간호중재와 간호결과 분석- 간이식 환자를 중심으로 - (Analysis of Nursing Interventions and Nursing Outcomes of the Patients with Liver Transplantation)

  • Purpose: This study was designed to investigate the survey of analyzing nursing interventions and nursing outcomes of the patients with liver transplantation. Method: The subjects of this study consisted of 83 patients and fifteen nurses who took care of them. The methods of this study were retrospective and descriptive survey. Result: The mean age of patients was 42.7 years and the subjects were 62.7% males. 56.7% of these patients were diagnosed with liver cirrhosis of B type. The mean duration of hospitalization was 48.6 days. Ninety-five nursing interventions were performed at least daily. The most frequent used interventions were "environmental management: comfort", "medication administration: parenteral", "cough enhancement", and " oxygen therapy". Even though SGOT level to identify nursing outcomes was decreased, SGOT was not within normal limits. Therefore, even after discharge of patients, There is a need to take care of them carefully. Conclusion: These findings revealed the significance and need of nurse practitioners who performed professional nursing intervention for the patients with liver transplantation. Especially, it is necessary needs to develop the nursing intervention programs for comfort.

Behavioral Symptoms in Nursing Home Residents with Dementia: Developing a Nursing Practice Model

  • Purpose: Behavioral symptoms in dementia (BSD) are one of the most disturbing behaviors to caregivers and a major reason for nursing home placement. Behavioral symptoms are often treated with psychotropic drugs (PD), however, the effect of such drugs for the frail elderly dementia patient is not certain because of their critical adverse effects. Theoretical model applicable to nursing practice for BSD in nursing homes, which is essential in guiding and evaluating such interventions, is absent. This article presents the process of developing a theoretical model of BSD in nursing homes. Method: Using Walker and Avants' theory synthesis method, three behavior models and two system models were incorporated into the proposed model to provide the theoretical and analytical explanation of the relationships between PD usage, its determinants, and BSD. Results: Resident variables and nursing home variables related to the two focal concepts (i.e., PD usage and BSD) were identified. Resident variables include demographical characteristics such as age and gender, and dementia-compromised functions such as cognitive and functional impairment. Nursing home variables include facility characteristics such as ownership type and size, and physical and psychosocial environment. Conclusion: The proposed model suggests that fulfillment of resident unmet needs through improvement of physical and psychosocial environment may produce better health outcomes of nursing home residents with BSD. Assessment and intervening environmental triggers of such behaviors are also suggested to be prior to the PD usage.

표준화 환자를 활용한 임상수행능력 평가방법 개발 및 적용효과: 복부수술 후 환자간호 (Effects and Development of Clinical Competency Evaluation using Standardized Patients among Nursing Students: Based on Abdominal Surgical Patients)

  • Purpose: This study was to develop the clinical competency evaluation, and to examine the effects of the developed evaluation by comparing it with existing evaluation on clinical competency, communication skill, and self-efficacy of nursing students. Methods: Design was a randomized control group post-test design. The subjects were 102 senior nursing students(Experimental group: 48, Control group: 54) at K university in seoul, Korea. The experimental group took the clinical competency evaluation using standardized patients and the control group took the existing evaluation using a doll. The clinical competency and communication skills were measured by evaluators, and self-efficacy was self reported by the nursing students. Results: The experimental group had higher scores in clinical competency, communication skills, and self-efficacy than those in the control group. Conclusion: Through these results, practice education of nursing education programs need to activate the clinical competency evaluation using standardized patients.

간호등급 상향에 따른 직접간호활동, 간호업무성과와 직무만족 비교 (Differences of Upgrading Nurse Staffing in Nursing Care Activity, Work Performance Outcomes, and Job Satisfaction)

  • Purpose: This study examined differences in nursing care activity, work performance outcomes, and job satisfaction associated with upgrading nurse staffing of a nurse-to-patients ratio. Methods: Descriptive design was used in this study. In total, 148 medical and surgical nurses were recruited from one university hospital. Three instruments were used for data collection: Scale of Nursing Care Activity, Nurses' Work Performance Outcome Measurement Scale and Nurses' Job Satisfaction Scale. Data were analyzed by using descriptive statistics, t-test, ANOVA, and paired t-test. Results: There were significant differences in nursing care activity (t=-5.06, p<.001), in work performance outcomes (t=-5.46, p<.001) and in job satisfaction (t=4.61, p<.001) when the grading for the nursing staff was changed from three to two indicating increasing number of nurses. Conclusion: The findings from this study showed that there were more nursing care activities, better work performance outcomes, and higher job satisfaction when numbers of nurses were increased. The changes in the scale to evaluate nursing staff influence nursing activities, work performance and job satisfaction.

