비만관리프로그램이 초등학교 비만아동의 비만도, 신체조성, 체력, 지식 및 식행동에 미치는 효과 (The Effects of Obese Program on the BMI, Body Composition, Physical Fitness, Knowledge, Eating Behaviors in Obese Elementary School)

  • Purpose: The purpose of this study was to identify the effects of the obese program on BMI, body composition, physical fitness, knowledge, eating behaviors among the elementary school obese children. Method: They ranged from the third to sixth grades with over 30% body fat ratio. The subjects consisted of 31 obese children who participated in the obese program and 34 obese who did not participate in the program. The contents of the programs for nine weeks from contained obese education, counseling and exercises once a week. The program lasted from Oct. 8 to Dec. 22, 2001. The Data were analyzed using SPSS program. Result: Flexibility was significantly higher for the program participants than the nonparticipants at the post-test. Agility was significantly higher for the program participants than the nonparticipants at the post-test. Knowledge were significantly higher for the program participants than the nonparticipants at the post-test. Conclusion: This study provides evidence that obese program is effective in changing the flexibility, agility, knowledge in obese elementary children over 9 weeks.

학령기 아동을 위한 멀티에이전트 비만관리 프로그램의 효과 (The Effects of a Multi Agent Obesity Control Program in Obese School Children)

  • Purpose: The purpose of this study was to identify the effects of a multi agent obesity control program in obese school children. This program was composed of strategies to modify diet and exercise habits and to change cognitive behavior variables(stress, coping, and self-efficacy). Method: The subjects were 40 obese school children who participated in our project voluntarily via homepage, TV, newspaper, public paper and school official documents. The program was implemented daily for 4 sessions per day for ten days from August 16 to 26, 2004. The daily program consisted of exercise therapy, dance therapy, cognitive behavior therapy and aroma therapy. The data was analyzed by paired t-test using the SPSSWIN program. Result: There was a significant decrease in children's waist-hip ratio(p=.04) and in children's stress(p=.00) after the program. There was a significant increase in children's self-confidence after the program(p=.02) and a significant decrease in children's diet habit after the program(p=.02). Conclusion: This study provides evidence that a multiagent obese control program is effective in changing waist-hip ratio, stress, self-confidence, and diet habits in obese school children.

건강체중아 및 비만아의 체중조절 경험, 식습관, 운동습관 및 자아존중감 비교 (Comparison of Weight Control Behavior and Self-esteem between Healthy Weight and Obese Children)

  • The study compared eating and physical activity related behaviors and self-esteem between healthy weight and obese children by presenting 175 primary school students in Busan City and Gyeongsang province with a self-reported questionnaire and Coopersmith's self-esteem inventory. The questionnaire was composed of 25 items, weighted primarily by a Likert scale. The self-esteem inventory presented to the students comprised 25 'Yes' or 'No' response questions to different statements. The study found obese children were more likely to think they always had to control their weight (p=0.000), reportedly measuring their weights significantly more than the healthy weight children. Also the study found that obese children are significantly more likely than healthy weight children to go on a diet, however neither group were successful in losing weight as the duration of the diet in 79.5% of the total sample lasted no longer than one week. In comparison to healthy weight children, obese children reported that they consumed fewer snacks during the day, avoided snacking subsequent to an evening meal and exercised more frequently for as long as physically possible. Interestingly, we found no difference of reported self-esteem between groups, though the obese group were more likely to answer that their parents did not understand them (p=0.055) Based on these findings, we concluded that the obese children who participated in the study were more aware of their body weights than the healthy weights children. It may be necessary to investigate further the relationship between self-esteem and participants' weights while considering other variables such as personality and body image.

비만 남아를 위한 최적 규격치 설정 및 사이즈 스펙 개발 - 초등학생 4~6학년을 중심으로 - (A Study on the Optimal Sizing System for Obese Children - Focusing on 4~6 Grade Elementary School Boys-)

  • As the population of overweight and obese children is rising rapidly around the world, there are many researches on purchasing and wearing children's clothing and optimal sizes, but researches on obese children are still inadequate. This study was carried out on 192 obese children over 75% in BMI. The purpose of the study was to set up the optimal interval of sizing system using the loss function which would be a guide for obese children for selecting ready to wear of suitable size. Introducing a loss function, which reflects how much the purchasing desire changes according to the difference, we formulate the problem and suggest a procedure to determine the optimal standard sizes minimizing the loss. These results were as follows ; In size chart of top's, 4 sizes had been determined by a loss function, had covered more than 91.1% of all subjects. In size chart of bottom's, 5 sizes had been determined by a loss function, had covered more than 87.0% of all subjects.

