Purpose: The purpose of this study was to present the outcomes of the district visiting nursing services. Method: We analyzed nursing records and inspected public health center data for evaluating nursing tasks. and conducted a telephone survey of 651 district inhabitants. Results: According to the result, the coverage of the district population, the rate of new registration and overall program activities increased. Meanwhile, accessibility of visiting health care, the level of health problem management, personal cognition and satisfaction concerning visiting nursing care increased, but there was little change in personal perception of health improvement. Furthermore, the status of client management was improved. In the analysis of participation in national cancer screening program, we found that there was twice higher participation in the district than others. Conclusion: We suggest that this district visiting nursing system takes responsibility for district inhabitant health management. Based on the study, we also suggest that the government and local autonomous entities should increase administrative and financial supports to execute the district-based visiting nursing system in wider areas.
Purpose: The purpose of this study was to identify the current management status and the policy issues of visiting nursing programs at public health center in urban area. Method: A questionnaire surveys was conducted on all public health nurses in 25 health centers in Seoul. Result: The major results were as follows: The characteristics of the visiting nurse showed that the average age was 41.1 years, and total nursing experience was 122 months. Their visiting nursing experience was only 30 months. The recipients of visiting nursing services consisted of 60.1% females, and 32.5% of the recipients were over 65 years. The major health problems of the recipients were hypertension, diabetes mellitus, arthritis and psychiatric problems. The visiting frequency was an average of 5.4 times per day, and 357 families were managed per visiting nurse. The major problems of visiting nursing programs were the shortages of visiting nurse manpower. The most important related policy issue was being in charge of a workload, which was not related to the visiting nursing services. Conclusion: By analyzing work for visiting nurses and identifying weak points, this research can present recommendations to be practically used as baseline data for establishing policy in relation to vitalizing visiting nursing programs.
Purpose: The study aimed to identify plans to revitalize occupational health nursing at industrial worksites. Methods: First, a literature review was conducted to derive the questions. Next, we used the Delphi method with two rounds to obtain experts' opinions. The 15 expert participants were seven occupational health nurses and eight professors from nursing colleges. Results: The analysis of opinions indicated that occupational health nurses should be competent in clinical nursing care to perform health management and health promotion activities of workers. It is necessary to develop high-quality occupational nursing services that can prevent and manage occupational diseases and work-related illnesses. Moreover, an improved system for stable employment of these nurses should be implemented. Conclusion: This study confirmed that occupational health nursing is an independent and important area for improving workers' disease prevention and health promotion. It will provide basic data for initiating occupational health nursing and expanding the role of the occupational health nurses.
Purpose: This study used MDS-HC 2.0 to identify health problems in the elderly who live alone. Method: The subjects of this study were 100 living-alone elders aged over 65 in Daegu city. This study was performed through home visiting interviews during the period from the 1st of September to the 30th of September 2005. Analysis in this study was made using SPSS Version 11.0. Results: Frequent health problems in the living-alone elderly were: preventive heath measure (90), health promotion (80), social function (78), visual function (75), depression and anxiety (61), pain and communication disorder (60), cognition (55), environmental assessment (48) and oral health (45) in order of frequency. Informal services were provided to 73% of the subjects for emotional support. IADL services were provided to 57% of the subjects, and ADL services to 66%. Conclusion: Nursing intervention programs based on health problems from MDS-HC 2.0 should be provided in order to cope with individual health problems and living-alone elders needs. The results of this study suggest that MDS-HC 2.0 is applicable to help decide nursing interventions for the elderly who live alone in community.
Purpose: The purpose of this study is to identify clubhouses general characteristics, core services, funding sources and costs in Korean Clubhouse Model, and to compare with Korean and international clubhouses. We explored the annual budget, cost per member, and cost per visit for 1 year. Methods: The data were collected from 14 Korean clubhouses and analyzed using descriptive statistics and Spearman's rank correlation with the SPSS 14.0 program. Results: The average of clubhouse operating period was 8.2 years. There were an average of 40.4 active members; among them, 84.1% were schizophrenia. In addition, there were an average of 5.8 staff and 15.3 services in each clubhouse. Cost estimates were as follows: annual budget (excluding housing) $223.633, cost per member $5,704, and cost per visit $21.35. There were significant difference among the annual budget, number of staff, number of service, and active members, but hours of Work-Ordered Day and social activities hours were not statistically significant. Conclusion: Findings provide a more understanding of operations, programs, and costs of Korean clubhouses.
