두개하악장애환자의 전치부접촉과 측모두부형태간의 관계에 관한 연구 (A Study on the Correlation between Anterior Tooth Contacts and Cephalometric Profile in Patients with Craniomandibular Disorders)

  • 80 patients who presented at Wonkwang University Dental Hospital with craniomandibular disorders were collected for this study. To observe the occlusal contact pattern such as contact numbers, contact force and presence or absence of anterior occlusal contact, the author used T-Scan system (Tekscan, Inc, U.S.A.) with are computerized occlusal analysing system. And to study the correlation between craniofacial profile and occlusal contact pattern, cephalogram were also taken, The cephalometric items related to growth pattern, jaw bone relation and denture pattern were measured and analysed according to routine method by computerized program. The obtained data were statistically processed with SPSS/PC+ package about anterior contact pattern and its craniofacial relationship. The obtained results were as follows : 1. In terms of growth pattern, patients without anterior tooth contacts showed a tendency to downward growth of craniofacial profile. The value in this subjects were significantly different from the value of patients with anterior tooth contacts in items of low gonial angle, Jarabak ratio, SN to GoMe angle, FMA, occlusal plane to mandibular plane angle and ramus height. 2. In terms of jaw bone relationship, patients without anterior tooth contacts showed a tendency to backward growth of craniofacial profile. The value of this patients were significantly different from the value of patients with anterior tooth contacts in items of SNB, ANB, mandibular plane to anterior cranial base ratio, SNPo, NAPo and APDI items. 3. But in denture pattern, no statistically significant difference by the presence or absence of anterior tooth contacts were showed between this patients groups. 4. From this study, it could be proposed that anterior open bite in the patients with craniomandibular disorders would be originated from not dental discrepancy but skeletal discrepancy.

  • The understanding the natureof occlusal tooth contacts of natural dentition is important for correct diagnosis and treatment of diseases developed in stomatognatic system. Several investigator have studied the distribution of tooth contacts in maximum intercuspation and have reported contact locations with respect to the tooth position. Many methods have been clinically applied for the occlusal analysis in the intercuspal position. However, there are few quantative methods. This study analyzed the new software version of the T-Scan system to record and analyze occlusal contact balance in the anterior-posterior and right-left directions. Six time moment statistics and five moment statistics were calculated in the midsagittal and the incisal axes of the occlusal plane. In the present study, informed consent was obtained from 100 subjects with natural dentitions. The results were as follows ; 1. The mean of the dental arch length & width were 48.78, 65.32mm in whole population, 49.09mm, 65.50mm in males, 48.78mm, 64.63mm in females, respectively. 2. The mean of TLR & PLR were 0.193mm(left), 0.311mm(left), respectively. Therefore, the distribution of tooth contacts was bilaterally symmetric. 3. The mean of TFB & PFB were 29.168mm, 29.055mm, and that of LFB & RFB were 29.627mm, 29.587mm, respectively, and the qualitative center of occlusal contacts was the firtst molar. 4. The mean of LL & RL were 31.666mm, 31.377mm, respectively, and the quantitative center of occlusal force was the first molar. 5. The mean of LF & RF were 60.237N, 59.276N, respectively and Left-right moment was 72.491Nmm. Therfore, the distribution of occlusal force was bilaterally balanced.

Effects of a new type of clear overlay retainer on occlusal contacts

  • The popularity of clear overlay retainers (CORs) has increased recently because of their advantages such as better esthetics, cost effectiveness, easy fabrication, and good compliance. However, a deficiency in posterior occlusal settling is a reported limitation of CORs. The aim of this study was to evaluate the posterior occlusal contact changes in a new type of clear orthodontic retainer called Oral-treaper (OTP), which consists of three layers and has stronger mechanical characteristics than do conventional retainers. Three patients who completed fixed orthodontic treatment received OTP as a removable retainer. Cast models were fabricated after the removal of fixed appliances (T1) and after 4 to 11 months of using the retainers (T2). We evaluated all the cast models to compare the post-orthodontic settling pattern during the use of the OTPs. The depth of occlusal contacts was evaluated using color maps. The OTP did not prevent vertical settling in all patients but resulted in an improvement in posterior occlusal contact points.