성인여성의 성인애착, 책임귀인, 자아존중감이 친밀한 관계의 심리적 폭력에 미치는 영향 (Effect of Adult Attachment, Attribution of Responsibility, and Self-esteem on Psychological Violence from Intimate Partner in Women)

  • 본 연구의 목적은 성인여성의 성인애착, 책임귀인, 자아존중감이 친밀한 관계에서 심리적 폭력에 미치는 영향을 파악하기 위하여 시도되었다. 본 연구는 서술적 조사연구로 성인여성을 대상으로 자가보고식 설문지를 이용하여 수집된 137명의 자료를 분석하였다. 연구결과 연령(36세 이상)과 연애경험(4회 이상), 책임귀인의 정도가 높을수록 심리적 폭력피해가 높게 나타났다. 또한 연령(36세 이상)과 연애경험(4회 이상), 성인애착이 높을수록 심리적 폭력 가해가 높게 나타났다. 반면 자아존중감은 영향을 미치지 않는 것으로 나타났다. 따라서 안정형 성인애착 증진과 적대적인 귀인 감소를 위한 중재방안을 개발하여 성인여성의 심리적 폭력을 줄이기 위한 노력이 필요하다.

성인간호학의 실시간 온라인 수업에 대한 학습만족도 영향요인 (Factors Influencing Learning Satisfaction for Real-Time Online Classes in Adult Nursing)

  • 본 연구는 실시간 온라인 성인간호학 수업에 대한 학습만족도의 영향요인을 확인하여 간호학 교과목의 온라인 강의 설계 및 운영에 기초자료를 제공하고, 새로운 교육 패러다임을 준비하고자 수행되었다. 연구대상자는 일개 대학 간호학과에서 실시간 온라인 성인간호학 수업을 수강하고 있는 3학년 105명을 대상으로, 2020년 6월 20일부터 7월 30일까지 구조화된 온라인 설문지를 통하여 자료수집을 시행하였다. 자료의 분석은 t-test, One-way ANOVA, Scheffe test, Person's correlation coefficients, Hierarchical multiple regression analysis 으로 분석하였다. 연구결과, 대상자의 학습몰입은 3.07점(5점 척도), 수업참여는 3.46점(5점 척도), 학습만족도는 3.88점(5점 척도)으로 나타났다. 학습만족도는 학습몰입(r=.41, p<.001), 수업참여(r=.56, p<.001)와 양의 상관관계가 있는 것으로 분석되었다. 또한, 본 연구 대상자의 학습만족도에 영향을 주는 요인은 수업참여(��=.47), 성인간호학 수업에 대한 흥미정도에서 매우 높음(��=.21), 높음(��=.20) 순이었으며, 설명력은 34%로 분석되었다(F=14.53, p<.001). 따라서 본 연구를 토대로 간호대학생의 학습만족도를 향상시키기 위한 주요요인은 수업참여로, 적극적인 수업참여를 이루어지기 위한 방안 및 교육방법을 개발하여 간호대학생의 학습성과 및 학습목표의 효율적인 달성을 기대해 본다.

Nursing Outcomes Classification(간호결과 분류체계)의 한글 명명화에 대한 타당성 연구 (Standardization and Validity study of Korean Nomenclature of NOC)

  • The purpose of this study was to develop and test the validity of the standardized Korean nomenclature of Nursing Outcomes Classification (NOC), developed by Johnson & Maas at the University of Iowa. The four phases of the study were: (1) translation of the NOC into Korean by the research team, (2) four nursing professors and eight nurses with various clinical backgrounds reviewed each nomenclature taking into consideration definitions and nursing activities. The modified Delphi method was used to determine the most appropriate nomenclature for each term; (3) Twenty four academic and clinical experts in nursing were given a questionnaire to rate each Korean nomenclature using a 5 point Likert scale ranging from very inappropriate to very appropriate; (4) the team determined the most appropriate Korean nomenclature for each class of the NOC. The mean validity score of 190 items was 4.54, but several nursing outcome had a score lower than 4.0. They included 'adherence behavior(3.3)', 'ambulation : walking(3.57)', 'transfer performance (3.57)', 'caregiving endurance potential(3.57)'.

