긴급치료격리병동의 평면유형 (A study on the Types of Urgent Isolation Ward)

  • Purpose: In response to the rapid spread of COVID-19 in 2020, the government supported facilities and equipment through the 'Urgent Isolation Ward Expansion Project'. Design and remodeling of efficient negative pressure isolation facilities had to be done in a short period of time, and the performance gap between facilities was very large because the types of hospitals and wards of existing medical facilities were diverse. In order to secure the stability of isolation wards between medical facilities and reduce the facility gap, guidelines for planning isolation wards considering the diversity of each hospital should be appropriately presented. In consideration of these points, this study aims to provide basic data for future remodeling guidelines for each plan type of the negative pressure isolation ward first. Methods: We analyzed the plans before and after the change of 13 case hospitals that performed the urgent care bed expansion project for COVID-19 confirmed patients. Before the remodeling, the current status of the facility was analyzed according to the type of corridor, the location of the nursing station, and the location of the elevator. After remodeling, the flow of medical staff and patients, the flow of entry and exit of clean and contaminated items, and the space of negative pressure and non-negative pressure areas. Results: The ward type was divided into three types according to the corridor type and room arrangement: double loaded corridor type with two side wards, race track type with one side ward, and race track type with two side wards. Based on these three types, the standard floor plan type of the isolation ward was proposed in terms of the location of the elevator bank and Nurse station. Implications: When the existing general ward is converted into a negative pressure isolation ward, this study can be a basic data to present customized guidelines for each ward type.

Barriers to Effective Communication about Advance Care Planning and Palliative Care: A Qualitative Study

  • Purpose: The purpose of this study was to identify barriers to effective conversations about advance care planning (ACP) and palliative care reported by health care and community-based service providers in Massachusetts, USA. Methods: This qualitative research analyzed open-ended responses to two survey questions, inquiring about perceived barriers to having conversations about ACP and palliative care with patients and consumers. Data were collected between November 2017 and June 2019 from nine organizations in Massachusetts, including health care provider organizations, health insurers, community-based organizations, and a nursing education institution. Two researchers reviewed and coded the responses and identified common themes inductively. Results: Across 142 responses, primary barriers to ACP included hesitation and lack of understanding and knowledge, discomfort and resistance among service providers, lack of staff knowledge, difficulties with followup, and differences in ACP policies across regions. Common barriers to palliative care were misconceptions about palliative care and lack of knowledge, service providers' lack of preparedness, and limited policy support and availability. Challenges relevant to both ACP and palliative care were fear and discomfort around serious illness discussions, lack of knowledge and awareness, discussions that occur too late, and cultural and language barriers. Conclusion: Health care practitioners and community-based professionals reported consumer-, service provider-, and system-level barriers to facilitating conversations about ACP and palliative care with patients experiencing serious illness. There is a need for more tools and support to strengthen service providers' ACP and palliative care competencies and to promote a structured approach to health care planning conversations.

요양병원 입원급여 적정성 평가 결과를 활용한 요양병원 입원환자의 장기입원 관련 요인 탐색 연구 (An exploratory study of factors related to long-term hospitalization of inpatients using the quality assessment data for long-term care hospitals)

  • Purpose: The purpose of this study was to analyze the factors associated with long-term hospitalized patients in long-term care hospitals using the quality assessment data for long-term care hospitals by the Health Insurance Review. Methods: Among 1,376 long-term care hospitals, frequency analysis and descriptive statistics were used to analyze the characteristics of these hospitals. Multiple linear regression was conducted to examine the associations between infrastructure characteristics, medical personnel characteristics, health outcomes and the proportion of long-term hospitalized patients. Results: The research findings indicate that the number of patients per doctor, the number of patients per nurse, and the number of patients per nursing staff were positively associated with the proportion of long-term hospitalized patients. Among health outcomes, a higher proportion of patients with more than a 5% weight loss compared to the previous month and the proportion of patients showing improvement in ADL, were more likely to have a lower proportion of long-term hospitalized patients. However the proportion of diabetic patients with HbA1c test results within the appropriate range was positively associated with the proportion of long-term hospitalized patients. Conclusion: The present study results provide fundamental data for the establishment of policies for long-term care hospitals. Based on this study, it is important to suggest screening methods for unnecessary long-term hospitalizations, such as sufficient medical personnel to improve the quality of care in long-term care hospitals. It is also necessary to clearly separate the roles of medical institutions and long-term care facilities and implement policies to support patients' social reintegration.

