Early and Late Complications after Inguinofemoral Lymphadenectomy for Vulvar Cancer

  • Background: We aimed to determine the frequency of early and late complications following groin surgery for vulvar cancer and analyze possible risk factors. Materials and Methods: This retrospective cohort study included 99 women who underwent for vulvar cancer. The early (${\leq}1$ month) complications were wound infection, breakdown and lymphocyst and late (>1 month) complications were lower limb lymphedema, incontinence and erysipelas. The risk factors for developing each of the complications were analyzed with regression analysis. Results: In the entire cohort, 29 (29.3%) women experienced early and 12 (12.1%) had late complications. Wound complications including infection and breakdown were the leading early complications (23.2%). In the multivariate analysis, both obesity (body mass index ${\geq}30kg/m^2$) and advanced age (${\geq}65years$) were found as independent predictive factors for early complications. Obese women of advanced age had 6.32 times more risk of experiencing any of the early complications, when compared to non-obese and young women (55.6% vs 8.7%). The most common late complication was lower limb lymphedema (10.1%) that was more frequently seen in young women. However, neither age nor lymph node count were significantly associated with the occurrence of lower limb lymphedema. Conclusions: More than 40% of the women suffered from postoperative complications after inguinofemoral lymphadenectomy in the current study. While advanced age and obesity were the significant predictors for any of the early complications, there was no identified risk factor for lower limb lymphedema.

비만아동의 비만정도에 따른 생화학적 상태와 식습관 및 식행동에 관한 연구 (The Difference of Biochemical Status , Dietary Habits and Dietary Behaviors according to the Obesity Degree among Obese Children)

  • This study was conducted to find biochemical status, dietary habits and dietary behaviors according to the degree of obesity among obese children, and to provide baseline data for nutrition education. The number of subjects was 64 obese children(Mildly obesed : MI 19, Moderately obese : Mo 30, Severely obese : SI 15) participated in '98 Summer Nutrition Camp'. The results of this study were as follows. 1. Weight and body fat rate was increased according to obesity degree(p<0.05). Total cholesterol and TG also was increased according to obesity degree but it was statistically significant. 2. In dietary habits, it wasn't statistically significant but MI tended to eat fast and to eat the snacks before sleeping. MO tended to skip the breakfast. SI tended to eat more snacks and dinner and they tended to overeat. 3. In cognition of self-weight, most obese children(93.7%) worried about their weight but 73.4% of obese children thought that it was possible for losing weight. There were no difference by the obesity degree. 4. In dietary behaviors, there were no statistically significant difference but MI and SI than MO tended to had worse dietary behaviors. 5. In life styles, SI than the others have tendency to have more regular exercise but they had more indoor activities than outdoor activities. 6. In foods intake frequency, SI than MI and MO ate instant foods more frequently and MI than the others ate hot and salty foods more frequently(p<0.05). With these result, MI and SI than MO tended to have undesirable dietary habits and behaviors. Many obese children worried about the weight but they had positive thought about losing weight. SI tended to do regularly exercise but they had more indoor activities than outdoor activities. Therefore, only with this study, it is difficult to say that there is the clear difference by the obesity degree. Because, in this study, we can have some difference among these groups, we should study more about these difference for effective, systemic and practical nutrition education in the future.

The Effects of Low-Calorie Diet with Raw-Food Formula on Obesity and Its Complications in the Obese Premenopausal women

  • Recently interests on raw-food diets are rapidly increasing in relation to chronic diseases prevention in Korea, but studies on raw-food diets have been hardly performed by nutritionists. This study was performed to investigate the effects of low-calorie diets using a raw-food formula in the form of freeze-dried powder on obesity and its complications in the obese women (body mass index (BMI) $\geq$ 25kg/㎡) for eight weeks. Forty premenopausal women (mean BMI 28.04kg/㎡, mean age 28.33years old) participated in this diet intervention, and were controlled by eating 1 regular meal, 1-2 snacks and 2 raw-food formula (140kcal/pack) meals a day within the 1500-1300kcal ranges. Anthropo-mentric measurements, body compositions, physical exercise, and obesity-related risk factors were assessed before (the initial), during (the 4th week) and after (the 8th week) the study. All the data was analyzed by paired t-test, repeated measures ANOVA, and nonparametric rank test at p<0.05 level. Obesity was significantly increased during this study, and it was decreased in weight (-4.59%, p<0.000), BMI (-4.56%, p<0.000), body fat percent (-6.18%, p<0.000), fat mass (-10.19%, p<0.000), waist and hip circumferences(-5.69%, p<0.000 and -2.55%, p<0.000) and WHR (-3.24%, p<0.000). Energy expenditure of physical exercise was increased as much as 70kca1/day during the study (p=0.000), but it did not have any correlations with weight loss and changes of body compositions. Biochemical measurements including blood triglyceride(p <0.006) and leptin(p<0.000) levels were significantly decreased, LDL cholesterol level was increased(p<0.05), but all the blood lipid levels were in the normal ranges. Fatty liver echogenicity and menstrual irregularity were improved after the diet intervention(p<0.000 and p<0.034). In conclusion, this B-week low-calorie diet intervention using raw-food formula was effective for obese premenopausal women in reducing obesity and its risk factors so as not to proceed towards comorbidities. However, the variation of blood lipid levels should be observed for a longer Period.

