병원 간호사의 프리셉터 경험에 관한 포커스 그룹 연구 (Focus Group Study on Hospital Nurses' Lived Experience of Being a Preceptor)

  • Purpose: The purpose of this study was to describe hospital nurses' lived experience of being a preceptor in Korea. Methods: The data were collected in three focus group interviews with 15 nurses in one general hospital. The phenomenological analytic method suggested by Colaizzi was used to analyze the data. Results: Five theme clusters and 11 themes emerged from the analysis. The five themes include "inevitable rite of passage for an authentic nurse", "facing new role and situations that were difficult to handle", "preceptee-centered preceptorship", "healing connection with colleague nurses", and "enhanced status with competence". Conclusion: The findings of the study provide deep understanding on hospital nurses' positive as well as negative experiences of being a preceptor and should inform the development of a more integrative preceptorship program to empower both preceptees and preceptors.

전국 병원간호사의 임금구조와 임금수준 결정요인 분석 (Nurse Wage Structure and Its Determinants in Hospital Industry)

  • Purpose: The purpose of this study was to investigate the wage structure of hospital nurses in Korea and to analyze the factors that determine the level of nurses' wages. Methods: First, a mobile survey on nurses' wages was conducted with a total of 3,742 nurses working in hospitals. A literature review was also done. Second, a regression model was applied to analyze the determinants of nurses' wages. Results: The average monthly wage of 3,742 nurses based on the survey was KRW 3,588,000 and the standardized monthly income of 3,742 nurses was estimated to be KRW 3,364,000 as of the end of 2018. The results of regression analysis(R2=61.7%) showed that gender, nursing experience, designated night duty, ownership, and number of beds were statistically significant variables for nurses' wages at 1% significance level. The monthly wage of nurses working in total nursing care wards was not significantly different from those working in other wards at 5% significance level. Conclusion: The substantial difference in nurses' wages according to job career, hospital size, region, etc may result in the low proportion of nurses with more than three to five years of service experience and the high turnover of less-experienced workers. Consideration should be given to adjust the gap in wage level and a standard nurse wage system could be effective measures.

병원간호사의 간호근무환경과 직무 스트레스가 건강문제에 미치는 영향 (The Effects of Nursing Work Environment and Job Stress on Health Problems of Hospital Nurses)

  • Purpose: The purpose of this study was to identify the effects of nursing work environment and job stress on health problems of hospital nurses. Methods: The subjects were 200 nurses working in S general hospital in Gyeongnam, and the data were collected using organized questionnaire from Jan 10 to 25, 2015. The Korean version of the practice environment scale of nursing work index, the instrument for job stress, and the Korean version of Todie Health Index for health problem were used for measurement. The collected data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation, and multiple regression. Results: The nursing work environment was found to be slightly negative, and the job stress was found to be high. There were significant correlation among nursing work environment, job stress, and health problems. In addition, it showed that the nursing work environment and job stress of nurses were factors affecting their health problems. Conclusion: The nursing work environment and job stress are influencing factors on the health problems of hospital nurses. Multi-faceted efforts to create a positive nursing work environment are required. Further researches related to association between the nursing work environment and health problem of nurses are needed.

간호사의 입원환자 욕창 사정에 대한 정확성 (Accuracy of Nurses' Assessments of Pressure Ulcers in Hospitalized Patients)

  • Purpose: The purpose of this study was to examine the accuracy of general nurses' assessments of pressure ulcers in hospitalized patients. Methods: A total of 129 nursing records of assessments on pressure ulcers were analyzed. Assessment records of pressure ulcers by general nurses were compared to those by Wound, Ostomy, Continence Nurses (WOCN) on the same pressure ulcers. A WOCN of a nursing unit was a nurse certified by the hospital after completion of a formal WOCN course and passing a cyber education course, both offered by the hospital. The formal WOCN course was taught by an internationally certified WOCN. The inter-rater reliability among WOCNs was 98.2%. General nurses in this study did not receive a structured pressure ulcer education. Results: The accuracy for nursing assessment of pressure ulcers by general nurses compared to WOCNs' were evaluated in various ways and resulted in as follows; the existence of pressure ulcer 91.4%, site of pressure ulcer 85.3%, stage of pressure ulcer 85.3%, Braden scale 36.3%, size of pressure ulcer 51.9~64.3%, details of pressure ulcer 0~100%. Conclusion: The structured education about pressure ulcer assessment is important to enhance the accuracy of pressure ulcer assessment of hospitalized patients by general nurses.

