Development of Performance Measures Based on Nursing Process for Admission and Discharge Care (간호과정 적용 평가도구 개발 -입원 및 퇴원 시 간호를 중심으로-)

  • Purpose: This study was conducted to develop standards ensuring nursing process-based care of patient admission and discharge, to develop a performance measurement tool evaluating the care applied according to the standards, and finally to determine validity of the standards and the tool. Methods: The standards and the tool were reviewed by a panel of experts and refined based on the panel's suggestions. Validity of the standards and the tool were examined through surveying a total of 302 hospital nurses. Results: The mean validity scores of the performance measurement standards and the tool were 4.11 and 4.09, respectively, out of 5.00. So the performance measurement standards and the tool in this study were found to be acceptable in evaluating quality of nursing care provided at patient admission and discharge. Conclusion: This result indicates that the performance measurement standards and the tool developed in this study are valid instruments to monitor and improve quality of nursing care for patient admission and discharge.

Development of Nursing Intervention Standards for Home Hospice Patients (가정호스피스 대상자를 위한 간호중재 표준서 개발)

  • Purpose: The purpose of this study is to develop a set of standardized nursing interventions and their associated nursing activities according to the NIC system to guide home care nurses in performing nursing intervention activities for hospice patients. Method: This study was a descriptive survey that first identified frequently used & specialized hospice nursing interventions based on the NIC systems and next validated a set of standardized nursing interventions and their associated nursing activities. One hundred chart records of home hospice patients were used to identify nursing interventions. Also, thirty-nine hospice specialized nurses participated twice using the Delphi technique to test the content validity of the standards. Results: Among the nursing interventions, 19 important nursing interventions and 418 associated nursing activities were selected after two rounds of Delphi technique by hospice specialized nurses. The mean content validity of the final nursing activities was 0.82. In this paper, only the 3 most frequently used nursing interventions and 3 hospice specialized nursing interventions with their associated nursing activities are presented since space is limited. Conclusion: The nursing intervention standards will be a basis for home hospice nurses to improve quality of hospice care for hospice patients.

Development of an evaluation tool for the quality of nursing care in neurosurgical intensive care unit patients (신경외과 중환자 간호의 질평가 도구개발)

  • To meet standards for high quality of care and satisfied customers, an evaluation tool about nursing care is necessary. And, We need to evaluate our practice continuously for the improvement in quality and outcomes. This study was intended to develop an evaluation tool about nursing care in NSICU, and was progressed of 3 steps; development, content validity verification and reliability verification. Data were obtained and analysed from Feb. To April, 2000. Development process of the study was as follows A preliminary list was made item by item on the basis of clinical Experience, literature review and patients' record review. Then the standards, criteria and indicators of preliminary evaluation tool were set by 5 clinical nursing panel, and their content validity was reviewed by 27 ICU nurses. Finally, an evaluation tool was developed and verified the reliability at c-university hospital located in Kwang-Ju. The results of this was as follows 1) The evaluation tool of this study developed 8 standards, 39 criterias and 106 indicators. The standards were divided into two dimensions. One was process dimension which contained 4 standards(26 criteria), The other was outcome dimension which contained 4 standards(13 criteria). 2) the Average content validity of the tool was 3.39 at standards, 3.55 at criteria and 3.51 at indicators. 3) Interrater reliability of the tool is r=.7993(p<.001) & internal consistency reliability ${\alpha}$ is .6031 4) Scores of NSICU Patients who participated in this study were 57 at total mean score, 58 at process mean score and 56 at outcome mean score The evaluation tool developed in this study seems to be useful in evaluation nursing practice appropriately for the improvement of nursing care in NSICU. I hope that this evaluation tool can be used effectively in NSICU as an intervention for the improvement of quality control.

Development of Standards and Criteria for Accreditation of a Baccalaureate Nursing Education Program: Reflections on the Unique Characteristics of the Nursing Profession (간호학 특성을 반영한 4년제 간호교육 평가인정 표준 및 기준 개발)

  • Purpose: The purpose of this study was to determine characteristics of nursing science and identify expected outcomes from baccalaureate graduates, and to develop accreditation standards and criteria reflecting the characteristics of the nursing profession. Methods: A methodological research design was utilized in this study. Related literature reviews and the nursing education program goals and objectives of the 99 nursing schools in Korea were analyzed. A cross-sectional survey research design was utilized to test the validity of the developed characteristics of the nursing profession and their accreditation criteria and standards. The face validity was from the advisory committee and public committee hearing. Results: The characteristics of nursing science were defined with five concepts including humanity, scientific knowledge, professionalism, therapeutic relationship, and facilitating well-being. The expected outcomes from graduates were identified as providing holistic nursing, critical thinking, establishing professionalism and leadership, construction of a therapeutic relationship, and skilled nursing practice. Finally6 standards and 14 criteria reflecting the unique characteristics of the nursing profession were developed for accreditation. These proposed accreditation standards and criteria are a challenge to promote the quality of nursing science.

