Development of Nursing Intervention Protocol for Childhood Cancer at Early Diagnosis Stage (소아암 환자의 초기 진단단계의 간호중재 프로토콜 개발)

  • The main purpose of this methodological study was to develop an assessment tool and intervention protocol for child and family with childhood cancer at early diagnosis stage. The assessment tool and intervention protocol was developed by extensive literature review and consultation with experts. Review of nine domestic and sixty-six international journal articles were done to identify stress, interventions, coping strategies and adjustment of children with cancer and their family. Results were as follows; First, assessment at the early diagnosis stage need to include information on patient, family, and patient/family attitude toward diagnosis and treatment. Second, intervention protocol for children with cancer includes control physical symptoms, manage the side effects of chemotherapy and diagnostic or therapeutic procedures, control emotional responses, provide support and information, assist decision-making and adjust to environment. Third, intervention protocol for family includes controlling emotional responses, provision of informations, inducing family support to patient, improving family cohesion, supporting siblings and supporting spiritual growth. In conclusion, the early diagnosis stage in cancer treatment is important for child and family since this stage greatly affects the overall adjustment of child and family to live with cancer. Therefore, pediatric nurses need to be sensitive to the need of patient/family and systematically manage their needs at this stage.

A Study on the Nursing Needs and Satisfactions of Early Postpartum Women (초기 산욕부의 간호요구도와 만족도에 관한 연구)

  • This study was carried out to identify the differences between nursing needs and levels of satisfaction of postpartum women during the early postpartum period. The goal of this study was to obtain needed to develope the nursing quality for postpartum women. The subjects were 87 postpartum women who had vaginal delivery at 2 general hospital and 2 maternity hospital in the Ulsan City. The period for the data collection was from June 1 to 29, 1999. The data was gathered using an 98 items questionnaire which was a modified version of a questionnaire the developed by Y. J. Chun. Results found are as follows : 1. The general characteristics of the subjects : The majority of subjects were 25-29 yrs. (57.5%), high school and college graduates(96.5%), unemployed(75.9%), middle ranged economics(93.1%), had no religion(43.7%), male baby(52.9%), birth weight 3-3.5Kg(49.4%), wanted pregnancy(96.6%), no abortion history(50.6%), had antenatal care(89.7%), had prenatal education(32.2%), postnatal education(42.5%), intended breast feeding (46.0%), had resonable self confidence about self(20.7%), infant care(36.8%) and nuclear family pattern(82.3%). 2. The level of nursing needs of overall nursing care(3.93) and the levels of satisfaction(3.86) was relatively high. The nursing needs by category of nursing care, the highest need was in the emotional and psychological care(4.09), and the lowest need was in physical need(3.73). The satisfactions by category of nursing care, the highest satisfaction was in emotional and psychological care(4.11) and the lowest satisfaction was in education in self care(3.64). The significant differences between nursing needs and satisfactions were found on education in infant care(p=.005), and education in self care(p=.020). 3. Among items of physical care, 'accurate medication and treatment(4.21)', 'control of postpartum hemorrhage(4.13)', 'pain control and care of episiotomy wound (4.12)' and 'regular observation of postpartum conditions(4.09)' showed high nursing needs. 'Accurate medication and treatment(4.31)', 'regular observation of postpartum condition(4.24)', 'control of postpartum hemorrhage(4.22)' and 'pain control and care of episiotomy wound(4.12)' showed high satisfaction levels. 4. Among items of emotional and psychological care, 'personal treatment(4.32)', kind and faithful care(4.30)', 'detailed explanation on the treatment or nursing care(4.25)', 'adequate draping during the care and treatment(4.23)' and detailed explanation on a doubt(4.13)', showed high nursing needs, 'personal treatment(4.52)', 'kind and faithful care(4.45)', 'detailed explanation on a doubt(4.24)', 'detailed explanation on the treatment or nursing care(4.21)' and 'adequate draping during the care and treatment(4.18)' showed high satisfaction of nursing care. Difference between the level of nursing needs and satisfaction was significant except item of 'early contacts with their baby and breast feeding'. 5. Among items of environmental care, the highest level of need and satisfaction were on the items of 'neat bedding and clothes(4.05, 4.21)' and 'room cleansing or care of room(4.01, 4.28)'. Differences between the level of nursing care and satisfaction were 'room cleansing of care of room'. 6. Among items of educational needs on self care, 'sitz bath method(4.22)', 'high risk symptoms to immediate clinic visits(4.13)', 'the timing of tub bath(4.05)' and 'good secretion of breast milk(4.03)', showed high nursing needs, 'sitz bath method(4.22)' showed high satisfaction of nursing care. Differences between the level of nursing care and satisfaction were 'the timing of hair shampoo', 'the timing of tub bath', ' the method of pad change', 'postpartum exercise', 'good secretion of breast milk', 'maintenance of breast figure', 'contraindicated drugs in postpartum women', 'kegel exercise' and 'breast self examination'. 7. Among items of educational needs on infant care, 'immunization of infants(4.36)', 'symptoms of sickness to immediate clinic visits(4.28)'. 'safety and emergency care(4.28)', 'umbilical care(4.26)', 'feeding times and intervals(4.24)', 'normal growth and development of infant(4.24)' and 'infection control(4.22)', showed high level of nursing care, 'immunization of infants(4.21)', 'feeding times and intervals(4.17)', were high satisfaction items showed significant differences between the level of nursing care and satisfaction. 8. Relationship between nursing needs and levels of satisfaction among postpartum women were as follows : 1) Physical area : There were no significant differences in the level of nursing needs, but satisfaction levels were significantly different among 'working mothers', 'baby's weights', 'baby's sex', 'planning of feeding' and 'routes of receiving postpartum informations'. 2) Emotional and psychological area : The level of nursing needs were significantly different in the area of 'confidence in self care'. Satisfaction levels were significantly different among 'baby's sex', 'baby's weights', 'the confidence of infant care' and 'working mother'. 3) Environment area : There were on significant differences in the level of nursing needs, but satisfaction levels were significantly different among 'maternal age' and 'baby's weights'. 4) Education in self care : The level of nursing needs were significantly different among 'parity history' and 'type of family formation'. Satisfaction levels were significantly different among 'working mother', 'baby's sex', 'antenatal care', 'postpartum education', 'planning of feeding', 'routes of receiving postpartum informations'. 5) Education in infant care area : The level of nursing needs were significantly different among 'parity history'. Satisfaction levels were significantly different among 'baby's sex', 'receiving postpartum education or not' and 'working mother'.

