Rescue of a periodontally compromised tooth by non-surgical treatment: a case report

  • Purpose: This article describes a case of the successful non-surgical management of a periodontally compromised maxillary premolar. Methods: A combination therapy, including root planing, occlusal adjustment, and tooth splinting, was applied. Clinical and radiographic examinations were performed during the 16-month follow-up period. Results: All periodontal parameters were improved. There were dramatic decreases (3-6 mm) in the probing pocket depth, tooth mobility, and marginal bone loss. Interestingly, gradual resolution of the periapical radiolucency and alveolar bone regeneration were observed in the radiographs, and the periodontal condition was maintained during the follow-up period. Conclusions: Within the limits of this study, these results demonstrate the importance of natural tooth preservation through proper periodontal treatment and occlusal adjustment of the periodontally compromised tooth, which is typically targeted for tooth extraction and dental implantation.

가철성 의치의 유지관리 (Maintenance care for removable denture)

  • There are many kinds of maintenance care services for removable denture patient. Adjustment of the denture base and occlusion should be performed regularly not only for pain relief but also for maintaining the denture function. Direct and indirect relining are needed frequently in specific cases including mandibular distal extension case or non-symmetric residual teeth situation. Surface treatments for metal and resin are essential in the relining procedure. Clinical process for the denture repair is similar to indirect relining which needs inter-occlusal registration. Especially, the peridontal maintenance care and caries prevention are most important way to preserve the abutment teeth in partial edentulism. Moreover, the caring method for the denture and the tissue should be instructed to the denture patient.

  • The author devised a simple technique for duplicating occlusal contour of natural teeth on the prepared abutments using hard stone index obtained from the study cast of natural teeth. Throughout this author devised method, the following advantages are studied. Firstly the dentist can obtain an ideal occlusal wax patterns which have arrived at the physiologic state through a long period of adjustment to various habits and functional movements. Secondly the construction procedure is simple, and thirdly it is easy to adjust wax pattern of nomal natural teeth which duplication is required.

How adjustment could affect internal and marginal adaptation of CAD/CAM crowns made with different materials

  • PURPOSE. Recently introduced hybrid and reinforced glass ceramic computer-aided design/computer-aided manufacturing (CAD/CAM) materials have been used for full-coverage restorations. However; the effect of adjustment and type of materials on internal and marginal adaptation are unknown. This study aimed to evaluate and compare the marginal and internal adaptations of crowns made of three different CAD/CAM materials before and after adjustment. MATERIALS AND METHODS. One acrylic resin maxillary first molar was prepared and served as the master die. Thirty-six restorations were fabricated using CAD/CAM system (CEREC Omnicam, MCXL) with three materials including lithium disilicate (IPS e.max CAD), zirconia-reinforced lithium silicate (Suprinity), and hybrid ceramic (Enamic). Internal and marginal adaptations were evaluated with the reference point matching technique before and after adjustment. The data were analyzed using mixed ANOVA considering α=.05 as the significance level. RESULTS. The effect of adjustment and its interaction with the restoration material were significant for marginal, absolute marginal, and occlusal discrepancies (P<.05). Before adjustment, Suprinity had lower marginal discrepancies than IPS e.max CAD (P=.18) and Enamic (P=.021); though no significant differences existed after adjustment. CONCLUSION. Within the limitations of this study, crowns fabricated from IPS e.max CAD and Suprinity resulted in slightly better adaptation compared with Enamic crowns before adjustment. However, marginal, axial, and occlusal discrepancies were similar among all materials after the adjustment.