12주 복합운동 프로그램이 비만 중년여성의 심혈관계 기능과 심전도 기반의 ST 분절, QRS군에 미치는 영향 (Effect of a 12-Week Combined Exercise Program on Cardiovascular Function and Electrocardiogram Based ST Segment, QRS in Obese Middle-Aged Women)

  • 본 연구는 12주 복합운동 프로그램이 비만 중년 여성의 심혈관계 기능과 심전도 기반의 ST 분절 및 QRS에 미치는 영향을 알아보기 위하여 중년 여성 24명을 무작위 배정하였다. 결과에 대한 분석은 집단 내 및 기간·집단 간 차이 검증을 실시하였다. 심혈관계 기능에서 심근 산소소비량은 실험군에서 안정단계와 운동 중 최고 강도 단계에서 유의한 감소가 나타났으며, 안정 시 집단·기간 간 상호작용 역시 나타났다. 심박수 회복률은 실험군에서 회복기 2분에 유의한 감소가 나타난 반면 대조군에서는 반대로 유의한 증가가 나타났다. 수축기 혈압에서는 실험군이 유의하게 감소한 것으로 나타났으며, 집단·기간 간 상호작용 역시 나타났다. 이완기 혈압은 실험군 대조군 모두 변화가 없었으며, 맥압에서는 회복기 3분 단계에서 실험군에서만 통계적으로 유의한 감소가 나타났다. 심전도의 경우 QRS 높이는 실험군이 안정 시, 회복기 3분, 6분, 9분에서 대부분 유의한 증가를 나타낸 반면 QRS 기간은 실험군 대조군 모두 유의한 차이가 없었으며, ST 분절 높이는 실험군에서 안정 시에 유의하게 증가한 것으로 나타났다. 따라서 본 연구의 결과를 종합해 보면 12주 복합운동 프로그램은 비만 중년여성의 심혈관계 기능과 심전도에서 심실의 수축력 향상과 심근의 허혈을 개선하는데 긍정적인 효과가 있다고 사료된다.

  • 스텝운동(bench-step aerobics: BSA)이 과체중 및 비만 우리나라 성인 여성을 위한 효과성, 안전성 및 실행 가능성을 갖춘 운동 프로그램이라는 것이 다수 연구자들에 의해 시사되어 왔다. 그러나 그 효과성은 일부 생리적 효과에 한하여 검증되었을 뿐 심리적 효과에 대해서는 거의 연구되지 않았다. 따라서 본 연구는 스텝운동 프로그램이 우리나라 과체중 및 비만(체질량지수 ${\geq}\;2\;kg/m^2$) 성인 여성에게 미치는 심리적 효과를 알아보기 위해 수행되었다. 이러한 목적을 달성하기 위해 보건소 방문자와 지역신문 및 보건소 홈페이지 홍보를 통해 모집된 여성 중 편의 추출한 운동군 15명과 대조군 13명의 분노(anger), 운동관련 자기 효능감(exercise self-efficacy), 운동관련 정서(exericse-related affect), 그리고 신체상(body image)을 운동 프로그램 전후에 측정하여 비교하였다. 자료 수집은 12주 스텝운동 프로그램(2006년 8월${\sim}$11월) 전과 후에 보건소에서 이루어졌다. 운동군에게 적용된 스텝운동 프로그램은 중강도[개인의 예비 심박 수(Heart Rate Reserve : HRR)의 $40/50{\sim}50/60%$]로 $45{\sim}60$분 동안 주 3회 이루어졌으며, 상기 운동 강도 및 운동 시간은 대상자의 적응도에 따라 12주에 걸쳐 점진적으로 증가되었다. 12주간의 스텝운동 후 운동군의 신체상이 유의하게 개선되었으며, 이는 대조군에서의 변화와도 유의한 차이를 보였다. 분노, 운동관련 자기 효능감, 그리고 운동관련 정서 또한 개선되는 경향이었으나 통계적으로 유의한 변화는 보이지 않았다. 이로써 우리나라 과체중 및 비만 성인 여성 대상 스텝운동의 심리적 효과가 부분적으로 지지되고 있으나, 현재까지 이루어진 관련 연구가 거의 없고 본 연구 또한 연구 설계상의 제한점을 갖고 있으므로 연구결과의 일반화를 위한 추후 연구가 요구된다. 특히 Floor effect 및 Ceiling effect 등에 의한 효과를 배제하기 위해 분노 수준이 상대적으로 더욱 높거나 운동관련 정서 수준이 상대적으로 낮은 과체중 및 비만여성을 대상으로 반복 연구를 수행할 필요가 있다.

