한국의 가정건강관리 연구에 관한 고찰 -의료기관 및 지역사회 가정간호와 보건소 방문보건을 중심으로 - (A Review on Home Health Care in Korea - Hospital, Community, and Public Health Center Based Home Health Care Nursing-)

  • Purpose: This study was conducted to provide fundamental information for policy integration of the home health care system. Methods: Focused review was performed from 8 studies out of 48 literature related to home care needs. Results: Females utilized home care based on hospital and public health centers at a higher rate than males. While there had been more rate of the elderly above 60 year-old at hospital and community based home care, on the other hand below 45 year-old at public health center. The most common disease amongst users in all three types was cerebrovascular disease, the second was cancer based at hospitals, hypertension and arthritis at public health centers. There were more needs for those using artificial respiratory equipment, cancer patients, and puerpera. However, the needs for hospital and community based home care were very similar. Conclusions: Due to similar disease characteristics but varied severity at the three home care based settings, they should be managed with flexibility. Home care curriculum should be developed according to common disease type, and home care services should be strengthened for its expertise.

척수장애 남자 환자의 성재활 지식, 성생활 만족 및 성재활 교육 요구 (Knowledge, Satisfaction, and Education Needs in Sexual Rehabilitation of Male Patients with Spinal Cord-Injury)

  • Purpose: We measured the knowledge, satisfaction, and education needs in sexual rehabilitation of male patients diagnosed with a spinal cord injury. Methods: We recruited 104 patients from a general rehabilitation hospital. Data were collected between April 4 and May 9, 2008. Knowledge of sexual rehabilitation, satisfaction, and educational needs were measured using self-report questionnaires. Results: Points for knowledge of sexual rehabilitation were 6.75 out of 20, sexual satisfaction was 3.02 out of 5, and demand for sexual rehabilitation education were 3.54 out of 5. Most (93.27%) of the patients wanted to have a sex life, and many (75%)were willing to participate in sexual rehabilitation education. There were no differences in knowledge or satisfaction based on general characteristics. However, sexual satisfaction of the university graduates was higher than the lower education group. Conclusion: Male patients with spinal cord disorders are interested in a sex life and demand sexual rehabilitation education. These findings should support the development of sexual rehabilitation programs.

임상간호사의 연구관련 활동 실태와 관련요인 (Research Related Activities and Its Related Factors in Clinical Nurses)

  • Purpose: This study was don(B to describe research-related activities and attitudes toward research, barriers to and support needs for undertaking research in clinical nurses. Method: Data were collected by a questionnaire from 238 clinical nurses with over one year clinical experience working at 2 university hospitals. Results: Research related activities included courses or lecture about Nursing Research 85.7%, journal reading at least once every 2 to 3 months 30.0%, memberships in academic societies 29.4%, participation in academic conferences 45.0%, conducting research 45.4%, research utilization 24.6%. The score for attitudes toward research was 3.08(range 1-5). The score of barriers to undertaking research was 3.37(1-5) and the score for support needs for undertaking research 4.14(1-5). Attitudes toward research significantly correlated with barriers to undertaking research(r=.- 36, p=.000). Barriers to undertaking research significantly correlated with support needs for conducting research(r=.23, p=.000). Nurses with experience in conducting research had more negative attitudes toward research(t=-2.130, p=0.034) and more barriers to undertaking research than those without experience in conducting research (t=2.194, p=0.029). Conclusion: These results suggest that it is necessary to increase positive attitudes toward research in clinical nurses and nursing organizations need to provide strong supports for nurses conducting research.

Educational Status and Needs of Premature Birth Prevention and Its Association with Preconception Health Behavior among Women of Childbearing Age in Korea

  • Purpose: The aim of this study was to investigate the educational status and needs of premature birth prevention, and to identify factors associated with preconception health behaviors. Methods: The study design was a crosssectional descriptive study. Data were collected through an online questionnaire survey, and the subjects were 192 women of childbearing age in Korea. Data were analyzed using the Mann-Whitney U test, Kruskal-Wallis test, Spearman's correlation coefficients, and multiple regression. Results: The proportion of subjects who received education on premature birth prevention was 8.9%, and 75.5% of subjects answered that they needed education on premature birth prevention. They demanded education through online media, small groups, cases, cartoons (webtoon) with stories, pictures, and videos. A related factor of preconception health behavior was self-efficacy for high-risk pregnancy health care (β=.20, p=.012), which accounted for 8.2% of the total variance related to preconception health behavior. Conclusion: There was a need for more development of education programs to prevent premature birth for women of childbearing age. Its education programs should be applied with online, small group activities using various educational media. It is also required to promote preconception health behavior through self-efficacy for high-risk pregnancy health care.

