한국 중년 남성의 연도별 체질량지수, 신체계측치, 식품 및 영양소 섭취량 변화 : 국민건강영양조사 결과(2001~2011) (Body Mass Index, Anthropometric Factors, and Food and Nutrient Intakes of Korean Middle-Aged Males by Year : Based on 2001~2011 Korea National Health and Nutrition Examination Survey (KNHANES))

  • This study investigated anthropometric and biochemical characteristics, food and nutrient intakes, and changes in body mass index (BMI) of Korean middle age men (40~49 yrs). Data on BMI, anthropometric (height, weight, waist circumference, body mass index, blood pressure), and biochemical (total cholesterol, HDL-cholesterol, LDL-cholesterol, triglyceride, hemoglobin, GOT, GPT) characteristics, food and nutrients intakes, nutrient density, and index of nutrient quality were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES, 2001, 2005, 2011). Subjects were divided into three groups by year. Weight, BMI, and hemoglobin increased significantly by year. All anthropometric factors except height showed a positive relation with BMI by year. Consumption of barley, chinese noodle, confections, soybean milk, egg, ice cream, pizza, fried foods, banana, coffee, and raw rice wine increased significantly by year. Intake ratios of nutrients except water and fiber were over 100% of KDRI generally. INQ according to intakes of protein, calcium, phosphorus, iron, vitamin A, thiamine, riboflavin, and niacin per 1,000 kcal increased significantly by year. Distribution of BMI status of subjects showed a significant difference by exercise. The findings of the present study show that BMI of subjects increased by year, and intakes of all nutrients except calcium and riboflavin showed aspects of overnutrition by year. Intakes of fruits decreased while intakes of raw rice wine and coffee increased by year. Therefore, practical and systematic nutrition programs are required to maintain optimum BMI and appropriate intakes of nutrients to maintain a desirable lifestyle and improve dietary life.

Factors Associated with the Use of Gastric Cancer Screening Services in Korea: The Fourth Korea National Health and Nutrition Examination Survey 2008 (KNHANES IV)

  • Objective: Despite government efforts to increase participation in gastric cancer screening, the rate is still suboptimal in Korea. Therefore, we explored barriers to and predictors of gastric cancer screening participation among a nationally representative sample. Methods: We used the Health Interview Survey sub-dataset derived from the Fourth Korean National Health and Nutrition Examination Survey 2008 (KNHANES IV) to evaluate participation in gastric cancer screening and factors associated with attendance in individuals age ${\geq}40$ years. We enrolled 4,464 subjects who completed the questionnaire and were not previously diagnosed with gastric cancer. Four groups of factors were considered potential predictors of gastric cancer screening in a multivariate analysis: sociodemographic, health behavior, psychological and cognitive, and dietary factors. Results: Overall, 41.3% complied with the gastric cancer screening recommendations. Younger age, lower education level, living without a spouse, frequent binge drinker, and current smoker were significantly associated with less participation in gastric cancer screening. Conclusions: To improve participation in gastric cancer screening, more focused interventions should be directed to vulnerable populations, such as groups with low socioeconomic status or unhealthy behavior. In addition, there should be new promotional campaigns and health education to provide information targeting these vulnerable populations.

한국 노인의 건강요인 및 구강건강요인과 우울증 유병여부의 관련성 (Relationship between health factor, oral health factor and prevalence of depression in Korean elderly)

  • Objectives: The purpose of the study is to investigate the relationship between health factor, oral health factor and revalence of depression in Korea elderly by data from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Methods: The subjects were 10,938 elderly over 65 years old by rolling survey method and data were extracted from the 5th Korea National Health and Nutrition Examination Survey(KNHANES) 2012. Data of 1,421 elderly were finally selected and analyzed using kstrada. The questionnaire consisted of socio-demographical characteristics of the subjects, health factors, oral health factors and depression. The oral health factors included subjective perception of oral health, alcohol consumption, exercise, smoking, and toothache. Results: Health factors influencing on the prevalence of depression were subjective health(p<0.001), stress(p<0.001), and alcohol consumption(p<0.05). Oral health factors related to the prevalence of depression were subjective oral health(p<0.05), annual dental checkup(p<0.01), and speaking problem(p<0.01). Conclusions: It is necessary to develop the oral health promotion program for the elderly and help them maintain good quality of life and mental health.

The Effect of Chewing Difficulty on Osteoarthritis in Korean Adults: The 8th Korean National Health and Nutrition Examination Survey (KNHANESVIII-1, 2), (2019~2020)

  • The purpose of this study was to analyze the effect of chewing difficulty on osteoarthritis in Korean adults. Data from the National Health and Nutrition Examination Survey for two years from 2019 to 2020. The collected data were analyzed using the SPSS (21.0) program using composite samples, and the mean crossover analysis and logistic return analysis were performed. An analysis of the effects of chewing difficulty on osteoarthritis in korean adults showed that those with chewing difficulty had a 2.02 fold higher risk of osteoarthritis (95%CI=1.56~2.42) and a statistically significant difference (P<0.05). The association between the chewing difficulty on osteoarthritis could be confirmed, and measures improve of the oral health and preventing osteoarthritis should be prepared.

