임상간호사의 간호근무환경, 공감피로 및 공감만족이 소진에 미치는 영향 (Effects of Nursing Practice Environment, Compassion Fatigue and Compassion Satisfaction on Burnout in Clinical Nurses)

  • Purpose: The purposes of this study was to identify the effects of nursing practice environment, compassion fatigue and compassion satisfaction on burnout in clinical nurses. Methods: Participants selected for the final analysis were 208 nurses working in 2 general hospitals in Busan and Masan. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation and Hierarchial Multiple Regression with SPSS/WIN 18.0. Results: Compassion fatigue had a significant positive effect on burnout, while Compassion satisfaction had a negative effect on burnout, but Nursing practice environment had no effect on burnout. The explained variance for burnout was 61% and compassion satisfaction was the most significant factor in burnout of nurses. Conclusion: The results of this study indicate that to reduce burnout in clinical nurses it is necessary to develop programs to increase nurses' compassion satisfaction and decrease compassion fatigue.

간호 대학생이 인식하는 실무현장의 효과적 환경을 위한 간호리더자의 역할유형분석 (The role type analysis of nursing leader for effective environment at the practice field that nursing students recognize - Q methodology -)

  • 간호 대학생은 임상실습기간동안 실무현장에서 다양한 학습을 하게 되며 특히 간호 리더자의 역할에 대한 중요성과 그 효과에 대해 인식하게 된다. 본 연구에서는 간호대학생이 실습 과정에서 경험한 효과적 실무환경을 위해 간호리더자의 역할에서 중요하게 작용하고 있는 부분이 무엇인지를 분석하여 그 유형을 분류하고자 한다. 이에 본 연구방법은 간호 대학생이 심층적으로 어떻게 인식하고 있는지를 파악하기 위해 Q방법을 적용하여 분석하였다. 연구 참여자는 간호대학생으로 실습 경험이 있는 자로 30명으로 구성하였고, Q표본은 간호리더자의 역할유형과 관련된 문헌, 임상실습을 경험한 학생과의 인터뷰를 통해 최종 표본을 36개의 진술문으로 구성 하였으며, QUANL PC프로그램으로 주요인을 분석하여 관련 유형을 분류하였다. 연구결과로 간호학생들이 인식하는 간호리더자의 역할유형은 4개의 유형으로 나타났으며, 제1유형은 "정의와 냉철함 유지형", 제2유형은 "동기부여를 위한 환경조성 형", 제3유형은 "인격존중 및 능력신장형", 제4유형은 "신뢰성을 통한 인간관계 중심형"으로 분류되었다. 본 연구 과정을 통해 간호학생이 실습과정에서 경험하는 실무현장의 효과적 환경을 위해 간호 리더자의 역할을 파악하여 추후 실무 환경을 위해 리더자의 역할의 중요성을 제안하고 이를 개선하는데 근거자료를 마련하고자 한다.

간호사의 간호근무환경과 이직의도 관계 : 워라벨(일과 삶의 균형)의 매개효과 (Relationships among Nursing Practice Environment and Turnover Intention of Hospital Nurses: The Mediating Effect of The Work-Life Balance)

  • 본 연구는 간호사의 간호근무환경과 워라벨 및 이직의도 간의 관계를 알아보고, 간호근무환경과 이직의도의 관계에서 일과 삶의 균형의 매개효과를 파악하기 위한 서술적 조사연구이다. 연구 대상자는 2020년 10월 5일부터 10월 30일까지 편의추출하였고, 3개의 종합병원에 재직 중인 병원간호사를 대상으로 자기보고식 설문방법으로 자료수집 하였다. 자료분석은 SPSS/WIN 23.0 프로그램을 사용하여 t-test, ANOVA, Pearson's correlation coefficient, 회귀분석으로 분석하였다. 연구결과, 간호근무환경과 이직의도 간의 관계에서 워라벨은 부분 매개하는 것으로 나타났다(Z=-3.55, p<.001). 따라서 간호사의 이직률을 감소시키기 위해서는 간호사의 근무환경을 긍정적으로 개선하고 워라벨을 증진하기 위한 조직 차원의 인적자원관리 전략과 더불어 이를 위한 프로그램 개발 및 적용이 필요하다. 본 연구결과를 토대로 간호사의 간호근무환경을 긍정적으로 인식하고 워라벨을 향상시켜 간호사의 이직률을 감소시키기 위한 기초자료로 제공하고자 한다.

