The objective of this study was to confirm the validity of classification of dental disability by measuring the dental treatment time required for disabled patients and identify the disability type that requires more of chair time. As a result of measuring a total of 123 patients who were admitted to the Seoul dental hospital for the disabled on Oct. 2014, I was able to make the following conclusions: 1. For dental counseling and check-up, the chair time did not show significant differences between the different types of disability. 2. For periodontal treatment, restorative treatment, root canal treatment, and surgical treatment, the difference of the chair time for dentally disabled patients was significantly longer than that of non-dentally disabled patients. 3. The difference in the total chair time for prosthetic treatment was statistically insignificant. However, each of the prosthetic treatment steps did show statistically significant differences.
Purpose: Dental implants present several advantages over other tooth replacement options. However, there has been little research on masticatory function in relation to implant treatment. Therefore, the aim of the present study was to evaluate the improvement of masticatory function two weeks after implant restoration. Methods: Masticatory ability was evaluated with the subjective food intake ability (FIA) and objective mixing ability index (MAI) methods. Fifty-four subjects with first and second missing molars completed the study. The subjects were asked to complete a self-reported questionnaire about 30 different food items, and to chew wax samples 10 times both before and two weeks after implant restoration. A total of 108 waxes were analyzed with an image analysis program. Results: Dental implant restoration for lost molar teeth on one side increased the FIA score by 9.0% (P<0.0001). The MAI score also increased, by 14.3% after implant restoration (P<0.0001). Comparison between the good and poor mastication groups, which were subdivided based on the median MAI score before implant restoration, showed that the FIA score of the poor group was enhanced 1.1-fold while its MAI score was enhanced 2.0-fold two weeks after an implant surgery. Conclusions: Using the FIA and MAI assessment methods, this study showed that masticatory function was improved two weeks after implant restoration. In particular, the enhancement of masticatory function by implant restoration was greater in patients with relatively poor initial mastication than in those with good initial mastication.
Objective: To three-dimensionally elucidate the effects of occlusal hypofunction on the periodontal ligament and alveolar bone proper of rat molars by micro-computed tomography (micro-CT). Methods: Occlusal function in the molar area was restricted by attaching an anterior bite plate on the maxillary incisors and a metal cap on the mandibular incisors of 5-week-old male Wistar rats for 1 week. The periodontal ligament space and alveolar bone proper around roots of the mandibular first molar were assessed by histology and micro-CT. Results: The periodontal ligament space was narrower and the alveolar bone proper was sparser and less continuous in the hypofunction group than in the control group. Further, both the volume of the periodontal ligament and the volumetric ratio of the alveolar bone proper to the total tissue in the region of interest were significantly lower in the hypofunction group (p < 0.05). Conclusions: Occlusal hypofunction induces atrophic changes in the periodontal ligament and alveolar bone proper of rat molars.

Objective: To measure aspartate aminotransferase (AST) activity in the pulp of teeth treated with fixed appliances for 6 months, and compare it with AST activity measured in untreated teeth. Methods: The study sample consisted of 16 healthy subjects (mean age $25.7{\pm}4.3$ years) who required the extraction of maxillary premolars for orthodontic reasons. Of these, 6 individuals had a total of 11 sound teeth extracted without any orthodontic treatment (the control group), and 10 individuals had a total of 20 sound teeth extracted after 6 months of orthodontic alignment (the experimental group). Dental pulp samples were extracted from all control and experimental teeth, and the AST activity exhibited by these samples was determined spectrophotometrically at $20^{\circ}C$. Results: Mean AST values were $25.29{\times}10^{-5}U/mg$ (standard deviation [SD] 9.95) in the control group and $27.54{\times}10^{-5}U/mg$ (SD 31.81) in the experimental group. The difference between these means was not statistically significantly (p = 0.778), and the distribution of the AST values was also similar in both groups. Conclusions: No statistically significant increase in AST activity in the pulp of mechanically loaded teeth was detected after 6 months of orthodontic alignment, as compared to that of teeth extracted from individuals who had not undergone orthodontic treatment. This suggests that time-related regenerative processes occur in the dental pulp.
본 연구는 올바른 구강보건교육과 칫솔질 교육을 통해 아동들의 구강건강에 관한 관심을 증진시키고 구강관리법의 변화와 개선 정도를 알아보고 더 나아가 좀 더 효율적인 구강보건교육의 방법을 모색하고자 하였다. 강원도 태백시 보건소에서 2003년 4월 1일부터 12월 16일까지 구강보건교육에 참가한 631명의 미취학 아동들을 대상으로 시행하였다. 매회 약 20-30명 정도를 대상으로 약 30분 정도 시청각 자료를 이용한 치과의사의 직접설명과 아동들이 참여놀이를 이용하여 구강보건교육 후 칫솔질 교육과 불소 겔 도포를 시행하였으며, 불소도포 과정 중 대기하는 아동들은 구강보건교육 비디오를 시청하도록 하였다. 구강보건교육의 효과와 아동들의 구강 건강관리의 변화를 알아보기 위하여 하반기 교육 후 아동들의 부모와 선생님들을 대상으로 설문지를 작성하게 하고 그 결과를 수집, 분석하였다. 아동들은 보건소에서의 전문가 구강보건교육을 시행한 후 치아우식증에 대한 지식이 향상되었으며, 치과 치료에 대한 두려움이나 공포가 감소하는 변화가 있었다. 많은 아동들이 올바른 칫솔질 방법을 습득하였으며 칫솔질 횟수의 증가를 보였다. 또한 칫솔질에 대한 재미와 흥미를 유발하는 효과도 나타내었다. 아동들은 구강보건교육을 통해 치아에 좋은 음식과 해로운 음식을 구별 할 수 있는 능력을 가지게 되었고 미약하나마 식습관의 변화를 발견할 수 있었다. 본 조사를 통해 미취학 아동의 구강보건교육은 매우 실효성이 있는 것으로 판단되며 가정에서나 유치원, 치과에서의 구강보건교육의 확대가 요구된다.