  • The purpose of this study was to test whether moderate alcohol consumption has any positive effect on lowering blood pressure among Koreans. Study subjects were Korean adults 20 years or older (n=5,234) who participated in the 2001 Korean National Health and Nutrition Examination Survey (KNHANES). Analysis of variance and analysis of covariance were used to construct univariate and multivariate models relating alcohol consumption to blood pressure for the analysis. After adjustment for possible covariates, drinkers (regardless of drinking level) had significantly higher systolic and diastolic blood pressure, compared with never-drinker for male subjects. Diastolic and systolic blood pressures were also significantly elevated with the drinking frequency and amount of alcohol intake among male subjects. For the female subjects, only diastolic blood pressure was significantly associated with the alcohol consumption at multivariate model. however, low level alcohol consumption did not show any sign of lowing effects on blood pressure. The result implies that moderate alcohol consumption did not have any positive effect on lowering blood pressure among Koreans for either sex.

제6기 국민건강영양조사를 토대로 분석한 한국 성인의 당뇨병과 빈혈 관계 (Relationship between Diabetes Mellitus and Anemia in Korean Adults-Based on the Korean National Health and Nutritional Examination Survey VI)

  • Anemia, defined as a reduction in the hemoglobin concentration of blood, is common in diabetes mellitus (DM) patients, can be potentially caused by diabetes complications such as nephropathy. Recent research suggests that diabetes mellitus (DM) itself may be a major risk factor of anemia. However, there are few Korean studies on the relationship between diabetes mellitus (DM) and anemia. This study was performed to investigate the association between anemia and diabetes mellitus (DM) in Korean adults. A total of 10,151 Korean adults over aged 19 years (4,422 male, 5,729 female) were selected from the participants of the Korean National Health and Nutrition Examination Survey VI (KNHANES VI). Korean adults with anemia had a higher prevalence of diabetes mellitus (DM) than the normal adults (11.4% vs. 30.4% in male, 8.8% vs. 9.4% in female). The unadjusted odds ratio (OR) for anemia was greater in adults with DM than in normal male (OR=3.28; 95% CI: 2.27~4.73). After adjusting for other risk factors including age, education, family income, smoking, drinking, and menstrual status, anemia and diabetes were not associated (OR=1.33; 95% CI: 0.84~2.09). Similarly, there was no association between anemia and diabetes in female. In conclusion, this study shows that the prevalence of anemia is similar in diabetic and non-diabetic Korean adults after adjusting for multiple risk factors. Further research is required to elucidate the mechanism of anemia caused as a consequence of diabetes mellitus (DM).

한국 유아의 아연 섭취 수준 평가를 위한 데이터베이스 구축 (Development of a Zinc Database to Estimate the Zinc Intake Levels in the Korean Toddlers and Preschool Children)

  • Objectives: The objective of this study was to develop a zinc database (DB) to estimate the intake levels of zinc in Korean toddlers and preschool children using the data from the Korea National Health and Nutrition Examination Survey (KNHANES). Methods: A total of 3,361 food items for the DB representing the usual diet of Korean toddlers and preschool children were selected based on KNHANES (2009~2013) and the food composition table of Rural Development Administration (RDA). The existing values of zinc in foods were collected from the latest food composition tables of RDA (9th revision) and the US Department of Agriculture (legacy release). The zinc contents were filled preferentially with these collected values. The missing values were replaced with the calculated values or imputed values using the existing values of similar food items from the data source. The zinc intake levels of Korean toddlers and preschool children were estimated using KNHANES and zinc DB Results: A total of 1,188 existing values, 412 calculated values, and 1,727 imputed values were included in the zinc DB. The mean intake levels of zinc for 1-2-year-old children and 3-5-year-olds were 5.17 ± 2.94 mg/day and 6.30 ± 2.84 mg/day, respectively. There was no significant difference in the zinc intake levels between boys and girls in each group. Conclusions: This newly developed zinc DB would be helpful to assess the zinc nutritional status and investigate the association between the zinc intakes and related health concerns in Korean toddlers and preschool children.

Usual intake of dietary isoflavone and its major food sources in Koreans: Korea National Health and Nutrition Examination Survey 2016-2018 data

