Symptom Experiences and Coping Strategies among Multi-ethnic Solid Tumor Patients Undergoing Chemotherapy in Malaysia

  • Background: This study was performed to assess patient symptoms prevalence, frequency and severity, as well as distress and coping strategies used, and to identify the relationships between coping strategies and psychological and physical symptoms distress and demographic data of cancer patients. This cross-sectional descriptive study involved a total of 268 cancer patients with various types of cancer and chemotherapy identified in the oncology unit of an urban tertiary hospital. Materials and Methods: Data were collected using questionnaires (demographic questionnaire, Medical characteristics, Memorial Symptom Assessment Scale (MSAS) and Brief COPE scales and analyzed for demographic, and disease-related variable effects on symptom prevalence, severity, distress and coping strategies. Results: Symptom prevalence was relatively high and ranged from 14.9% for swelling of arms and legs to 88.1% for lack of energy. This latter was the highest rated symptom in the study. The level of distress was found to be low in three domains. Problem-focused coping strategies were found to be more commonly employed compared to emotion-focused strategies, demonstrating significant associations with sex, age group, educational levels and race. However, there was a positive correlation between emotion-focused strategies and physical and psychological distress, indicating that patients would choose emotion-focused strategies when symptom distress increased. Conclusions: These findings demonstrate that high symptom prevalence rates and coping strategies used render an improvement in current nursing management. Therefore development of symptoms management groups, encouraging the use of self-care diaries and enhancing the quality of psychooncology services provided are to be recommended.

A systematic review of the keystone design perforator island flap in the reconstruction of trunk defects

  • The keystone design perforator island flap can be utilized in the repair of trunk defects. A systematic review was carried out to identify the complication rates of the use of this flap to treat such defects. The MEDLINE, Embase, Cochrane Library, and PubMed Central databases were searched for articles published between January 2003 and December 2018 that reported the use of keystone design perforator island flaps in the repair of trunk defects. Study selection was conducted in alignment with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement. Eight articles involving a total of 54 flaps satisfied the inclusion criteria. The most frequently reported cause of trunk defects was oncologic resection (64.4%). The overall complication rate was 35.2%, and complications included infection (11.1%), wound dehiscence (7.4%), delayed healing (7.4%), and partial flap loss (1.9%). The keystone design perforator island flap is associated with a high success rate and low technical complexity. Despite minor complications, keystone design flaps could be a preferred choice for trunk reconstruction.

Lack of Relationship of Egg White Intake with Occurrence of Leukopenia in Gynecologic Cancer Patients during Chemotherapy

  • Egg white intake during chemotherapy is common advice for cancer patients for the prevention of leukopenia. However, the benefit is uncertain. We conducted this prospective study to identify the relationship of egg white intake for gynecologic cancer patients who received carboplatin and paclitaxel and the occurrence of leukopenia. Between January 2014 and January, 2015, 81 patients were interviewed regarding their intake of egg whites before receiving subsequent chemotherapy. The basic data, the details of egg white intake and the grade of leukopenia in the previous cycle were recorded. The mean age was 54.1 years and 80% of the patients had a diagnosis of ovarian or endometrial cancer. The patients were interviewed at cycles 1-3 in 45 cases, 4-6 in 45 cases and 7-9 in two cases. Subsequent dose reduction was found in 6.2% and granulocyte-stimulating growth factors was given at 4.9%. All the patients ate egg whites with variations in the number of eggs per day as follows: less than one (3), one to two (56), three to four (14) and five to six (8). Over 70% were recommended by nurses to eat egg whites and about 63% of patients received other supplemental food. Some 44.1% of the patients who ate less than or equal to two eggs per day and 36.4% who ate more than two eggs per day developed grade 2-4 leukopenia, P = 0.61. In conclusion, the data did not provide evidence in support of the conclusion that a greater egg white intake could significantly reduce the occurrence of leukopenia.

Factors Associated with Death Acceptance among Thai Patients with Advanced Cancer

  • Patients with advanced cancer cope with various issues, especially psychological symptoms, such as anxiety about death. Previous research from various countries indicates that most advanced cancer patients perceive distress before death, which means they cannot accept death peacefully. The purpose of this study was to examine the factors associated with death acceptance of Thai patients with advanced cancer. The participants were 242 adult patients with advanced cancer from three tertiary hospitals in Bangkok, Thailand. The research instruments consisted of 7 questionnaires: a personal information questionnaire, the death anxiety questionnaire, Buddhist beliefs about death questionnaire, the unfinished task questionnaire, general self-efficacy scale, family relationship questionnaire, and death acceptance scale. These instruments were tested for their content validity by a panel of experts. All instruments except the first one were tested for reliability, and their Cronbach's alpha coefficient were .86, .72, .74, .74, .89, and .70, respectively. Descriptive statistics, Pearson's correlation, Point-biserial correlation coefficient, and stepwise multiple regression were used to analyze the data. The results revealed that a total of 42% of variance of death acceptance among Thai patients with advanced cancer was explained by death anxiety, Buddhist beliefs about death, and self-efficacy. Knowing these factors provides further information for nurses to help patients to cope with their death. From this crucial knowledge, nurses can develop interventions in decreasing or controlling anxiety about death, promoting Buddhist beliefs about death, and enhancing self-efficacy of advanced cancer patients. Therefore, a better quality of life and good death can be achieved.

