Objective: The number of three-piece maxillary osteotomies has increased over the years; however, the literature remains controversial. The objective of this study was to evaluate the skeletal stability of this surgical modality compared with that of one-piece maxillary osteotomy. Methods: This retrospective cohort study included 39 individuals who underwent Le Fort I maxillary osteotomies and were divided into two groups: group 1 (three pieces, n = 22) and group 2 (one piece, n = 17). Three cone-beam computed tomography scans from each patient (T1, pre-surgical; T2, post-surgical; and T3, follow-up) were used to evaluate the three-dimensional skeletal changes. Results: The differences within groups were statistically significant only for group 1 in terms of surgical changes (T2-T1) with a mean difference in the canine region of 3.09 mm and the posterior region of 3.08 mm. No significant differences in surgical stability were identified between or within the groups. The mean values of the differences between groups were 0.05 mm (posterior region) and -0.39 mm (canine region). Conclusions: Our findings suggest that one- and three-piece maxillary osteotomies result in similar post-surgical skeletal stability.
본 연구는 장기요양시설 노인의 삶의 질에 관련된 요인을 알아보기 위하여 조사대상자의 인구사회학적 특성, 건강관련행위특성, 구강건강영향지수(OHIP-14), 신체적 기능(ADL, IADL) 및 정신적 기능(CES-D, MMSE-K)과 삶의 질(WHOQOL-BREF)과의 관련성을 분석하였다. 조사대상은 장기요양인정자로 판정받은 장기요양시설급여노인 602명으로 하였으며, 2016년 5월 1일부터 6월 30일까지의 기간 동안에 구조화된 설문지를 이용한 직접 면접조사와 구강검사를 실시하였다. 연구결과, 조사대상자의 삶의 질은 남성보다 여성에서, 생활비조달을 본인이 하는 경우보다 자녀가 조달하거나 정부의 보조를 받는다는 군에서, 요양기간이 길수록, 비음주군보다 음주군에서, 규칙적인 운동을 한다는 군보다 하지 않는다는 군에서, 식사를 규칙적으로 한다는 군보다 하지 않는다는 군에서, 주관적인 건강상태가 건강하다는 군보다 건강하지 않다는 군에서, 1일 칫솔질 횟수가 적을수록, 구강건조증이 없다는 군보다 있다는 군에서, 구강건강영향지수(OHIP-14)가 낮을수록, 신체적 기능(ADL, IADL)이 낮을수록, 정신적 기능(CES-D, MMSE-K)이 낮을수록 유의하게 낮았다. 특히 삶의 질은 다른 요인보다 건강관련행위특성과 정신적 기능(CES-D, MMSE-K)에 의해 더 큰 영향을 받고 있는 것으로 나타났다. 따라서 장기요양시설 노인의 삶의 질을 증진시키기 위해서는 바람직한 건강관련행위를 비롯하여 우울 및 인지기능장해를 예방하기 위한 노력이 필요할 것으로 생각된다.
Objectives : The aim of this study was to evaluate the relation between the state of dental health(number of natural teeth) and nutritional status of Korean elderly using Korean Dietary Reference Intakes, which was an objective standard for nutritional intake based on database of Korea National Health and Nutrition Examination Survey, large scale of sample obtained by the government. Methods : Complex sampling procedure was used to analyze the fourth data(2007-2009) of Korea National Health and Nutrition Examination Survey. When preparing planning file, the estimator of variance as a stratification variance (variance name : kstrata), population of survey unit as a cluster variance(variance name : PSU), and previous exam and nutritional related weighted as a weighted were analyzed by considering reproduced survey and nutritional related total weighted. Complex samples chi-square test was used to estimate the relation between number of natural teeth and inadequate intake and relation factors included in the model were analyzed by complex samples logistic regression analysis. Results : The group of edentulous had a higher risk to intake less nutrient, except calcium, riboflavin and vitamin C than recommended level comparing to reference group which had natural teeth more than 20(phosphate : OR=1.763; 95% CI=1.273-2.443, thiamine : OR=1.748; 95% CI=1.276-2.395, protein : OR=1.610; 95% CI=1.213-2.138). Conclusions : The number of teeth in Korean elderly over 65 years old had a relation with nutritional status in this investigation. Especially, intake level of nutrients was different between the edentulous group and the reference group. Therefore, dental health care is needed from young and middle age to keep health dental condition for through whole life as well as old age. Although the dental condition of the aged is not good, it is evitable to educate them about the relation between dental health and nutritional ingestion to take balanced nutrition, we think.
