완도지역 성인 및 노인의 영양소 섭취 실태조사 (A Study on Nutritional Intakes in Elderly People in Wando Area)

  • The purpose of this study was to investigate nutrients intakes and nutritional quality of Adults and Elderly People in an island area (Wando). A three-day dietary intakes survey, using a 24 hour recall method was obtained from 187 subjects aged 46 to 84 (mean age 65.3) living in an island area (Wando). Nutient intakes were analyzed using CAN-Pro soft program and compared to Korean RDA. The quality of nutrients was assessed by analyzing nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ). The average daily mean energy intakes were 1869.0 kcal for males and 1943.9 kcal for females, respectively. Daily intakes of protein for males and females were 28.0 and 30.4 g and those of fat were 31.5 and 28.51 g, respectively. Nutrient consumed below $75\%$ of Korean RDA was protein, vitamin A, Ca and Zn in both males and females. Average CPF ratio of males and females were 78.8 : 6.0 : 15.1 and 80.0 : 6.4 13.4, respectively. Energy intake ratio from protein was significantly higher in over 60 years males. Carbohydrate dependency decreased with age. Protein dependency increased with age. Nutrient adequacy ratio (NAR) of energy, protein vitamin A and vitamin E were increased with age in males. The mean adequacy ratio (MAR), an index of overall dietary quality, were not significantly different by age group. Average MAR for males and females was 0.77 and 0.78, respectively. The index of nutritional quality (INQ) showed the tendency to decrease with age. Especially, there were significantly decreases in INQ of all nutrients, except protein, with age. Based on these results, it is evident that people in the island area did not consume enough nutrients. Specially, dietary intake of protein, vitamin A, vitamin E, and Ca were not adequate. 일부 사춘기 여중생의 혈청 Leptin 함량과 영양소섭취상태 및 혈당, 혈청지질과의 상관관계 연구 (The Study of Relationship among Serum Leptin, Nutritional Status, Serum Glucose and Lipids of Middle-school Girls)

  • The purpose of this study was to investigate the relationship among urn leptin, nutritional status, and urn lipids, glucose of 99 middle school girls residing in Kyluggi-do, Korea. Subjects were assigned to one of three groups such as underweight(BMI < 20, n=38), normal(20$\leq$BMI<25, n=36) and over weight(BMI$\geq$25, n=25) groups. The nutritional status of the subjects was evaluated by anthropometric measurements, questionnaires, 24-hr dietary recalls and blood anal),sis. The results were as follows : The mean age, body weight, height and BMI were 13.93 years, 55.19kg, 158.40cm and 21.96kg/㎡ respectively. The mean menarche age of the subjects was 12.64 years and the mean daily energy intake was 94.22% off RDA for Koreans. The energy ratio from carbohydrate fat and protein was 63: 24: 13. Underweight group showed the lowest intakes of oils and fats among three groups(p<0.05). The mean serum levels of leptin, glucose, total cholesterol, HDL-cholesterol. LDL-cholesterol and triglyceride were 10.57ng/ml 78.80mg/dl, 12.29mg/dl, 48.07mg/dl, 93.50mg/dl and 78.61mg/d1, respectively. The mean atherogenic index was 2.37. There were significant increases in serum leptin, glucose, LDL-cholesterol, triglyceride and atherogenic index of subjects with increases in their BMIs. Serum leptin showed a significantly positive correlation with BMI(p<0.001), body fat percent(p<0.001). When BMI was adjused, serum leptin showed a significantly positive partial correlations with intakes of animal protein(p<0.05), milks and animal foods(p<0.05), Serum leptin showed a significantly positive correlation with serum glucose(p<0.05) total cholesterol(p<0.01), LDL-cholesterol(p<0.001), triglyceride(p<0.001) and atherogenic index(p<0.001), however, it had significantly negative correlations with HDL-cholesterol(p<0.05). Therefore, these results suggest that the increase of serum levels of leptin may elevate the risk of cardiovascular disease and lipids and some nutrient intakes may be related with serum leptin levels. 구강악안면영역의 치성감염으로 인한 근막간극 감염에 대한 회귀적 연구 (THE RETROSPECTIVE STUDY OF INTRAFASCIAL INFECTION FROM ODONTOGENIC INFECTION IN ORAL AND MAXILLOFACIAL REGION)