  • Temporomandibular joint disorder(TMJD) was mainly characterized with joint pain, motion limitation, joint sound, resulted from pathologic conditions in temporomandibular joint and around tissue. As temporomandibular joint is one of decisive factors determining the occlusion, disorders in temporomandibular joint may cause the occlusal changes. The causes of occlusal changes related with TMJD can be classified into 2 categories; (1) those related to progression of disorder, 2) those related to treatment of the disorder. The clinical manifestation of occlusal changes depend on their causes and affected site. Therefore, whenever possible, treatment should be directed to the relief of the underlying causes, However, it is not always possible to relieve the underlying conditions. Moreover, some occlusal changes may remain irreversible even after the considerable improvement in clinical symptoms. Regarding the treatment of the permanent occlusal changes, it has been reported that the extensive occlusal treatment including occlusal adjustment, prosthodontic treatment, orthodontic treatment should be applied. Here, we present with a case report of occlusal change caused by the progressive temporomandibular joint disorder, together with introducing the intermaxillary traction appliance as the possible treatment option.

하악변위로 인한 개방교합 환자의 보철치료: 증례보고 (Approach to prosthetic treatment for patients with open bite due to mandibular displacement: Case report)

  • 개방교합이 있는 환자는 치아접촉 및 수직피개의 감소를 나타내며 교합평면의 부조화, 저작 곤란, 언어 장애, 외모 변화 및 정상보다 낮은 교합력을 가지는 경우가 많다. 교합이 완성된 성인에서 측두하악관절장애로 인해 발생한 개방교합은 원인요소의 제거 또는 관절상태의 안정화가 먼저 이루어진 후 교합 수정이 이루어져야 하며, 이를 위해 교정치료, 교합조정, 보철 치료, 외과적 치료법 등이 있다. 본 증례보고는 교합이 완성된 성인에서 하악의 변위로 인해 개방교합이 발생한 두 명의 환자에서 교합 변화의 원인을 추정해 보고 원인에 따라 다른 치료 접근방법에 대해 살펴 보는 것이다. 관찰한 환자 중 한 환자는 측두하악관절의 안정화 후 교합조정을 통하여 기능적인 면과 심미적인 면에서 만족스러운 결과를 얻을 수도 있었기에 보고하는 바이다.

임플란트 보철물의 식편압입: 교합적 원인분석과 대처 (Food impaction related with trauma from occlusion)

  • Food impaction between the implant prostheses and adjacent teeth is the most frequently observed problem. It may be caused by the migration of the adjacent teeth. This symptom may be observed at the mesial aspect of implant prostheses especially, and related with the multiple contributing factors including teeth vitality and antagonist. Idal proximal contact with optimal strength and shape should be made for preventing the food impaction. Shape of customized abutment and prosthesis should have optimal emergence profile. Long duration from the extraction to the delivery of implant prostheses, the adjacent teeth and antagonist teeth may have possibility of occlusal interferences. Remained teeth mobility can induce the food impaction regardless of interproximal contact strength. Occlusal adjustment to remove occlusal interferences can be a method for enhancing the stability of interproximal contact.

두부의 자세 변화가 초기 교합접촉에 미치는 영향 (The Effect of Head Posture Change on Initial Occlusal Contacts)

  • The purpose of this study was to evaluate effect of head posture change on initial occlusal contacts through measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture. Two special devices were designed and constructed. Mandibular movement replicator was used to assess reliability of the K6 diagnostic system(MKG; Myo-tronic Inc, Seatle, USA) and head posture calibrator was used to maintain the constant head posture during experiment. We measured difference of distance between initial occlusal contact and maximum intercuspal position with MKG in upright, supine, 45 degrees extension, 30 degrees flexion, 30 degrees right and left bending postion of the head. The Frankfurt horizontal plane was used as a reference plane. 21 adults aged from 23 to 25 were selected, who have normal or class I molar relationship, and have no symptoms on TMJ and masticatory muscles, and have restorations less than 3 surfaces on each tooth, and have no other prosthetic restoration. The obtained results were as follows : The mean absolute distances between initial occlusal contact and maximum intercuspal postion were 0.39(0.18mm in the upright position, 0.65(0.37mm in the supine position, 0.59(0.33mm in the 45 degree extension, 0.70(0.53mm in the 30 degrees flexion, 1.12(1.10mm in the 30 degrees right bending and 1.94(0.67mm in the 30 degrees left bending of the head. The positions of the initial occlusal contacts have a tendency to locate anterior, left and inferior to maximal intercuspal position in upright position, posterior and inferior in supine position and 45 degrees extension, anterior and inferior in 30 degrees flexion, right and inferior in 30 degrees right bending, and left and inferior in 30 degrees left bending of the head. There were significant differences among the initial occlusal contacts in each head postures(P<0.0001). Therefore, we need to check initial occlusal contacts in the altered head posture during occlusal analysis and adjustment of occlusal appliance and dental occlusion for diagnosis and treatment of temporomandibular disorder.