본 연구는 제9차(2013)청소년건강행태 온라인조사를 바탕으로 우리나라 청소년 총 66,951명을 대상으로 청소년의 건강위험요인과 구강건강과의 관련성을 확인하고, 구강건강에 미치는 위험도를 알아보고자 하였다. 수집된 자료는 통계 분석 프로그램인 PASW Statistics ver. 18.0을 사용하여 분석하였고 주요 결과는 다음과 같다. 남학생은 33,777명(50.5%), 여학생은 33,174명(49.5%)으로 남학생 비율이 높았고, 학년 분포는 중학생과 고등학생 모두 1학년이 가장 높았다. 학업성적은 '하위권'이 24,468명(36.6%)으로 가장 높았고, 경제 상태는 중위권이 32,374명(48.4%)으로 가장 높았다. 어머니 학력은 고졸이 가장 높았다. 성별에 따른 6가지 구강증상경험 분포의 차이를 알아본 결과 '치아가 깨지거나 부러짐'은 성별의 차이가 유의하지 않았고(p=0.135), '먹을 때 치아가 아픔', '치아가 욱신거리고 쑤심', '잇몸이 아프고 피가 남', '혀와 입 안쪽 뺨이 아픔'은 여학생이 남학생보다 더 많이 경험한 것으로 나타났다(p<0.001). 그러나 '입 냄새가 남'은 남학생(52.5%)이 여학생(47.5%)보다 더 많이 경험한 것으로 나타났다(p<0.001). 건강위험요인과 구강증상경험과의 관련성 분석결과 음주, 흡연, 학교폭력 경험, 주중 인터넷 사용이 관련성이 있었으며 로지스틱 회귀분석 결과 음주경험이 있는 대상자의 구강증상경험 위험도가 OR 1.33배, 흡연 경험이 있는 대상자의 구강증상경험 위험도가 OR 1.2배로 높아졌으며 인터넷 사용시간은 '1시간 이내 사용'보다 '7시간 이상' 사용자가 구강증상경험 위험도가 OR 1.25배 높아졌으며 학교폭력 경험도 경험이 없는 대상자에 비해 '3~4번' 경험이 있는 대상자의 구강증상경험 위험도가 OR 1.54배 높아짐을 알 수 있었다. 따라서 본 연구 결과를 토대로 청소년의 올바른 건강습관을 도모하고, 건강위험요인을 적절히 중재하여 청소년의 구강건강을 증진시킬 수 있는 포괄적 건강증진 프로그램개발에 기여할 수 있을 것이다.
이 연구는 구강기능향상운동을 통한 의치사용 노인들의 구강기능의 향상과 의치만족도 및 구강건강관련 삶의 질과의 관련성을 파악하고자 2013년 9월부터 2014년 4월까지 경상북도 청송군 관내 경로당 13개소에서 실시되었다. 구강기능향상운동 프로그램 실시군 79명과 미실시군 71명을 대상으로 구강기능, 의치만족도, 구강건강관련 삶의 질(OHIP-14)을 조사하여 다음과 같은 결과를 얻었다. 구강기능은 실험군에서 타액분비량이 사전보다 사후에 $1.53m{\ell}$ 유의하게 증가하였고(p<0.001), 개구량은 사전보다 사후에 0.33 cm 유의하게 증가하였다(p<0.01). 또한 발음은 사전보다 사후에 6.91회 유의하게 증가하였고(p<0.001), 미각 기능도 0.52점 유의하게 증가하였으며(p<0.001), 실험군과 대조군의 타액분비량, 개구량, 발음의 개선 정도 및 미각의 두 군 간 변화량은 유의한 차이가 있었다(p<0.01). 의치만족도는 실험군에서 고정기능 만족도가 사전에 비해 사후에 0.32점 유의하게 증가하였고(p<0.01), 일반적 치료 만족도는 사전에 비해 사후에 0.26점 유의하게 증가하였으며(p<0.01). 의치만족도 총점은 사전에 비해 사후에 0.22점 유의하게 증가하였다(p<0.01). 실험군과 대조군의 저작기능 만족도, 고정기능 만족도, 일반적 치료만족도, 의치만족도 총점은 두 군 간의 변화량이 유의한 차이가 있었다(p<0.05). 구강건강관련 삶의 질은 사회적 불리와 구강건강관련 삶의 질의 총점에 있어서 두 군 간의 변화량에 유의한 차이가 있었다(p<0.05) 이상의 연구결과로 구강기능향상운동이 의치사용 노인의 구강기능 개선과 의치만족도를 높이는데 긍정적인 영향을 미친다는 것을 확인하였으며, 향후 노인들의 구강건강관련 삶의 질을 향상시킬 수 있는 방안들이 모색될 필요가 있다.