1. Introduction Community medicine with the concept of comprehensive medical care and an ideal medical care delivery system not only for an individual or family but for the whole community has emerged. In April 1970, the Presbyterian Medical Center started a hospital based community health service project in order to improve the health of the people in rural areas. Prior to commencing a comprehensive medical care system, a family survey was needed. The major objective of this survey was to obtain information concerning the people and their environment so as to be able to plan and implement a comprehensive medical care program in Soyang-Myun. 2. Survey Method An interview using a family record form was carried out for each household. Prior to beginning, the members of the survey team were trained in interviewing techniques for three days. In the past, many dentist were interested mainly in the mechanical aspects such as tooth preparation and retainer types for making dental restoration. But, these days, the concept of restorative treatment emphasizes the importarce of gnatic system and masticatory muscles in addition to oral cavity. So, the current considerations for the fixed prosthodontic treatment include the stabilization of temporomandibular joint and neuromuscualr system and the relationship of periodontal ligament and occlusion. To achieve the above objectives, occlusal splint has been used as one of the mouth preparations for restorative treatment. The objectives of occlusal splint are as follows; 1. To use as preliminary application for periodontal-occlusal treatment 2. To provide proper vertical dimension 3. To control abnormal habits and parafunction 4. To treat the temporomandibular disease and myofascial pain 5. To establish the new therapeutic position In some cases, the patients had improper vertical dimension and occlusal interferences caused by prostheses reconstucted using centric relation recorded without considering the health of TMJ and manticatory muscle. And these prosthesis act as primary source that cause pathologic phenomenon in periodontal ligament, muscles and TMJ. Physiologically, in order to make the treatment occlusion guided by proper centric relation method, the method should be guided after the use of occlusal splint for some period. The main objective of prosthetic treatment is to maintain the function and health of stomatognathic system. So, one of the most important things that have to be performed by clinicians is a clinical ability to do the correct diagnosis and treatment planning based on the stomatognatic function and occlusion.
Objectives: The objective of this study was to examine the current state of dementia-related education for dental hygiene degree programs in Korea, as well as to develop a dementia-related curriculum for dental hygiene programs suitable for the social characteristics of Korea. Methods: The study was conducted between April and November 2018. First, the current state of dementia-related education for dental hygiene programs in colleges throughout Korea and dementia-related education for other health care-related fields were investigated. Based on the initial findings, the basic content of the preliminary curriculum was constructed. Second, based on the opinion of the dementia-related curriculum development committee comprising 10 professors of dental hygiene, the operations of dementia-related courses and relevant details were constructed. Third, these operations and relevant details were assessed and revised based on focus-group interviews. Fourth, the dementia-related curriculum was developed based on the study findings and literature review. Results: The name of the course in the developed curriculum was set as "Elderly Dental Hygiene and Practicum." The course was established as a "major elective," and was offered as two units with two instructional sessions of two hours each. The learning goal for this course was acquiring the methods for understanding and managing the characteristics of the elderly. A consensus was reached regarding conducting the theoretical and practical lessons on some of the dementia-related content. The dementia-related curriculum comprised 10 "required contents" which is to be conducted over three weeks of theoretical lessons and two weeks of practical lessons. Conclusions: With the growing elderly population, the development and operation of the curriculum for geriatric oral health interventions are warranted in dental hygiene education. However, continued discussions and improvements are needed on the extent of educational content, considering various dementia-related symptoms and general geriatric systemic diseases.
Objective: To determine and compare the frequency distribution of various arch shapes in ethnic Malays and Malaysian Aborigines in Peninsular Malaysia and to investigate the morphological differences of arch form between these two ethnic groups. Methods: We examined 120 ethnic Malay study models (60 maxillary, 60 mandibular) and 129 Malaysian Aboriginal study models (66 maxillary, 63 mandibular). We marked 18 buccal tips and incisor line angles on each model, and digitized them using 2-dimensional coordinate system. Dental arches were classified as square, ovoid, or tapered by printing the scanned images and superimposing Orthoform arch templates on them. Results: The most common maxillary arch shape in both ethnic groups was ovoid, as was the most common mandibular arch shape among ethnic Malay females. The rarest arch shape was square. Chi-square tests, indicated that only the distribution of the mandibular arch shape was significantly different between groups (p = 0.040). However, when compared using independent t-tests, there was no difference in the mean value of arch width between groups. Arch shape distribution was not different between genders of either ethnic group, except for the mandibular arch of ethnic Malays. Conclusions: Ethnic Malays and Malaysian Aborigines have similar dental arch dimensions and shapes.

