Effect of Smoking Cessation Program for Workers in Large Size Company -Using Motivational Enhancement Counseling- (대규모 사업장 근로자를 위한 금연 프로그램의 효과 - 동기강화 상담(Motivational Enhancement Counseling)의 적용 -)

  • Purpose: The purpose of this study was to examine effects of smoking cessation program in large size company using motivational enhancement counseling. Methods: A quasi-experimental design was conducted. 72 workers in intervention group and 65 workers in control group participated in 7 large size companies. Smoking cessation program consisted of a campaign, smoking cessation education, and 6 weeks motivational enhancement counseling. The motivational enhancement counseling was tailored to the individual's stage of change. Stages of smoking behavior, smoking cessation rate, nicotine dependence, and confidence of smoking cessation were assessed after intervention. Results: The results showed that intervention had a significant impact on improving stages of smoking behavior and smoking cessation rate, and impact on decreasing nicotine dependence, compared to those of a control group. Conclusion: The effects and feasibility of smoking cessation program including motivational enhancement counseling were revealed in the large size company. These results might provide information that can be used in improving cultures of smoking cessation for workers.

Effects of an Integrated Breast Health Program according to Stages of Breast Cancer Risk Appraisal (유방암 위험평가 단계에 따른 통합적 유방건강관리 프로그램의 효과)

  • Objectives: The current study evaluated the effects of an integrated breast health program according to levels of breast cancer risk appraisal on knowledge on breast cancer, early detection behaviors, and diet patterns and attitudes in Korean healthy women. Method: A nonequivalent control group pre-posttest design was used. A total of 413 women aged 40-59, registering at the Life Long Health Center in two cities, were classified into intervention groups of 179 women and control groups of 234 women. The integrated breast health program included education, counseling on breast cancer, early detection behaviors, and appropriate diet with multimedia and individual practice session using breast models, reflecting characteristics of each level according to levels of risk appraisal. The knowledge on breast cancer, early detection behaviors, and diet were investigated using questionnaires at baseline and three months after intervention. Results: In both normal and borderline-risk group, intervention groups reported significantly higher scores of knowledge on breast cancer and higher stages of BSE behaviors than control groups. Conclusion: The results showed positive effects on knowledge and early detection behaviors of breast cancer in normal and borderline-risk groups. Further studies should investigate longitudinal effects of the intervention program on dietary change.

The Effects of Psychosocial Intervention on Depression, Hope and Quality of Life of Home-Based Cancer Patients (심리사회적 중재 프로그램이 재가 암 환자의 우울, 희망 및 삶의 질에 미치는 효과분석)

  • Purpose: The purpose of this study was to identify the effects of psychosocial intervention on depression, hope and quality of life of home-based cancer patients. Methods: The study design was a nonequivalent control group pretest-posttest design. Data were collected from September 21 to November 13, 2009. The subjects consisted of 81 cancer patients randomly selected who were registered at four public health center in Daegu, Korea. The 39 subjects in the experimental group received a psychosocial intervention and the 42 subjects in the control group received the usual nursing care. The weekly psychosocial Intervention protocol was comprised of health education, stress management, coping skill training and support (60 min) for eight weeks. Data were analyzed by using the SPSS/WIN 12.0 program. Results: Depression (F=23.303, p<.001) scores in the experimental group were significantly less than that of the control group. Further, hope (F=58.842, p<.001) and quality of life (F=31.515, p<.001) scores were significantly higher than those reported by the control group. Conclusion: The findings indicate that the psychosocial intervention was an effective intervention in decreasing depression and increasing hope and quality of life of home-based cancer patients.

  • Purpose: This study was conducted to examine the effects of the oral care program for improving swallowing function of the elderly using welfare centers on depression, self efficacy, subjective oral health status and swallowing related quality of life. Methods: The intervention in this study was systematically developed through the six stages of Intervention Mapping Protocol (IMP) and was based on Mead's symbolic interaction theory and Bandura's self efficacy theory. A non equivalent control group pre and post-test design was conducted on a total of 37 elderly people (experimental group: 19, control group: 18) from D city. The oral care program was administered to the experimental group once a week for five weeks, totaling five times, and the exact program ran for 50 minutes. The collected data were analyzed using the SPSS/WIN 25.0 program. Results: Following the intervention, subjective oral health score, and swallowing related quality of life were significantly improved in the experimental group. Conclusion: The oral care program presented in this study was found to be effective in increasing subjective oral health status, and swallowing related quality of life for the elderly using welfare centers.