환자안전 전담인력의 환자안전관리 역량강화 방안: 질적연구 (Measures to Strengthen Patient Safety Management Competencies for Patient Safety Coordinators: A Qualitative Research)

  • Purpose: This study aimed to identify strategies to enhance the competencies of patient safety coordinators in Korea. Methods: Fourteen participants from nine hospitals were interviewed between May and November 2022. Qualitative content analysis was used to analyze the data. Results: As for the strategies to enhance patient safety management competency, 3 themes and 11 sub-themes were derived. The first theme was 'Having individual competence as a patient safety coordinator', and the sub-themes were 'Communication skills with members', 'Flexible thinking from multiple perspectives', and 'Preparing for administrative work competencies that they had not experienced as a nurse.' The second theme was 'Responding strategically to promote improvement activities', and the sub-themes for it were 'Multi-angle approach to the problem', 'A careful approach so as not to be taken as criticism in the field', 'Increasing the possibility of improvement activities through awareness', 'Activating the network between patient safety coordinators', and 'Expanding learning opportunities through patient safety case analysis.' The third theme was 'Obtaining support to facilitate patient safety activities', and the sub-themes for this were 'Improving staff awareness of patient safety', 'Providing a training course for nurse professional of patient safety', and 'Expanding the manpower allocation standard of patient safety coordinators.' Conclusion: This study explored personal competencies such as document writing and computer utilization capabilities, focused on ways to improve the field of patient safety management, and emphasized the need for organizational and political support.

심폐소생술금지 결정 시점에서의 임상적 특성: 일개 종합병원 종양내과 사망한 암환자를 대상으로 (Clinical Characteristics of Oncologic Patients with DNR Decision at a Tertiary Hospital)

  • 목적: 본 연구의 목적은 심폐소생술금지 지시에 동의한 시점에서 말기암환자의 임상적 특성을 확인하기 위한 후향적 서술조사 연구이다. 방법: A 종합병원 종양내과에서 2013년 1월부터 12월까지 사망한 환자 중에서 DNR 지시에 동의한 197명을 대상으로 전자의무기록을 조사하였다. 조사 내용은 입원 당시 대상자의 일반적 특성과 임상적 특성, DNR 지시에 동의한 시점에서의 대상자의 상태를 활력징후, 의식상태, 시행 중인 치료였으며, 수집된 자료는 SPSS win 21.0 프로그램을 이용하여 분석하였다. 결과: 대상자의 평균 나이는 58.7세였고, 남자가 121명(61.4%)이었고, 소화기계 암환자가 94명(47.7%)으로 가장 많이 포함되었다. 대상자의 평균 재원기간은 19.61일이었고, DNR 지시에 동의서를 작성한 시점은 입원일로부터 평균 12.95일이었으며, 사망까지 이른 시간은 평균 6.66일이었다. DNR 동의시점에서 대상자의 66.0%가 호흡기계부전을 가지고 있었으며, 불안정한 활력징후를 147명(74.6%)에서 보였으며, 의식수준의 변화가 있는 91명(46.2%)에서 있었다. DNR 동의서 작성 당시 대상자의 의식 상태는 명료 53.8% (106명), 기면 20.8% (41명), 혼돈 16.8% (33명), 혼수 8.6% (17명)이었다. 결론: 대부분 DNR 결정과 관련된 설명이나 의사 결정 시기에서 환자의 의식수준, 활력증후, 산소포화도가 불안정한 상태였으며, 대부분 위중한 상태 또는 환자의 판단능력이 상실된 시점에서 이루어지면서 환자의 참여가 제한되었다. 말기암환자의 자율성 향상 및 적절한 치료 중단 시점을 규정하기 위한 제도적인 장치 마련이 시급하다.

간호사의 태움 개념분석 (Concept Analysis of Tae-um in Nurses)

