Development and Evaluation of a Nursing Handoff Protocol for Intensive Care Units (중환자실 간호 인수인계 프로토콜의 개발 및 평가)

  • Purpose: This study aimed to develop a nursing handoff protocol for intensive care units and test its relevance. Methods: This is a methodological research to develop a protocol. A preliminary protocol was developed by composing items and testing content validity through literature review and experts' review. We revised and complemented the preliminary protocol following practical relevance assessment of 38 intensive care unit nurses at a university hospital to test content validity and to assess practical relevance of the final protocol. Results: On the basis of the content validity test for the final protocol, 40 items were adopted. The scores for the practical relevance of the final protocol increased significantly for items such as accuracy of handoff, reduction of handoff-related errors, convenience in using the protocol, reduction in handoff time, and simplification of handoff. Conclusions: The nursing handoff protocol for intensive care units in this study is expected to improve nursing performance with a standardized handoff in intensive care units, promote patient safety, and improve communication among the medical staff.

Evaluation of a Self-efficacy-based Basic Life Support Program for High-risk Patients' Family Caregivers (자기효능 증진 기본생명소생술 프로그램의 효과 평가 -심정지 고위험 환자 가족을 대상으로 -)

  • Purpose: The purpose of this study was to evaluate a Self-efficacy-based Basic Life Support (SEBLS) program for high-risk patients' family caregivers on cardiac arrest. The SEBLS program was constructed on the basis of Bandura's self-efficacy resources as well as the International Liaison Committee on Resuscitation's '2000 Guidelines for CPR and ECC'. Method: The effect of the SEBLS program on emergency response self-efficacy and emergency response behavior such as BLS(Basic Life Support) knowledge and BLS skill performance was measured by a simulated control group pretest-posttest design. Study subjects were38 high-risk patients' family caregivers(20 experimental subjects and 18 control subjects) whose family patients were admitted to a general hospital in Incheon, Korea. Result: 1. Emergency response self-efficacy was significantly higher in the experimental subjects who participated in the SEBLS program than in the control subjects. (t=8.3102, p=0.0001). 2. For emergency response behavior, BLS knowledge (t=5.6941, p=0.0001) and BLS skill performance (t=27.8281, p=0.0001) was significantly higher in experimental subjects than in control subjects. Conclusion: A SEBLS program can increase emergency response self-efficacy and emergency response behavior, and could be an effective intervention for high-risk patient's family caregivers. Long-term additional studies are needed to determine the lasting effects of the program.

Development and a Psychometric Evaluation of Cardiovascular Disease-Specific Quality of Life Scale for Koreans (한국 심혈관질환 특이형 삶의 질 측정도구 개발 및 평가)

  • Purpose: Health-related quality of life (HRQOL) in patients with cardiovascular disease in Korea has rarely been studied, mostly due to the lack of a psychometrically validated disease-specific instrument. The purpose of this study was to develop and validate a cardiovascular specific-HRQOL questionnaire (CD-QOL). Method: The CD-QOL was developed and validated as follows; item generation, pilot study, and psychometric tests. Patients were recruited from three-university hospitals. The patients were asked to complete the preliminary questionnaire comprising the content-validated items, SF-36, and CES-D. The NYHA and KASI classifications were used to classify the functional performance of the patients. The data was analyzed using correlation, factor analysis, multidimensional scaling, multitrait/multi-item matrix, ANOVA, and Cronbach's alpha. Result: Preliminarily, thirty-nine items were generated. Factor analysisextracted a five-factor solution with a total of twenty-two items. One item was deleted based upon the MDS. The remaining items were moderately correlated with the subscales of the SF-36 and associated with depression measured with the CES-D. The mean scores of patients in NYHA and KASI class I were significantly higher than those in NYHA and KASI class II or/and III, which suggested patients with better functional performance were likely to have a better HRQOL. Cronbach's alphas of the total and subscales were all greater than 0.70. Conclusion: The CD-QOL is a easily applicable instrument with excellent psychometric properties of content, criterion, factorial, convergent, and known-groups validity, and internal consistency reliability in Korean patients with cardiovascular disease.

