Impact of Eating Psychopathology, Obsessive-Compulsion and Depression on Self-Harm Behavior in Patients with Eating Disorders (섭식장애 환자의 섭식장애 증상, 우울, 강박성이 자해행동에 미치는 영향)

  • Purpose: The purpose of the study was to investigate psychological factors such as eating psychopathology, depression, and obsessive-compulsion that might influence self-harm behavior in patients with eating disorders. Methods: Patients with eating disorders (n=135) who visited "M" clinic for eating disorders participated in the study. Data were collected from March to August 2007 using the Eating Disorder Inventory-2, Beck Depression Inventory, Maudsley Obsessional-Compulsive Inventory, and Self-Harm Inventory (SHI). Results: The participants scored high on self-harm as well as on depression and obsessive-compulsion. On the SHI, a high frequency of self harm behavior such as 'torturing self with self-defeating thoughts', 'abused alcohol', 'hit self', and 'suicide attempt' were found for the participants. There were significant correlations between most eating psychopathology variables, depression, obsessive-compulsion, and self-harm behavior. 'Interoceptive awareness' (eating psychopathology), depression, and 'checking' (obsessive-compulsion) were significant predictors of self-harm behavior. Conclusion: Future interventions for patients with eating disorders should focus on assessing the possibility of self-harm and suicidal attempts, especially in those patients with high levels of eating psychopathology, depression, or obsessive-compulsion. Early intervention for depression and obsessive-compulsion could contribute to preventing self-harm and suicide in patients with eating disorders.

The relationship between hostility and obsessive-compulsive symptoms: Focused on the moderating effect of impulsivity (적대성과 강박증상과의 관계: 충동성의 조절역할을 중심으로)

  • This study examined the relationship between hostility and obsessive-compulsive symptoms to clarify the differential role of overt hostility and covert hostility on obsessive-compulsive symptoms. In addition, this study examined whether impulsivity has a moderating effect on the relationship between hostility measures and obsessive-compulsive symptoms. The Buss Durkee Hostility Inventory (BDHI), Revised Obsessive Compulsive Inventory (OCI-R), and Barratt Impulsivity Scale (BIS) were used to measure hostility, obsessive-compulsive symptoms, and impulsivity, respectively. Data were collected from 150 online university students and analyzed using the correlation and moderated multiple regression model. The result showed that overt hostility was positively correlated with obsessive thoughts; covert hostility was positively correlated with obsessive thoughts and compulsive behavior. In addition, the regression results, which set the hostility variables as the predicting variable, revealed covert hostility to increase obsessive thinking and compulsive behavior, whereas overt hostility had no significant effect on both variables. Impulsivity was found to function as a moderator in the prediction of covert hostility on obsessive thought. With the result, the implications and limitations of this study are discussed.

Voxel-Based Morphometry Study of Gray Matter Abnormalities in Neurodegenerative Disease with Obsessive-Compulsive Behaviors

  • Objectives : Obsessive-compulsive(OC) symptoms have yet to be directly studied in neurodegenerative conditions involving behavioral changes. To examine regional abnormalities in the brains of dementia patients with OC symptoms, we assessed the gray matter density using voxel-based morphometry(VBM). Methods : We performed brain magnetic resonance imaging(MRI) with VBM analysis in 106 dementia patients with OC behaviors. In this study, OC behaviors were investigated in patients with neurodegenerative disease using the modified Manchester Behavior Questionnaire. Results : The OC behavior scores were correlated with structural brain volume using VBM. The total OC symptom score correlated negatively with the volume of both putamens, the right middle orbitofrontal gyrus, both anterior cingulate cortices, and the left insula(p<0.001, uncorrected). No gray matter reductions were associated specifically with the OC symptom sub-categories. Conclusions : Our results suggest that abnormalities in these brain regions may play an important role in the pathophysiology of OCD in neurodegenerative disease. This is the first lesion study to investigate the neural basis of OCD behaviors in neurodegenerative disease.

