Patients' Experiences of Sensations After Breast Cancer Surgery in Korean Women (유방암 수술 환자의 감각 변화에 대한 연구)

  • Purpose: The purpose of this study was to obtain information about the experiences of sensations after breast cancer surgery characterized by prevalence, frequency, and severity, distresses, and disturbances in ADL using the BSAS (Breast Sensation Assessment Scale) and to develop the standardized Korean BSAS. Methods: Thirty two women from 3 to 100 days after breast cancer surgery (BCS) completed Roberta's BSAS. The 18 sensations of BSAS were translated using the references of Lee's Korean Pain Rating Scale and English-English, English-Korean dictionaries and the consultation from two native Americans, one bilingual permanent resident of the States, and one nursing professor. The Korean-translated BSAS has high reliability in test-retest. Likert type 4-point scale and 100 mm VAS were used for assessment. Results: Certain sensations remained prevalent (tender, pull, pain), frequent (numb, nag, throb), severe (throb, shoot, numb), causing distress (throb, penetrate, shoot), and influencing on ADL (throb, penetrate, nag). The most frequently experienced sensation other than BSAS was itching. There was little difference in the prevalence of symptom experiences between sentinel lymph node biopsy and axillary lymph node dissection. Conclusion: The women after BCS are suffering from neuropathic sensations. The Korean- translated BSAS could be used in effectively assessing breast sensations after BCS in Korean women.

Factors associated with the organizational citizenship behavior of nurses in small- and medium-sized hospitals (중소병원 간호사의 조직시민행동 관련요인)

  • Purpose: This study aimed to determine the levels of burnout, self-efficacy, intention to stay, and organizational citizenship behavior (OCB) among nurses in small- and medium-sized hospitals, and factors related to nurses' OCB. Methods: A targeted sampling method was adopted, whereby 185 clinical nurses employed at 1 of 4 hospitals were enrolled in the study. OCB was measured with a tool developed by Podsaoff et al. Associated factors, including nurses' general characteristics, burnout, self-efficacy, and intention to stay, were measured using a structured survey. Data were analyzed by frequencies, t-test, ANOVA, Duncan's multiple test, Pearson Correlation Coefficient and multiple stepwise regression. Results: Levels of OCB in nurses was on average 3.52 on a scale of 1 to 5, exhibiting a positive correlation with self-efficacy and intention to stay, and a negative correlation with burnout. The final related factors were self-efficacy, burnout and intention to stay with a predictive and explanatory power of 52% for OCB. Conclusion: To increase the level of OCB of nurses, an environment in which such behavior is encouraged needs to be established by introducing measures related to administrative support and task efficiency that reduces mental burnout, while increasing self-efficacy and intention to stay.

Patient한s Experiences about their Nurses한 Healing Relations (입원환자의 간호사에 대한 치유적 관계형성 경험에 관한연구)

  • The purpose of this study was to understand how patients experience their nurses, healing relation-ships. The 33 participants were patients who had been admitted to a general nursing unit of a university hospital in Seoul for more than one week, understood the study purpose and agreed take part, were fully conscious, able to communicate and over 20 years of age , their health was improving and they were ready for discharge. The data for this descriptive study using ground theory approach were collected by direct interviews with patients using five main questions derived from the literature and clinical experiences about their experiences of their nurses, healing relationships during hospitalization. Van Kaam’s method was used to analyse the data audio-recorded. Interpretation was enhanced consultations with two supervisors and one head nurse with a master’s degrees in nursing science, one nursing professor and one nursing docotoral candidate. Twenty - four patients said that they had formed a healing relationship with a nurse and nine said their relationship was not healing relationship. Six categories emerging from the characterizations of the healing relationships were valuable gratifying comforting trusting close and sympathetic. Descriptions of non - healing relationships were classified as none and poor. For this group of hospitalized patients nurses’ healing relationships were defined as valuable gratifying comforting trusting close as sym-Pathetic. Since the data suggest a change in the experiences during hospitalization, a gronded theory approach is recommended for further study.