  • Infections in oral and maxillofacial region are relativley common and self-limiting, but in some cases, infections spread to adjacent hard and soft tissue and to cause any complication, even threaten life. So we made retrospective study of patients with interfascial infection who had been hospitalized and been treated by surgical treatment in Dankook university about 10 years. We reviewed the charts of patient with interfascial infection from 1995 to 2005. The result were as follows: 1. In gender & age distribution, male(54.2%) & fouth decade were most frequently. 2. The most common cause of infection was dental caries(55.2%) and the most of involving teeth was lower posterior teeth(44.1%). 3. Submandibular space is most frequently involving space and most infection involved mainly one space. 4. The patients with systemic disease were 38.2%. Diabetic mellitus was 87.2% of systemic diease. The admission period was 19.5 days in systemic disease. 5. The most microorganism in culture was Streptococcus Viridans(36.2%) in all patient. Klebsiella Pneumoniae was found most in Diabetic Mellitus. 6. The patient were mainly treated I&D on admission day. Of them 5(1.1%) patients were received tracheostomy. 7. Serum albumin, CRP and body weight are associated with Nutritional Risk Index(NRI). High risk patient group according to NRI classification showed higher rate of complications & mortality. 8. The patients with complication were 28(6.7%) persons. 4(0.9%) patients were expired. Nutritional Risk Index was helpful to predict the prognosis. When interfascial infection starts to spread, we must pay attention to airway management. Fluid therapy with nutritional may support to healing of wound.

Development of Small Farms in the Agro-Industrial Complex

  • Modern small farms are important link components in the structure of the world agro-industrial complex. It ensures the food and nutritional sustainability of the country exclusively at the local regional level. The purpose of the research is to examine the role of farming in ensuring nutritional security and food stability based on the analysis of the Food Sustainability Index (FSI). Research methods: modeling, abstraction, analogy, analysis, synthesis, formalization, logical abstraction, theoretical cognition, systematization and classification, abstract-logical, etc. Results. Having analyzed the Food Sustainability Index for 2018, it has been established that there is a lack of a clear relationship between the pace of economic development and the level of food and nutritional sustainability. In addition, this study has identified the countries with the largest number of small farms, as well as the number of farms within the region. The correlation between the size of the farm and the area of agricultural land that it cultivates has been determined. The problems faced by small farms in the process of their activity have been analyzed. The programs implemented in the field of agro-industrial complex development by international profile institutions have been systematized. Particularly, the regional structure of agricultural development programs under the guidance of IFAD is defined, as well as the areas to which they are directed. Specific measures taken by governments to stimulate the development of small farms have been outlined. Reasonable conclusions have been formed based on the study. The direction of future research is seen in the assessment of the export potential of small farms in terms of range, volume of export deliveries and geographical direction of movement of their products.

Selection and optimization of nutritional risk screening tools for esophageal cancer patients in China

  • BACKGROUND/OBJECTIVES: Malnutrition has multiple impacts on surgical success, postoperative complications, duration of hospital stay, and costs, particularly for cancer patients. There are various nutrition risk screening tools available for clinical use. Herein, we aim to determine the most appropriate nutritional risk screening system for esophageal cancer (EC) patients in China. SUBJECTS/METHODS: In total, 138 EC patients were enrolled in this study and evaluated by experienced nurses using three different nutritional screening tools, the Nutrition Risk Screening 2002 tool (NRS2002), the Patient-generated Subjective Globe Assessment (PG-SGA), and the Nutrition Risk Index (NRI).We compared sensitivity, specificity, positive and negative likelihood ratios, and Youden index generated by each of the three screening tools. Finally, cut-off points for all three tools were re-defined to optimize and validate the best nutritional risk screening tool for assessing EC patients. RESULTS: Our data suggested that all three screening tools were 100% sensitive for EC patients, while the specificities were 44.4%, 2.96%, and 59.26% for NRS 2002, PG-SGA, and NRI, respectively. NRI had a higher positive likelihood ratio as well as a higher area under the receiver operating characteristic curve compared to those of NRS 2002 and PG-SGA; although, all three tools had null negative likelihood ratios. After adjusting the cut-off points, the specificity and accuracy for all tools were significantly improved, however, the NRI remained the most appropriate nutritional risk screening system for EC patients. CONCLUSIONS: The NRI is the most suitable (highest sensitivity and accuracy) nutritional risk screening tool for EC patients. The performance of the NRI can be significantly improved if the cut-off point is modified according to the results obtained using MedCalc software.

