We aimed to provide evidences for developing non-pharmacological intervention in older adults with Mild Cognitive Impairment(MCI) by integrated literature review. The final 16 papers were selected as a result of an integrated literature analysis. All of them are focused on strengthening cognitive activities, while Korean studies have often merged emotional activities such as music therapy and laughter therapy rather than physical activities such as exercise therapy, international studies have combined physical activities rather than emotional activities. The effects of non-pharmacological intervention differed according to the outcome variables. The primary variables were cognitive function and depression, and secondary variables were found to have effects on physical function, activities of daily living (ADL), and self-efficacy. This study contributes to a multidisciplinary approach that can be applied in the clinical field through the development of various non-pharmaceutical intervention for the prevention of dementia in the older adults with MCI.
[Purpose] Recent studies have shown that COVID-19 is often associated with altered gut microbiota composition and reflects disease severity. Furthermore, various reports suggest that the interaction between COVID-19 and host-microbiota homeostasis is mediated through the modulation of microRNAs (miRNAs). Thus, in this review, we aim to summarize the association between human microbiota and miRNAs in COVID-19 pathogenesis. [Methods] We searched for the existing literature using the keywords such "COVID-19 or microbiota," "microbiota or microRNA," and "COVID-19 or probiotics" in PubMed until March 31, 2021. Subsequently, we thoroughly reviewed the articles related to microbiota and miRNAs in COVID-19 to generate a comprehensive picture depicting the association between human microbiota and microRNAs in the pathogenesis of COVID-19. [Results] There exists strong experimental evidence suggesting that the composition and diversity of human microbiota are altered in COVID-19 patients, implicating a bidirectional association between the respiratory and gastrointestinal tracts. In addition, SARS-CoV-2 encoded miRNAs and host cellular microRNAs modulated by human microbiota can interfere with viral replication and regulate host gene expression involved in the initiation and progression of COVID-19. These findings suggest that the manipulation of human microbiota with probiotics may play a significant role against SARS-CoV-2 infection by enhancing the host immune system and lowering the inflammatory status. [Conclusion] The human microbiota-miRNA axis can be used as a therapeutic approach for COVID-19. Hence, further studies are needed to investigate the exact molecular mechanisms underlying the regulation of miRNA expression in human microbiota and how these miRNA profiles mediate viral infection through host-microbe interactions.
Purpose: The purpose of this study was to identify barriers to effective conversations about advance care planning (ACP) and palliative care reported by health care and community-based service providers in Massachusetts, USA. Methods: This qualitative research analyzed open-ended responses to two survey questions, inquiring about perceived barriers to having conversations about ACP and palliative care with patients and consumers. Data were collected between November 2017 and June 2019 from nine organizations in Massachusetts, including health care provider organizations, health insurers, community-based organizations, and a nursing education institution. Two researchers reviewed and coded the responses and identified common themes inductively. Results: Across 142 responses, primary barriers to ACP included hesitation and lack of understanding and knowledge, discomfort and resistance among service providers, lack of staff knowledge, difficulties with followup, and differences in ACP policies across regions. Common barriers to palliative care were misconceptions about palliative care and lack of knowledge, service providers' lack of preparedness, and limited policy support and availability. Challenges relevant to both ACP and palliative care were fear and discomfort around serious illness discussions, lack of knowledge and awareness, discussions that occur too late, and cultural and language barriers. Conclusion: Health care practitioners and community-based professionals reported consumer-, service provider-, and system-level barriers to facilitating conversations about ACP and palliative care with patients experiencing serious illness. There is a need for more tools and support to strengthen service providers' ACP and palliative care competencies and to promote a structured approach to health care planning conversations.
This study was conducted to investigate the structural relationship between depression, self-esteem, and social withdrawal that affect academic helplessness in adolescents. The study sample was 2,265 first-year students of high school in the fourth year (2021) data from the Korean Children and Youth Panel Survey 2018. In this study, data were analyzed by a structural equation model based on the partial least squares method using SmartPLS 3.0. Results indicated that depression and social withdrawal had a significant positive effect on adolescents' academic helplessness, and self-esteem had a significant negative effect. In addition, social withdrawal had a significant positive mediating effect on the relationship between depression and academic helplessness. Therefore, it is required strategies that consider the relationship between depression, self-esteem, and social withdrawal to approach adolescents' academic helplessness.

This study was analyzed using a mixed methods approach to clarify the concept of compassionate communication competencies of psychiatric nurses. In the theoretical phase, the literature published from 2000 to 2022 was collected and 38 articles were analyzed. For the fieldwork phase, in-depth interviews were conducted with eight psychiatric nurses from December 1 to December 28, 2022. In the final analysis phase, the dimensions and attributes of psychiatric nurses' compassionate communication competence were identified and conceptualized. Based on the attributes identified in the theoretical and fieldwork phases, the definition of psychiatric nurses' compassionate communication competence was synthesized into five dimensions and 12 attributes. Therefore, psychiatric nurses' compassionate communication competence refers to the skills and abilities of psychiatric nurses to use active listening and empathic skills for effective communication based on compassion and understanding of the target, to be sensitive to the thoughts and feelings of the target, to accurately convey what the target wants to express, to respect the target, and to empower the target.
