This study attempts to investigate the relations between the types of communication, the job satisfaction. and the nursing performance of the staff nurses. The subjects of this study were the 114 nurses working in general wards, intensive care units, and the emergency rooms of the general hospital in Kyongbuk province. They were asked to answer the questionnaires as regards the types of communication and job satisfaction, and their performance of nursing, and these were evaluated by the head nurses, and their direct seniors. The data were collected for 10 days from the September 17 to 26, 2002. According to the purpose of the study, descriptive analysis, ANOVA. Pearson correlation coefficient, and stepwise multiple regression were applied for the data analysis using SPSS 10.0. The results are as follows : 1. The types of communication of staff nurses showed that the upward communication was $3.12\pm0.56$. the downward communication was $3.56\pm0.42$. the horizontal communication was $2.98\pm0.44$. and the informal communication was $2.59\pm0.49$. It showed that the most frequent communication was the downward communication. 2. In terms of the job satisfaction, there was significant difference according to the length of the period working for one ward (F=5.95, p=0.003), 3. The performance of nursing was significantly different according to the age (F=23.52, p=0.000), the working duration as a nurses (F=21.9, p=0.000), the types of nursing unit (F=5.33, p=0.002), and the working duration in present nursing unit (F=15.87, p=0.000). 4. In terms of communication type, the job satisfaction was associated with quality of the upward communication (F=6.23, p=0.003), the downward communication (F=5.78, p=0.004), and the horizontal communication (F=11.73, p=0.000) There were no association between the types of communication and the performance of nursing. 5. There was a positive correlation between the communication and the job satisfaction. And the downward communication (r=0.398) showed the greatest correlation with the job satisfaction. There was no statistically significant correlation between the communication and the performance of nursing. And the upward communication (r=-0.058) showed a negative, though not significant correlation with performance of nursing. 5. 본 연구는 국내 요양병원의 의료서비스 품질과 관계의 질 및 환자 충성도 간의 영향 관계를 검증하는 데 목적이 있다. 이를 위해 총 324명의 환자를 대상으로 설문조사를 실시하였으며, 조사 결과는 다음과 같다. 첫째, 요양병원 의료 서비스품질이 관계의 질에 미치는 영향을 분석한 결과, 의료 서비스품질변수 중에서 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판은 관계의 질에 유의미한 정(+)의 영향을 미치는 것으로 나타났으며, 진료비는 유의미한 영향이 없었다. 둘째, 요양병원 환자와 병원 간의 관계의 질이 환자 충성도에 미치는 영향을 분석한 결과, 관계의 질 변수 중에서 신뢰와 관계몰입은 모두 환자 충성도에 유의미한 정(+)의 영향을 미치는 것으로 나타났다. 셋째, 요양병원 의료 서비스품질이 환자 충성도에 미치는 영향을 분석한 결과, 의료 서비스품질 변수 중에서 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판은 환자 충성도에 유의미한 정(+)의 영향을 미치는 것으로 나타났으며, 진료비는 유의미한 영향이 없었다. 넷째, 요양병원 의료 서비스품질과 환자 충성도의 관계에서 관계의 질의 매개효과를 분석한 결과, 요양 진료비를 제외한 요양시설장비, 요양의료진, 요양서비스시스템, 요양시설 평판 모두 환자와 병원 간의 관계의 질을 매개로 하여 환자 충성도에 긍정적인 영향을 미치는 것으로 나타났다.

Background: Cervical cancer is a major preventable cancers. The, current study aimed to assess relevant knowledge and attitude of female students and hospital staff in Iran. Method: This cross-sectional study was conducted in Medical and Nursing faculties and hospitals of East-Azerbaijan Province of Iran. Participants were medical and paramedical female students and female staff in hospitals selected by stratified random sampling techniques. Tools for data collection were questionnaires for which validity and reliability had been verified (${\alpha}=0.8$). Descriptive and inferential statistics were used to analyze data with SPSS.16. Result: Response rates were 71 % (426 from 600) and 63.5% (254 from 400) for students and staff, respectively. Some 29.1% admitted that they had no information about cervical cancer, only 70 (10.3%) thinking their knowledge as high, 360 (52.9%) as intermediate, and 237 (34.9%) as low. While 93% of participants considered cervical cancer as a severe health problem, the only statistically significant relationships with knowledge were for education (p<.001) and occupation (p<.001) variables. Conclusion: Given the importance of the roles of medical students and personnel as information sources and leaders in health and preventive behavior, increasing and improving their scientific understanding seems vital. Comprehensive and appropriate education of all people and especially students and personnel of medical sciences and improving attitudes towards cervical cancer and its monitoring are to be recommended.