Development and Effects of a Person-Centered Fall Prevention Program for Older Adults with Dementia in Long-Term Care Hospitals: For Older Adults with Dementia and Caregivers in Long-Term Care Hospitals (요양병원 치매노인을 위한 대상자 중심 낙상 예방 프로그램의 개발 및 효과: 치매노인과 요양보호사 대상)

  • Purpose: This study examined the effects of a person-centered fall prevention program for older adults with dementia in long-term care hospitals. Methods: A nonequivalent control group pretest-posttest design was used. The study sample included 42 older adults with dementia (experimental group: 21, control group: 21) and 42 caregivers (experimental group: 21, control group: 21). The program comprised 48 sessions held over 12 weeks and included exercise intervention with resistance and balance, dance walking (45~60 min, three times/week), cognitive and emotional intervention (35~50 min, once per week), and person-centered fall prevention education (10 min, once per week). The program for caregivers consisted of six educational sessions (i.e., fall prevention competency enhancement and person-centered care strategy education, 80 min, once per week) for six weeks. Data were collected before participation and 12 weeks after program completion from February 18 to May 12, 2019. Data analysis was conducted using the chi-square test, t-test, and Mann-Whitney U test with SPSS/WIN 21.0. Results: The experimental group of older adults with dementia showed significant improvement in physical and cognitive functions, and a decrease in depression, and behavioral and psychological symptoms, when compared with the control group. caregivers in the experimental group exhibited significant improvement in fall-related knowledge and person-centered care of older adults with dementia compared to the control group. Conclusion: The study findings indicate that this program was effective as a nursing intervention for fall prevention among older adults with dementia in long-term care hospitals.

The Effects of a Secondary Stroke Prevention Program on the Health Risk Indicators and Self-Care Compliance of Stroke Patients (뇌졸중 이차예방교육이 건강위험지표와 자가간호수행에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of a secondary stroke prevention education program on the health risk indicators and self-care compliance of stroke patients. Methods: A non-equivalent control group pretest-posttest design was used to select the participants. Subjects were 54 stroke patients (27 in the experimental group and 27 in the control group) hospitalized in a K university hospital in D city, Korea. Health risk indicators and self-care compliance were measured both for a baseline, as well as after intervention. The data was analyzed using a chi-square test, paired t-test and ANCOVA. Results: There were significant differences in systolic blood pressure, diastolic blood pressure, fasting blood sugar, weight and self-care compliance. Conclusion: The results of the study indicate that an educational secondary stroke prevention program is effective for health risk indicators and self-care compliance of patients. Therefore it can be used as an effective nursing intervention in clinical practice.

The Effectiveness of a Cultural Competence Training Program for Public Health Nurses using Intervention Mapping

  • Purpose: This study evaluated the effects of a cultural competence training program for public health nurses (PHNs) using intervention mapping. Methods: An embedded mixed method design was used. Forty-one PHNs (experimental: 21, control: 20) and forty marriage migrant women (MMW) (20, in each group) who were provided nursing care by PHN participated in the study. The experimental group was provided with a four-week cultural competence program consisting of an eight hour offline and online course, e-mail newsletters and social networking services (BAND). Transcultural Self-efficacy (TSE) of the PHNs, client-nurse trust, and satisfaction with nursing care of MMW were measured. Ten PHNs in the experimental group were interviewed after the experimental study. Results: The experimental group showed a significantly greater improvement in TSE, client-nurse trust, and satisfaction with nursing care than did the control group. Six themes emerged from qualitative data: (a) Recognizing cultural differences, (b) Being interested in the multicultural policy, (c) Trying to communicate in MMW's own language, (d) Providing medical information using internet and smart phone, (e) Embracing culturally diverse people into society, and (f) Requiring ongoing cultural competence training. Conclusion: Cultural competence training enabled PHNs to provide culturally competent care and contribute to MMW's health outcomes.