ICU Nurses'Noncompliance of Critical Care Nursing Standards (중환자실 간호사의 중환자 간호실무표준 불이행에 대한 조사)

  • Purpose: This study was to determine ICU nurses noncompliance of critical care nursing standards in order to provide basic data for education aimed at improvement in practice and evaluation of quality of nursing care in ICU. Methods: Data was collected from 150 nurses who work for three educational hospitals which have more than 800 beds and located in Gyeonggi province using questionnaires from September 2007 to October 2007. Results: The highest categories of noncompliance of critical care nursing standards were admission care($2.71\pm.60$) and infection control($2.31\pm.70$). The main causes of noncompliance critical care nursing standards that nurses regarded as causes were lack of attention(80.7%). ICU nurses mainly reported their noncompliance to charge nurses(89.3%) within 30minutes (84.7%). The reasons they reported were to solve problems rapidly and correctly, to reduce a harm to patients, and to prevent making noncompliance again. The reasons they didn't report were that they thought it might be not a problem and there was no change of patients conditions. Conclusion: ICU nurses noncompliance of critical care nursing standards was determined, therefore it can be used for prevention of further noncompliance.

A comparative study on the 4-year college and 3-year college nursing curriculum (대학 및 전문대학의 간호교육과정 비교 연구)

  • This study analyze and find problems in the 4-year and 3-year college nursing curriculum through comparison and analysis of each college curriculum according to the Nursing Education Standards Criteria from the Korean Nurse Association, 1994. Analyzed areas and results are as follows : 1. Nursing education philosophy To analyze Nursing Education philosophy, we reviewed the categaries of Person, Health, Nursing, Environment. In of 4-year colleges, 50% of them were presenting definitions of Person that were the same as the Nursing Education Standards. But in the definitions of Environment and Health, they presented limited contents. In the 3-year college, we could not find any statement of Nursing Education Philosophy similar to Nursing Education Standards. 2. Nursing education purpose In 4-year colleges, they stated aboict only 3 aspects and others werelacking in correspondance with Nursing Education Standards. In 3-year colleges, some aspects were sincere, but when they were compared to the standards they were very limited. 3. Nursing education curriculum As we analyzed 4-year and 3-year college nursing curriculum, We found that 3-year colleges use curriculum from 4-year colleges in a modified and condensed form. Thus, it is considered that 3-year colleges have considerable burdens because they have to teach all the credits within 3-years that is normally taught in the 4-year college. Therefore, we can suppose that 3-year colleges have more problems in the quality. And, the majority of 4-year and 3-year college curriculums don't include recent concepts such as health promotion, prevention, and rehabilitation.

Criteria for Critique of Qualitative Nursing Research (질적 연구평가 기준)

  • Gradually, there has been increase in the use of qualitative research methods in nursing research. Nursing scholars are using qualitative research methods to explore the essence of nursing and discover its meaning. However, there has been lack of standards for evaluating qualitative nursing research. Often nursing researchers are applying quantitative research evaluation standards to qualitative nursing research. Thus, there has not been any notable qualitative research done to date. In order to improve the quality of nursing research done to discover new knowledge for the nursing, & for development of nursing theory, criteria for critiquing qualitative research should be established. Therefore, this researcher introduced standards as criteria for critiquing qualitative research which are based on literature reviews and re search experiences. The suggested criteria are developed several question's as follows : 1) description of the research phenomena, 2) significance, 3) research purpose, 4) research question, 5) assumptions, 6) researcher's abilities, 7) selection of research samples, 8) data collection, 9) human subjects, 10) data analysis, 11) description of researcher's results, 12) literature review. Follow-up of concrete questions for each of the criterias have been developed.

Relationship between Standard Precautions Knowledge and Standard Precautions Compliance of Nursing Students: The Meadiating Subjective Norms (간호대학생의 표준주의 지식과 표준주의 수행 간의 관계: 주관적 규범 매개효과)

  • This study was conducted to investigate the mediating effect of subjective norms in the relationship between standards precautions knowledge of and of standards precautions compliance in nursing students. The subjects of the study were 195 senior nursing students who completed clinical practice at three universities located in J province. As a result of the study, subjective norms showed a partial mediating effect between standards precautions knowledge and standards precautions compliance. And the explanatory power (R2) explaining the standards precautions compliance was 35%. Therefore, in order to promote the standards precautions compliance in nursing students, it is necessary to develop standards precautions knowledge and prepare effective strategies to induce subjective norms through it.