Purpose: This study aimed to provide basic data for future infectious disease prevention educational programs targeting preschoolers by verifying the effects of the 2023 Home-visiting Infectious Disease Prevention Class Project for Preschoolers. Methods: The study is a descriptive survey that analyzed educational satisfaction of kindergarten teachers and the activity reports of visiting teachers. The effectiveness of the education on the children's side was evaluated with a quasi-experimental approach using pre- and post-test design. Data collection was conducted from September 6 to November 10, 2023. The data from 96 kindergarten teachers, 100 activity reports written by visiting teachers, 56 children in the experimental group, and 50 children in the control group were analyzed. Results: As a result of analyzing the satisfaction survey results of the kindergarten teachers and the activity reports submitted by the visiting teachers, all questions received a very positive evaluation with 4 or more out of 5 points. In addition, the children in the experimental group showed statistically significant positive changes both in hand-washing accuracy and their awareness of immunity. Conclusion: The Home-visiting Infectious Disease Prevention Class Project for Preschoolers is of great educational significance and is expected to be a useful educational intervention that can improve infection prevention behaviors and show practical preventive effects in preschoolers so that they can be prepared for various infectious diseases in the future.
For the flexible and rational distribution of limited existing health resources based on measurements of individual risk, the socalled Risk Approach is being proposed by the World Health Organization as a managerial tool in maternal and child health care program. This approach, in principle, puts us under the necessity of developing a technique by which we will be able to measure the degree of risk or to discriminate the future outcomes of pregnancy on the basis of prior information obtainable at prenatal care delivery settings. Numerous recent studies have focussed on the identification of relevant risk factors as the Prior infer mation and on defining the adverse outcomes of pregnancy to be dicriminated, and also have tried on how to develope scoring system of risk factors for the quantitative assessment of the factors as the determinant of pregnancy outcomes. Once the scoring system is established the technique of classifying the patients into with normal and with adverse outcomes will be easily de veloped. The scoring system should be developed to meet the following four basic requirements. 1) Easy to construct 2) Easy to use 3) To be theoretically sound 4) To be valid In searching for a feasible methodology which will meet these requirements, the author has attempted to apply the“Likelihood Method”, one of the well known principles in statistical analysis, to develop such scoring system according to the process as follows. Step 1. Classify the patients into four groups: Group $A_1$: With adverse outcomes on fetal (neonatal) side only. Group $A_2$: With adverse outcomes on maternal side only. Group $A_3$: With adverse outcome on both maternal and fetal (neonatal) sides. Group B: With normal outcomes. Step 2. Construct the marginal tabulation on the distribution of risk factors for each group. Step 3. For the calculation of risk score, take logarithmic transformation of relative proport-ions of the distribution and round them off to integers. Step 4. Recently in Korea, application of the soil nailing is gradually extended to the sites of excavations and slopes having various ground conditions and field characteristics. Design of the soil nailing is generally carried out in two steps, The First step is to examine the minimum safety factor against a sliding of the reinforced nailed-soil mass based on the limit equilibrium approach, and the second step is to check the maximum displacement expected to occur at facing using the numerical analysis technique. However, design parameters related to the soil nailing system are so various that a reliable design method considering interrelationships between these design parameters is continuously necessary. Additionally, taking into account the anisotropic characteristics of in-situ grounds, disturbances in collecting the soil samples and errors in measurements, a systematic analysis of the field measurement data as well as a rational technique of the optimum design is required to improve with respect to economical efficiency. As a part of these purposes, in the present study, a procedure for the optimum design of a soil nailing excavation wall system is proposed. Focusing on a minimization of the expenses in construction, the optimum design procedure is formulated based on the genetic algorithm. Neural network theory is further adopted in predicting the maximum horizontal displacement at a shotcrete facing. Using the proposed procedure, various effects of relevant design parameters are also analyzed. Finally, an optimized design section is compared with the existing design section at the excavation site being constructed, in order to verify a validity of the proposed procedure.