The Study on the Development and Effectiveness of Parent Role Education Program I -Focusing on the six-months results- (영아의 건강증진을 위한 부모역할훈련 프로그램의 효과 -생후 6개월의 결과를 중심으로-)

  • Recent research indicates that the new mothers want to learn about childrearing, and have burden in care of infants. Also it indicates that the new mothers say the lack of confidence and knowledge about infant care. This study was a prospective longitudinal reseach developing parent role education program and evaluating the effectiveness of this program for mother-infant interaction, childrearing environment and infant development. Longitudinal quasi-experimental design was used. The subjects were the healthy infants weighing over 2,500gm at birth, whose gestational age was more than 37weeks, and their mothers. The sample consisted of 19 mother-infant dyads for intervention group and 18 dyads for control group. Data were collected from March 15th to December 14th in 1999. For the intervention group received programmed education that was consisted of discharge education, telephone counselling, and home visiting for maternal education. The data were analyzed using chi-square test and t-test to test the equivalence of two groups, and the effectiveness of intervention program. This study was focused on the results of six months time point. The results were as follows: 1. There was significant difference in the mean score of mother-infant interaction(NCAST) between two groups. Intervention group showed higher scores in the subscales of sensitivity to cues, cognitive growth fostering, and caregiver total. 2. There was no significant difference in the mean score of child rearing environment (HOME) between two groups of six-month-infants. But when each subscale of HOME was examined, intervention group showed higher scores in the dimensions of opportunities for variety in daily stimulation, maternal involvement with child, and emotional, verbal response. 3. Six-month-infants of the intervention group showed significantly higher GQ in the Griffiths mental development scale. In conclusion, the maternal education program was effective in promoting the mother-infant interaction, organizing the childrearing environment, and fostering the infant development. These results were very meaningful that we found parent role education necessary for normal infants' mothers, and nurses can make a great contribution in promoting health of infants and mothers.

An Integrative Literature Review of Interventions for School-aged Overweight and Obese Children (학령기 과체중, 비만 아동 중재에 관한 통합적 문헌고찰)

  • Purpose: This study aimed to identify the characteristics of an intervention program for school-aged overweight and obese children through an integrated literature review. Methods: We used five databases (MEDLINE [PubMed], CINAHL, Embase, DBpia, and RISS) to search for studies published between January 1, 2013, and April 18, 2018. The studies were reviewed according to specific inclusion and exclusion criteria. Results: Finally, nine studies were selected and classified into three types: school-based, community-based, and home-based. Specific intervention methods were identified, including exercise, diet, exercise and diet, and cognitive behavioral therapy. Conclusion: Most studies have shown the effectiveness of obesity intervention programs. However, the variables, tools, and methods of measuring the effectiveness of intervention varied, making it difficult to compare the intervention studies. Therefore, future studies should develop objective and integrated measurement tools and evaluate the effects with long-term follow-up.

Development and Evaluation of a Dysphagia Assessment Tool and an Intervention Program for the Elderly in the Long-Term Care Facilities (노인요양시설 노인의 연하곤란 사정도구, 중재프로그램 개발 및 평가)

  • The purpose of this study was to develop a dysphagia assessment tool and a intervention program for elderly in the long-term care facilities and to evaluate its effect. The dysphagia assessment tool consists of 20 items was developed through literature review, previous studies and discussion with nursing/medical members. The intervention program consisted of 72 items and participants for evaluation were 50 elders who were registered at long-term care facilities in 4 regions of Seoul. Collected data were analyzed using SPSS Statistics 18.0 and Spearman's Correlation, t-test, and $x^2$-test. After the application of the dysphagia assessment tool and intervention program, the weight in experimental group was increased (t=4.913, p=.000) and the total time of swallowing was reduced (t=-4.557, p=.000) than control group. These findings were statistically significant. Considering these results, by applying the dysphagia assessment tool and the intervention program, the lives of the elderly in long-term care facilities are expected to be improved and emergency situations such as aspiration pneumonia and asphyxia will be decreased.
