Effect of a Multidisciplinary and Individualized Educational Program on the Need for Caring among Patients with Cancer under Radiotherapy (다학제적 개발 교육 프로그램이 방사선 치료를 받는 암환자의 돌봄 요구도에 미치는 효과)

  • This study was conducted to examine the effect of an individualized educational program by multidisciplinary staffs on the need for caring among patients with radiotherapy. Methods: A quasi experimental study with one group was designed to examine the effect. A total of 48 adult patients were recruited from a university hospital in G-city, Korea from July, 2006 to June, 2007. The level of patients' need for caring during radiotherapy was assessed at the pre- and post-education by a trained research assistant. The level of patients' need for caring on radiotherapy was measured with a 20-item questionnaire. Results: Paired t-test showed that the level of patients' need was reduced at the post-test compared to pre-test (t=3.40, p=0.002). The level of need was higher among the older than 65 yr (F=4.82, p=0.034), and patients who had education years less than 10 yr (F=4.40, p=0.042) and not a spouse (F=5.97, p=0.019) at the pre-test, while there were no difference according to participants' characteristics at the post-test. Conclusions: This multidisciplinary and individualized education program was effective on reducing the level of need for caring during radiotherapy. Therefore, for long-term self-management, further educational strategies based on patients' needs through multidisciplinary teamwork need to be developed and applied.

Roles and Functions of the Rehabilitational Nurse Practitioner Expected by Nurses and Doctors in Rehabilitation Hospital (재활전문간호사에게 기대되는 역할과 기능)

  • Purpose: This study aims to identify the role and function of the RNP(rehabilitational nurse practitioner) expected by nurses and doctors. Method: This study was a survey. The data were collected 188 nurses and 21 doctors who worked for disabled patients in the rehabilitation hospital during months of June, 2004 and August, 2005. Results: 98.4% of nurse and 61.9% of doctors agreed at opening of RNP course. The major role of RNP expected by nurses were educator, counsellor and case manager. The major role of RNP expected by doctors were direct care, self care promoter & exercise and emotional care. There was a significant difference about the need for opening of RNP course and major role and function of RNP between the group of nurses and doctors. Conclusion: The results of this study showed that the need for opening of RNP was identified and the major role of RNP was educator, counsellor, case manager and direct care. So there is a need for further research about major role of RNP related to various setting including rehabilitation hospital, nursing home, home care etc.

Implementing Evidence into Practice for Best Dementia Care

  • Purpose: The purpose of this paper was to discuss the need for translation studies in dementia care and current translational endeavors, and to provide recommendations for evolving evidence-based dementia care. Methods: A literature review yielded current evidence and translational efforts. Results: Dementia care interventions need to be implemented at various service levels. Barriers to translation include evidence gaps, lack of the use of a conceptual framework to explain the implementation process, and unsupportive funding mechanisms for applying innovations. Conclusion: There is clear evidence of the need for and benefits of evidence-based dementia care for patients with dementia, family caregivers, and care professionals. The urgent need now is finding ways to advance translational activities and facilitate future research into translation science.

A Study of the Nursing Needs of Hospitalized medical Patients by Means of Nursing History Form (간호력(Nursing History)을 통해 본 내과 입원환자의 간호요구)