Risk factors for childhood pneumonia: a case-control study in a high prevalence area in Indonesia

  • Background: Acute respiratory infections (ARIs), especially pneumonia, remain a major cause of infant mortality worldwide. In Indonesia, pneumonia is the second most common cause of infant and toddler deaths. Exclusive breastfeeding and basic immunization can protect infants and children from contracting pneumonia. Purpose: Our goal was to assess the risk factors for childhood pneumonia in regions with a high prevalence of pneumonia in Indonesia. Methods: This case-control study was conducted between March and April 2019. A total of 176 infants and toddlers aged 10-59 months were enrolled and selected from among patients who visited the community health center. Cases of pneumonia were diagnosed clinically based on the World Health Organization guidelines, and the control was nonpneumonia. Results: The risk factors for the diagnosis of pneumonia included no or nonexclusive breastfeeding (odds ratio [OR], 7.95; 95% confidence interval [CI], 3.52-17.94), incomplete basic immunizations (OR, 4.47; 95% CI, 2.22-8.99), indoor air pollution (OR, 7.12; 95% CI, 3.03-16.70), low birth weight (OR, 3.27; 95% CI, 1.19-8.92), and a high degree of wasting (OR, 2.77; 95% CI, 1.06-7.17). Other variables such as nutritional status (height-for-age z score), age, sex, and educational status of the mother were not risk factors for pneumonia. Conclusion: No or nonexclusive breastfeeding, incomplete basic immunizations, indoor air pollution, a history of low birth weight, and severe malnutrition were risk factors for childhood pneumonia. Breastfeeding was the dominant factor, while sex modified the relationship between exclusive breastfeeding and the incidence of pneumonia.

Predictor of Liver Biochemistry Improvement in Patients with Cytomegalovirus Cholestasis after Ganciclovir Treatment

  • Purpose: Cholestasis resulting from cytomegalovirus (CMV)-induced hepatitis manifests in 40% of patients with a CMV infection. Ganciclovir treatment in children with CMV infections has proven to be highly effective. Until now, there are very few studies have identified predictive factors for liver biochemistry improvement after ganciclovir therapy. This study aimed to identify the predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Methods: A retrospective cohort study was conducted using medical records from Dr. Sardjito General Hospital Yogyakarta, Indonesia from 2013 to 2018. CMV cholestasis was confirmed based on serum CMV IgG and IgM positivity and/or blood and urine CMV antigenemia positivity. Incomplete medical records and other etiologies for cholestasis, such as biliary atresia, choledochal cyst, metabolic diseases, and Alagille syndrome, were excluded. Patient age at cholestasis diagnosis and ganciclovir treatment, duration of CMV cholestasis, history of prematurity, central nervous system involvement, and nutritional status were analyzed and presented as an odds ratio (OR) with a 95% confidence interval (95% CI). Results: CMV cholestasis with ganciclovir therapy was found in 41 of 54 patients. Multivariate analysis showed that a shorter duration of CMV cholestasis (OR: 4.6, 95% CI: 1.00-21.07, p=0.04) was statistically significant for liver biochemistry improvement after 1 month of ganciclovir treatment. The remaining factors that were analyzed were not significant predictors of liver biochemistry improvement in patients with CMV cholestasis after ganciclovir treatment. Conclusion: A shorter duration of CMV cholestasis is the predictor of liver biochemistry improvement after 1 month gancyclovir treatment.

Prevalence and Risk Factors of Functional Gastrointestinal Disorders in Infants in Indonesia