간호대학생이 경험한 무례함과 임상실습교육환경이 임상실습스트레스에 미치는 영향 (Effects by Incivility Experienced and Clinical Learning Environment of Nursing Students on Clinical Practice Stress)

  • 본 연구는 간호대학생이 경험하는 무례함과 임상실습교육환경이 임상실습스트레스에 미치는 영향을 확인하기 위해 시도하였다. 연구대상자는 임상실습 경험이 있는 3, 4학년 간호대학생 204명이며, 자료는 구조화된 설문지를 배부하고 자가보고하는 형식으로 2019년 4월 1일부터 4월 30일까지 수집되었다. 자료 분석을 위해 SPSS 22.0 program을 이용하여 기술적 통계, t-test, ANOVA, Pearson 상관계수, 다중회귀분석을 실시하였다. 본 연구의 결과는 다음과 같다. 간호대학생이 경험한 무례함의 평균은 1.58점, 임상실습교육환경의 평균은 2.79점, 임상실습스트레스의 평균은 3.33점으로 나타났다. 임상실습스트레스는 간호대학생이 경험한 무례함과 정적 상관관계, 임상실습교육환경과는 부적 상관관계가 있는 것으로 나타났다. 간호대학생이 경험한 무례함과 임상실습교육환경은 부적 상관관계가 있는 것으로 확인되었다. 임상실습 중 경험한 무례함과 임상실습교육환경은 임상실습스트레스에 유의한 영향요인으로 나타났으며 22.5%의 설명력을 보였다. 본 연구결과를 바탕으로 간호대학생들의 임상실습스트레스를 효과적으로 관리하기 위해서는 임상현장에서 경험하는 무례함을 감소시키고 임상실습교육환경을 개선하는 노력이 필요 할 것이다.

간호간병통합병동과 일반병동의 간호서비스 만족도와 중요도, 간호 근무환경, 간호사 재직의도 비교 (Comparing Satisfaction and Importance of Nursing Care Nursing Work Environment, Nurse's Intention to Work between Comprehensive Nursing Care Unit and General Nursing Care Unit)

  • Purpose: This study was aimed to identify the difference in satisfaction and importance of nursing care between patients in comprehensive nursing care unit (CNCU) and general nursing care unit (GNCU). It also confirms the difference between practice environment of nursing work and nurse's intention to work. Methods: Nursing care satisfaction and importance levels were measured from 202 patients. Practice environment of nursing work and nurse's intention to work were measured from 54 nurses. Results: The satisfaction level was higher in the CNCU in comparison to the GNCU (p<.001). There was no significant difference between the importance and satisfaction level of nursing care for patients at the CNCU (p=.973), whereas in the GNCU, patients' satisfaction level was lower than the importance level (p<.001). The score for practice environment for nursing work was higher in the CNCU than in the GNCU (t=3.34, p=.002). The nurse's intention to work in the CNCU was higher than that of the GNCU, but there was no statistically significant difference. Conclusion: Through the survey, the paper suggests that comprehensive nursing care is a service type that satisfies the nursing demand (importance) that patients consider important. Results from nurses showed no significant differences.

간호 대학생의 기본 간호 실습 경험에 대한 연구 (A Study on the Experience of Fundamental Nursing Practice)

  • The purpose of this study was to understand and to explain how were nursing students experienced and accepted the fundemental nursing practice. In addition to, the results of this study are attempted to contribute for offer of basic data in projecting and accomplishing to promote quality practice education. The participants were 79 freshmen of S College of Nursing in kyungi-do. They presented record of feeling and thinking on their the fundemental nursing practice experience. The data were collected from 29, J une to 10, July in 1998. Collected data was analyzed by means of Van Kaam's phenomenological method. The results of this study was founded 423 descriptive expression and they were grouped under 42 common factors and they were grouped under 9 categories. By means of the frequency on the categories. the higher category is Anxiety. next Solemn. Flutter. Pride. Usefulness, Recognition of reality in nursing-system. Lack of practice environment. Self-accusation. Comprehension of nursing spirit were founded. 5 common factors. Tension. Difficulty, Dread. Apprehension, Burden were grouped under Anxiety. 7 common factors, Pledge, Memory, Importance of practice, Sincerity, Restriction of dress, Acceptance, Active attitude were grouped under solemn. 5 common factors, Interest, Strange, Beanimated, Waiting, Curiosity were grouped under Flutter. 5 common factors, Conceit, Self-confidence, Skilled, Worth, Accomplishment were grouped under Pride. 6 common factors, Acknowledge of nursing affairs, Expectation of furture, Fascination of nursing. Acquirement of disposition of nurse, Association of injection, Actual feeling of dept. of nursing were grouped under Recognition of reality in nursing-system. 4 common factors, Lack of practice time, Many persons of practice, Lack of practice instrument, Lack of reality were grouped under Lack of practice envirnment. 5 common factors, Inconvenient, Reflection, Loss of pride, Shyness, Feeling sorry were grouped under Self-accusation. 3 common factors, Utility, Connection of practice and theory, Various experience were grouped under Usefulness. 2 common factors, Comprehension on the dignity of human, Comprehension on a point of view of patient were grouped under Comprehension of nursing spirit. In conclusion, the following recommendation should be necessary a supplementary study to approach on the type of students that has a firm view and care about client prior to clinical nursing practice.