일 무료요양시설 거주 여성 노인의 심리사회적 욕구에 대한 구성주의 연구 (The Constructivist Study on Psychosocial Needs of Elderly Female Residents living in Nursing Home)

  • 본 연구의 목적은 요양원 거주 노인들의 심리사회적 욕구를 탐색하고 이들을 돌보는 일에 효과적으로 개입하기 위한 것이다. 본 연구의 참여자는 지방 소도시의 한 요양원에 거주하는 노인들 중, 인지적으로 건강하고, 의사소통이 가능하며, 거주한지 1년이 넘는 65세 이상의 여성노인 8명으로 의도적 표집방법을 통해서 선정되었다. 본 연구는 구성주의 접근(constructivist inquiry)을 바탕으로 한 사례연구로서 자료 수집은 심층면접을 통하여 이루어졌으며, 자료 분석은 지속적인 비교방법(constant comparison method)이 이용되었다. 참여자들의 심리사회적 욕구는 충족된 욕구와 충족되지 못한 욕구, 숨겨진 욕구로 분류되었으며, 욕구 표현의 소극성이 또 다른 범주로 드러나, 연구결과는 총 4개 영역으로 범주화되어졌다. 먼저, 충족된 욕구는 편안함과 종교생활과 관계된 욕구였으며, 충족되지 못한 욕구는 음식 문제와 의미 있는 활동에 대한 결핍으로 나타났다. 참여자들이 구체적으로 인식하지 못하지만, 충족될 수 없는 숨겨진 욕구로는 정서적인 관계 욕구였다. 마지막으로, 욕구 표현의 소극성은 참여자들 대부분이 요양원내에서 그들의 욕구를 표현하는데 소극적이어서 분류된 것인데, 이는 스스로를 불쌍하게 여기는 부정적인 자기인식과 적극적인 욕구표현은 비난을 받을 수 있다는 암묵적 동의, 참여자의 독립과 의존사이의 갈등이 그 원인으로 분석되었다. 본 연구의 결과는, 요양원내 노인거주자들의 심리사회적 복지를 향상시키기 위한 사회 복지적 개입을 실천하는데 중요한 토대가 된다. 또한, 시설 생활에 대한 부정적인 인식이 현존하는 가운데, 자칫 감춰질 수 있는 시설노인들의 목소리에 귀 기울이고 이들의 경험을 드러냄으로써, 요양원이 노인들의 대안적인 삶의 공간으로서 사회 속에 자리매김하는 것을 돕는다.

21세기를 향한 전문대학 간호교육과정 개발 - 요구사정, 개념틀 개발을 중심으로 - (Nursing Curriculum Development for the 21st Century - Need assessment and development of conceptual framework -)

  • In order to prepare for the coming twenty-first century and to meet changing social demands and health needs, it is necessary to improve the curriculum through development of new conceptual frameworks. The purposes of this study were : 1) to assess the needs of the curriculum of the students, the faculty, the graduates, and the nursing supervisors and head murses ; 2) to develop the conceptual framework which includes philosophy and goals of nursing education based upon needs assessment ; 3) to develop level objectives ; 4) to identigy the contents of the curriculum ; 5) and to develop the faculty. The curriculum was assessed and developed from April, 1995 to March, 1996 through twenty four weekly meetings, two seminars, and two workshops. The process and results of this study are as follows : 1. A needs assessment of the present curriculum was done of the 194 students, 177 graduates, 14 faculty members, and 60 nursing administrators in 5 main areas (objectives, planning and organization, teaching-learning process and methods, evaluation, revision of curriculum). The results showed that there were many descrepancies between expectations and actual situations in all the groups and in all the areas. This implies that there was a necessity for total curriculum revision. 2. To develop the conceptual framework, 1) the core concepts were identified(man, health, environment, and nursing) on the basis of the existing educational philosophy and educational objectives of our school, elicited by group discussions using the nominal group method, one of the needs assessment methods, the philosophy and objectives were restated. 2) Six essential componednts were indentified for the conceptual framework from the restated philosophy and objectives ; nursing process, communication, professional roles, client, health, and nursing. The vertical theread consists of the client and health/nursing ; and the horizontal thread consists of nursing process, communication, and professional roles. 3. The contents of the curriculum were selected on the basis of the educational objectives and organized according to the conceptual framework. 4. The level objectives were then restated. It is expected that the objectives of our school will be accomphished through developing the courses, choosing and Implementing, more effective teaching-learning methods, and evaluating the efficacy of changes implemented. The most important factor will be to continually upgrade the faculty and their teaching skills.

중환자실 간호사의 임종간호 어려움과 임종간호 교육요구 조사: 혼합연구방법 (Difficulties in End-of-Life Care and Educational Needs of Intensive Care Unit Nurses: A Mixed Methods Study)

  • 목적: 중환자실 간호사가 경험하는 임종간호 어려움에 대해 확인하고 임종 간호 시 필요한 교육요구도를 확인하기 위한 혼합연구이다. 방법: 대상자는 중환자실에 근무하는 간호사로 106명은 설문지를, 19명은 초점집단인터뷰로 연구가 진행되었다. 자료분석은 SPSS win 23 프로그램을 이용, 서술 통계, Independent t-test, ANOVA, Mann-Whitney U test, $scheff{\acute{e}}$ test 이용하여 분석, 초점집단인터뷰는 내용 분석 방법을 사용하였다. 결과: 중환자실 간호사는 임종간호에 대해 업무적, 심리적으로 어려움을 느끼고 있었으며, 임종간호에 대한 체계적인 교육에 대한 요구와 일부 요구된 교육 내용을 확인하였다. 결론: 이러한 결과를 바탕으로 만들어진 임종간호 교육프로그램을 통하여 중환자실 간호사의 임종간호의 어려움을 감소시키고, 나아가 환자 및 보호자가 중환자실에서 평화로운 임종기를 보내는데 도움이 되었으면 한다.