초등학교 비만아동 및 정상체중아동과 어머니의 온도 적응성과 착의 행동의 관계 (Relation of Adaptability to Air Temperature and Wearing Behavior between Obese and Normal Weight Elementary School Children and Their Mothers)

  • This study was conducted to investigate the relation of adaptability to the temperature and wearing behavior in both obese and normal weight children and their mothers' guidance. A questionnaire was used and the subjects were elementary school children in the 4th to the 6th grade (1,501) and their mothers (1,459) living in Seoul, Busan, Daegu, Gwangju, and Andong cities. The children were divided into two groups: a normal weight group (NWG, 1,192) and an obese group (OG, 309). There was no relation between children's adaptability to temperature and their mothers'. OG's mothers were more affirmative than NWG's mothers in purchasing children's clothing which suited them. The more NWG's mothers purchased children's clothing which suited them, the more NWG chose clothes that were conscious of body figure, while OG's mothers chose children's clothing which suited them regardless of OG's consideration of the body figure. It is suggested that an educational program of wearing behavior of obese children and their mothers as well as various measurements for the questionnaire should be developed for more intensive studies in this field.

중등도 이상 비만아동의 영양소 섭취, 영양지식, 식습관 및 생활태도 조사 (Nutrient Intakes, Nutritional Knowledge, Food Habits, and Lifestyle Behaviors of Obese Children)

  • The purpose of this study was to investigate the nutrient intakes, nutritional knowledge, food habits, and lifestyle behaviors of moderately or severely obese children living in the Kyonggi-do area. The subjects in this study were 37 obese children with an obesity index higher than 130% in the forth to sixth grades of elementary school. Anthropometric measurements, 24-hour recall of dietary intakes, and a survey on nutritional knowledge, food habits, and lifestyles behavior were conducted in the 37 obese children (25 boys and 12 girls). The average age, body weight, BMI, and R$\ddot{o}$hrer Index were 12.4 years, 68.6 kg, $29.6kg/m^2$, and 194.9 in boys and 12.7 years, 65.7 kg, $29.8kg/m^2$, and 201.8 in girls, respectively. The percentage of correct answers on nutritional knowledge and nutritional attitude score/max score were 85.8% and 23.0/50 in boys and 87.0% and 17.5/50 in girls, respectively. Except for calcium (85.3%) in girls and calcium (62.7%) and folic acid (83.3%) in boys, the average daily nutrient intakes percentages of nutrients were greater than the DRIs values (107.9~327.3% in boys and 103.0~416.0% in girls). This study showed the existence of some problems associated with obesity, such as higher frequency of skipping breakfast, irregular meal times, unbalanced diet, lack of intake of some nutrients, low nutritional attitude, lack of physical activity, and time management in obese children. These results suggest that a systematic education program including nutritional education on balanced diet, good eating habits and behaviors, and importance of eating breakfast and meal regularity must be emphasized to improve nutrient intakes and food habits in obese children. Obese children also require a more concentrated program that includes physical activity, weight reduction, and weight maintenance strategies to improve their life habits.

Subtle inflammation: a possible mechanism of future cardiovascular risk in obese children

  • Purpose: The risk of cardiovascular disease (CVD) has been shown to be associated with systemic inflammation in obese adults with metabolic syndrome (MetS). The aims of this study were to evaluate the prevalence of MetS and its relation to inflammatory markers in obese Thai children. Methods: A cross-sectional study was conducted. Children with history of endogenous obesity, chronic diseases, drug ingestion, and any acute illness within 2 weeks prior to enrollment were excluded. Their fasting blood glucose (FBG) levels, oral glucose tolerance tests, insulin, lipid profiles, and selected inflammatory markers, including interleukin-6, tumor necrosis factor-alpha, and high-sensitivity C-reactive protein (hs-CRP) levels, were tested. Results: In this study, 58 obese Thai children (female, 20; male, 38) with a mean body mass index z score of $5.1{\pm}2.2$ were enrolled. The prevalence of MetS and prediabetes was 31% and 17.2%, respectively. None of the children had diabetes. FBG levels, 2-hour glucose levels, and lipid profiles were not statistically different between those with and without MetS. However, obese children with MetS had higher insulin levels and homeostasis model assessment of insulin resistance values. Elevated hs-CRP levels were found in 69% of the cases, although it was not statistically different between the 2 groups. Conclusion: We described a substantial prevalence of MetS in Thai obese children. Regardless of MetS status, two-thirds of the obese children had elevated hs-CRP level, indicating subtle ongoing inflammatory process. This chronic inflammation feasibly predisposes them to CVD in the future, even in children without MetS.