Long-term Results of Stereotactic Psychosurgery (뇌정위적 정신수술의 장기 추적 결과)

  • Objective : Stereotactic psychosurgery is known as one of effective means of treating in some medically intractable psychiatric illness. However, it is unfamiliar and it's long-term clinical result has not reported in our country. The authors evaluated its long-term results of pscychosurgery and discussed its neuroanatomical basis. Methods : Since 1993, eight patients underwent stereotactic psychosurgery for medically intractable psychiatric illnesses. All were referred from psychiatrist of these disorders, one was aggressive behavior, five were obsessive-compulsive disorders(OCD), and two were depression with anxiety disorders. Bilateral amygdalotomy and subcaudate tractotomy were done for aggressive behavior, and limbic leukotomy was done for OCD and depression with anxiety. The results of OCD were evaluated with with YBOCS(Yale-Brown obsessive compulsive scale) and VAS (visual analogue scale), CGI(clinical global impairment) in OCD, and OAS(overt aggression scale), MMS, WAIS were checked for the evaluation of aggressive behavior. Hamilton depression scale(HAMD) was used for evaluation of depression. Ventriculography was used in the first five patients and MR-guided stereotaxy was used in recent three cases for localization of target. The lesions were made with radiofrequency lesion generator. Results : With long-term follow up(mean 45 months) in five OCDs, mean YBOCS declined from 34 to 3(n=5). All returned to previous social life. In OAS scores of aggressive behavior during six-year follow up, scores declined from 8 to 2 with clinical improvement. In two patients with depression with anxiety, HAMD declined from 28.5 to 16.5(n=2). There was no operative mortality and no significant morbidity except one case of mild transient urinary incontinence. Conclusion : With these long-term results, authors assumed that stereotactic psychosurgery could be one of safe and effective mtherapeutic methods in several medically intractable psychiatric illness.

A Case Report : Hypersexuality and Obsessive-Compulsive Behaviors in a Stroke Patient with the Left Mesial Frontal Cortex and Both Basal Ganglia Lesion (좌측 정중 전두엽과 양측 기저절 경색 후 과잉성욕 증상과 강박 증상을 보인 환자 1례)

  • Hyposexuality after stroke has been frequently observed, but hypersexuality as a sequela of stroke has not been commonly documented. We report a patient who exhibited hypersexuality and obsessive-compulsive behaviors after stroke in the region of the left mesial frontal cortex and both basal ganglia. At 2 months after stroke, he visited psychiatric unit due to these symptoms. His motor function was almost full recovered. He was treated with fluvoxamine and perphenazine. With two-month medication, his hypersexuality and obsessive-compulsive behavior disappeared. This case may indicate that basal ganglia-thalamocortical circuit plays an important role in the mediation of sexual behavior and obsessive-compulsive behavior. Since changes in sexual activity may not be spontaneously reported, a systemic inquiry into patient's sexual functioning after infarction in frontal lobe or basal ganglia is warranted.

The psychological factors of creation of virtue women based on the relationship between Mrs. Park in and Samganghaengsildo's the virtuous women volume (<열녀함양박씨전>의 박씨와 『삼강행실도: 열녀편』의 관계를 통해 본 열녀제작의 심리적 요인)

  • The author analyzed the psychological factors which affected on the way of Mrs. Park in the became a virtuous woman and compared these psychological factors with a virtuous women in Samganghaengsildo based on the relationship between Mrs. Park in and Samganghaengsildo's the virtuous women volume. First of all, it's about the power of authority on women. The volume of Samganghaengsildo's virtuous women was about the group of women who were recognized as virtuous women by the king who had absolute power at the time period. The volume of Samganghaengsildo's virtuous women was very well known for women. This book had a huge impact on women since the government authority educated the women using this book. In addition, the incredible events which were written in Samganghaengsildo indicated that the behavior of virtue was the right thing to follow since even super power recognized it as a good thing to do and the woman might be punished when she didn't follow the behavior of virtue. Second, it was based on the similarity of circumstance and uncertainty for the future. Mrs. Park in showed similar behavioral patterns with a virtuous woman in Samganghaengsildo since she took her role model from the virtuous woman. When Mrs. Park lost her husband, she was not sure about her future and the volume of Samganghaengsildo pointed out that there was another world after the death and she would meet her husband again after the death. In addition, the incorrect information that she would feel better psychologically when she followed her dead husband instead of living herself. Third, it is about the resemblance of obsessive mind and self internalization. The examination of the record of Mrs. Park showed that Mrs. Park tried to internalize integrity, loyalty, and virtue of virtuous women in Samganghaengsildo and became herself like virtuous women. In addition, the woman's obsessive mindset remarkly reflected in Samganghaengsildo. For instance, Mrs. Park in held to the time of death of her husband and the writer who wrote about the behavior of Mrs. Park also emphasized that the time of death of Mrs. Park was closed to the time of the death of her husband. This explained that the obsessive mindset of women in the epic of virtue women could have influence on the women's mind who tried to show the similar behavior with a virtue woman and internalize it. Finally, the people of the day also were not free from it.