원주지역 초등학교 아동의 영양지식과 식생활 태도에 관한 연구 -급식학교와 비급식학교 아동의 비교- (A Comparative Study on Nutritional Knowledges and Dietary Behaviors of Children in Elementary School by School Lunch Program in Won-Ju Province)

  • This study was to investigate the children's understanding level of nutritional knowledge and the degree of knowledge application into the actual living not only in the with-lunch school but also in the without lunch school. Having expected their obtaining of nutritional knowledge and practical applicating, we compared the with-lunch school children's understanding level of nutritional knowledge, dietary attitude, and completeness of diet life with those of without-lunch school children. In addition to this, we surveyed healthiness, Physical condition, and BMI (Body Mass Index) and compared these factors. The results are as follows. 1. Almost all of the children were standard in physical condition, however they recognized themselves fatty than normal. Mealtime consumption(p<0.05), BMI(p<0.05), and diet attitude points(p<0.001) showed significant difference in the children who regarded the themselves healthy. 2. High correlation was observed between parent's physical shape and mealtime consumption (p<0.05), quantity of eating food (p<0.001), children's BMI(p<0.001) and diet attitude(p<0.05) respectively. 3. The points of itemised nutritional knowledge test was different in accordance with the children's year grade (p<0.05), children's understanding level of health and physical condition(p<0.05), parent's physical shape (p<0.05), and mother's education level(p<0.05) respectively. The points of nutritional knowledge test in both schools showed no difference except the item of vitamin.

[재출판] 위암 수술 후 외래환자의 영양상태 평가: 5가지 영양검색도구의 비교연구 ([Republished study] Assessing Nutritional Status in Outpatients after Gastric Cancer Surgery: A Comparative Study of Five Nutritional Screening Tools)

  • Objectives: This study examined the characteristics of patients according to nutritional status assessed by five nutritional screening tools: Patient-Generated Subjective Global Assessment (PG-SGA), NUTRISCORE, Nutritional Risk Index (NRI), Prognostic Nutritional Index (PNI), and Controlling Nutritional Status (CONUT) and to compare the agreement, sensitivity, and specificity of these tools. Methods: A total of 952 gastric cancer patients who underwent gastrectomy and chemotherapy from January 2009 to December 2012 were included. The patients were categorized into malnutrition and normal status according to five nutritional screening tools one month after surgery. The Spearman partial correlation, Cohen's Kappa coefficient, the area under the curve (AUC), sensitivity, and specificity of each two screening tools were calculated. Results: Malnutrition was observed in 86.24% of patients based on the PG-SGA and 85.82% based on the NUTRISCORE. When NRI or CONUT were applied, the proportions of malnutrition were < 30%. Patients with malnutrition had lower intakes of energy and protein than normal patients when assessed using the PG-SGA, NUTRISCORE, or NRI. Lower levels of albumin, hemoglobin, total lymphocyte count, and total cholesterol and longer postoperative hospital stays were observed among patients with malnutrition compared to normal patients when NRI, PNI, or CONUT were applied. Relatively high agreement for NUTRISCORE relative to PG-SGA was found; the sensitivity was 90.86%, and the AUC was 0.78. When NRI, PNI, and CONUT were compared, the sensitivities were 23.72% for PNI relative to NRI, 44.53% for CONUT relative to NRI, and 90.91% for CONUT relative to PNI. The AUCs were 0.95 for NRI relative to PNI and 0.91 for CONUT relative to PNI. Conclusions: NUTRISCORE had a high sensitivity compared to PG-SGA, and CONUT had a high sensitivity compared to PNI. NRI had a high specificity compared to PNI. This relatively high sensitivity and specificity resulted in 77.00% agreement between PNI and CONUT and 77.94% agreement between NRI and PNI. Further cohort studies will be needed to determine if the nutritional status assessed by PG-SGA, NUTRISCORE, NRI, PNI, and CONUT predicts the gastric cancer prognosis.