20대 비만여성에서 6주간 복합운동을 통한 신체구성과 균형능력의 융합적 분석 (Convergence analysis of Body Composition and Balance after 6 Week Combined Exercise in 20's Obese Women)

  • 본 연구 목적은 20대 비만 여성에서 6주간의 복합운동 적용 시 신체구성변화가 균형능력에 어떠한 영향을 미치는지 알아보고자 하였다. 자발적 참여의사를 밝힌 20대 비만 여성 21명을 Control(no exercise, n=7), Group I(aerobic, n=7), Group II(combined, n=7)으로 무작위 배정하여 매회 50분간 1주일에 5회 총 6주간 시행하였다. 운동 전, 후 및 측정시기에 따른 세 집단 간 신체구성과 균형변화를 SPSS 18.0 ver.을 이용하여 통계분석 하였다. 연구결과, 운동 전에 비해 Group I, II는 체중과 체지방률이 감소하였고, Group II에서만 근육량의 증가와 체질량 지수가 감소하였다(p<0.05). 균형변화는 Group II에서 정적, 동적균형 모두 향상하였으나, Group I은 정적균형만 향상하였다(p<0.05). 따라서, 복합운동프로그램은 신체구성 변화와 근육량 증가를 유발시켜 균형능력 향상에 긍정적 효과를 나타낸 것을 확인할 수 있었다.

복합운동의 운동유형 순서변화가 비만여성에게 미치는 영향 (The Effect of Changing the Order of Exercise Types on Body Composition and Blood Lipid in Obese Women)

  • 본 연구는 복합운동 유형 순서 변화에 따른 생리적, 생화학적 변화를 비교 하고자 40대 비만여성(n=18)을 대상으로 12주간 복합운동을 실시하였다. 복합운동 유형은 유산소성 운동과 저항성 운동그룹 A(n=9) 그리고 저항성 운동과 유산소성 운동그룹 B(=9)로 구성하였다. 체지방량의 경우 A그룹의 평균변화량이 B그룹 평균변화량보다 더 많이 감소하였고 통계적으로 유의한 차이를 보였다(p<0.01). 혈당의 경우는 A그룹보다는 B그룹이 더 많이 감소하였으며 통계적으로 유의한 차이를 보였다(p<0.01). 따라서, 비만 중년 여성의 체중감량을 위한 신체구성 변화 유도를 위해서는 복합운동의 순서 변화에 따른 운동유형이 선택적으로 사용되어질 수 있다.

영양교육과 운동중재 프로그램이 성인비만여성의 신체성분과 식이섭취, 혈중지질 및 기초체력에 미치는 효과 (2) - 비만관리 프로그램의 참여율과 프로그램 효과와의 상관성 - (The Effect of Nutrition Education and Excercise Program on Body Composition and Dietary Intakes, Blood Lipid and Physical Fitness in Obese Women (2) - Relationship between Participation Rates and Effectiveness of Obesity Management Program -)

  • This study examined how the participation rates contribute to subjects' outcomes after a nutrition education (once/week) and exercise (3 times/week) intervention program for 12 weeks in 44 obese female subjects. The subjects were divided into two groups: high-participator (HP) or low-participator (LP) groups according to their participation levels based on the median visit value (30 visits out of a total of 48 visits). Daily nutrient intakes assessed by a 24-hour recall, body composition, blood lipid profiles and physical fitness were measured at baseline and after 12 weeks. After the intervention, weight, BMI, percent body fat, and the waist-hip ratio were significantly decreased in the HP group. In addition, the HP group had a greater rates of changes in weight (HP group: -6.6% and LP group: -3.1%, p < 0.01), BMI (-7.0% and -3.2%, p < 0.01), percent body fat (-7.8% and -4.2%, p < 0.05), and waist-hip ratio (-3.1% and -0.4%, p < 0.01) compared to the LP group. Energy-adjusted protein, fiber, potassium, vitamin $B_1$, vitamin $B_6$, and vitamin C were significantly increased in the HP group. The HP group showed a significant increase in HDLcholesterol and a decrease in blood pressure, total cholesterol, LDL-cholesterol, triglycerides, and atherogenic index (AI). While the muscle endurance, muscle strength, power, agility and flexibility were significantly increased in the HP group, agility and balance were improved in the LP group. In addition, dietary behavior score was significantly increased and stress score decreased in the HP group compared to the LP group. High participation rates were correlated with the improvement of the nutrient density (protein, fiber, potassium, Vitamin $B_1$, Vitamin $B_2$, Vitamin $B_6$ and folate), anthropometric parameters (body weight, BMI and percent body fat), serum lipid profiles (HDL-cholesterol, Triglyceride, and AI) and physical fitness (muscle endurance) adjusted for age, menopausal status, alcohol drinking, nutritional supplement use and baseline BMI. Our results suggest that high participation in nutrition education and exercise program was effective not only for weight reduction but also for the improvement of physical fitness in obese women.