측두하악 장애환자에서 두부자세 변화가 초기 교합접촉에 미치는 영향 (The Effect of Head Posture Change on Initial Occlusal Contact in Temporomandibular Disorder Patient)

  • The purpose of this study was to evaluate an effect of change on head posture initial occlusal contacts with measuring the distances between initial occlusal contacts and maximum intercuspal position at different head posture in TMDs patient. For this study, 24 patients from age 13 to 36 were selected, they were examined health history taken, patients who have sign and symptoms of TMDs were examine before the study. For the normal group, 21 adults from age 23 to 25 were selected. They have normal or class I molar relationship, and have no other prosthetic restorations. Difference on distance between initial occlusal contact and maximum intercuspal position with mandibular kinesiograph$(MKG^R)$(K6 diagnostic system, Myo-tronic Inc, USA) in upright, supine, 45$^{\circ}$ extension, 30$^{\circ}$ flexion position of the head were measured. The Frankfort horizontal plane was used as a reference plane. The results were as follows : 1. There were significant differences between initial occlusal contacts of the normal and patient group on upright position and 30$^{\circ}$ flexion of the head(p<0.05, p<0.01) 2. The position of the initial occlusal contacts have a tendency to place anterior and inferior to maximal intercuspal position in upright position and 30( flexion of the head as well as posterior and inferior in supine position and 45$^{\circ}$ extension of the head in the normal and patient groups. 3. There were significant differences among the initial occlusal contacts between uptight and supine position; upright and 45$^{\circ}$ extension of the head(p<0.05); supine position and 30$^{\circ}$ flexion of the head, .and 30(flexion and 45$^{\circ}$ extension of the head in the patient group(p<0.01) The result have shown that after treatment on the supine position, it may be necessary to check occlusal contact on the upright position as well ass flexion of the head. It may need careful adjustment in occlusal condition on upright position of TMDs patient.

Modified Twin Blocks에 의한 성장기 아동의 II급 부정교합의 치료증례 (A CASE REPORT ON TREATMENT OF CLASS II MALOCCLUSION WITH TWIN BLOCKS IN GROWING CHILD)

  • The Twin Blocks technique was developed by Dr. William Clark of Scotland during the early 1980's. Twin Blocks are an uncomplicated system that incorporates the use of upper and lower bite blocks. These blocks reposition the mandible and redirect occlusal forces to achieve rapid correction of malocclusions. They are also comfortable and the patients wear them full-time-inducing eating time. Occlusal forces transmitted through the dentition provide a constant proprioceptive stimulus to influence the rate of growth and the trabecular structure of the supporting bone. The features of Twin Blocks mean easier and quicker treatment. The occlusal inclined plane is the fundamental functional mechanism of the natural dentition. Twin blocks are bite blocks that effectively modify the occlusal inclined plane to induce favorably directed occlusal forces by causing a functional mandibular displacement. Upper and lower bite blocks interlock at a $45^{\circ}$ angle and are designed for full-time wear to take advantage of all functional forces applied to the dentition including the forces of mastication. The patients who were treated with modified Twin Blocks, and following results were observed: 1. Large overjet and deep overbite were corrected. 2. Class II molar relationship was changed into Class I. 3. Labial inclination of upper incisors was corrected by adjustment of labial bow of upper bite block. 4. The profiles of two patients were improved by anterior displacement of mandible.