The study is to identify the status of the ward of public hospital for integrated nursing care service. Integrated nursing service has expanded nationwide from 2016 but 10 out of 34 public hospitals are currently unable to operate as nurses due to the shortage. Analyze each floor plan and space program of ward and provide the basic data for the construction of the architecture plans for Integrated nursing care service. For this study, selecting 14 wards of public hospital in Korea and then analyzes space organization, distance from NS to bedroom and NS to core(Elevator) and spatial hierarchy of them. Among the 14 public hospitals, there are 7 types of double-loaded corridor, 4 types of double corridor and 3 types of single corridor. If NS is placed centrally, the control of the outsider and the patient's control would be efficient, but most of NS are lean on one side of floor. the placement of NS should be independent of individual arrangements, but it should be smooth alignment with patient spaces, staff spaces, and public spaces. This study could serve as basic research for the architectural plan for future integrated nursing care in ward of public hospital.
Purpose: The relationship between the staff area and the patient's private area is the key factor in designing the structure and the environmental characteristics of ward spaces in a psychiatric hospital. Recent research has found that for the purpose of treatment and securing privacy, psychiatric patients need to be in an open space of relief rather than closed confined environment and under the watch of nursing staffs. Methods: A survey at three kinds of wards in a private psychiatric hospital in Japan was conducted in October 2002. These wards include an acute ward, a psychiatric convalescence ward, and a stress care ward. All three kinds of wards have the same structure. At each ward, spatial preferences of the 145 psychiatric inpatients were surveyed and data concerning the patient's diagnostic category, symptoms, and activities of daily living were recorded. Results: The patients in the stress care ward prefer to stay in private spaces than public spaces. On the other hand, in the acute ward the patients seem to have a preference between managed public spaces where are monitored by nursing staffs and their private rooms where the nurse station is close. In addition, the patients in the psychiatric convalescence ward spend most of their time in the public space, such as the hallways or the day room. Implications: Base on this research, the spaces at the acute ward that could be monitored by the nursing station serves effectively as a safety space for patients was concluded. However, in the stress care ward, the patients may perceive the monitoring by the nursing staff as interruption or nuisance to their relaxation. In order to design an ideal healing environment for psychiatric patients in psychiatric ward, it is important to consider how environmental characteristics of space affect the environmental sense of patients in each ward.
Purpose: The purpose of this study was to examine the effects of structured nursing intervention for caregivers on maintenance of intravenous (IV) infusions in infants. Methods: The structured nursing intervention was developed through the processes of interviews with nurses from pediatric wards. To identify the effects of the developed nursing intervention, a non-synchronized non-equivalent control group pretest-posttest design study was conducted with caregivers from the pediatric ward from a hospital in Seoul. Of 100 admitted infants, 50 caregivers were assigned to the intervention group and received the structured nursing intervention for maintenance of intravenous infusions in infants. The others were assigned control group and received routine care. Data were collected on patient factors, IV insertion factors, treatments, and IV related complications. The form developed for the structured nursing intervention was used by staff nurses. Results: Compared to the control group, IV insertion frequency in experimental group infants was significantly lower and IV related complications decreased (p<.05). Conclusion: These results suggest that the structured nursing intervention for caregivers on maintenance of intravenous infusions may have effects on maintenance of intravenous infusions in infants, and decreasing IV related complications. This nursing intervention can be used to improve IV related problems of admitted infants.