How Healthy is the Health related Informations brocated by TV News? (TV 뉴스에 보도된 건강관련 정보의 건강성과 해독성)

  • Television news programs are becoming significant source of health information. This study aims at investigating the current state of health coverage of the prime time news program in Korea. Data were collected from KBS 9 0'clock news in the period of thirteen months. from December 1. 1998. to November 1. 1999. The data were analyzed using content analysis method. and the reliability degree was 99.7% according to the Holsti's inter-coder reliability test. The current research classified 489 health related news items into 49 sub-categories and five health categories through content analysis. Some of the basic results of this study are as follows. 1. The frequency according to health category, health maintenance promotion(57.3%) topped followed by disease prevention(23.2%), disease treatment(14.9%), life ethics(4.0%), and growth development(0.6%). 2. According to human developmental age. for the most part(80.1 %) is applicable to the entire range of human developmental age. 3. Health maintennance promotion category take top of health category by the rate of 57.3% and contain 20 sub-categories. 4. News items in the life ethics category, which had six sub-categories. occupied only four percent of the total health related news. News in the growth development category included two sub categories and occupied 0.6% of the total news items. 5. In disease prevention and disease treatment category, infectious disease(33.2%) showed the highest percentage according to the WHO's international disease classification system. Disease prevention occupied 23.2% and contained eleven sub-categories while disease treatment occupied 14.9% and included ten sub-categories. Television news coverage on health showed a wide variety of selection in terms that they are reporting various issues. This study, however, found that some news items were confusing and failing in presenting scientific evidences. It is suggested that the television coverage on health could be beneficial to most of viewers in receiving important health information and guidelines, only if they are utilizing their own sound discretion in consuming those news.

  • Purpose: This study was to investigate sexual behavior, pregnancy and contraception knowledge among teenage. Method: The subjects were 627 of teenaged girls from 14 to 19 years old who lived in a capital city. Data was collected by questionnaire surveys used the convenience sampling. The instrument used for this study was contraception knowledge tool. The collected data was analysed by using descriptive statics, chi-square, t-test, ANOVA, Duncan test for post hoc with SPSS program. Result: The finding of Sex experience was kiss and petting experience 196(32%), sex intercourse 44(7.2%) and pregnancy 6(1%). Average of contraception knowledge was 3.8 score(total 15). Statically significant general characteristics variable related to sex experience were age, school type, smoking and alcohol use. Statically significant general characteristics variable related to contraception knowledge were age, school type, previous sex education and contraception education. Conclusion: These results of this study provided health professional informations to develop intervention programs to reduce teenage pregnancy.