The purposes of this study are to delineate a profile of the state of a stroke patient's adaptation at 3 months after hospitalization and to explore the relationship between the level of adaptation and the variables which influence the adaptation of hemiplegic patients. To these ends, theoretical framework was derived basically from the stress adaptation model. The basic assumption underlying the level of adaptation is influenced by the presenting focal, contextual and residual stimuli. This group of stimuli is further operationalized and represented by a perception of stress. which is the perceived effect of the disability and by the mediating variables such as sociodemographic factors as an external conditioning variables and perceived social support and hardiness personality characteristics as an internal intervening variables. The dependent varibales in this study is the level of physical, psychological and social adaptation and is hypothesized to be a function of the interaction between 3 sets of variables namely, the perceived disability effect, external conditioning variables and internal intevening varibles. A total of fourty three subjects from 3 general hospitals in Seoul were observed and interviewed with the aid of 7 structured instruments. The data were collected twice on each subject : first at the pre-discharge period arid at 3 months post-discharge from hospital for the second time. The study was carried out for the period from February to August, 1988. The instruments used for the study include 4 existing scales and 3 scales developed by the researcher for this study. They are : 1) The ADL dependency scale and the scale of the clinical physical functions for the assessment of physical adaptation. 2) the SDS(self report of depression) to measure the level of psychological adaptation. 3) The scale for the amount of social activities for the measurement of the level of social adaptation. 4) The scale for the perceived effect of disability for the measurement of the focal stimuli. 5) The health related hardiness scale and the perceived interpersonal support self evaluation list(ISEL) for the measurement of the hardiness personality character and the perceived social support. The data obtained were analyzed using percentage, oneway ANOVA, Pearson coefficients correlation and stepwise multiple regression. The findings provide valuable information about the present level of physical adaptation at 3 months after discharge. The patient revealed a decreased ADL dependency and lowered limitation of physical function as compared with pre - discharge state. Psycholcgically, the average degree of depression at follow up was within normal range of depression. Socially, the amount of social activities was very low. The one way ANOVA and the correlational analysis revealed the relationship between the 3 sets of variables and the adaptation level as follows : 1) The perceived disability effect was related to the degree of the depression and the amount of social activities but was not related to the physical adaptation. 2) Among the sociodemographic variables, sex and education were related to the difference of ADL dependency and the change of physical function. These factors indicate that women more than men and educated more than the less educated were found more independent. The education was also related to the degree of depression suggesting that the higher the educational level, the more well adapted the patients were both physically and psychologically. Age, marital status and job state were not found to be related to the patient's adaptation level. 3) Among the internal intervening variables, the health related hardiness characteristic was related to the differences of ADL dependency, physical functions and the social activities, indicating that the higher the hardiness character the higher the level of physical and social adaptation. 4) The perceived social support, another internal intervening variable, was related to the degree of depression and the social activities. This data suggest that the higher the perception of social support, the better adapted the patients were psychogically and socially. In summarizing the results of the correlational analysis, the level of physical adaptation was influenced by sex, the years of education and the hardiness character. The level of psychological adaptation was influenced by the years of education, the perceived disability effect and the perceived social support. And the level of social adaptation was influenced by the perceived disability effect, the hardiness character and the perceived social support. The stepwise multiple regression analysis shows findings as follows : 1) The most important factor to explain the difference of ADL dependency was sex, indicating females were more independent than males. 2) The most important factor to explain the difference of physical function and the degree of depression was the patient's education level. 3) The strongest explaining factor for the amount of social activities was perceived self esteem(one of the subconcepts of perceived social support). Thus the most important factors influencing the level of adaptation were found to be sex, education, the hardiness character and self esteem. From the above findings, the significance of this study can be delineated as follows : 1) Corroboration of the assumed relationship between the various variables and the adaptation level as suggested in the conceptual model. 2) Support for the feasibility of the cognitive approach for nursing intervention such as hardness character training, counselling and teaching for self-care in the chronic patients.

The purpose of this study was to investigate the effects of case-based learning about infants and toddlers on healthcare department students, using a video in an emergency care environment. A total of 57 students from a healthcare department of D university in J city were enrolled. They were divided into two groups: The experimental group (n=29) and the control group (n=28). This study is pre-post designed with a non-equivalent control group. The experimental group received a 1-week education for a duration of 3 weeks (3 sessions in total) with 180 minutes per session. The control group received a traditional curriculum of lecture. Before and after the education, we measured the knowledge and skill confidence of emergency care toward infants and toddlers, the academic self-efficacy, and problem solving ability. Data collection and intervention were carried out from November to December of 2014. Data were analyzed with x2-test, paired t-test, unpaired t-test with SPSS version 20.0 Program. The experimental group showed a significantly higher improvement of skill confidence of emergency care toward infants and toddlers (P<001), as well as preferred task difficulty among sub-items of academic self-efficacy (p=.029), approach avoidance style (P=.001), and problem solving confidence (p=.040) among sub-items of problem solving ability on preference compared with the control group. In this study, a case-based learning was verified to be an effective teaching method to enhance professional competency of healthcare department students. The findings from this study suggest that a case-based learning using various educational contents should be developed, expanded, and carried out to promote better learning.
