Integrated Literature Review of Infection Control of Nursing Care Workers in Long-term Care Facilities (장기요양시설에 근무하는 요양보호사의 감염관리에 관한 통합적 문헌고찰)

  • Purpose: This study aims to conduct an integrated literature review of infection control studies conducted by nursing care workers in long-term care facilities in Korea. Methods: Through the domestic search engines RISS and KISS, seven articles were selected by searching for theses and academic journals published in Korea from 2008 to January 2020. Results: In total, six research studies and one intervention study out of seven studies were analyzed. Measurement tools for examining the knowledge and performance of infection-related care workers consisted of skin infection, aspiration pneumonia, urinary tract infection, and hand hygiene. Factors affecting nursing care workers' infection management performance were infection management knowledge, education level, health status, and importance awareness. Conclusion: This study showed it is necessary to develop a tool that can accurately measure nursing care workers' infection management knowledge and performance. In addition, it is necessary to develop an intervention program for nursing care workers' infection control.

Actual Disinfection and Sterilization Control in Korean Healthcare Facilities (국내 의료기관의 소독과 멸균 관리 실태)

  • Purpose: This study was done to investigate the status of disinfection and sterilization in healthcare facilities. Method: A survey of 193 Korean healthcare facilities was conducted from February 8 to March 7, 2013. Data were analyzed using descriptive statistics, ${\chi}^2$ test, Fisher's exact test, one-way ANOVA, Scheffe with SPSS WIN 18.0. Results: Of the healthcare facilities 93.2% had specific guidelines for disinfection/sterilization, but only 47.9% had a committee on disinfection/sterilization for decision-making, less than half (42.7%) conducted regular monitoring of actual practices, while 83.9% had established procedures for recovery in case of problems with the disinfection process and 89.0% kept records and archives of disinfection practices. Cleaning process, selection of chemical disinfectants and process of disinfection and sterilization were found to be inadequate in some healthcare facilities. Perception score for adequacy of medical instruments was 8.10, environmental disinfection was 7.20, and sterilizer management was 8.45 out of a possible 10. Conclusion: Compared to larger institutions, smaller healthcare facilities had less effective disinfection and sterilization management systems, while some facilities showed inadequate practices for medical equipment and general sterilization. Better academic and state-level support is recommended for smaller facilities in order to establish a better system-wide management system.

A Delphi Study for Developing a Person-centered Dementia Care Online Education Program in Long-term Care Facilities (장기요양시설 인간중심 치매케어 온라인 교육 프로그램 개발을 위한 델파이 조사연구)

  • Purpose: There has been a growing recognition that person-centered care enhances the quality of life of nursing home residents with dementia. This study was conducted to develop a person-centered dementia care online education program for direct care staff in long-term care facilities. Methods: Delphi method with expert group was used to validate contents. We developed 61 draft items based on literature review. Twenty experts participated in consecutive three round surveys including 5-point Likert scale questions and open-ended questions. Based on experts' opinions, the content validity ratio for content validity and the coefficient of variation for stability were calculated. Results: Three-round Delphi surveys and additional feedback from the expert panel established a consensus of core contents: 1) dementia (7 categories), 2) person-centered care (6 categories), 3) communication (8 categories), and 4) behavioral and psychological symptoms of dementia (6 categories). Specific sub-categories in each category were differentiated according to the job qualifications (65 sub-categories for registered nurses, 64 sub-categories for nursing aids, and 41 sub-categories for personal care workers). Conclusion: This delphi study identified person-centered dementia education curricula, in which the person-centered approach should be a key policy priority in Korean long-term care system. Now it is urgently needed to develop education programs utilizing online platforms that enable efficient and continuous learning for long-term care staff, which can contribute to behavior changes in the person-centered dementia care approach and improvement of care quality in long-term care facilities.