Parental Abuse and Maladjustment Perceived by Middle School Student (중학생이 지각한 부모의 학대와 자녀의 부적응)

  • This study investigated the relationship between parental abuse and maladjustment per ceived by middle school student. The subjects were 448 middle school students in Kwangju. The main results of this study were as follows ; 1. There were found significant differences according to background variables in parental abuse and maladjustment. Physical abuse was found significantly different according to gender of the child and unreasonable rearing behavior of the parents, but emotional abuse was found significantly different according to job of the father and unreasonable rearing behavior of the parents. And, neglect was found significantly different according to educational level of the father and unreasonable rearing behavior of the parents. Social withdrawal was found significantly cliff erent according to gender of the child, family income and unreasonable rearing behavior of the parents, but hyperactivity was found significantly different according to gender of the child and unreasonable rearing behavior of the parents. Agression and retreat were found significantly different according to gender of the child and unreasonable rearing behavior of the parents. Obsessive-compulsion was found significantly different according to unreasonable_ rearing behavior of the parents. 2.There were significantly positive correlation between parental abuse and maladjustment. 3. The findings of multiple regression analysis demonstrated that neglect, physical abuse, family income, gender of child were found to be important variables in predicting social with drawal. Neglect, unreasonable rearing behavior of the parents, phsical abuse, gender of child were found to be important variables in predicting hyperactivity. Neglect, emotional abuse, and unreasonable rearing behavior of the parents were found to be important varialbes in predicting aggression.   • By observing the psychotherapy process expressed by an adolescent with obssessive compulsive disorder, the author suggests that the main problem of OCD is not merely a product of an unconscious process as traditionally thought, but a learned problem solving process as a method surviving the more realistic conflicts, and therefore the treatment should be directed to more or less conscious and realistic issues and conflicts, family therapy should be offered too.

  • This study investigated the relationship between the types of parental abuse and maladjustmental behaviors of adolescents. The subjects were 448 junior and senior middle school students. Statistics were frequencies, percentile, Pearson's r-coefficient, and regression analysis. The main results of this study were as follows ; 1. There were significantly positive correlation between the types of parental abuse and maladjustmental behaviors. 2. The variables, neglect, unreasonable rearing behavior of the parents, and physical abuse were found to be the important variables in predicting social withdrawal. Neglect, unreasonable rearing behavior of the parents, and sex of the child were found to be the important variables in predicting hyperactivity. Neglect, emotional abuse, and unreasonable rearing behavior of the parents were found to be the important variables in predicting aggression. Sex of the child, emotional abuse, and unreasonable rearing behavior of the parents were found to be the important variables in predicting retreat. And, neglect and unreasonable rearing behavior of the parents were found to be important variables in predicting obsessive-compulsion.

Treatment of Morsicatio Buccarum by Oral Appliance: Case Report

  • Morsicatio buccarum is a condition caused by chronic cheek biting. It means frictional hyperkeratosis by repetitive cheek biting, which may be associated with obsessive compulsion. Clinically it presents as rough, shaggy, whitish, often peeling surface. We report a 9-year-old female patient who complained oral ulceration on both buccal mucosa. The initial diagnosis was oral candidiasis because of whitish plaques which were peeled off. Topical antifungal agent was ineffective and the symptom did not disappear. By incisional biopsy, she was diagnosed with morsicatio buccarum. A soft oral appliance was placed in the patient's oral cavity. Although morsicatio buccarum improved significantly when the appliance was used, the oral lesion had a tendency to recur, when the patient stopped using the appliance. In case of recurrence, cognitive behavioral therapy for obsessive-compulsive disorder is needed for the fundamental treatment.