Influence of Stress Experience on Change of Attention (스트레스 사건의 경험이 주의변화에 미치는 영향)

  • For a man to maintain attention, he needs to keep a certain level of arousal. An inordinate increase or decrease in the level of arousal eventually has a negative influence on attention. Precedent research has shown that the degree of attention changes when an experience of stress is related to anxiety resulting in a rise in arousal. This research was done to examine this hypothesis by looking at the 27 female students, 14 of whom had failed in the annual examination. The results of the investigation are as follows : The stress of failure in the examination was seen to raise the level of physiological arousal. Although pulse and electromyography showed no significant change, further inquiries should be made based on other types of methodology. In spite of the rise of arousal, the performance of selective task was degraded. This suggests those students failed to give moderate attention to given information for that kind of task. But the exact reason of that failure was not identified : that is it was difficult decide whether they gave too much attention to the anxiety brought about by stress. Performance of integral tasks, however, did not show any degradation. Judging from these results, stress seems to exert significant influence on attention in the selection of the appropriate information among the various options given. This offers an important hint in relation to the health care situation where nursing information is offered. Clients who receive nursing information in stressful situations may have difficulty in separating and selecting this helpful information from other options which they have acquired through their life experience. The content and terminology of nursing information may be strange and unintelligible to clients, although they are quite familiar and distinct to nurses. So, it is desirable for nurses to give, in addition and at the same time when nursing information is given, some certain related information as devices for selection, instead of merely giving nursing informations as such. So far it is not clear whether the concepts of information processing theory can be suitably applied to nursing. However, it is obvious, according to this research, that the quality of attention is disturbed in the stress situation. This is why further inquiries should be made into attention in practical nursing situation.

Experience of the Married Women on Contraception (가임기 기혼여성의 피임경험)

  • The study is to reveal what the married women has experienced while they were on the contraception. Data were gathered for four mothers beginning on October, 1996. Interviewees were 22 women. Data were analyzed by Colaizz's phenomenological data analysis method. Three basic structures of contraception experience were derived. Those are 'Conflict in choosing contraceptive methods', 'Dilemmar in sexual relationship', and 'Responsibility concerning contraception'. Contraception was a major task of the married women, and they difficulties, because sexual relation was usually initiated by husband and contraception by husband was negatively perceived in Korea culture. Conflict were caused by inavailability of information, side effects of contraceptive methods, and limitation of the contraceptive methods. Interviewees perceived that responsibility of contraception was on them, but they expressed feeling of anger against they burden by contraception. Most interviewees took initiation of contraception to keep their body healthy. Some of the interviewees got husband's cooperation through negotiation and shared responsibility with husband. Suggestions were as follows ; (1) Sex education, especially on communication about sex, is necessary to the married women. (2) Informations on contraception should be provided in the private environment. (3) Studies on husband's contraception experience are required to promote husband's cooperation.

A Study of Nurses' Knowledge, Attitude on the Nurses' Law and Nurses' Perception on the Causes, Coping Patterns with the Nursing Accidents (간호사의 간호관련법에 대한 지식$\cdot$태도 및 간호사고에 대한 원인$\cdot$대처양상에 관한 연구)

  • Recently the request of the patients to participate in the medical courses has been expanding due to elevated sense of right on the people's health. merchandised medical treatment by mass supply, human right declaration of the patients, generalized medical informations by the mass media and the change of human relation between the medical personnels and the patients. Under these phenomena the accident by the nurses have been increasing by the area of the nurses having been expanded and their independent roles having been increased. Such nursing accidents are the important subject which the professional occupation of the nurses has been facing but legal protective capability of the nurses has been very weak. Therefore this study has examined the degree of the experience of the nursing accident that happens in the clinical nursing scenes in the general hospital to provide the basic materials for the protection and the counter measures of the nursing accidents. The following is the conclusion based by the above examination. 1) The general characters of the subjects of this study is that they are mostly single in their twenties and graduate from nursing college. Their total clinical career is above 5 years$(44.8\%)$ and their current clinical parts' career is between 1-3 years$(40.1\%)$. So these facts suggest that most hospitals has taken the working rotation policy on nurses. 2) The level of nurses' knowledge on the nursing law is accurate partially but isn't it patially. So it is suggested that nurses need the accurate information and education about the nursing law. But the nurses' attitude is very approved of the establishment of a unilateral nursing law. 3) The relation between the demographic characters of the subjects and their attitudes on the nursing law shows that there is no significant differences except the relation between the attitude 6(the sufficient level of education on nursing law in formal education course) and age. total clinical career. 4) The perception of the nurses shows that the cause of the nursing accident has been due to the heavy work$(78.2\%)$. short of professional knowledge and skill$(60.2\%)$, discordance with Doctors. patients and patients' families. They report the accident to the head nurse first$(81.8\%)$ and within 30 minute$(75.1\%)$. The hour of nursing accident frequently happened is regardless of service hour with $49.4\%$ in response rate. the highest rate. and the nursing accident happens in the night more than the daytime. Even though most nurses think that they are themselves responsible for nursing accident. it is found that the chief cause of the nursing accident is due to the nurses' heavy work$(78.2\%)$. So the causes of nursing accidents is analysed. it may be suggested that the endeavor of hospital and nursing organizations to decrease nursing accidents is very important. 5) The coping patterns of patients with nursing accidents are mostly active attitude such as a violent words$(69\%)$. sue or accusation$(36.4\%)$, monetary compensation $(35.6\%)$ except a understanding cases$(38.7\%)$. But the coping patterns of hospitals with nursing accidents are mostly to investigate the accurate cause.