Validation of self-reported height and weight in fifth-grade Korean children

  • Height and weight are important indicators to calculate Body Mass Index (BMI); measuring height and weight directly is the most exact method to get this information. However, it is ineffective in terms of cost and time on large population samples. The aim of our study was to investigate the validity of self-reported height and weight data compared to our measured data in Korean children to predict obese status. Four hundred twenty-two fifth-grade (mean age $10.5{\pm}0.5$ years) children who had self-reported and measured height and weight data were final subjects for this study. Overweight/obese was defined as a BMI of or above the 85th percentile of the gender-specific BMI for age in the 2007 Korean National Growth Charts or a BMI of 25 or higher (underweight : < 5th, normal : ${\geq}5th$ to < 85th, overweight : ${\geq}85th$ to < 95th). The differences between self-reported and measured data were tested using paired t-test. Differences based on overweight/obese status were tested using analysis of variance (ANOVA) and linear trends. Pearson's correlation and Cohen's kappa were tested to examine agreements between the self-reported and measured data. Although measured and self-reported height, weight and BMI were significantly different and children tended to overreport their height and underreport their weight, the correlation between the two methods of height, weight and BMI were high (r = 0.956, 0.969, 0.932, respectively; all P < 0.001), and both genders reported their overweight/non-overweight status accurately (Cohen's kappa = 0.792, P < 0.001). Although there were differences between the self-reported and our measured methods, the self-reported weight and height was valid enough to classify overweight/obesity status correctly, especially in non-overweight/obese children. Due to bigger underestimation of weight and overestimation of height in obese children, however, we need to be aware that the self-reported anthropometric data were less accurate in overweight/obese children than in non-overweight/obese children.

Influence of subclinical hypothyroidism on metabolic parameters in obese children and adolescents

  • Background: Subclinical hypothyroidism (SH) is a common condition in obese children. However, its effect on glucose and lipid metabolism in obese children remains controversial. Purpose: The present study aimed to investigate the association between SH and metabolic parameters. Methods: A total of 215 obese children and adolescents aged 6-18 years were included in this retrospective cross-sectional study. The patients' anthropometric measurements such as thyrotropin (TSH), free thyroxine (fT4), fasting plasma glucose, and insulin levels, as well as homeostasis model assessment for insulin resistance (HOMA-IR) index, and lipid profiles were evaluated. The patients were allocated to the SH group (fT4 normal, TSH 5-10 mIU/L) (n=77) or the control group (fT4 normal, TSH<5 mIU/L) (n=138). The glucose and lipid metabolisms of the 2 groups were compared. Results: SH was identified in 77 of 215 patients (36%). Mean body mass index was similar in both groups. The mean serum insulin, HOMA-IR, and triglyceride (TG) levels were higher and the mean high-density lipoprotein cholesterol level was lower in the SH group than in the control group (P=0.007, P=0.004, P=0.01, and P=0.02, respectively). A positive correlation was observed between TSH level and insulin level, HOMA-IR, and TG level. Conclusion: SH was identified in some of the obese children and adolescents. A clear association was observed between SH, insulin resistance, and dyslipidemia in obese children.

소아 지방간 환자에서 체중조절에 의한 간기능 개선 효과 (Improvement of Liver Function by Weight Control in Children with Fatty Liver)

  • Obese children have increaed in Korean in recent years as socieconomic status have got more higher than past years. Obese children have many health problems and some kinds of complications. Obese patients who have fatty liver with abnormal liver function should be managed with weight control. This study was intended to investigate the effect of weight control on hepatic abnormalities of obese children with fatty liver. We managed 6 obese children who had fatty liver with abnormal liver finction for weight control containing behavior modification, diet and exercise during 3 months. After 3 months, we compared body weight, liver function test, dietary intake and exercise. Body mass index(BMI) showed significant decrease from 28.1$\pm$2.3kg/$m^2$ to 26.2$\pm$1.7kg/$m^2$ (p<0.01). Aspartate aminotransferase(AST) showed significant improvement from 56$\pm$15IU/l(p<0.01). Alanine aminotransferase(ALT) also showed significant improvement from 108$\pm$38IU/l to 33$\pm$9IU/l(p<0.01). Therefore if we can rule out the other causes of hepatic abnormalities in obese children with fatty liver, these patients showed benefit by weight control as it can improve both obesity and liver functions.