Objective: To investigate the long-term effects of maxillary skeletal expander (MSE) treatment on functional breathing. Methods: Objective measures of breathing, the peak nasal inspiratory flow (PNIF), and peak oral inspiratory flow (POIF), and subjective measures of breathing, the visual analog scale (VAS) and nasal obstruction symptom evaluation (NOSE) survey, were used to investigate the long-term effects of MSE in functional breathing. Seventeen patients, mean age 19.4 ± 3.9 years treated at the UCLA Orthodontics Clinic were assessed on their functional breathing at 3 timepoints: pre-expansion (T0), post-expansion (T1), and post-orthodontic treatment (T2). Results: Immediately after expansion (T1), all the objective functional breathing values were significantly increased in comparison to T0 (P < 0.05). The VAS total, VAS right and VAS left were significantly lower at T1 in comparison to T0 (P < 0.05). At 26.8 ± 3.9 months after MSE expansion (T2), PNIF total, PNIF right, PNIF left, and POIF were significantly higher when compared to T0 (P < 0.05). Also, VAS total, VAS right and VAS left were significantly lower at T2 when compared to T0 (P < 0.05). Additionally, there was a positive correlation between PNIF and the magnitude of expansion at anterior nasal spine and zygomaticomaxillary point (ZMA). There was a positive correlation between total VAS and the magnitude of expansion at the ZMA. There were no significant changes for the NOSE subjective breathing measurement at all time comparisons. Conclusions: Overall, MSE treatment produces an increased objective and subjective airway improvement that continues to remain stable in the long-term post expansion.

Objectives: The objective of this study was to investigate the presence of oral lesions in human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) patients in a descriptive cross-sectional study, and to establish their presence according to levels of CD4+ cells (including the CD4+/CD8+ cell ratio). Materials and Methods: A total of 75 patients infected with HIV were included. Oral lesions were observed and classified using World Health Organization classification guidelines. Potential correlations between the presence and severity of oral lesions and CD4+ cells, including the CD4+/CD8+ cell ratio, were studied. Results: The most frequent oral lesion detected was oral pseudomembranous candidiasis (80.0%), followed by periodontal disease (40.0%), herpetic lesions (16.0%), hairy leukoplakia (16.0%), gingivitis (20.0%), oral ulceration (12.0%), Kaposi's sarcoma (8.0%), and non-Hodgkin's lymphoma (4.0%). The CD4+ count was <$200cells/mm^3$ in 45 cases (60.0%), between $200-500cells/mm^3$ in 18 cases (24.0%), and >$500cells/mm^3$ in 12 cases (16.0%). The mean CD4+ count was $182.18cells/mm^3$. The mean ratio of CD4+/CD8+ cells was 0.26. All patients showed at least one oral manifestation. Conclusion: There was no correlation between the CD4+/CD8+ cell ratio and the presence of oral lesions. The severity of the lesions was more pronounced when the CD4+ cell count was less than $200cells/mm^3$.
Objectives: The objective of this work was to investigate the hospital nursing care of oral and maxillofacial health in jeju province. Methods: 438 Registered nurses(RN) who were working at each of secondary hospitals in Jeju province had responded to the questionnaire. The data were analyzed via frequency analyses and one-way ANOVA to assess the state of RN on hospital nursing care of oral and maxillofacial health. Results: The class of education on density in formal density that marked '0 hour' and '1-3 hours' were 73.5% and 19.9%, respectively. The class of refresher training on density that marked '0 hours' and '1-3 hours' were 92.9% and 6.6%, aggregately 99.5%. The nursing education on appearance after tumor of maxillofacial area that marked 'formal education' and 'none' were 45.2% and 52.1%, respectively. The score of question 'function, effect and side effect of hexamedin gaggle' was $2.68{\pm}0.95$ by Likert 5-point scale. Likewise, the scores were $2.82{\pm}0.88$ on question 'management of removal denture', $2.83{\pm}0.95$ on question 'preventive dental treatment before anticancer therapy', $2.88{\pm}0.86$ on question 'function of saliva', $2.96{\pm}0.99$ on question 'oral management of tube feeding patient', $3.13{\pm}1.00$ on question 'bacterial endocarditis from oral microflora', $3.36{\pm}0.89$ on question 'dysphagia' and $3.62{\pm}1.03$ on question 'aspiration pneumonia'. RN replied that 'lack of knowledge' and 'delay of cooperation' formed 53.7% and 33.3% respectively, on question 'problem in dental consultation other diseased patient'. Conclusions: From this study, it is necessary for RN and student of nursing science to be educated on the oral and maxillofacial nursing. Authors suggest further co-study and nation-wide research.