A Study on the Assessment of Dementia Patients in Community (지역사회 치매환자 사정도구 개발을 위한 일 고찰)

  • As concerns about dementia patients increase gradually in society, the nursing of them becomes more and more important. For intervening in dementia patients effectively, the most fundamental thing is to understand their characteristics and to increase the ability of assessing them. The assessment data supply us with the foundations of Nursing diagnosis and nursing plan. The reaseons why the assessment data are especially important to dementia patients are as follows; First, dementia patients have great differences among them in intellectual, physical ability. And their condition varies from time to time. Second, the intervention without planning results in the deficiency of consistence, and it rather embarrasses and makes dementia patients uneasy. Third, nursing of dementia patients requires participation of multi-disciplinary team because of characteristics of that disease. The development of an unitary assessment tool is essential for all members of the team to have close relations between them and care for dementia patients. Fully understanding the importance of assessment data, this study investigate the content to be included in the assessment before developing assessment tools to be used in community. It points out that the assessment should include the informations about patients, their family, and main care-giver. And it sets up items that can supply with detailed information on each person. In the future it is necessary to transform the contents of this study to more concrete items and develope the assessment tools for dementia patients. And recording paper for follow up should be supplemented to check any changes in family caring for dementia patients and describe the changes in detail. To make assessment tools is one of the fundamental works in controlling dementia patients. Therefore the official assessment tools should be provided as soon as possible. This study aimed at presenting the guiding principles for developing that assessment tools.

Developing the Joint Employment System of Occupational Nurse Practitioners for the Minor Enterprises in Korea (part 1) (보건관리자 공동채용 모형 개발 (I) -산업간호사 의견을 중심으로-)

  • The occupational health services for the minor enterprises are poor comparing with that of large enterprises in Korea. In 1997, Korean government announced officially the law on joint employment system of occupational health practitioners to solve the problems. This study is to develop the model on the joint employment system of occupational nurse practitioners for minor enterprises in Korea based on the opinions of occupational health nurses (OH Ns). The data were collected by questionnares from October to November in 1997. The number of subjects was 210 occupational health nurses who had participated the mandatory nursing education program provided by the Korean Occupational Health Nurses Association(KOHNA). The response rate was 47.1%. The contents of questionnaires were general characteristics, occupational & job characteristics of OHNs, and the views on the joint employment system. The SAS-PC program was used for the statistical analysis. The results were as followed, (1) The proper number of industies was 3 and the appropriate number of workers was 448 for one joint employed occupational nurse practitioner (J EONP). (2) 44.7% of the OHNs wanted that the Ministry of Labor monitored the way and content of contraction b/w JEONP and minor emterprises, 82.6% of the OHNs wanted that KOHNA managed the employment informations. (3) The OHNs of occupational health agencies showed more positive attitude than the OHNs of private industries on the joint employment system. (4) 88.3% of the OHNs wanted the education training for JEONP. Especially in occupational hygiene and safety control. (5) The OHNs expected the role expansion of Occupational Health Nursing by JEONP system. At the same time they worried the adverse effects. For the successful execution of this system, it is necessary the cooperation among the Ministry of Labor, KOHNA, the minor enterprises, and the OHNs.

Uncertainty and Transfer Anxiety of Pediatric Patients' Mothers upon Discharge from the Pediatric Intensive Care Unit (소아중환자실 퇴실 시 환아 어머니의 불확실성과 전동불안)

  • Purpose: This study was done to investigate the level of uncertainty, transfer anxiety, and to identify the participant's characteristics those influence them by targeting the mothers of transferred patients from the pediatric Intensive Care Unit (PICU) to the general ward. Methods: This study was a descriptive correlational study. The participants were 94 mothers of children who had been scheduled to transfer from the PICU. Results: The item mean score for uncertainty was 2.16 (4 scale), transfer anxiety was 2.15 (4 scale). There were statistically significant positive correlation between uncertainty and transfer anxiety (r=.591). The level of uncertainty significantly differed according to education level (p=.035), duration of marriage (p=.043), and monthly income (p=.022). The level of transfer anxiety significantly differed according to religion (p=.044), duration of marriage (p=.043), and severity of illness at transfer (p=.047). Conclusion: In order to reduce the transfer anxiety and uncertainty of the mothers of patients, efficient communication with medical staff and clear informations are needed to reduce ambiguity. Also, standard protocol should be developed for improving communication among medical staffs.