  • 본 연구는 간호사의 태움 개념분석연구로 개념의 속성을 확인하고 명확한 개념 정의를 내리고자 시도되었다. Walker and Avant의 개념분석 방법을 사용하였다. 간호사의 태움의 속성은 조직문화, 피해자인 미숙한 신규 간호사, 가해자인 선임 간호사, 가르침의 변질, 언어적, 신체적 폭력경험, 심리적, 신체적 증상으로 도출되었다. 간호사의 태움의 선행요인은 간호업무 과다, 높은 노동 강도, 직무 스트레스, 인력 부족, 열악한 근무 환경, 신규 간호사의 업무 능력이 기대에 미치지 못함, 조직의 위계질서, 힘의 불균형이었으며 결과는 신체적, 심리적 증상, 간호업무 효율 저하, 불신 증가, 이직의도 증가, 환자 간호의 질 하락으로 나타났다. 간호사의 태움의 선행요인을 해결하기 위해 활동 간호인력 확충을 위한 근로조건 개선, 사회적지지 프로그램 제공, 의사소통과 상호협력 관계를 지향하는 공동체 문화가 조성되어야 한다. 이후 간호사의 태움에 대한 더 많은 질적 연구와 태움의 속성이 잘 반영된 도구개발 연구, 태움 문화 개선을 위한 프로그램 개발 연구를 제언한다. 본 연구는 태움의 선행요인을 제시하여 해결방안을 제안함으로써 신규간호사 적응 프로그램을 위한 이론적 근거를 제공했다는 점에서 의의가 있다.

혈액투석환자의 사회적 지지와 건강정보이해능력이 치료순응도에 미치는 영향 (Influence of Social Support and Health Literacy on Treatment Adherence in Hemodialysis Patients)

  • 본 연구는 주기적으로 혈액투석을 시행하고 있는 환자를 대상으로 사회적 지지와 건강정보이해능력의 정도를 파악하고 치료순응도에 미치는 영향을 조사하기 위해 시도된 서술적 연구이다. 연구대상자는 지방 Y시에 소재한 2개 종합병원 인공신장실에 등록된 혈액투석 치료를 받은 지 1년이 경과 한, 만 40세 이상 투석환자 140명이며, 자료수집기간은 2019년 11월 1일부터 12월 31까지이다. 수집된 자료는 SPSS/WIN 26.0 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test, one-way ANOVA, Scheffe test, Pearson의 상관계수, 다중회귀분석을 실시하였다. 연구 결과, 사회적 지지 하위영역인 가족 지지는 친구 지지와 의료인 지지에 비해 치료순응도에 더 유의한 영향을 주는 것으로 나타났다. 건강정보이해능력에 있어 세 가지 하위영역인 기능적, 의사소통, 위기 건강정보이해능력 모두가 치료순응도와 매우 높은 상관관계를 보여 건강정보이해능력이 높을수록 치료순응도가 높은 것으로 나타났다. 사회적 지지와 건강정보이해능력은 치료순응도에 유의한 영향을 미치는 변수로 나타났고 69.6%의 높은 설명력을 보였다. 따라서 혈액투석환자의 치료순응도를 증진시키기 위해서는 사회적 지지를 기반으로 건강정보이해능력을 향상시키는 교육을 제공하는 프로그램 개발이 필요하다.

한 대학병원의 수술실 이용 지연요인과 개선방안에 관한 연구 (Delayed use of Operating Rooms in a University Hospital)

  • Advanced surgical technology demands more precise, meticulous, and time-consuming procedures. In addition, the patient's preference of tertiary health providers makes over crowding of the University Hospitals. Therefore, it has been necessary to maximize utilization of the operating room of such hospitals to accommodate these requirements. This study, targeting 1,302 surgical cases performed in 22 operating rooms at a university hospital in Seoul from October 8 to November 1, 2001, analyzed reasons for delay, and factors that caused delayed use of operating rooms. This study also assessed that the rate of operating room use would increase if the sources for possible reform were improved. 1. Among total of 1,302 cases of surgery, the incidence of surgeries in which there were no time delays and no factors for delay were discovered is 71.4% or 930 cases: the incidence in which surgeries were delayed was 28.6% or 372 cases. 2. As results of logistic regression for delay, procedures involving women were delayed 1.4 times more frequently than those of men. Compared to Department A, Department B was 1.8 times more likely to be delayed, and Department H was 0.4 times less likely to be delayed. Regional anesthesia was 2.4 times more likely to be delayed than general anesthesia, and surgeries that PCA was applied were 0.6 times less likely to be delayed than those when it was not. Surgeries performed on the Thursday were 1.7 times more likely to be delayed than those performed on the Monday. Compared to surgeries performed between 07:00-07:59, those performed between 08:00-08:29 were 4.3 times higher. 3. The reasons for delay were related to surgeon, surgical department, patient, anesthesia, administrative system, sick ward, and support services. Among these, 5,755 minutes for 276 delayed cases could be resolved easily, and resolving delays of 3,320 minutes for 131 cases would be more difficult. Among the causes for delay that could be improved, delays due to patient's transfer and surgeon's factor were the most common, 21.6% and 17.4% respectively. 4. If resolvable delays are improved, pre-anesthesia room is administered, and regional anesthesia and PCA are done ahead of time, use of emergency operating rooms will increase, we can increase overall utilization by 4.09%, we will save 744 minutes a day, we can reduce the time the operation room is used after 4 PM by 35%, and we can resolve the operation cancellations due to insufficient operating rooms. For the increase in the use of operating rooms, we need to maximally decrease the delays that could be improved, by allocating block time based on used totals hours of elective cases, giving accurate information on surgery schedule, voluntary cooperation by staff participating in surgeries in reducing delay time, and the hospital management's will to improve delay.