  • BACKGROUND/OBJECTIVES: Accumulating evidence has shown the beneficial effects of isoflavone on health. There is limited information on the usual isoflavone intake for Koreans. This study examined the usual intake of total isoflavone and its major food sources in Koreans according to age and gender. SUBJECTS/METHODS: The dietary intake data of 21,271 participants aged 1 yrs and older from the Korea National Health and Nutrition Examination Survey (KNHANES) VII 2016-2018 were analyzed. The average isoflavone intake was estimated based on the 24-h dietary recall data in KNHANES and the isoflavone database from the Korea Rural Development Administration (RDA) and literatures. The usual isoflavone intake was estimated by applying the ratio of within- and between-participant variance estimated from the 2009 KNHANES data to the 7th KNHANES (2016-2018) data. The variance of the isoflavone intake was calculated using MIXTRAN macro with intake data for two days in the 2009 KNHANES. Complex sample analysis with stratified variables and integrated weights was conducted. RESULTS: The mean total isoflavone intake in the Korean population aged 1 yrs and older (n = 21,271) was 139.27 mg/d, which was higher than the usual intake of 47.44mg/d. Legumes were a major contributing food group (91%), with arrowroot being a major individual contributor to the isoflavone intake (67.2%), followed by 21.3% of soybean, 5.4% of bean sprouts, and 2.1% of tofu. The usual isoflavone intake was highest in the participants aged 50 to 64 yrs old and increased with age until 50 to 64 yrs and then decreased with further increases in age. The usual isoflavone intake of participants aged 65 yrs and older was higher for men than for women, showing gender differences. CONCLUSIONS: The usual dietary intake of isoflavone varied according to age and gender in the Korean population. This study showed that the usual isoflavone intake was lower than the average isoflavone intake. The difference between percentiles of the usual isoflavone intake was similarly smaller than the average intake. An estimation of average intake can be hindered by the occasional consumption of foods high in isoflavones, suggesting that the usual intake estimation method can be more appropriate. Further research will be needed to establish isoflavone dietary guidelines regarding the effects of isoflavone intake on health outcomes.

흡연 노인 남성의 흡연기간에 따른 신체적, 생화학적 특성, 영양소섭취 및 골밀도 차이 : 2008~2011년 국민건강영양조사 자료 분석 (Anthropometric, Biochemical Characteristics, Nutrient Intakes and Bone Density by Smoking Period in Elderly Male Smokers: Analysis of Data from Korea National Health and Nutrition Examination Survey (KNHANES), 2008~2011)

  • This study investigated anthropometric and biochemical characteristics, nutrient intakes, and bone density of Korean elderly men (over 65 yrs). Data on bone density and anthropometric (height, weight, waist circumference, body mass index and blood pressure) and biochemical (total cholesterol, high-density lipoprotein (HDL)-cholesterol, low-density lipoprotein (LDL)-cholesterol, triglyceride and hemoglobin) characteristics, nutrient intakes, and nutrient density were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES, 2008~2011). Subjects were categorized into smoking and non-smoking groups, and smoking groups were divided into three groups by smoking period (under 20 yrs, 21~40 yrs and over 41 yrs). Serum triglyceride concentrations of the smoking group aged 21~40 yrs were higher than those of other groups (P<0.01), whereas other biochemical factors were not different. Intake ratios of energy, protein, phosphorus and sodium in subjects were over 100% of Dietary Reference Intake for Koreans (KDRI). Nutrient densities according to intakes of thiamin, riboflavin, and niacin per 1,000 kcal were significantly different among the groups (P<0.05). Bone density of subjects decreased according to smoking period (T-score of total femur in non-smoking group -0.3108, and -0.2918, -0.4941, -0.6847 in smoking group, respectively). Ratio of osteoporosis was 38.1% in the non-smoking group and 44.4%, 51.1%, and 64.0% in the smoking group, respectively. The findings of the present study show that smoking may be associated with bone health, higher ratio of osteoporosis, and low nutrient density in elderly men. Therefore, practical and systematic non-smoking programs are required to improve the bone density of elderly men as well as maintain healthy bone levels and desirable lifestyle.

서울지역 20~30대 성인여성의 직업유무에 따른 골밀도와 영향요인: 제5기 국민건강영양조사자료 분석, 2010~2011 (Various Factors Affecting the Bone Mineral Density in Korean Young Adult Women: Data from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2011)

  • This study investigated factors affecting the bone mineral density (BMD) of Korean young adult women (20~30 yrs) living in Seoul, according to the state of employment. Data on BMD and anthropometric and biochemical measurements were obtained from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V) (2010~2011). BMD of the working group (WG) was generally higher than that of the non-working group (NWG). The prevalence of osteopenia, and osteoporosis between the groups was 29.0/31.5%, and 12.9/18.5% from 20~29 yrs as well as 25.5/24.1%, and 20.2/20.5% from 30~39 yrs, respectively. From 20~29 yrs, prevalence of osteopenia and osteoporosis in the WG was lower than that of the NWG. Alkaline phosphatase activity of the WG was lower than that of the NWG. In the WG, from 20~29 yrs, total femur BMD showed significant positive correlations with waist circumstance and BMI. In the NWG, from 20~29 yrs, total femur, femoral neck, and lumbar spine BMDs showed significant positive correlations with weight and fat mass, whereas they showed significant negative correlations with vitamin D. In the WG, from 30~39 yrs, total femur and femoral neck BMDs showed significant positive correlations with weight and free fat mass. The results of this study show that BMD of young adult women is insufficient regardless of their employment status. Therefore, practical and systematic programs are required to improve the BMD of young adult women as well as to maintain healthy bone levels.