A Correlational Study on the level of Importance & performance of postpartal Care and its Relationship with Women's Health Status (출산경험 여성의 산후관리 중요도, 수행정도 및 건강상태와의 관계 연구)

  • This correlational study sought to find the relationship among women's health status and the level of importance & performance of postpartal care. One hundred thirty three women who live in Seoul and rural area including hospitalized in a general hospital and midwifery clinic were studied from 1st April, 1998 to 25th April, 1998 for 25 days. Data analysis consisted of frequency, percentage, Pearson Correlation Coefficiency, t-test, ANOVA and Sheffe test as a post hoc, using SPSS. The results of analysis were as follows ; mean age of respondents was 31.9 years and mean number of children was 1.8. The most of family type was nuclear family and lived int apartment. Mean frequency of pregnancy was 2.7 times and most women delivered at local clinic, general or University hospital. Mean period of after delivery was 53.7 month. The level of importance and performance of traditional postpartal care (Sanhujori) was more higher than hospital postpartal care. The level of importance, performance of postpartum care and health status had significant relationship. The higher level of importance was, & the higher level of performance was higher, and the higher degree of health status. The factors related to health status were postabortal sahujori period after abortion, nuclear family, the evaluation of sahujori, whether women and followed the caregiver's advice well or not and whether they have physical symptoms or not, at the level of $5{\sim}0.1%$ of significance statistically. The factors related to the level of importance of postpartal care were the number of child, present health status and health status of pre-post of delivery, deliver place and the opinion of effective postpartum care method at the level of $5{\sim}0.1%$ of significance statistically. The factors related to the level of performance of postpartal care were delivery place, the opinion of whether they can do at the hospital or not and whether they have physical symptom or not at the level of $1{\sim}0.1%$ of significance statistically. In conclusion, this finding reconfirmed the relationship among women's health status and the postpartal care. It provides a challenge to the health professional caregivers to research continually and repeatedly and confirm the conceptual model of Sanhujori, reestablish effective and integrative postpartal caring system which contains oriental and western paradigm for women's life long health toward the 21C.

A Study on the Operating Status of Community-based Home Health Care Centers (일개 지역사회 중심 가정간호사업소의 운영실태 및 운영방안)

  • Purpose: This study was done to evaluate the operating status of community-based home health center for revitalization of the centers. Method: In this study performance data including organization plan and service utilization plan were evaluated according process theory. Target of evaluation was the community-based home health care center. Results: The vulnerable part of the organization was the information system and financial resources. The home health center introduced PDA (Personal Digital Assistants) in 2005, however home health care nurses did not make full use of it. This service received full support from Seoul city and local government and there were no other sources of income. The vulnerable part of service utilization was service expansion and standardization due to vulnerability of organizational aspects. Conclusions: The home health care center provides high quality services to underprivileged people. In the future, these services should be provided with equity for continuous health care for this population.

Evaluation of Organizational Effectiveness as to Types of Maternal-Child Health Management Program in Public Health Centers (보건소 모성과 어린이 건강관리사업의 조직유형별 효과성 평가)

  • Purpose: The purposes of this study are; to classify executive organizations engaged in MCH programs at 23 advanced public health centers according to each characteristic; evaluate the effectiveness of organizations engaged in MCH programs. Method: This study analyzes me data on the present conditions and evaluates the organizational effectiveness of MCH workers at 23 advanced public health centers. The organizational effectiveness of MCH organization is assessed by both MCH workers and clients who have received MCH services at the three health centers selected from me 23 advanced public health centers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Results: The results are as follows. The executive organizational systems for MCH services are classified into three types: Type I (provides MCH and HP services by MCH org.); Type II(provides MCH and HP Services by HP org); and Type III(provides MCH Services by MCH org. and HP services by HP org.) at leading MCH Health Centers. The evaluation of the organizational effectiveness of MCH org. shows that Type II is the most effective in terms of teamwork, autonomy, service quality, and resource utilization. According to clients' evaluation of organizational system in the field of MCH, Type II is also me most effective. Conclusion: This study suggests that the executive organization for the MCH and HP services for maternity and children should be established in accordance with lifetime health programs in order to efficiently utilize the limited MCH and HP resources at public health centers.

Development of Job Satisfaction Scale for Clinical Nurses (임상간호사의 직무만족 측정도구 개발)

  • Purpose: This study was performed to develop the Job Satisfaction Scale for Clinical Nurses (JSS-CN) and verify its validity and reliability. Methods: A preliminary 42-item version of the JSS-CN was developed through literature reviews and in-depth interviews. The draft scale was developed using thirty-seven items selected following content validity evaluation. Finally, thirty-three items with response options on a 5-point Likert scale were selected based on internal consistency reliability and construct validity. Subsequently, the test-retest reliability and convergent validity of the JSS-CN were verified. Results: Six factors, namely, recognition from the organization and professional achievement, personal maturation through the nursing profession, interpersonal interaction with respect and recognition, accomplishment of accountability as a nurse, display of professional competency, and stability and job worth, were identified, which explained 59.7% of the total variance. The JSS-CN's Cronbach's ${\alpha}$ for the total scale was .95, and the intra-class correlation coefficient was .90. The correlation coefficient between the scores of the JSS-CN and Slavitt's scale was .75, and that between the JSS-CN and job performance was .53. Conclusion: Results showed that the JSS-CN has good reliability and validity. Therefore, it is concluded that the JSS-CN could be a useful tool for the measurement of the job satisfaction of clinical nurses in Korea.