  • Development and Evaluation of the Effectiveness of a Web-Based Learning System Program for Nursing Education on Peritoneal Dialysis Ji-Soo Yoo(Professor, College of Nursing, Yonsei University, Seoul, Korea) Seong-Mi Moon(Doctoral Candidate, College of Nursing, Yonsei University, Seoul, Korea) The purpose of this study was to develop a nursing education program using a web-based learning system to teach peritoneal dialysis and to evaluate the effectiveness of the program. The program was developed in two steps. The first step was to develop the content using CVI, and the second was to program it using HTML. The program which was developed was evaluated with 50 nurses of whom 48 responded to the questionnaire. The results are as follows: Of the subjects, 85.4% responded that they were satisfied with this web-based learning system and 87.5% stated that they would recommend this system to nurses who are in other nursing education programs. Of the educational material charac- teristics, the item that the subjects evaluated as most valuable was "accuracy of the content". For system related charact- eristics, the item to which the subjects gave the most positive response was "clarity of the system tool marking" (81.3%). In conclusion, it was verified that a web-based learning system is an effective and satisfactory method for nursing education.

A study on the actual condition of practical apprenticeship training for university(college) nursing education and developing reform measure in Korea (우리나라 간호교육기관의 현장실습교육 실태 파악 및 개선방향 모색에 관한 연구)

  • The practical apprenticeship training is important and takes part in one fourth of the curriculum in university(college) nursing education. The quality and quantity of practical training influence the quality of nursing student ability. This study have planed to find out actual condition of practical apprenticeship training for university(college) nursing education and develop the reform measure in Korea. The questionnaires were sent for directors of nursing department in university(college) and the directors of nursing in hospitals which were chosen by systemic sampling and have over 500 beds. The response rate was 48.4% in total. The results were as follow : 1. There were 2 kinds of training system ;subject training and comprehensive training. The comprehensive training was done frequently in college education. There were gaps of subject between the lecture and the practical training education for professor, part time lecturler and assitant. 2. There was the shortage of training fields especially in college education. 3. The training content by university(college) personnel were care conference, orientation, ward rounding. The content by hospital nursing pernnels were bedside nursing skills and nursing process application. The headnurse acted the important role in training education. For improving the actual condition of training apprenticeship in university(college) nursing education, it is necessary that the Ministry of Education should make the accreditation system on the preparation of training hospitals for university(college) nursing education. The Academy Society for Nursing Education can do the central and coordination role to improve the training apprenticeship for nursing education in Korea.

  • Purpose: The purpose of this study was to analyze research in the Korean Journal of Rehabilitation Nursing and to suggest further perspective for rehabilitation nursing research. Method: In this study we reviewed the contents of 134 articles published in this journal from its beginning year 1998 to 2004. Results: The majority of subjects were adult patients. The type of disease were Cerebrovascular Accident, Neuromuscular disease, Cancer patients, Urogenital disease such as Chronic Renal Failure, Spinal cord injury. Domain of research almost was client, type of research were survey(34.3%), quasi experimental study(20.9%), correlation study(19.4%). Nursing intervention of experimental study was exercise(39.0%), education(17.1%). Place of data collection was hospital (58.2%), position of researcher was professor(67.2%), method of data collection was questionnaire(50.7%). Conclusion: There has been a lack of written consent, child rehabilitation, measurement tool development, qualitative research. More studies about rehabilitation nursing are needed to extend the role and fields for professional nursing.

Updates of Evidence-Based Nursing Practice Guidelines for Central Venous Infusion Therapy (근거기반 중심정맥 주입요법 간호실무지침 개정)

  • Purpose: This study was conducted to update nursing practice guidelines for intravenous infusion published in 2017. Methods: The guideline update process was carried out using 22 steps developed by NICE and SIGN. It was agreed to update domains related to central venous infusion therapy. Contents related to peripheral infusion would be updated later. Results: Updated guidelines for central venous infusion therapy consisted of 6 domains and 195 recommendations. The number of recommendations by domain was 11 for general instruction, 14 for central vascular access devices (CVAD) and add-on devices, 13 for nursing management before insertion of CVAD, 30 for management during insertion of CVAD, 51 for management after insertion of CVAD, and 76 for complications. A grade was 29 (14.9%), B grade was 87 (44.6%), and C grade was 79 (40.5%) in the strength of recommendations. A total of 37 (19.0%) recommendations were newly developed and 23 (12.3%) previous recommendations have been modified. The newly developed recommendations were mainly related to the infection control methods. Conclusion: The updated guideline is focused on safe maintenance of central venous infusion therapy. Through this guideline, it is hoped to minimize the occurrence of complications and improve the standardization and efficiency of nursing practice.