병원간호사의 인지적 정서조절전략이 전문직 삶의 질에 미치는 영향 (The Effects of Cognitive Emotion Regulation Strategies on the Professional Quality of Life in Hospital Nurses)

  • 본 연구는 병원 간호사의 인지적 정서조절전략과 전문직 삶의 질의 정도를 확인하고 전문직 삶의 질에 영향을 미치는 요인을 알아보기 위해서 수행되었다. 연구 대상자는 서울 소재 일개 대학병원에서 근무하고 있는 일반 간호사 176명으로, 2020년 3월 3일에서 16일까지 구조화된 온라인 설문지를 통하여 자료수집 하였다. 자료 분석은 SPSS WIN version 25.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson's correlation coefficient, Multiple regression analysis 로 분석하였다. 연구 결과, 공감만족에 영향을 미치는 요인은 적응적 인지적 정서조절전략(β=.501, p<.001)이었고, 설명력은 24.8%였다. 소진에 영향을 미치는 요인은 적응적 인지적 정서조절전략(β=-.435, p<.001), 부적응적 인지적 정서조절전략(β=.427, p<.001), 학력(학사)(β=.188, p=.046)이었으며, 설명력은 36.8%였다. 대상자의 이차 외상성 스트레스에 영향을 미치는 요인으로는 부적응적 인지적 정서조절전략(β=.502, p<.001)이었으며, 설명력은 24.4%였다. 이상의 연구결과를 토대로 적응적 인지적 정서조절전략을 습득하고 부적응적 인지적 정서조절전략을 덜 사용하도록 하는데 초점을 맞추어 병원 간호사의 전문직 삶의 질 향상을 도모하기 위한 프로그램 개발이 필요하다.

병원 이용자의 의료서비스 만족도 조사 (Patient's Satisfaction with Medical Care Services in Hospital)

  • The purpose of this study was to determine factors influencing patient satisfaction with medical services in hospital, which is classified into environmental aspect, human services and procedural services. Based on the results of literature review, the study focused on effects of social-demographical factors on patient satisfaction. The environmental aspect of medical care services included medical equipment and facilities, hygiene, ventilation, heating and air-conditioning, waiting and resting space, ward space and parking facilities. Procedural service included registration process, bill payment, waiting time after registration, examination and prescription as well as appointment process. Human services consisted of physicians listening to stories of patients, examination duration, physicians' explanation and physicians' service. As for nurses, explanation about disease, examination procedure and results, kindness and nursing care were evaluated. Services provided by other staff members were also evaluated. Patient satisfaction, defined as individual attitude toward medical service as a whole, was measured using a questionnaire. A total of 700 in-or out-patients were surveyed in 6 hospitals with more than 300 beds in North Gyeongbuk Province. 1. The level of patient satisfaction varied with characteristics of patients. Male patients and those in their 30s had a low level of satisfaction. Dissatisfaction level was positively related to education level but negatively related to economic condition. 2. As for patient satisfaction with medical service providers and other employees in hospital, satisfaction level with physician's explanation about treatment was higher. But dissatisfaction levels with treatment duration and the lack of explanation about examination procedures were high, calling for improvement. Dissatisfaction level with nursing care was high, calling for training of nurses for better service. Given the low level of satisfaction with human services, hospital employees need to be trained to improve their service. 3. It Was found that administrative service was also a significant factor influencing patient satisfaction in addition to medical service. It is therefore important for hospitals to provide patients with prompt and convenient procedural service. 4. Environmental factors such as medical equipment and amenity facilities also affected patient satisfaction. Thus environmental condition, procedural service and human service are all important to improve medical service in hospital. In summary, procedural service was the most significant factor for patient satisfaction. The level of satisfaction in patients was also affected by human service and environmental condition. It is therefore necessary to take patient-oriented approach in providing medical service in an effort to improve patient satisfaction. The finding of a lower level of satisfaction with human service signifies the need for training of healthcare providers and other hospital employees for better services. The introduction of advanced management programs is also needed to improve procedures that patients go through in hospitals.