Psychometric Evaluation of a Six Dimension Scale of Nursing Performance and Student Nurse Stress Index Using an Objective Structured Clinical Examination - Modules for Asthma and Type 1 Diabetes (객관구조화 임상시험을 활용한 간호수행능력의 Six Dimension Scale과 간호학생 스트레스 평가지수의 도구 평가-천식 및 1형 당뇨 모듈을 중심으로)

  • Purpose: The study purposes were to describe the process of developing the Korean versions of the Six Dimension Scale of Nursing Performance (Six-D) and Student Nurse Stress Index (SNSI) and psychometric evaluation of the two measurements. Methods: This was a methodology study using a descriptive cross-sectional design with 51 nursing students in 4th year of university. Internal consistency reliability was assessed using Cronbach alphas. Construct validity was determined by exploring correlations among Six-D, SNSI, objective structured clinical examination (OSCE), self-efficacy and grade point average (GPA). Results: Internal consistency reliability of Six-D and SNSI was acceptable with Cronbach's ${\alpha}$ of .95 and .82. Correlation analysis to determine construct validity revealed that Six-D presented positive correlations with OSCE (r=.109~.272) and self-efficacy (r=.005~.161) and negative correlation with GPA (r=-.246~-.394), although all were not statistically significant. SNSI presented all negative correlations with OSCE (r= -.007~-.238), self-efficacy (r=-.246~-.394), and GPA (r=-.092~-.426) and were mostly statistically significant except OSCE. Conclusion: Six-D needs more evidence to confirm validity to predict observed clinical competency and theoretical relationships with self-efficacy and GPA. However, SNSI presented trends of expected relationships with relevant variables. Therefore, further research is recommended in testing validity of Six-D with other student populations.

The Influence of Compensation Justice on Job Satisfaction and Organizational Commitment Perceived by Hospital Personnels (종합병원 종사자의 보상공정성 지각이 직무만족과 조직몰입에 미치는 영향)

  • Purpose: The purpose of this paper was to identity the effects of distributive justice and procedural justice on job satisfaction and organizational commitment in hospital personnels. Method: The subjects were 181 employee who were working at 4 hospitals in Daegu and Gyeongbuk. Data was collected using structured questionnaire from Oct 4 to 24, 2007. The collected data were analyzed using the SPSS WIN 12.0 program for descriptive statistics, t-test, ANOVA, Pearson correlation coefficient, and Stepwise multiple regression. Result: The mean score of distributive justice was 3.24, procedural justice was 3.05, job satisfaction was 2.73 and organizational commitment was 2.65. The distributive justice(37.5%) and gender(1.6%) explained 39.1% of the variables in the job satisfaction. The organizational commitment was significantly influenced by distributive justice(27.8%), age(8.9%), procedural justice(2.8%) and gender(2.4%). These factors explained 41.8% of the variables in the organizational commitment. Conclusion: The distributive justice is the most important factor in promoting job satisfaction and organizational commitment. Therefore, It is necessary to operate proper compensation system based on fair performance evaluation of workers in hospital.

Job Analysis of the Nurses Who Work in Customized Visiting Health Care Services using DACUM Technique (맞춤형 방문건강관리사업 내 방문간호사의 직무분석)

  • Purpose: This study was performed to analyze the visiting nurses' job using Developing a curriculum (DACUM). Methods: The DACUM committee with 6 visiting nurses participated in the workshop. In this workshop, the visiting nurses role was defined and their duties and tasks were identified. After content validation via the experts, the pilot test was conducted to 78 nurses. Results: A total of 13 duties and 82 tasks were identified on the DACUM chart which represented the importance, difficulty, and frequency of tasks with alphabet A, B, and C as its higher degree. Regarding duties, 'visiting nursing for high risk group' is the highest, while the lowest was 'public welfare events'. Regarding to tasks, 'discovering a new patient' was the highest, while 'selecting patients and offering medical supplies (nutritional supplement, patch)' was the lowest one. Conclusion: The results showed that visiting nurses working in the visiting health care service center were doing more various duties and tasks than those working according to 'Act on Long-term Care Insurance for the Aged'. The results can be used to develop training programs for visiting nurses and evaluation-scale of their job performance.