Effect of Virtual Reality Program for Alleviating Behavioral and Psychological Symptoms of Dementia Patients (치매 환자의 행동심리 증상완화를 위한 가상현실 프로그램의 효과)

  • Purpose: This study examined the impact of a virtual reality intervention program based on psychological needs on behavioral and psychological symptoms, apathy, and quality of life (QOL) in patients with dementia or mild cognitive impairment living in nursing facilities. Methods: This study is nonequivalent control group pretest-posttest design of quasi-experimental study. The study collected data from November 18, 2020 to July 24, 2021 from patients with dementia or mild cognitive impairment (30 in the experimental group and 30 in the control group) at three nursing facilities in G city using self-reporting and caregiver-informant reporting methods. The analysis employed the chi-square test, Fisher's exact test, paired t-test, independent t-test, Wilcoxon signed rank test, Mann-Whitney U, repeated measures ANOVA, GEE, using SPSS/WIN 27.0. Results: The severity of behavioral and psychological symptoms (Wald 𝛘2 = 2.68, p = .102) and the care burden of caregivers (Wald 𝛘2 = 1.72, p = .190) were not significant and was no significant time and group interaction effect (Wald 𝛘2 = 0.63, p = .426, Wald 𝛘2 = 0.52, p =. 471). The difference in apathy and QOL score were statistically significant for the group-time interaction (F = 43.65, p < .001; F = 4.35, p = .041). Conclusion: The virtual reality intervention program of this study shows a positive effect on the apathy reduction and QOL of patients with dementia or mild cognitive impairment residing in nursing facilities.

A Study on Patients' Perception of Nurse's Behavior in Protecting Patient Privacy (간호사의 환자 프라이버시 보호행동에 대한 환자의 인식도)

  • Purpose: The purpose of this study was to investigate the patient's perception of the nurse's behavior in protecting patient privacy and to make suggestions for medical facilities to increase protection of patient privacy. Method: The research was a survey study The data on protection of privacy in primary nursing, for physical privacy, of patient information and in private conversations were collected in October 2005 from 187 patients in a university hospital. Frequencies, means, t-test, ANOVA, and $x^2$-test were used to analyze the data. The SPSS 13.0 for Windows program was used. Results: The mean score for patients' perception of nurse protection of their privacy was 3.33. There were significant differences in perception of protecting patients' privacy according to gender for private conversation, according to level of education for all but physical nursing and for number of admissions for total score and for primary nursing. Conclusion: The results of this study suggest the following: a) Institutional polices and nursing guideline should be clearly stated as to the nurses' duty to protect patient privacy. b) Medical facilities should be arranged in a way that allows for the protection of patients' medical information, and should focus on patient privacy. c) It is necessary of nurses to receive periodic in-service education on protecting patient privacy.

Wound Care Needs of Hospital-based Home Care Nursing at Elderly Care Facilities (노인요양시설의 간호인력과 비간호인력간 상처관리에 대한 병원중심 가정간호 요구도 비교)

  • Background: The purpose of this study is to investigate the needs for wound care services in the elderly care institutions by analyzing hospital-based home care nursing in select facilities in Korea. Methods: A total of 92 staffs at the elderly care institutions, located throughout the country, completed self-report questionnaires between February 1 and November 31, 2009. SPSS ver. 17.0 was used for data analysis, regarding frequency and chi square. The 5% level of significance was the critical level for acceptance of the study's hypotheses. Results: Results were as follows; group A and group B were rated as 'high' and 'very high', respectively, in care of simple pressure ulcer, care of complicated pressure ulcer, stitch out, care of simple surgical wound, ostomy care, complicated ostomy care, simple dressing and complicated dressing. Conclusion: Political decisions intended to promote visits by hospital- based home care nurses to the elderly care institutions would be a prudent course.

A Structural Equation Model of Quality of Life in Nursing Home Residents (노인요양 시설 입소 노인 삶의 질 구조모형)

  • Purpose: This study was done to develop a predictive model for quality of life of elderly residents in long-term care facilities (LTCF). The study was based on Brofenbrenner's ecological system theory and a literature review. Methods: Data were collected using a convenience sample of 205 elderly residents in 2 nursing homes located in D city and 1 nursing home located in K province. The exogenous variables were individual factors, family support, and facility environmental factors. The endogenous variables were self-esteem, accommodation adaptation and quality of life of elderly residents in LTCF. Collected data were analyzed through structural equation modeling using AMOS 20.0. Results: Eleven of the twelve hypotheses were supported, but the hypothesis that facility environment factors effect self-esteem was not supported. Quality of life of elderly residents in LTCF was explained first by facility environmental factors, followed by self-esteem, individual factors, accommodation adaptation, and family support with an explanatory power of 83.0%. Conclusion: To improve the quality of life of elderly residents in LTCF, the service and environment preparation provided by facilities is important, and it is necessary to provide emotional counseling to improve the self-esteem of these elders.