T-scan을 이용한 성인정상교합자의 하악운동시 치아접촉양상에 관한 연구 (A STUDY OF THE OCCLUSAL CONTACT PATTERN DURING MANDIBULAR MOVEMENTS OF ADULT WITH NORMAL OCCLUSION)

  • This study was accomplished to analyse and compare the occlusal contact patterns during eccentric mandibular movements in adult with normal occlusion. 50 subjects(male 27, female 23), who had natural occlusion and no symptom of temporomandibular disorder, were selected. Teeth contact patterns during mandibular eccentric movements were recorded and the distribution of tooth contacts in maximum intercuspation analysed by T-scan system. And then, tooth contact numbers recored by T-scan and silicone bite registration at centric occlusion were analysed and compared. The results obtained were as follows : 1. Antero-posteriorly, the qualitative center of occlusal contacts in centric occlusion were in the first molar areas, but there was a slight deviation in left-right directions. Thus, distribution of occlusal contacts were not bilaterally symmetric. 2. During the mandibular movements from centric occlusal position to right lateral and left lateral directions, the frequency that maxillary canine joined in lateral guidance was relatively high, but pure canine protected occlusion or pure group function occlusion had small frequency. 3. During mandibular protrusive movement, one or more maxillary central incisors frequently joined in protrusive guidance. 4. During mandibular eccentric movements, working and balancing side premature contact was observered in relatively high frequency. 5. In centric occlusal position, the numbers of occlusal contacts recorded on T-scan were relatively smaller than on silicone bite registration.

비우식성 치경부병소를 가진 구치부 치아의 교합 및 치주상태에 관한 연구 (Occlusal and Periodontal Status of Teeth with Non-carious Cervical lesions)

  • A non-carious cervical lesion(NCCL) is the loss of tooth structure at the cementoenamel junction level that is unrelated to dental caries. This study was to evaluate the occlusal and periodontal status of teeth with non-carious cervical lesions. We evaluated 105 teeth with non-carious cervical lesions in 35 subjects aged 38-75 years and characterized them based on the shape and dimension, plaque retention, bleeding on probing(BOP), probing pocket depth(PPD), occlusal status, brushing type, hypersensitivity and wear facet. The results of this study were as follows 1. No significant association was observed between cervical lesions and occlusal contact in lateral excursions. 2. No significant difference occurred in plaque retention, PPD, BOP between teeth with and without cervical lesions. 3. Test teeth had a significantly higher percentage of hypersensitivity and occlusal wear facet than teeth without cervical lesions. 4. Wedge shaped lesions had a significantly higher percentage of plaque than saucer shaped lesions. 5. Teeth with plaque were found to have significantly deeper PPD than teeth without plaque retention in cervical regions. 6. Teeth with occlusal contacts were found to have significantly deeper PPD than teeth without occlusal contacts. 7. No significant association was observed between cervical lesions and PPD independent of plaque retention and occlusal contacts Although more knowledge is necessary, our results suggest that occlusal contact and bacterial plaque may influence on periodontal tissue, but NCCL is not directly associated with periodontal health

COMPUTERIZED T-SCAN SYSTEM을 이용한 정량적 교합분석방법에 관한 연구 (A PRELIMINARY STUDY ON QUANTITATIVE ANALYSIS USING THE COMPUTERIZED T-SCAN SYSTEM)

  • The purpose of this preliminary report was to describe the operating procedure of T-scan system and to identify the location, timing and force of occlusal contact in patient with normal occlusion using computerized T-scan system. From the preliminary observation , the author obtained the following results. 1. T-scan system displayed 2 dimensional and 3 dimensional description of occlusion: contact locations, timing (sequence) and forces of occlusal contacts. 2. The T-scan sensor was the most important part of the T-scan system. 3. The data of T-scan system cannot be stored in computer diskett. 4. The T-scan system is thought to be the most effective system to detect occlusal contacts and can be applied to the followings : occlusal diagnosis, occlusal equilibration, crown and bridge restorative procedures, denture adjustment, implant procedures, splint adjustment, laboratory procedures, periodontal treatment, orthodontics, TMJ treatment and patient education etc.

교합안정장치 사용후 교합력 및 교합접촉의 변화양상에 관한 연구 (Changes of bite force and occlusal contacts after stabilization splint therapy)

