경두개방사선사진과 측방 개별화 단층방사선사진을 이용한 하악과두의 형태와 위치에 관한 연구)

  • The purpose of this study was to observe mandibular condyle shape and position in an asymptomatic population. In order to carry out this study, 142 temporomandibular joints in 71 adults(35 males, 36 females), who were asymptomatic for temporomandibular joint disorders and had no history of prosthodontic or orthodontic treatments, were selected, and radiographed using the Accurd-200 head holder(Denar Co., U.S.A) for transcranial radiograms and the Sectography(Denar Co., U.S.A) for lateral individualized corrected tomograms. Mandibular condyles were classified morphologically and evaluated in positional relationships with articular fossae and articular eminences at centric occlusion and 1 inch mouth opening. The obtained results were as follows; 1. In the classification of mandibular condyle shape, the convex type was more prevalent in transcranial radiograms and tomograms taken at medial, central, and lateral locations. 2. In the mandibular condyle position at centric occlusion, the mandibular condyles were placed posterior to the center of articular fossae in transcranial radiograms and anterior to the center of articular fossae in tomograms taken at medial, central, and lateral locations. 3. In the mandibular condyle position in right and left TMJs at centric occlusion, the mandibular condyles were placed bilateral asymmetric relationships to the articular fossae in transcranial radiograms and tomograms taken at medial, central, and lateral locations. 4. In the mandibular condyle position at 1 inch mouth opening, the mandibular condyles were placed anterior to the articular eminences in transcranial radiograms and tomograms taken at central location and posterior to the articular eminences in tomograms taken at medial and lateral locations.

  • The prime objective of this study was to analyze the positional relationship of the lips to the anterior teeth according to age, sex, and both sides of the dental arch in 381 healthy Korean adults with normal occlusion ranging from 17 to 70 years over old. The results were as follows; 1. In the position of the corner of the mouth to the teeth, the range from center of canine to mesial side of first premolar was revealed the incidence of 81~87%, the distal side of canine was the hiqhest frequency of 39~43%. 2. A straight type (76~79%) of lip line was prevailed against the curved type in rest position. 3. In the exposed type of anterior teeth to upper lip in rest position, type III(50~52%) Grade 2 (26~29%) was revealed the highest frequency of occurences. 4. No significant difference in both sexes and sides of the dental arch was observed. 5. Lip position to the teeth with aging was showed the tendency toward straight type in lip line and toward reduction of the number of exposed type of upper anterior teeth to upper lip, and the corner of the mouth was placed posteriorly.

The Size, Form of Dental Arch and Occlusion in Dental Hygiene Students in Korea (한국 치위생과 학생의 치열궁 크기 및 형태와 교합)

  • The purpose of this study was to examine the size, form of dental arch and occlusion type in college students in our country and the relationship of the factors. The subjects in this study were 210 selected dental hygiene students. The collected data were analyzed by a statistical package PASW 18.0. When their size, form and occlusion of dental arch were analyzed, the inter-canine width of the maxillary was 34.38 mm, and the inter-first molar width was 52.05 mm. The canine depth was 8.60 mm, and the first molar depth was 28.69 mm. As for the mandibular, the inter-canine width was 26.42 mm, and the inter-first molar width was 44.83 mm. The canine depth was 5.54 mm, and the first molar depth was 24.38 mm. Concerning the form of dental arch, the percentage of normal dental arch in the maxillary stood at 29.0, and that of crowding stood at 60.5. The percentage of spacing stood at 10.5. In the case of the mandibular, the percentage of normal dental arch stood at 29.0; crowding, 55.7; and spacing, 15.2. In relation to occlusion, the percentage of normal occlusion stood at 16.7. As to malocclusion, class I that accounted for 55.7 was most common, and class II and class III respectively accounted for 20.5 and 7.1. When the size of dental arch was compared according to the form and occlusion of it, dental arch was largest (45.95 mm) in size when the form of dental arch in the inter-first molar width of the mandibular was spacing. The size of dental arch was 44.73 mm when its form in the same region was normal, and that was 44.58 mm when its form in the same region was crowding (p=0.032). Regarding the relationship between the form and occlusion of dental arch, crowding was most common when there were class I, II and III of malocclusion both in the maxillary and mandibular.

Implementation and estimation of the urophonography measurement system for diagnosis of Lower urinary tract symptom (하부요로계 폐색 진단용 요류음 계측 시스템의 구현 및 평가)

  • A sensor and measuring system were implemented to measure both uroflow and urophonography signal during urination for diagnose the lower urinary tract symptom. The implemented system was composed of the uroflow sensor using the load-cell, the urophonography measurement sensor so as stethoscope type, pre-processing part for sensors signal detection, amplifier and filter, system control parts and PC measurement program. A simulator of the lower urinary system that is experimental equipment implemented for evaluate the developed system. The two signals were measured using implemented system and analyzed these signal by means of time domain and frequency domain for extraction of the characteristic parameter which can most effectively reflected by the occlusion of the lower urinary system. Furthermore two signals were measured and analyzed with the subject of 5 healthy adult for clinical application possibility of the implemented system. As a result, the most influence bandwidth of effect according to occlusion of the lower urinary system is $253{\sim}282$ Hz of the urophonography signal.

  • Two hypertensive men with DeBakey type III dissection were admitted due to acute onset of leg ischemia.One patient had ischemia of both legs,The other patient had ischemia of the right leg.Angiograms showed occlusion of aortic bifurcation in one patient and occlusion of right common iliac artery and right renal artery in the other patient.The first patient who had ischemia of both legs was relieved by axillo-bifemoral bypass operation and the second patient with right leg ischemia by femoro-femoral bypass.The dissection of the aorta was successfully managed by conservative measures including hypotensive medication.The bypass grafts was functioning well one year later.The aortic dissection should not be overlooked as an etiology of acute onset of ischemia of the lower extremities.

Estradiol Valerate Exerts Neuroprotective Effects in Ischemic Rat Brain when Administered after Middle Cerebral Artery Occlusion

  • Stroke occurs when local thrombosis, embolic particle or the rupture of blood vessele interrupts the blood floe to the brain. $\beta$-estradiol 17-valerate has been reported to exert neuroprotective effects when administered before an ischemic insult. Recently, the pathophysiology of cerebral ischemia has been studied extensively in rat with various methods. In the present study, we investigates whether $\beta$-estrodiol 17-valerate can protect against brain injury. RNA sample were extracted from the hippocampus of female rat, reverse-transcription in the presence of [$\alpha$32p] dATP. Differential gene express-ion profiles were revealed (Bone morphogenetic protein type 1A receptor, Protein disulphide isomerase, Leukemia inhibitor factor receptor, cytochrome bc- 1 complex-x core P, thiol-specific antioxidant protein). RT-PCR was used to validate the relative expression pattern obtained by the cDNA array. The precise relationship between the early expression of recovery genes and stroke is a matter of luther investigation.