The Purpose of this study was to explore the condition of periodic medical examination and the health care services of industries in order to offer some basic data on developing industrial nursing care. To achieve this goal a self - administered questionnaire (developed by the academic affairs of community health nursing) was provieded to the nurses in 56 industries from Dec. 10, 1992 to Jan. 20, 1993. The statistical computer package, SPSS, was used to manipulate the data along with T-test and ANOVA. The results were as follows : 1. General characteristics: The greater part of the industries were manufacturing company, and below 300 employees of industry were 55.4%. The shift system was mostly one shift(66.1%) and three shift(23.2%), and 50.0% them organized the Industrial Safety and Health Commitee. Average a number of employees was 631 person. 2. Periodic Medical Examination: Most of the workers were receiving periodic medical examination from the designated hospital (95.71%). From the about 12.89% were gone through a colse medical examination. In colse medical examination 58.41% were decided 'C' and 36.73% were decided 'D'. About 6.23% off those who had any clinical findings were work-time shortening(7.84%), work-transition(8.12%), recoverating at home. The majority of the workers receive the result of the periodic medical examination individually (78.5%). 3. Special medical examination: The rate of those who are receiving special medical examination were 76.82% and about 8.24% were decided 'C' and 1.23% were decided 'D' Those who had any health problems were receiving follow-up checking (9.10%) and medical treatment while working (15.04%). The health managers in the company can consult (85.7%) those who had any suspicious sign and symptoms of occupational disease. 4. Health care services: The average score of health care services were 17.57 point out of 28 point, and the score was lower in health assessment and environmental hygiene than medical diagnosis and health education. There were significant differences in environmental hygine (F=3.72, P=0.017), health care services(F=3.94, P=0.013) according to the size of the size of the industries The other's significancy is not shown by any type of industrial nurse. The level of health care services were higher in the wokers who had better health and showed no singificant differences(T=-0.73, P=0.470).
Purpose : The purpose of this study was to analyze the nursing care needs which can properly cope with patients' needs in emergency room, and grasp the patients' satisfaction level which can assess the care quality. It was aimed to check the level of today's care and provide basic data for care performance and care standard. Method : Data were collected from 192 patients in general hospitals in Daegu. The results are as followed. Results : First, the result of analysis conducted on the degree of overall patients' need for nursing care revealed that the need for nursing information (4.21+57) was the highest, followed by the order of information of patient's condition (4.05+53), emotional support (3.98+47), and nursing participation (3.65+54). Secondly, our study revealed that there was a significant negative correlat interrelation between care need and care satisfaction (r=.516, p<.01), indicating the higher the overall emergency room patients' nursing care need, treatment and nursing information, patient's condition, emotional support and nursing participation were, the lower the actual feeling of their satisfaction turned out. Conclusion : The study shows that treatment and nursing related information is the most highly needed and the patients' satisfaction level relies on actual care participation.
Purpose : For improvement of nursing services in ambulatory unit, ambulatory nursing activity was identified and the required time by nursing activity was measured. Method : The data were collected from 4 tertiary hospitals and 2 secondary hospitals. The method of data collection was used the self-reporting method. The 123 nurse-work days and 137 nurse-aid-work days in 6 hospitals were analyzed. Results : As a result, 11 ambulatory nursing domains and 79 ambulatory nursing activities were confirmed. Above 50% of subjects performed the general outpatient affair domain. The percentage of total time spent in each domain were as follows : general outpatient affair domain, 58.3%, patient education ${\cdot}$ counseling domain, 10.8%, therapeutic care domain, 8.6% and so on. And the average hours were measured by activity and domain. Conclusion : For the improvement of ambulatory nursing care, the strategies that keep the legal standard of number of ambulatory nurse and exchange the nursing-aid for the nurse is needed. And it is also needed to standardize the ambulatory nursing services.
Purpose: This study was conducted to survey children's health status and need of customized visiting health care services in one province. Methods: The participants in this study were 237 caregivers of infants and preschoolers. Data were collected at the participant's home or public health center. Results: Many of the children did not receive developmental screening tests or dental examinations. In the beneficiary group, the prenatal checkup rate and children's vaccination rate were lower, and caregivers had more health problems than the other groups. On the assessment of home safety, unsafe conditions were more frequently found in the beneficiary group. The caregivers in the beneficiary group showed lower child rearing confidence than the other groups, and wanted customized visiting health care services most in the areas of developmental screening, regular health check-up and counseling, nutritional supplementation, and oral health care. Conclusion: These results indicate that it is necessary for children and parents in poverty to be provided with professional home visiting interventions for the promotion of child health and prevention of developmental problems and diseases. These findings can be used for developing future customized visiting health care service programs for infants and children in this community area.
Purpose: This study was performed to identify the key elements for the improvement of healthcare services for foreigners in Korea. Method: Delphi technique was used for this study. As the members of an expert panel for this study, 32 healthcare professionals, who were physicians, nurses, administrators, and care coordinators, with at least 6 months of experiences in international clinics and healthcare services in five metropolitan areas in Korea participated. Data collections were conducted three times from August to October, 2009. The priority and the importance were analyzed using descriptive statistics in SPSS Win 15.0. Result: The key element selected most frequently by the experts was the 'Healthcare providers' abilities for foreign languages' followed by 'Guidelines for facing medical accidences and disputes', 'Information and guide for healthcare services written in English', 'Informed consent preventing medical disputes', 'System of healthcare service fees for foreigners'. Conclusions: The key elements for the improvement of healthcare services for foreigners in Korea were mostly the requirements for effective communication with the foreign clients and the systemic support. The key elements identified in this study can be applied usefully for the development of strategies to improve the quality of healthcare services for foreigners.