  • Purpose: Information regarding functional gastrointestinal disorders (FGIDs) in infants is currently lacking in Indonesia. This study aimed to describe the prevalence and risk factors of FGIDs in infants aged 6 weeks to 4 months in Indonesia. Methods: This cross-sectional study of 433 infants was conducted between September 2018 and February 2020. Information on FGIDs was collected using the Infant Gastrointestinal Symptom Questionnaire and the Feeding Practice and Gut Comfort Questionnaire. Adapted Rome IV criteria were used to define the FGIDs. Results: The prevalence of regurgitation was 26.3%; 16.8% of the infants presented cryingrelated symptoms and 5.5% exhibited constipation. The statistical analyses revealed that constipation was associated with sex (odds ratio [OR], 2.74; 95% confidence interval [CI], 1.07-7.71; p=0.043), employment of the father (OR, 0.3; 95% CI, 0.12-0.77; p=0.01), and education of the mother (OR, 1.92; 95% CI, 1.07-3.51; p=0.031). Length at birth (OR, 0.74; 95% CI, 0.55-0.99; p=0.042) was associated with constipation. Length at visit (OR, 0.83; 95% CI, 0.76-0.91; p<0.001) was associated with regurgitation, and the weight at visit (OR, 0.58; 95% CI, 0.35-0.96; p=0.038) was associated with crying and/or colic. A history of parental FGIDs was associated with crying-related symptoms (OR, 2.12; 95% CI, 1.23-3.68; p=0.007). Conclusion: Regurgitation, crying, and constipation are common FGIDs in infants. Some parental and infant characteristics may be predictors for FGIDs. Further investigations are needed to evaluate the clinical relevance of our findings. Understanding the determinants of FGIDs will benefit healthcare professionals and parents to improve infant's quality of life and better manage these condition.

의과대학 교원 현황과 업적평가제도 특징 분석 (Current Status and Performance Evaluation Systems of Faculty in Korean Medical Schools)

  • The aim of this study is to analyze the current status and performance evaluation systems of faculty in Korean medical colleges and professional graduate medical schools (called medical schools). We developed a research tool based on previous studies and distributed it to 40 medical schools from July to October 2017. The response rate was 100%. We calculated the number of faculty members and analyzed the faculty evaluation systems and awareness according to national and private medical schools. As of 2017, the number of medical faculty in Korea was 11,111 (4,973 faculty were employed by their alma mater, which is 44.76% of the total), with non-medical doctor faculty accounting for 754 of the total. The medical schools reflect research achievements as most important for re-appointment and screening to promote faculty, and the area of education is secondary excepting clinical faculty of private medical schools. However, important issues in the faculty evaluation deal with the relevance of research achievement and the need for qualitative assessment. Some medical schools revised or have been revising the faculty evaluation system in areas such as minimum standards of education for promotion and separation of promotion and tenure review. Opening non-tenure track lines for faculty show positive effects such as increasing the number of positions for hire and easing the financial burdens of medical schools. Downfalls include inconsistencies between the responsibilities and actual practices of tenure not being available and the instability of faculty's status. In conclusion, medical schools need to prepare a faculty evaluation system that fits the position of faculty members and attempt to establish a reasonable compensation system.

  • Purpose: This study was aimed to develop and evaluate of a Problem-based Learning (PBL) in the course of Nursing Management and Ethics. Method: The design of the study was both methodological and one group only pre-post design. The sample included 61 senior students who are currently enrolled in Nursing management and Ethics course in college of nursing. Data regarding PBL evaluation were collected on the critical thinking and clinical reasoning using structured questionnaires during March to June, 2005. Data were analyzed using descriptives and paired t-test. Results: A total of three PBL packages was developed by the two faculty members and two teaching assistants who are majoring in nursing management. PBL packages that had been developed was applied to 61 senior students for three months. Critical thinking and clinical reasoning were measured twice pre and post the application of PBL packages. There were statistically significant differences in the critical thinking and clinical reasoning between the pre and post PBL application. Conclusion: PBL was considered to be effective in understanding the learning concepts in the Nursing Management and Ethics. Further research on the facilitative strategies and development model considering the characteristics of Nursing Management and Ethics course is needed.

간호사 국가고시 대비 기본간호학 문제집 문항의 변화분석 (Analysis of Test Items for National Qualifying Examination in Fundamentals of Nursing)

  • Purpose: The purpose of this study was to explore changes in the test items in the preparation book for the national qualifying examination. Test items which developed in the year 2000 and 2004 through nationwide faculty workshop in Fundamentals of Nursing were examined. Method: Test items for the national qualifying examination for 2000 and 2004 in Fundamentals of Nursing were analyzed using frequencies and percent. Results: The test items for the national qualifying examination for 2004 in Fundamentals of Nursing showed an increase in the number of test item over the year 2000. According to McGuire's taxonomy, domains of the test items in 2000 were recall(65.1%), interpretation (22.0%), and problem solving(12.9%) in that order. In 2004 items for the recall domain(74.7%) increased 9.6% above the level of 2000, and interpretation and problem solving domains showed a slight decrease. With regard to type of test items, the proportion of A type in 2004 was the same with 37.6% as 2000, and that of K type(61.5%) increased by 0.7% over the year 2000(60.8%). Conclusion: The test items for the national qualifying examination of 2000 and 2004 were not different except for the increase in the number of test items.