Background: The term 'hardcore' has been applied to use of smoking tobacco and generally referred to as the inability or unwillingness of regular smokers to quit. The component constructs of hardcore except nicotine dependence are product neutral. With the use of 'time to first chew' as a measure of nicotine dependence, hardcore definition can be extended to characterize smokeless tobacco users. Hardcore users respond less to tobacco cessation interventions, and are prone to tobacco induced diseases including cancer. Thus identifying hardcore users would help in estimate the burden of high risk population for tobacco induced diseases. Smokeless tobacco use is predominant and accounts for more than 50% of oral cancer in India. Hence, hardcore chewing information could be used for planning of tobacco and cancer control interventions. The objective of this study was to assess the prevalence and associated factors of hardcore smokeless tobacco use in India. Materials and Methods: Global Adult Tobacco Survey (GATS)-India 2010 data were analyzed to quantify hardcore smokeless tobacco use in India with following five criteria: (1) current daily smokeless tobacco use; (2) no quit attempt in the past 12 months of survey or last quit attempt of less than 24 hours duration; (3) no intention to quit in next 12 months or not interested in quitting; (4) time to first use of smokeless tobacco product within 30 minutes of waking up; and (5) knowledge of smokeless tobacco hazards. Results: The number of hardcore smokeless tobacco users among adult Indians is estimated to be 5% (39.5 million). This group comprises 23.2% of daily smokeless tobacco users. The population prevalence varied from 1.4-9.1% across different national regions of India. Logistic regression modeling indicated age, education and employment status to be the major predictors of hardcore smokeless tobacco use in India. Conclusions: The presence of a huge number (39.5 million) of hardcore smokeless tobacco users is a challenge to tobacco control and cancer prevention in India. There is an unmet need for a universal tobacco cessation programme and intensification of anti-tobacco education in communities.
연구목적: 치과공포는 주로 과거의 부정적인 치과 치료 경험에 기인하여 치과 방문 시 중재되지 않은 불안과 두려움의 감정을 의미하며, 성인에서는 치과치료의 필요함을 인식함에도 불구하고 진료회피로 이어져 구강건강의 악화를 야기할 수 있다. 따라서 본 연구는 일부 성인을 대상으로 치과공포 및 불안 수준을 측정하고, 그와 관련된 요인 및 이에 영향을 미치는 원인에 대해 파악하고자 수행되었다. 연구방법: Kleinknecht's의 치과 공포 척도(Dental Fear Survey scale)를 이용하였고, SPSS 21 프로그램(IBM SPSS Statistics 21)을 사용하여 분석하였다. 연구결과: 치과공포도가 높을수록 "치과병원 문을 열고 들어설 때 두려움"가 가장 높게 나타났으며, "치과의자에 낮아 치료를 기다릴 때 두려움", "치과의사를 쳐다볼 때 두려움", "치과기구가 돌아가는 소리가 들릴 때 두려움", "치과 진료실에서 병원 냄새가 날 때 두려움" 순으로 나타났다. 고안: 치과의사와 치과위생사는 환자가 느끼는 공포와 두려움을 이해하여 체계적인 의료서비스를 제공할 수 있도록 치과 불안에 대한 여러가지 중재법을 모색할 수 있어야 한다.
Objective: The purpose of this study was to investigate the relationship between the mandibular dental and basal arch forms in subjects with normal occlusion and compare them with those of Class III malocclusion using cone-beam computed tomography (CBCT). Methods: CBCT images of 32 normal occlusion (19 males, 13 females; 24.3 years) and 33 Class III malocclusion subjects (20 males, 13 females, 22.2 years) were selected. Facial axis and root center points were identified from the left to right mandibular first molars. Distances between the facial axis and root center points for each tooth were calculated, and 4 linear and 2 ratio variables were measured and calculated for each arch form. The variables were compared between groups by independent t-test. Pearson correlation coefficient was applied to assess the relationships between dental and basal variables within each group. Results: The mandibular dental and basal intercanine widths were significantly greater in the Class III group than in normal occlusion subjects (p < 0.05). The dental and basal intercanine widths as well as the dental and basal intermolar widths were strongly correlated in normal occlusion and moderately correlated in Class III malocclusion. Conclusions: The dental arch form demon strated a strong positive correlation with the basal arch form in the normal occlusion group and moderate correlation in the Class III malocclusion group. These results might be helpful for clinicians to have a better understanding of the importance of basal arch form in the alveolar bone.