체중관리 영양교육에 참여한 여대생의 Diet Quality Index-International (DQI-I)점수와 체중감소와의 상관성 (Diet Quality Index-International Score is Correlated with Weight Loss in Female College Students on a Weight Management Program)

  • This study was to evaluate the effectiveness of the weight control program for female college students. The program was composed of diet and behavioral modifications for 8 weeks. A total of 78 participants enrolled the weight control program. Upon completion of the program, 53 participants experienced weight loss and 25 did not. The intakes of carbohydrate and fat were significantly decreased in both groups. However, the total diet quality index-international (DQII) scores as well as individual scores such as variety scores for protein source and adequacy scores for vegetable, fiber, calcium and vitamin C and moderation scores for empty calorie food were increased significantly in weight loss group only. In the weight loss group, weight, BMI, body fat, percent body fat and waist-hip ratio were decreased significantly. In addition, compared to the weight gain group, the weight loss group had higher changes in weight (weight loss group: -2.6% vs weight gain group: 1.5%, p < 0.001), body fat (-6.0% vs 0.0%, p < 0.001), percent body fat (-3.1% vs -0.3%, p < 0.001), waist-hip ratio (-1.0% vs 0.5%, p < 0.001) and BMI (-2.6% vs 1.3%, p < 0.01). There was no difference in blood profiles between the two groups. The changes in DQI-I scores were significantly correlated with the changes in body weight (r = -0.239, p < 0.05) and BMI (r = -0.224, p < 0.05), indicating that effective nutrition education could help improve diet quality leading to successful weight management among female college students.

Glycemic index of dietary formula may not be predictive of postprandial endothelial inflammation: a double-blinded, randomized, crossover study in non-diabetic subjects

  • The emerging role of endothelial inflammation in diabetes has stimulated research interest in the effects of nutrition on related indices. In the current study we investigated whether the nutrient composition of dietary formula as reflected in glycemic index (GI) may be predictive of postprandial endothelial inflammation in non-diabetic subjects. A double-blinded, randomized, crossover study was conducted in non-diabetic subjects (n = 8/group). Each subject consumed three types of diabetes-specific dietary formulas (high-fiber formula [FF], high-monounsaturated fatty acid (MUFA) formula [MF] and control formula [CF]) standardized to 50 g of available carbohydrates with a 1-week interval between each. The mean glycemic index (GI) was calculated and 3-hour postprandial responses of insulin, soluble intercellular adhesion molecule-1 (sICAM-1), nitrotyrosine (NT) and free fatty acids (FFA) were measured. The MF showed the lowest mean GI and significantly low area under the curve (AUC) for insulin (P = 0.038), but significantly high AUCs for sICAM-1 (P<0.001) and FFA (P < 0.001) as compared to the CF and FF. The FF showed intermediate mean GI, but significantly low AUC for NT (P<0.001) as compared to the CF and MF. The mean GI was not positively correlated to any of the inflammatory markers evaluated, and in fact negatively correlated to changes in FFA (r = -0.473, P = 0.006). While the MF with the lowest GI showed the highest values in most of the inflammatory markers measured, the FF with intermediate GI had a modest beneficial effect on endothelial inflammation. These results suggest that nutrient composition of dietary formula as reflected in the GI may differently influence acute postprandial inflammation in non-diabetic subjects.

Effect of early oral nutrition supplement using Encover in patients undergoing hepato-biliary-pancreatic surgery

  • Backgrounds/Aims: Early recovery after surgery has become a popular trend. The aim of this study was to evaluate effect of nutritional intervention using Encover, an oral nutritional supplement, in patients undergoing hepato-biliary-pancreatic surgery. Methods: This single center, prospective case-control study was conducted in Gangnam Severance Hospital from September 2018 to April 2019. Through randomization, patients were divided into an experimental group (30 patients) and a control group (30 patients). At postoperative seven days, the experimental group was instructed to take two packs of Encover (JW Pharmaceutical, Seoul, Korea) daily for seven days. Body cell mass index was measured at seven days after surgery and 14 days after discharge and Patient-Generated Subjective Global Assessment (PG-SGA) was performed at 14 days after discharge. Results: Body cell mass index during outpatient follow-up was significantly decreased compared to that at discharge in both groups. However, the amount of body cell mass index showed no significant difference between postoperative seven days and outpatient follow-up in either group. During outpatient follow-up, the experimental group had a higher mean value of PG-SGA score than the control group (11.32 ± 3.46 vs. 9.48 ± 3.97; p = 0.037). Conclusions: Short-term Encover doses after surgery may not produce significant results in weight gain or other body cell mass index. Encover did not significantly affect other dietary conditions based on PG-SGA.