뇌졸중 환자의 자가간호 수행과 가족 부담감 및 교육적 요구에 관한 연구 (A Study on the Degree of Self-care Activities of Stroke Patients, the Family Burden, and the Educational Needs of Primary Caregivers)

  • The purpose of this study was to identify the degree of self-care activities of stroke patients, the family burden, and the educational needs of primary caregivers. A descriptive survey research was conducted in 97 stroke patients and their respective primary caregivers. Data were collected by the interviews using a structured questionnare from July 26th to August 5th, 1999. NIH stroke status was $12.15{\pm}8.40$, the degree of self-care activities of stroke patients was $26.85{\pm}9.39$. The score of objective burden of primary caregivers was $31.24{\pm}4.81$ and subjective burden was $24.30{\pm}6.99$. The score of the educational needs was $89.78{\pm}9.99$. We present several imformations about clients from these scores as follows: The patients were convalescent and their depentant level of self- care was middle range; The objective burden was higher than the subjective burden; And the educational needs of family caregivers were very high. The Elain Mattis Educational Wants of Family Caregivers of Disabled Adults Questionnaire was used to assess the educational needs of primary caregivers of stroke patients. The results of this research are offered to help nurses understand the feelings of uncertainty about the new and unexpected role that family caregivers face and to help nurses meet the needs of families preparing to care stroke survivors at home.

간호사가 지각한 암환자의 퇴원후 가정간호요구 (Nurse의s Perception in the Homecare Needs of Cancer Patient)

  • The purpose of this descriptive study was to identify the homecare needs of the discharged patient with cancer as perceived by nurses caring hospitalized cancer patients. At two hospitals in Gyeongnam, 74 nurses responded to an open-ended questionnaire consisting of four need categories : 1) educational & informational need, 2) physical need, 3) emotional need, 4) social need. Respondents were asked to list above ten needs of cancer patient in each category. Two researchers analyzed the data by content analysis method. The findings are summarized as follows : 1) A total of 1,417 need items were generated by nurses. The largest number of needs were in the educational & informational need category(475 items, 36.3%). Physical(414 items, 31.6%), emotional (237 items, 18.1%) need were the second, third largest, and social(184 items, 14.0%) need made up the smallest category. 2) In the educational & informational need category, there were seven subcategories of prognosis, diet & exercise, medication & pain, wound care, folk remedy, personal hygiene, comfort. The need items related to prognosis of cancer accounted for almost a half(48.2%) of the total. 3) In the physical need category, there were ten subcategories of personal hygiene, skin & tissue, nutrition, side effect on treatment, exercise, pain, elimination, equipment, comfort & safety, others. The largest number of needs were in subcategory of the personal hygiene(82 items, 19.8%). 4) In the emotional need category, there were four subcategories of emotional support related to disease, emotional support related to routine life, spiritual support, maintenance of relationship with nurse & doctor. The largest number of needs were in subcatgory of the emotional support related to disease(96 items, 40.5%). 5) In the social need category, there were five subcategories of support for social life, household management, legal support, the use of volunteer service, financial support. The largest number of needs were in support for social life subcategory(58 items, 31.5%).

중등학교 보건교사가 인지한 보건교과교육 필요성과 자신감 (Health Education Needs and Confidence of School Nurses in Korean Secondary Schools)

  • Purpose: The aim of this study was to determine the perceived needs and confidence of school nurses in health education at middle and high schools in Seoul, Korea. Methods: A descriptive survey questionnaire was developed by a research team to investigate the perceived needs and confidence across eight health education areas. A total of 329 school nurses at secondary schools in the metropolitan areas of Seoul participated. Descriptive statistics, the t-test, and ANOVA were conducted to analyze the data. Results: Generally high means in perceived education needs(Mean: 3.8~4.7/5) and health education confidence (Mean 3.3~4.5/5) were reported. However, there were significantly low means of health education confidence related to perceived needs in health education for all areas of health education. While injury prevention and emergency care education were the highest perceived educational needs and were also areas with high confidence of school nurses, mental health, social health, and sexual health were areas where nurses showed a significantly lower confidence. In general, nurses with a longer clinical experience and educational career had a higher confidence in injury prevention, emergency care, and mental health. Conclusions: Multi-level support, including the continuing education and proactive in-service training, for the school nurses needs to be developed to improve their teaching competency.