A Study on the Relationship between Perceived Family Support and Satisfaction of Clinical Practice by Nursing Students (간호학생이 지각한 가족지지와 임상실습만족도와의 관계)

  • Purpose: This study was to identify the relationship between the degree of family support and the degree of satisfaction in clinical practice by nursing students. Method: This study was designed to measure the level of satisfaction according to the content, guidance, circumstance, hours, and the evaluation of clinical practice. Four hundred sixty-five nursing students from 4 nursing colleges, and 1 baccalaureate program in G area were randomly sampled. An instrument consisting of 64 questions, developed by the researcher and a nursing professor, was used to gather data from March 10 through 31, 2008. The data was analyzed by descriptive statistics, t-test, ANOVA, and Pearson Correlation, using the SPSS 14.0 program. Result: The results were summarized as follows : Family support of nursing students showed a significant difference by religion (t=2.104, p=0.036) and satisfaction of major (F=8.010, p<0.001). The satisfaction degree of clinical practice by nursing students showed a significant difference by university or college (t=2.636, p=0.012), residence type (t=2.098, p=0.036), and satisfaction of major (F=5.779, p=0.003). The relationship between generally perceived family support correlated with the degree of satisfaction in clinical practice of nursing students (r=.199, p<0.001). Conclusion: It was found that a higher degree of satisfaction in clinical practice depends on a higher degree of family support. Therefore, nursing faculty should plan interventions to inspire satisfaction level of clinical practice by family support.

The Relationship Between Satisfaction of a Clinical Training Program for Nursing Faculty and Teaching Efficacy (교수임상연수 프로그램의 만족도와 교수효능감과의 관계)

  • Purpose: The purpose of this study was to assess the degree of satisfaction of the clinical training program and teaching efficacy and their relationships among nursing faculty who joined clinical training program. Methods: In this cross-sectional descriptive study, seventy three nursing faculty participated in the clinical training program sponsored by Korean Academy Society of Nursing Education from July 2010 to February 2012. Collected data were analyzed by t-test, $x^2$, and Pearson's correlation coefficient through the SPSS WIN12.0 program. Results: The mean score of satisfaction of the clinical training program was $3.88{\pm}0.56$, and $4.24{\pm}0.49$ for teaching efficacy. Teaching efficacy showed associations with Major area (F=2.78, p=.014) and Goal of participation (F=3.39, p=.039). The results presented that satisfaction of the clinical training program positively correlated with teaching efficacy (r=.56, p<.001). Conclusion: This study reports high satisfaction of a clinical training program and teaching efficacy for nursing faculty. In order to increase the satisfaction of the program, there is a need to develop a client-focused program that meets the goals of participation and major areas for nursing faculty.

Content Analysis of the Experiences of RN-BSN Students through an RN-BSN Education Course (RN-BSN 과정에 대한 학생들의 경험 내용분석)

  • Purpose: The purpose of this study was to explore experiences of RN-BSNstudents through an RN-BSN education course. Method: Data was collected through in-depth interviews and recordings about RN-BSN students' experiences in the education system. The data was analyzed in terms of Downe-Wamboldt's(1992) content analysis methodology. Result: Ninety significant statements were found concerning their experiences and their experiences were divided into three dimensions (intra personal, inter personal, & outer personal). The analysis scheme consisted of 15 categories and 26 subcategories. Conclusion: We found participants not only advanced their careers and improved their knowledge but also grew and transformed internally. In addition, this study can suggest a framework for planning curriculum content and learning methods in an RN-BSN course. In addition, the qualitative findings of this study can help the professor to understand and guide RN-BSN students.