한국 정신간호영역에서의 간호업무환경 측정도구의 타당화 연구 (Validation Study of the Practice Environment Scale of Nursing Work Index for Psychiatric Hospital Nurses in Korea)

  • Purpose: The purpose of this study was to test the reliability and the validity of the Practice Environment Scale of Nursing Work Index (PES-NWI) scale when applied to Korean psychiatric nurses working in the psychiatric hospital in Korea. Methods: Data from 224 psychiatric nurses working in the 6 psychiatric hospitals in Korea were analyzed using Cronbach's ${\alpha}$, item total correlation, and exploratory factor analysis. Results: The Cronbach's ${\alpha}$ for the 27 items from the PES-NWI were .93, and the item total correlation coefficients ranged from .38 to .64. Six factors which explained 61.2 of variance in the perception of practice environment, emerged; nurse professional role and development, nurse manager ability, leadership, and support of nurses, staffing and resource adequacy, nursing foundation for quality of care, collegial nurse-physician relations, co-worker's ability and qualification. Five derived factors reconstructed, and two factors were renamed. Conclusion: Findings demonstrate that not all but 27 items from PES-NWI is a reliable and valid scale when applied to Korean psychiatric nurses working in Korea. Also findings indicate that Korean psychiatric nurses perceive practice environment differently than American nurses but have some point of similarities with Asian nurses working in the United States.

간호대학생의 임상실습 중 경험하는 무례함의 정도와 스트레스 대처 전략이 간호전문직관 형성에 미치는 영향 (The Effect of the Degree of Incivility Experienced in Clinical Practice of Nursing College Students and the Stress Coping Strategy on the Formation of Nursing Professional Value)

  • 본 연구의 목적은 간호대학생이 임상실습 중 경험한 무례함 정도를 조사하고 무례함, 스트레스 대처전략이 간호전문직관 형성에 미치는 영향을 파악하는 것이다. 연구대상은 총 290명을 대상으로 삼았다. 연구도구는 무례함, 스트레스 대처전략, 간호전문직관을 사용하여 측정하였다. 자료 분석은 SPSS WIN 20.0을 이용하여 분석하였다. 연구결과 첫째, 일반적 특성 중 성별, 학년, 임상실습만족도, 임상실습환경만족도, 임상실습 중 무례함을 경험한 경우에 무례함 정도에 유의한 차이가 있었다. 또한, 간호전문직관의 차이는 전공만족도, 임상실습만족도, 임상실습환경만족도에 따라 유의한 차이를 보였다. 둘째, 무례함은 문제해결 중심대처(r=.36), 사회적지지 중심대처(r=.26), 회피중심대처(r=.25), 간호전문직관(r=-.16)과 상관관계가 있는 것으로 나타났다. 셋째, 간호전문직관에 유의미한 영향을 미치는 요인으로는 사회적지지 추구 대처(β=.23), 임상실습 환경 만족도(β=.22), 임상실습 만족도(β=.16)로 나타났으며, 총 설명력은 14% 이었다. 본 연구의 결과는 간호대학생의 간호전문직관 형성을 위해서는 적절한 스트레스대처전략, 만족스러운 임상실습환경과 임상실습 경험이 중요함을 시사한다.

Systematic Literature Review on Nursing Department Clinical Practice Research in Korea

  • Nursing majors are a combination of theoretical and practical education, and since the advisor cannot participate in all practices in clinical practice, the efficiency and effectiveness of practical education are limited in judging the results according to students' reactions. Therefore, the purpose of this study was to analyze the domestic literature on the efficiency of clinical practice of nursing college students.The analysis target was selected through PRISMA flow, and the results were derived through categorization and questioning based on content analysis. Looking at the results, research related to the adaptation of trainees was mainstream, and specifically, research related to the characteristics, conflicts, clinical practice environment, and interpersonal anguish of trainees was conducted. In addition, it was confirmed that active and systematic intervention of practical guidance professors was required, such as possible safety accidents, emotional labor problems, and dilemmas during the practice period. Through this study, it is suggested that a systematic framework for nursing and clinical practice should be prepared.

자기주도적 학습능력이 간호학생의 임상실습만족도에 미치는 영향 (A Study on The Influence between Self-directed Learning and Nursing Student's Satisfaction of Clinical Practice)

  • Purpose. This study was to identify the relationship between self-directed learning and nursing student's satisfaction of clinical practice. Methods. This study was designed to measure the level of satisfaction according to the contents, guidance, environment, time, and the evaluation of clinical practice. Two hundred thirty-one nursing student from baccalaureate program in H-gun was sampled. A scale consisting of 75 questions, developed by the researcher, was used to gather data from September 14 through 27, 2012. The data was analyzed by descriptive statistics, paired t-test, pearson correlation, and multiple linear regression using the SPSS WIN 18.0 program. Results. The results were summarized : Self-directed learning didn't show a significant different by grade(t=0.83, p=.934). Nursing student's satisfaction of clinical practice didn't show a significant different by grade(t=0.26, p=.798). The relationship between self-directed learning correlated with the degree of nursing student's satisfaction in clinical practice(r=.44, p<.001). The factors of self-directed learning described nursing student's satisfaction of clinical practice as $R^2$=.215(F=9.858, p<.001). Conclusions. It was found that a higher degree of satisfaction in clinical practice depends on a higher degree of self-directed learning. Therefore, nursing faculty should plan intervention to improve satisfaction level of clinical practice by fostering self-directed learning.