국내 흡연과 금연관련 간호 연구동향 및 미래 방향 (Analysis of Smoking and Smoking Cessation Related Nursing Research in Korea and its Future Direction)

  • Purpose: The purpose of this study was to analyze the contents and trend of smoking and smoking cessation related nursing research in Korea and to suggest the directions for future research. Method: Eight nursing professional Journals published by the Korean Academic Nursing Society and by 7 - adult, community, psychiatric, fundamental, obstetrics, pediatrics, administrative- nursing societies from beginning edition to October 2005 were selected. They were examined for the proportion of published research, participants, research designs, key words, measurement tools, and the intervention outcomes in the case of the experimental research. Result: Of the research analyzed, 87 studies were smoking and smoking cessation related research. The Journals, which had published the most number of smoking and smoking cessation research were J of Korean Academic Society, J of Korean Adult Nursing Society, J of Korean Community Nursing Society. The most popular research design was an descriptive design. The major concepts studied were related psychosocial variables. Among 87 studies, only 11 were experimental research. Conclusion: Research on smoking and smoking cessation in the nursing discipline are increasing. However more research to test the effectiveness of nursing intervention programs are needed.

하브루타 학습법을 활용한 성인간호학 교육이 간호대학생의 비판적 사고성향, 학습몰입 및 학습만족도에 미치는 영향 (The Effects of Adult Nursing Education on the Critical Thinking Disposition, Learning Commitment, and Learning Satisfaction of Nursing Students Using the Havruta Learning Method)

  • 하브루타는 사고의 과정개발을 위한 최상의 교수학습방법으로, 다양한 학생활동 중심의 교육방법이다. 본 연구는 하브루타 학습법을 활용한 성인간호학 교육을 간호 대학생에게 적용하여 비판적 사고성향, 학습몰입, 학습만족도에 미치는 효과를 규명하기 위하여 실시하였다. 자료수집은 2017년 9월부터 12월까지 U시 소재한 일개 대학의 재학 중인 2학년 간호 대학생 96명 중 실험군 47명, 대조군 49명으로 진행하였다. 본 연구는 비동등성 유사실험 연구이며, 자료 분석은 SPSS WIN 23.0 프로그램을 이용하여 기술통계, ${\chi}^2-test$, t-test, Two-way repeated ANOVA를 이용하여 분석하였다. 연구결과, 하브루타 학습법을 활용한 성인간호학 교육을 시행한 실험군과 시행하지 않은 대조군의 비판적 사고성향, 학습몰입, 학습만족도가 집단과 시점간의 교호작용에 유의한 차이가 없었으나, 하브루타 학습법을 활용한 성인간호학 교육을 시행한 실험군는 교육 후, 비판적 사고성향(md=.14, p=.045), 학습몰입(md=.27, p=.025), 학습만족도(md=.30, p=.031)가 통계적으로 유의하게 증가되었다. 따라서 하브루타 학습법을 활용한 성인간호학 교육은 간호 대학생의 비판적 사고성향, 학습몰입, 학습만족도를 향상시키는 효과적인 프로그램이라 할 수 있다. 이에 본 교육프로그램을 다양한 학년 및 교과목에 적용하여 프로그램의 효과를 확인할 필요가 있다.

중환자실 간호사의 연명의료에 대한 태도, 역할인식 및 간호 스트레스 (Attitude, Role Perception and Nursing Stress on Life Sustaining Treatment among Intensive Care Unit Nurses)

  • Purpose: The aims of the study were to investigate relationships among intensive care unit (ICU) nurses' attitude, role perception, and nursing stress related to life sustaining treatment (LST), and secondly, to identify factors influencing nursing stress about LST. Methods: Participants were 202 conveniently sampled ICU nurses from general hospitals in Korea with over 300 beds. From December 1, 2015 to January 31, 2016, data were collected using structured questionnaires. The questionnaire was designed to measure nursing stress related to LST. Content validity and reliability was established for the instrument. Results: Relationships were found between attitude and role perception, and between role perception and nursing stress about LST. Participants' role perception, gender, education level, and the experience of dealing with family members of patients receiving LST accounted for 13% of variance in nursing stress about LST. Conclusion: Results confirmed that ICU nurses' role perception affects nursing stress about LST. Accordingly, the nursing education programs related to LST should aim to enhance role perception of nurses, and strategies to reduce the nursing stress about LST of the nurses in ICU need to be further developed.
