Long-Term Efficacy of Anti-Tumor Necrosis Factor Agents in Pediatric Luminal Crohn's Disease: A Systematic Review of Real-World Evidence Studies

  • Purpose: To determine the long-term efficacy of the anti-tumor necrosis factor (TNF) agents, infliximab (IFX) and adalimumab (ADA), in pediatric luminal Crohn's disease (CD) by performing a systematic literature review. Methods: An electronic search was performed in Medline, Embase, and the Cochrane Library from inception to September 26, 2019. Eligible studies were cohort studies with observation periods that exceeded 1 year. Studies that reported time-to-event analyses were included. Events were defined as discontinuation of anti-TNF therapy for secondary loss of response. We extracted the probabilities of continuing anti-TNF therapy 1, 2, and 3 years after initiation. Results: In total, 2,464 papers were screened, 94 were selected for full text review, and 13 studies (11 on IFX, 2 on ADA) met our eligibility criteria for inclusion. After 1 year, 83-97% of patients were still receiving IFX therapy. After 2 and 3 years the probability of continuing IFX therapy decreased to 67-91% and 61-85%, respectively. In total, 5 of the 11 studies subgrouped by concomitant medication consistently showed that the probabilities of continuing IFX therapy in patients with prolonged immunomodulator use were higher than those in patients on IFX monotherapy. Conclusion: This review of real-world evidence studies confirms the long-term therapeutic benefit of IFX therapy in diverse cohorts of children with luminal CD. Moreover, it supports the view that combination therapy with an immunomodulator prolongs the durability of IFX therapy in patients who previously failed to recover following first-line therapy. The limited number of time-to-event studies in patients on ADA prevented us from drawing definite conclusions about its long-term efficacy.

Colonic Angioectasia in an Adolescent Boy with Hoyeraal-Hreidarsson on Long-Term Anabolic Steroid Therapy

  • Androgen therapy has proven efficacy in treating patients with bone marrow failure who are not candidates for bone marrow transplantation. Herein, we report on a case of colonic angioectasia secondary to oxymetholone use in an adolescent patient with Hoyeraal-Hreidarsson syndrome (HHS). A 13-year-old Caucasian male with HHS characterized by cerebellar hypoplasia, developmental delay, microcephaly, esophageal strictures and myelodysplasia presented with severe hematochezia from colonic angioectasia secondary to long-term oxymetholone therapy. These vascular lesions resolved spontaneously once this anabolic steroid was discontinued. While androgen therapy is often recommended for certain anemias and myelodysplastic syndromes, clinicians should be aware of the potential complication in developing these perceived uncommon colonic angioectasias. Moreover, pediatric gastroenterologists should familiarize themselves in identifying these vascular lesions by colonoscopy, especially among the high risk groups on long-term anabolic steroid therapy.

식이요법에 의한 간암환자의 일부증상의 호전 (Clinical Case Studies on Hepatoma Patients with Dietary Therapy)

  • This study was performed to evaluate the effects of high supplementary diet of vitamins, minerals, and polysaccharides on enhancing immunity and preventing from cancer in patients with hepatocellular carcinoma(hepatoma). Dietary therapy for patients was performed with dried yeast, wheat germ. Gonoderma extract, and vegetable juice. The vegetable juice was made of fresh green edible vegetables. Ten patients of total 30 patients were considered as improved state in tumor sizes, AFP, or symptoms. Most of patients complained of weight loss, fatigue, indigestion, anorexia before dietary therapy, but most of symptoms disappeared after dietary therapy. There was no significant side effects because of dietary therapy except for occasional slight diarrhea.

혈액투석환자의 식생활 실천에 관한 질적 연구 (Qualitative Study on Dietary Practices of Hemodialysis Patients)

  • This study was a qualitative investigation into hemodialysis patients' dietary practices. The purpose of this study was to explore the obstacles and requirements to maintain a recommended diet therapy in hemodialysis patients. Five patients undergoing hemodialysis in the renal chamber of the general hospital were interviewed individually. The interviews were based on an interview guide and analyzed by Giorgi's method of analysis. As a result of this study, five elemental factors and 12 subelemental factors were derived. Derived elements were "difficulty in dietary guidelines", "recognizing necessity of diet therapy", "awareness of importance of diet", "difficulty practicing diet therapy", and "looking for ways to practice diet therapy". Patients not only felt difficulties in practicing dietary guidelines but also recognized the need and importance of diet therapy. Patients seemed to have difficulty practicing meal therapy and eating with their families or others. They were also stressed by the limited selection of dietary components and rapid dietary changes before and after dialysis. However, patients showed a willingness to implement dietary management to improve their quality of life and to practice dietary therapy. In order to improve the practice of dietary management in hemodialysis patients, nutritional education should be focused on long-term dietary habits through continuous education and monitoring, not just one-off education. Moreover, patients should be educated that adherence to dietary control may be less burdensome on their families.

Oral Vancomycin Therapy in a Child with Primary Sclerosing Cholangitis and Severe Ulcerative Colitis

  • Primary sclerosing cholangitis (PSC), a rare progressive liver disease characterized by cholestasis and bile duct fibrosis, has no accepted, effective therapy known to delay or arrest its progression. We report a 15 year old female patient diagnosed with PSC and moderate chronic active ulcerative colitis (UC) who achieved normalization of her liver enzymes and bile ducts, and resolution of her UC symptoms with colonic mucosal healing, after treatment with a single drug therapy of the antibiotic oral vancomycin. We postulate that the oral vancomycin may be acting both as an antibiotic by altering the intestinal microbiome and as an immunomodulator. Oral vancomycin may be a promising treatment for PSC that needs to be further studied in randomized trials.

  • Objective: A case report of pain relief and fracture recovery by ortho-cellular nutrition therapy in a patient with 2nd, 3rd, and 4th metatarsal fractures (closed). Methods: A Korean woman in her 50s was diagnosed with 2nd, 3rd, and 4th metatarsal fractures (closed) with severe initial pain and fractures. Surgery was recommended but could not be performed immediately due to her situations at work. Results: With OCNT performed immediately after the fracture, the pain completely disappeared, and even during subsequent recovery, recovery was faster than in other fracture cases. Conclusion: OCNT may help patients with similar problems relieve symptoms and recover.

  • Objective: A case report on the improvement of symptoms in an osteoporosis patient using ortho-cellular nutrition therapy Methods: Currently a 70 years old Korean woman was diagnosed with osteoporosis in her late 50s around 2010 and continued to receive injection treatments once every three months. Her T-score did not improve, and fractures continued to occur in the ankles, wrists, shoulders, ribs, etc., even from light impacts. Results: The patient underwent an osteoporosis test in February 2019, which resulted in an average T-score of -2.5 or less, equivalent to being diagnosed with osteoporosis. OCNT has been performed since May 2019 (late 60s), and in the test received in April 2020, the T-score value improved to over -2.5 and was determined normal at the hospital. Since then, OCNT continued without any further injection treatments. Unlike before, even when she fell hard, she only suffered bruises without fractures, and no additional fractures have occurred. Conclusion: OCNT may help patients with similar problems relieve symptoms and recover.