간호사의 암성 통증관리 수행정도와 관련요인: 개인 및 병원 기관 요인 (Factors Affecting Nurses' Pain Management for Cancer Patients: Personal and Hospital Institution Aspects)

  • Purpose: The purpose of this study was to examine potential factors related to the management of cancer pain, that is, hospital institutional factors as well as personal aspects of nurses. Methods: This study was a descriptive research study in which 229 RNs working in 2 tertiary medical institutions in Seoul and 4 secondary medical institutions in Seoul, Incheon and Gyeonggi were surveyed. Results: It was found that nurses' knowledge about pain intervention, their working division and their knowledge about the use of analgesics had different effects on their pharmacologic interventions. These 3 variables explained 14.5% of the variance regarding pharmacologic interventions. On the other hand, nurses' knowledge about pain interventions and nursing organization were variables affecting non-pharmacologic interventions by the nurses. These two variables explained 22.1% of the variance regarding non-pharmacologic interventions by the nurses. Conclusion: The findings indicate that nursing organization, one of hospital institutional factors, had significant effects on non-pharmacologic interventions. Therefore, to increase effective pain management by nurses, an organizational system should be established such as placement of nurse practitioners, improvement of nurses' autonomy in pain management, and development and distribution of standardized guidelines.

한국형 중환자간호 분류도구 개발 (Development of Korean Patient Classification System for Critical Care Nurses)

  • Purpose: This study was performed to develop a valid and reliable Korean Patient Classification System for Critical Care Nurses (KPCSC). Methods: Tertiary and general hospitals with various levels of ICU nurse staffing were included. To verify interrater reliability, data collectors and staff nurses of 15 ICUs in 11 hospitals classified 262 patients. To verify construct validity, the staff nurses classified 457 patients according to KPCSC comparing difference by medical department and type of stay in ICU. For conversion index, 195 patients from 10 ICUs in 7 hospitals were classified and nursing time was measured by 174 nurses, 7 head nurses, 18 charge nurses, 37 nurse aids and 1 secretary. Results: The developed KPCSC has 11 categories, 82 nursing activities and 115 criterias. Reliability was found to have high agreement (r=.96). Construct validity was verified by comparing differences in medical department and type of stay in ICU. According to scores, four groups in the KPCSC were identified. One score on the KPCSC indicates 6.12 minutes of nursing time. Conclusion: The findings show that the KPCSC can be used to measure new and complex nursing demands including rehabilitation and the safety of ICU patients.

The perceptions and performance of family-centered care among pediatric nurses at a children's hospital in South Korea: a descriptive study

  • Purpose: This study aimed to examine pediatric hospital nurses' perceptions and performance of family-centered care. Methods: A descriptive study design was used. This study surveyed 162 nurses who worked at a single tertiary children's hospital in South Korea. The modified Family-Centered Care Scale was used to assess nurses' perceptions and performance of family-centered care. Barriers to the implementation of family-centered care were described in an open-ended format. Results: Pediatric hospital nurses had a higher score for perceptions (mean score=4.07) than for performance (mean score=3.77). The collaboration subscale had the lowest scores for both perceptions and performance. The perceptions of family-centered care differed significantly according to the nurses' clinical career in the pediatric unit and familiarity with family-centered care, while performance differed according to clinical career only. Perceptions and performance were positively correlated (r=.594, p<.001). Barriers to implementation included a shortage of nursing personnel, a lack of time, and the absence of a family-centered care system. Conclusion: To improve the performance of family-centered care, nurses' perceptions of family-centered care should be improved by offering education programs and active support, including sufficient staffing, and establishing systems within hospitals.