A Study on Working Conditions and Factors Related to Job Satisfaction of Nurses in the Long-term Care facilities for the Elderly (노인장기요양기관에 근무하는 간호사의 근무실태 및 직무만족도 영향 요인)

  • Purpose: Th study is to explore the working conditions and factors related to job satisfaction of nurses in long-term care facilities for the elderly. Methods: A total of 127 nurses working long-term care facilities, from June to November 2014, completed questionnaires. Face to face interviews were conducted. The questionnaire consists of a total of 32 questions, including 5 questions related to general characteristics, 15 questions related to working conditions, and 12 questions related to job satisfaction(likert scale of 1 to 5). Results: The majority of nurses were employed in institutional care facilities, in urban area, and in privately owned facilities. On average, employed nurses worked 170.7 hours per month. Only 40.9% of them got more than two million won for their monthly salary. They were satisfied with the nature and worth of their work and the workplace relationship, but not with their wages and promotion opportunity 40.2% of nurses reported satisfaction with job. In multiple logistic regression analysis, age was a statistically significant influencing factor on general job satisfaction. Conclusion: The findings suggest that policies must be to improve nurses' working conditions, for wages, and to support them with education and training for skills development.

Social Support and Hope of the Unwed Mothers in the Facilities (미혼모의 사회적 지지와 희망)

  • Social support may play an important role in relieving the feelings of guilt and shame of unwed mothers due to social insults and ignorance and in helping them from physical and psychological harm caused by them. In addition hope may encourage to seek out a new way of life. To determine if a relationship between perceived social support and hope exists in unwed mothers a descriptive study was conducted in the facilities providing basic data for promoting the health of unwed mothers. A demographic data sheet and two instruments were used : Indirectly Perceived Social Support Scale(IPSSS) developed by Park Jee Won (revised by Jo Moo Yong), Miller Hope Scale(adapted and revised by Lee Mi Ja), 95 unmarried mothers participated. The mean score of perceived social support was 3.17. Family support was the highest(3.47) and unwed father support was the lowest(2.53) in sub-scales. Material support(3.53) was the highest in family support. Emotional support(3.58) was the highest in friends support. Informational support(3.39) was the highest in facilities-concerned personal support. The mean score of hope was 3.41 : the satisfaction with self, others and life was the highest(3.82) and anticipation of future was the lowest(3.27) in sub-scale. There was not a statistically significant relationship between social support and hope. A statistically significant relationship was found between facilities-concerned personal support and hope. In conclusion, facilities-concerned positive encouragement for unwed mothers and a program promoting support and hope from families, friends and unwed father are necessary to promote hope and health in unwed mothers.

An Exploratory Study on the Use of Care Robots and Devices by Caregivers of Older Adults Residing in Facilities (시설 거주노인 돌봄 제공자의 돌봄 로봇 및 기기 활용에 관한 탐색적 연구)

  • The purpose of this study was to conduct an exploratory and descriptive survey study design to examine 1) the physical difficulty by care task, 2) overall occupation-related physical burden by the characteristics of caregivers and care receivers, and 3) the level of help by the use of care robots and devices among formal caregivers working in facilities for older adults. In this study, 308 formal caregivers working in facilities were recruited from 8 nursing homes and 5 geriatric hospitals between Aug. 2021 and Sept. 2022 in South Korea. There were significant differences in ADLs between nursing homes and geriatric hospitals: eating, dressing, hygiene, transfer to bed, transfer to the toilet and walking. For each care item, the items the caregivers had the most difficulty ranked first were bathing, excretion assistance, and mobility support. The difference in occupation-related physical burden according to general characteristics was found to have statistically significant differences in female and bad perceived health. Among the 205 caregivers who had experience using care robots and devices, pressure ulcer prevention was the most experienced one, and those also were the most helpful and exercise aids were the least helpful. In order to reduce the physical burden on care providers, it is necessary to develop and introduce a care robots centered on the care site. Furthermore, national level public support systems are required to enable facilities to actively utilize care robots and devices.