  • 본 연구는 조선대학교 치과대학 부속치과병원 교정과에 내원한 환자 중 악태모형을 제작하고 CPI (condylar position indicator) 에 의한 하악과두의 위치를 평가하여 중심위 -중심교합 편위양이 전후방 및 수직적으로 1.0 mm 이내, 측방으로 0.3 mm 이내인 정상 범주를 벗어나는 성인 여자 환자 16명을 대상으로 교합안정장치를 3개월간 장착하고, 이들을 장착 기간에 따라 장착 직전, 장착 1개월 후, 장착 2개월 후, 장착 3개월 후로 분류하여 교합안정장치의 장착에 따른 교합력과 교합접촉점 수의 변화 여부를 평가하기 위해 T-scan system을 이용하여 다음과 같은 결론을 얻었다. 1. 교합력의 변화는 교합안정장치 장착 직전에서 장착 1개월 후 사이에 유의성 있게 감소되었다 (P<0.05). 2. 교합력의 변화는 교합안정장치 장착 1개월 후부터 장착 3개월 후 까지는 유의한 차이를 보이지 않았다 (P>0.05). 3. 전치부 교합접촉점 수의 변화는 장착기간에 관계없이 유의성있는 차이가 없었다 (P>0.05). 4. 구치부 교합접촉점 수의 변화는 교합안정장치 장착 직전에서 장착 1개월 후 사이에 유의성 있게 감소되었다 (P<0.05). 5. 구치부 교합접촉점 수의 변화는 교합안정장치 장착 1개월 후부터 장착 3개월 후 까지는 유의한 차이를 보이지 않았다 (P>0.05). 6. 교합접촉점 수의 변화는 전치부에서 보다는 구치부에서 절대적으로 영향을 미쳤다. 이상을 종합해볼 때, 교합안정장치는 장착 1개월 후면 어느정도 중심위로의 하악골 안정을 기대할 수 있을것으로 사료된다.

임플란트 보철 교합의 임상적 고려 사항 (Clinical considerations for appropriate occlusion of implant restorations)

  • The appropriate occlusion is one of the most important factors for the long-term success of implant and its restorations. The purpose of this review is to investigate and define occlusal considerations to reduce failure of implant prostheses. The physiological movement of implants is markedly lower than that of natural teeth and they also lack in occlusal sensitivity. Proper occlusal pattern may be assigned to compensate for the biological disadvantages and occlusal contacts must be formed where the cantilever effect is minimized. Moreover, the long-term success of implants after osseointegration can be assured by reducing early occlusal loading to avoid implant overloading and selecting appropriate occlusion material. Occlusal overload was brought by the number and location of occlusal contacts, which are under the clinician's control. The concept of implant occlusion is based on the concepts derived from traditional prosthetics. Moreover, there are few evidence on the concept or design of implant occlusion. Several occlusal design was recommended for implant prosthesis. Mutually protected occlusion, group function occlusion and bilateral balance occlusion was recommended for the specific types of implant restorations. This article reviews proper design of occlusion for implant restoration and offers occlusion strategy clinically.

  • In an edentulous situation, the dentist must make several determinations when constructing artificial teeth. These include vertical and horizontal relationships of mandible with respect to the maxilla, occlusal form and position, vertical dimension, occlusal relationships during both centric closure and eccentric excursive movements. Artificial teeth are attached to a movable base resting on movable and displaceable living tissue subject to damage. They act as a unit; therefore, they must be arranged to function as a unit. Bilateral balanced occlusion is that stability of the denture is attained when bilateral contacts ex ist throughout all dynamic and static states of the denture during function. Lateral excursion in a balanced scheme implies simultaneous working side and nonworking side contact, while occlusal contacts are maintained on both anterior and posterior teeth as the mandible moves anteriorly into protrusion.

Computerized analysis of occlusal contacts in bruxism patients treated with occlusal splint therapy

  • PURPOSE. Occlusal splints are commonly used to prevent tooth wear caused by bruxism. However, the effects of splints on occlusion are still unclear. Although it is rarely alluded in literature, splints can provoke severe occlusal alterations and other complications. This study was aimed to identify differences in the responses of individuals with bruxism and healthy individuals to a full-arch maxillary stabilization splint in terms of occlusal changes. MATERIALS AND METHODS. Occlusal contacts in 20 (5 male, 15 female) bruxism patients and 20 (5 male, 15 female) controls with normal occlusion were evaluated before and after occlusal splint therapy. T-Scan III, a computerized occlusal analysis system, was used to simultaneously measure occlusion and disclusion times as well as left-right and anterior-posterior contact distributions before splint therapy and 3 months after therapy. Wilcoxon and Mann-Whitney U tests were used for statistical analyses (${\alpha}$=.05). RESULTS. No differences were found in the posterior contact of bruxism patients before and after stabilization splint treatment. However, differences in posterior contact were observed between bruxists and normal individuals prior to treatment, and this difference disappeared following treatment. CONCLUSION. The results of this study showed the use of a stabilization splint may not have an effect on occlusion. However, the area of posterior occlusal contact among bruxists was found to be greater than that of normal individuals. According to this study, the clinical use of splints may be harmless.