간호사의 조직 몰입에 관한 일 연구 (A study on the Clinical nurses' Organizational)

  • Purpose : The purpose of this study investigated the relationship between nurses' organizational commitment and their demographic characteristics. Method : The sample for this study consisted 356 nurses from 6 hospital at Seoul and Kyung Gi province, in Korea The Organizational Commitment Questionnaire developed by Mowday was used. The Pearson correlation coefficient, Tukey's Studentized Range Test were used for the statistical analysis by SAS program. Result : The Study was found that 1) The mean of nurses' Organizational Commitment was 3.01. 2) The Organizational Commitment was positive correlation with age, career and position of the whole nurses, but it was different from each hospital. 3) There was significant positive correlations among nurses' age, career, position and organizational commitment of nurses. 4) But the correlations between nurses' organizational commitment and religion, marrital status, nurses' age, career, position were different from each hospital. Conclusion : Therefore before each hospital want to improve it's own nurses' organizational commitment, hospital managers have to find factors which influence to the nurses' organizational commitment directly, indirectly.

간호사의 창의적 간호실무가 직무만족 및 조직몰입에 미치는 영향 (Effects of Creative Nursing Practice on Hospital Nurses' Job Satisfaction and Organizational Commitment)

  • Purpose: This study was done to: (a) examine relationships between creative nursing practice and job satisfaction, and organizational commitment that nurses perceive in a hospital, and (b) identify creative nursing practice factors affecting job satisfaction and organizational commitment. Method: Using self-report questionnaires including 25 items of the Creative Nursing Practice Index, a descriptive study was conducted with a sample of 387 nurses working in the nursing care units of a general hospital in Seoul, Korea. Data were analyzed, using independent t-test, ANOVA, Pearson correlation, and hierarchical multiple regression analysis. Result: The mean score for nurses performance of creative practice was $2.29{\pm}0.46$ (range 1 to 4). Creative nursing practice had positive correlation with nurses' job satisfaction (r=.427, p<.001) and organizational commitment (r=.433, p<.001). Creative nursing practice factors influencing job satisfaction were implementing customized care (${\beta}$=.158, p=.010), making new knowledge (${\beta}$=.257, p<.001), and searching ideas (${\beta}$=.209, p<.001). Nurses' organizational commitment was influenced by implementing customized care (${\beta}$=.192, p=.002), making new knowledge (${\beta}$=.158, p=.028), searching ideas (${\beta}$=.254, p<.001), sharing ideas (${\beta}$=.250, p=.003), and validating ideas (${\beta}$=.189, p=.036). Conclusions: The results indicate a need to develop nurses' creative competency to pursue quality care as well as increase nurses' job satisfaction and organizational commitment.

병원간호사의 교대근무 특성, 직장 내 폭력에 대한 주변인 유형과 누락된 간호행위와의 관계 (Associations Between Shift Work Factors, Workplace Violence Bystander Types, and Missed Nursing care of Hospital Nurses)

  • Purpose: This study describes the associations between shift work factors, workplace violence bystander types, and the missed nursing care of hospital nurses. Methods: A cross-sectional survey was conducted among hospital nurses using structured questionnaires related to shift work factors, workplace violence bystander types, and missed nursing care. The study participants were 199 nurses caring for adult patients and working in two tertiary hospitals. Results: The missed nursing care of the participants showed a statistically significant positive correlation with the facilitating bystander (r=.40, p<.001) and abdicating bystander (r=.28, p<.001) among the workplace violence bystander types. However, rest time during duty showed a significant negative correlation (r=-.16, p=.026). A regression model with control variables (department and clinical career), shift work factors, and workplace violence bystander types explained approximately 25% of missed nursing care of the study participants. Further, the facilitating bystander type influenced the missed nursing care of hospital nurses. Conclusion: As a result of examining the associations between shift work factors, workplace violence bystander types, and missed nursing care of hospital nurses, it was confirmed that missed nursing care was influenced by the facilitating bystander type. It is necessary to improve the nursing culture within the department so that nurses do not behave as negative bystanders to workplace violence.