  • Personalized Patient centered nursing care is increasingly difficult to achieve despite the fact that it remains one of our consistent goals. So, we must find away to individualized nursing care. One means, to achieve this is by use of the nursing history form, which has been developed to help the nurse make maximum use of her limited time with the patient, by obtaining systematically the information needed to plan his nursing care. The nursing history form can be used to collect data about individual nursing needs but also it lends itself to the collection of epidemiological data relevant to the needs of patient population. So this study was undertaken in an attempt to describe the general characteristics of the population studied, to find out their perceptions and expectations related to their illness and hospitalization, to find out specific basic needs and to examine the relationship between the patients nursing needs and demographic characteristics through the responses to the nursing history questions. The study population defined and selected was all the patients (70) who were admitted to Yonsei University Hospital from October 1 - 15, 1975. The direct interview method was used and the data were categorized by the investigator, according to the nature of responses to each question and were subjected to the percentile and the chi- square tests. The findings can be summarized as follows 11. General characteristics of the study population ; The population was made up mainly of urban patients who were highly educated. The age was equally distributed. 2. Patients, perceptions and expectations related to illness and hospitalization ; 88.6% of the patients knew about the reason for hospitalization but 64. 5% could state symptoms only. 37.5% recognized the cause of illness. Approximately three fourth of the patients expressed on expectation for early recovery. 60.0% of the patient responses indicated, that they expected less than 10 days of hospitalization. Of the total responses regarding hospitalization, 45.7% were negative about the post -hospitalization expectation, 45.7% expected to return to work. As a result of these findings, we can see that there is a great educational need, a psychological need and environmental need for adaptation to the hospital and a socio- economic need for the post - hospitalization period. 3. Specific basic needs : The physical needs include the problem of getting sufficient sleep (50.0%), difficulty in food intake(47.1%), problems with hospital diet (47.0%), abnormal condition of the skin(44.3%), problems with bowel elimination(35.7%), assistance with bathing(35.7%), pain(30.0%), difficulty in walking(30.0%) , difficulty in seeing(30.0%) problems in urinary elimination(20.0%) , and difficulty in hearing(10.0%), 4. Nursing needs and epidemiological characteristics ; Age distribution was related to the rendition of the mouth but no significant differences were observed statistically with the patients responses to the other nursing history questions according to the epidemiological variables of age, sex, education and residence.

The Knowledge and Learning Needs of Patients with a Permanent Pacemaker (영구형 심박조율기 이식 환자의 지식과 교육요구도)

  • Purpose: The purpose of this study was to examine the level of knowledge and learning needs of patients with a permanent pacemaker implantation. Methods: The participants were 86 patients with a permanent pacemaker implantation. Data were collected during April and May 2002 using a questionnaire survey on knowledge and level of learning need. The collected data were analyzed using the SAS program. Results: 1. The average knowledge score was 13.21. The knowledge level was significantly higher in participants under 50 years of age and for those with higher levels of education. The items with the highest knowledge score was 'permanent pacemaker needs regular check ups' and the lowest, 'a bath can be taken 1 month after stitches are removed'. 2. The total average score for learning needs was 60.78 and the mean item score was 3.04 as measured on a 4-point Likert scale. The item showing the highest learning need was 'electrical fields can affect the pacemaker's function and the lowest learning need was for sexual activity after permanent pacemaker implantation'. Conclusion: According to the above findings, it can be concluded that an intensive nursing education program should be developed for patients with a permanent pacemaker especially for those patients over 50 years of age and those with lower levels of education.

Use of Home Nursing Therapy and Need of Home Care Equipments (가정간호환자의 치료적 간호서비스 제공과 의료기구 사용 및 요구도)

  • This study was conducted with whole home care nurses nationwide to provide secondary analyzed data to understand on their usage of medical equipments and their need of them for a month. This study found that treatments given by home care nurses were nelaton catheterization, bladder washing/urethral washing, newborn care, exchange and care for nasogastric tube and suction in that order of frequency. Second, instruments and equipments used for home care were reported to be stethoscope, patient monitor, blood pressure measuring equipment, air flotation mattresses, beds for patients, mattresses, suctioning device sets, enteral feeding equipment and dressing set in that oder of frequency. Moreover, need assessment of medical instruments and equipments showed renal dialysis was most needed and patient monitor, blood pressure measuring equipment, enteral feeding equipment, solution and other supplies for renal dialysis and beds for patient were necessary in that order. In conclusion, the results of this study investigating special treatments and medical instruments and equipments used for home care patients and analyzing patients' need, were expected to be useful for expansion of application of long-term care insurance for the elderly and health insurance as well as for quality control of home care and development of medial instruments and equipments used at home.