  • Purpose : this study was to examine the effect of using a nursing standards for the quality of nursing care in musculo-skeletal patients. Method : The approach to nursing quality assurance evaluation was selected process-outcome framework. The subjects ware 100 case of musculo-skeletal patients in orthopedic surgery nursing care unit of E. university hospital in seoul. The data of a control group were collected from 23 December 2001 to 20 March 2002 and an experimental group were collected from 1 June to 25 August 2002. The instruments used for this study were a nursing care standards in musculo-skeletal disorder patients developed by the investigator and an evaluation tool for the quality of nursing care in orthopedic surgery patients developed by soon-ook Choi in 1995. It's reliability is Cronbach alpha=.8628. The data were analyzed by means of $x^1$-test, t-test, ANOVA with SPSS program. Results : This study are as fellows : 1. Scores of the quality of nursing care in standard 1 in the experimental group were higher than those of the control group with no significant difference(t=-8.793, p=.000), scores of the quality of nursing care in standard 2 in the experimental group were higher than those of the control group with significant difference(t=-8.793, p=.000) and standard 3 in the experimental group were higher than those of the control group with significant difference(t=-10.550, p=.000). scores of the quality of nursing care in standard 4 in the experimental group were higher than those of the control group with significant difference(t=-8.793, p=.000). scores of the quality of nursing care in standard 4 in the experimental group were higher than those of the control group with no significant difference(t=-1.833, p=.070). 2. Scores of the quality of nursing care in criteria 28 of 33 criteria in the experimental group were higher than those of the control group with significant difference in 12 criteria(p<.05 ). 3. Scores of the quality of nursing care in standard 1 to standard 4(process evaluation); the experimental group were higher than those of the control group with significant difference(t=-10.704, p=.000). scores of the quality of nursing care in standard 5(outcome evaluation); the experimental group were higher than those of the control group. but no significant difference(t=-1.833, p=.070). 4. Scores of the quality of nursing care in experimental group were higher than those of the control group with significant difference(t=-10.794, p=.000). Conclusion : Through this study, I'm sure that using a nursing care standards in musculo-skeletal disorder patients improve quality of nursing care and nursing care standards are effective nursing care.

A study of the impact of using a nursing care standards on the quality of nursing care in gastrectomy patients (위절제술환자의 간호실무표준 사용이 간호의 질에 미치는 영향)

  • Nursing standards determine the type and extent of services that are delivered to the patients and define quality care and communicate the institution's expectations of care. Thus, taking the standard of care and incorporating it into a welldefined indicator of excellent patient care becomes one of the first activities in setting up the nursing service's quality assurance process. The purpose of this study was to determine the impact of using a nursing care standards for the quality of nursing care in gastrectomy patients. The subjects were composed of fourty-two under going gastrectomy patients with stomach cancer in general surgery nursing care unit of K University Hospital in Pusan. The data was collected from January 3 to April 13,1996. The subjects were divided into a control group - those admitted from Jan.3 to Feb.12 and an experimental group those admitted from Feb.18 to April 13. The instruments used for this study were a nursing care standards in gastrectomy patients developed by the investigator and an evaluation tool for the quality of nursing care in abdominal surgery patients developed by Byoung-Sook Lee in 1995. The data was analized by means of chi-square test, t-test and Cronbach-alpha test with the SAS System. The result was as follows : The hypothesis, that scores of the quality of nursing care in the experimental group would be higher than that of the control group. was supported(t=-6.12, p=0.00). The detailed results of each standards of evaluation tool were as follows : The mean score of the experimental group was significantly higher than that of the control group in audit standard 1:'Collection of basic data of the patients', (t=-3.76, p=0.00). The mean score of the experimental group was significantly higher than that of the control group in audit standard 2 : 'Defining nursing diagnoses(or nursing problems)', (t= (-), p= (-) ). The mean score of the experimental group was significantly higher than that of the control group in audit standard 3:'Estabilishment of nursing care plan according to nursing diagnoses(or nursing problems)',(t= (-), p= (-) ). The mean score of the experimental group was significantly higher than that of the control group in audit stndard 4:'Implimentation of nursing care plan', (t=-2.38, p=0.01). The mean score of the experimental group was significantly higher than that of the control group in audit standard 8 : 'Increase of the knowledge of health related to surgery',(t=-2.40, p=0.01). No significant differences between the mean scores of the experimental group and that of the control group in audit standard 5 : 'Recover and maintain of the physical function', audit standard 6:'Prevention of the post-operative complication', audit standard 7 : 'Decrease of discomfort caused by operation', and audit standard 9 : 'Patient satisfaction in nursing care' were found. The standards of evaluation tool were devided into two dimension. One was process dimension which contains four standards(audit standard 1 to 4), the other was outcome dimension which contains five standards(audit standard 5 to 9). The mean score of the experimental group was significantly higher than that of the control group in process dimension (t=-12.30, p=0.00), but no significant difference between the mean scores of the experimental group and that of the control group in outcome dimension was found. From these results, it is concluded that using a nursing care standards in gastrectomy patients promotes quality of nursing care and nursing care standards of various fields are necessary for effective nursing care.