Purpose: To analyze the job of nursing unit managers working at women's hospital, using DACUM (developing a curriculum), DACUM is a method for analyzing job-focused competency. Methods: This study involved a descriptive survey. A DACUM workshop was held to define women's hospital nursing unit managers' role and identify their duties and tasks. For the workshop, a committee was formed consisting of 5 women's hospital nursing unit managers. Finally, after validation, the developed contents were made into a survey asking about nursing unit manager's duties and tasks. Results: Sixteen duties and 83 tasks were identified on the DACUM chart. The importance, difficulty, and frequency of the tasks were ranked in terms of A, B, and C, with A being the highest degree. Eight tasks received A's all in importance, difficulty, and frequency of performance. The 8 tasks were: 'taking over', 'taking care of seriously ill patients on handover', 'ward rounding', 'analyzing and resolving demands identified during handover and patient tour', 'reporting patient status during rounding', 'promoting breast-feeding', 'uterine contraction, and training for breast-feeding'. The duty with the biggest determinant coefficient (DC) was 'patients complaint management' (DC=7.09). Based on tasks, the one with the biggest DC was 'solving patient and patient guardian's complaints' (DC=7.53), followed by 'making infection control guidelines' (DC=7.5). Conclusion: When expanding the nursing staff of the hospital, women's hospitals nursing unit managers also need to use administrative functions as intermediaries to focus on the operation management of the entire hospital rather than direct nursing to suit their role.
Purpose: The purpose of this study was to analyze the effects of self-efficacy and collective-efficacy on job stress in the nursing staff. Method: This study surveyed 160 nurses in three general hospitals in the Seoul and Gyung-gi province for two months starting in September 2004. The questionnaire consisted of 54 questions about job stress, 10 questions about self-efficacy, and 7 questions about collective-efficacy. The answers were on a scale rating of 5. The answer sheets were analyzed with descriptive statistics, the t-test, ANOVA, the tukey test, the Pearson correlation coefficients and stepwise multiple regression using SAS version 8.2. Result: The average job stress rating of the nurses was 3.11. The average self-efficacy and the average collective-efficacy were 3.41 and 3.39, respectively. The age, working department, income level, shift-work and hospital have influence on job stress. Efficacies such that self-efficacy and collective-efficacy have influence on job stress. The much efficacy makes the less job stress. The stepwise multiple regression revealed that the significant predictor of job stress was working department and hospital. Conclusion: This study showed that collective-efficacy as well as self-efficacy reduces job stress, so nursing intervention methods should promote collective-efficacy. The collective-efficacy improvement program should be developed to improve job performance, to improve cohesion of nursing units, and to improve satisfaction on the job. The next research could be to develop collective-efficacy improvement programs for nursing units.
Purpose : The purpose of this study was to analyze the trend of the number of nursing staffs and skill mix and to assess the effectiveness of hospital nurse expansion policies in Korea. Methods : The trend of the number of nursing staffs and skill mix were analyzed using time series data, which composed of yearly series data from 1975 to 2009. The impact of hospital nurse expansion policies was estimated by autoregressive integrated moving average(ARIMA) intervention model. Results : The number of general hospital and hospital nurses per 100 beds was decreased in late 1980s and late 1990s due to rapid growth of beds. As a result of the number of nurse aids per 100 beds decreased, skill mix became high in general hospital but nurse ratio among hospital nursing staffs was about 50%. Expansion of new nurse and revised differentiated inpatient fee were only effective in expansion of hospital nursing staffs. But they had no effect in general hospitals. Conclusion : In Korea, a few policies related to expansion of hospital nurses have an effect on increasing the number of hospital nurse. Nevertheless, level of hospital nursing staffs is inferior to that of general hospital.
The purpose of this study is to analyze the characteristics of hospital organization structure, organization culture, organization conflict and the interrelation of influential factor in organization conflict. The data for this study were collected through a self-administered survey with a structured delivered to 755 subjects including several nursing staff members, administration staff members and medical technicians at six hospitals. The structural equation model was used to examine the relation of organization structure, organization culture and organization conflict. The results of the analysis in this study were as follows. First, organization structure had a positive influence on organization conflict, especially in terms of complexity and centralization. But, formalization had a negative influence on organization conflict. Second, organization culture had a positive influence on organization conflict, especially in terms of group culture, developmental culture, and hierarchical culture. But, group culture had a negative influence on organization conflict. Third, organization structure had positive influences, especially on organization culture complexity, centralization, and formalization. But, centralization had negative influences too.