Moderation and Mediation Effects of Self-efficacy and Perceived Behavioral Control on the Relationship between Knowledge of and Compliance with Handwashing in Nursing Students (간호대학생의 손씻기 지식과 수행의 관계에서 자기효능감과 지각된 행위통제의 조절 및 매개효과)

  • Purpose: The aim of this study was to examine the relationship between handwashing knowledge and handwashing compliance in nursing students and to identify the moderation and mediation effects of self-efficacy and perceived behavioral control on the relationship. Methods: The participants were 172 nursing college students who completed the self-report measures. Data were analyzed using t-test, ANOVA, and Pearson correlation coefficient, Multiple regression analysis was used to test moderation and mediation effects. Results: The score for knowledge of handwashing was higher in participants who had completed fundamentals of nursing practicum (t=-6.54, p<.001) and the score for compliance with handwashing was higher in participants who had completed clinical practicum (t=-3.81, p<.001) or fundamentals of nursing practicum (t=-5.97, p<.001). Self-efficacy showed a significant moderation effect (z=2.21, p=.027) but not a mediation effect while perceived behavioral control had a significant mediation effect but not a moderation effect on the relationship between knowledge of handwashing and compliance. Conclusion: Findings indicate that clinical practicum and fundamentals of nursing practicum were helpful to nursing students in acquiring knowledge and in practicing proper handwashing technique. These findings suggest that continuing and repetitive education programs on handwashing need to be enhanced and strategies to strengthen self-efficacy and perceived behavioral control need to be included in education programs.

Relationships between Critical Thinking Disposition, Professional Nursing Competence and Job Satisfaction in Clinical Nurses (임상간호사의 비판적 사고성향, 간호수행능력 및 직무만족도)

  • Purpose: This study was undertaken in order to investigate the relationships between critical thinking disposition, professional nursing competence and job satisfaction in clinical nurses. Method: A convenience sample of 221 registered nurses were obtained from one hospital in Seoul. The data were analyzed by descriptive statistics, t-test, ANOVA, Scheffe' test, Pearson correlation coefficient using SPSS WIN 14.0 program. Results: There were a statistically significant difference in the critical thinking disposition according to age, marital status, career length, and department. There were a statistically significant difference in professional nursing competence according to role, age, career length, current position, and department. There were a statistically significant difference in job satisfaction according to age, marital status, career length, and department. A significant positive correlation between critical thinking disposition and professional nursing competence, and job satisfaction was found. Conclusion: Critical thinking disposition, professional nursing competence and job satisfaction of clinical nurses are significantly related. To improve professional nursing competence and job satisfaction of clinical nurses, it need to use actively the critical thinking disposition with resonable salary. And it need to prepare the nursing education and strategy to increase the job satisfaction of clinical nurses.

The digital literacy, awareness, and educational needs of virtual reality among nursing students (간호대학생의 디지털 리터러시, 가상현실에 대한 인식 및 가상현실을 적용한 간호 교육 요구도)

  • Purpose: This study's aim was to investigate digital literacy, awareness of virtual reality, and educational needs of virtual reality-based nursing education among nursing students. Methods: Structured self-reported questionnaires were used to measure the nursing students' digital literacy, awareness, and educational needs of virtual reality. This study was conducted on 162 nursing students in Seoul between December 13 and December 24, 2021. The data were analyzed using descriptive statistics, t-test, and one-way ANOVA with SPSS/WIN 24.0. Results: The mean scores on digital literacy, awareness, and educational needs of virtual reality were 4.38±0.42, 4.71±0.64, and 4.08±0.61, respectively. Concerning the educational needs of the subjects, the need for safety education was the highest, followed by the need for oxygenation education, elimination education, and activity and exercise education. Significant relationships were found between digital literacy and the awareness of virtual reality (r=.34, p<.001), between digital literacy and the educational needs of virtual reality (r=.17, p=.028), and between awareness and the educational needs of virtual reality (r=.51, p<.001). Conclusion: This study suggests that it is necessary to develop a virtual reality-based nursing educational program that reflects the educational needs of nursing students.

Empathy, Awareness and Attitudes toward Violence among Elementary School Students (초등학생의 공감 정도와 폭력에 대한 인식 및 태도)

  • Purpose: The purpose of this study was to investigate empathy, awareness, and attitudes toward violence among elementary school students. Methods: The participants were 195 fifth and sixth grade students in Y elementary school. The data collection period was from June 24 to July 4, 2019. Results: Empathy scores significantly differed according to participants' gender and need for education on violence prevention. Attitudes towards violence (permissive and neglectful) significantly differed according to students' grade and need for education on violence prevention. Empathy was negatively correlated with permissive attitudes toward violence (r=-.26, p<.001) and neglectful attitudes toward violence (r=-.24, p=.001). Conclusion: The results of this study are expected to be utilized as basic data for education on violence prevention through empathy.
