노인요양시설의 평면 특성에 관한 고찰 (A Study on the Characteristics of the Floor Plan of Skilled Nursing Facilities for the Elderly in Rural Areas)

  • The purpose of this study is to clarify the actual conditions of the floor area of skilled nursing facilities in rural area. The thorough investigation and observation works were made from the viewpoints of floor area of the skilled nursing facilities. This article discusses about the basic condition and the floor area for daily living activities of the elderly in 3 skilled nursing facilities in west gyeongnam province. As the result, There is a wide gap between floor area ratio of skilled nursing facilities. The characteristics of the private establishment skilled nursing facilities in the west gyeongnam province were figured out through the analysis of spatial composition in skilled nursing facilities for the elderly. The skilled nursing facilities in west gyeongnam mainly focus on the living environment orientated toward the bedroom. The difficulty is to encourage the communication between the skilled nursing facility patients because the facilities are composed of small semi-public zone.

  • The purpose of this study is to clarify the actual condition of the spatial use patterns of the elderly in skilled nursing facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and behavioral places of the recuperating elderly. This article discusses about the basic characteristics of 194 elderly and the actual condition of the daily living activities of 24 elderly in 3 skilled nursing facilities. The results of this study are summarized as follows; There is no wide difference between the skilled nursing facilities for the dementia and the skilled nursing facilities on the characteristic of living behaviors of the elderly. The usage of behavioral places in recuperating facilities are influenced by the physical environments and the basic characteristics of the elderly.

노인요양시설에서 활용되는 간호중재의 중요도와 수행도 분석 (The Importance and Performance of Nursing Interventions Perceived by Nurses in Long-term Care Facilities for Elderly)

  • Purpose: This study was to identify nursing interventions which were perceived highly in the importance and performance of nursing interventions by nurses in long-term care facilities for elderly. Method: Data was collected from nurses working in long-term care facilities for elderly over 2 years who participated in continuous education by Korea Association of Senior Citizens Welfare Institution. Data was analyzed using mean and paired t-test to compare difference between the importance and performance of each nursing intervention. Results: Among 264 nursing intervention, 49 nursing interventions were considered highly important and performed very often by nurses. Especially, 11 nursing interventions had significant difference between the importance and the performance, which meant that nurses perceived them as the most important and they were not implemented often as much as that by nurses. Conclusion: The results of this study revealed that what kinds of the nursing interventions were perceived highly important and performed very often by nurses in long-term care facilities for elderly. These nursing interventions can be utilized in the development of standardized nursing intervention classification to be used for the long-term care facilities for elderly.

Common spaces such as day room, dining room plays an important role for their daily living activities, because that most of the elderly with dementia stay long during daytime. Layout of each private room and common space seems very crucial point in designing these facilities, in order to enable demented people to live their lives more independently.

  • Purpose: The aim of this study was to provide information on the extent and variations of elderly residents' nursing care needs, and provision of nursing care across long term care facilities. Methods: A nationwide survey was conducted on nurse managers from 1,041 long term care facilities, by e-mail or fax, from August 16 to September 30 in 2017. A self-reported questionnaire consisting of 5 domains was used to collect data. Results: Facilities with more than 30 residents were more likely to need skilled nursing services and to obtain the nursing staff such as a registered nurse and a nurse's aide. Awareness and satisfaction of hospital-based home nursing care was high in all facilities. In addition, there are some differences in nurse managers' perceptions of the level of healthcare resources and required action by facility size. Nurse managers of senior congregate housings were more likely to have considerable difficulty in dealing with healthcare needs of residents and recognizing the healthcare resource shortage. A majority of nurse managers agreed on the need to employ a registered nurse. Conclusion: This study confirmed that it is essential to increase nurse staffing level and to reform the long term care insurance for enhancing the accessibility of healthcare services, especially for the residents in small long term care facilities. There is also a need to provide diverse education and training opportunities for nursing staff working in long term care facilities.

경인지역 요양시설의 식사지원 서비스 실태 (Meal Services at Long-term Care Facilities in Kyung-In Area)

  • Purpose: This study was aimed at understanding meal services provided at long-term care facilities. Method: Interview survey was conducted using questionnaires at 254 facilities located in the Kyung-In area. Result: Of the residents, 37.4% were eating meals unassisted. Eating places included living rooms and residents' rooms in most facilities. Major noise source was television in 63.8%. Apron was applied to all elderly residents at mealtimes in 49.6% of the facilities. Half of the facilities used feeding utensils except for ordinary spoon and chopsticks. Of the facilities having individual prosthetic devices, dentures were applied before eating in 98%, glasses in 20.2% and hearing aids in 9.2%. Most facilities included the residents' favorite foods in menu: wheres, only 9.4% offered the menu which residents could choose. Conclusion: Standard guidelines and staff education for meal services need to be provided for elderly residents.

  • The study investigated the actual conditions of U.S. nursing-home facilities and Korean nursing homes and conducted a comparative analysis of those nursing facilities to propose the following suggestions for facility improvement: 1.) The number of sickbeds per nursing unit in Korea should be maintained a level of more than 30 beds like the United States, and each rest room in a residential facility should have an area of at least 4.2 square meters. 2.) Each room of a nursing home should be occupied by four or less persons for privacy. 3,) The minimum areas of residential rooms should be larger than 9.2 sq. meters for single rooms and 6.1 sq. meters for quadruple rooms. 4.) Nursing homes for the elderly need to improve rest room spaces and interior fishes to prevent them from falling down and getting hurt and thus help them live safely.

노인간호요양시설의 QMI 개발과 현장 실태분석 (Development of Nursing Home Quality Management Index & Application in the Field)

  • Purpose: Expecting a new system of nursing security for elderly in operation in 2007, a substantial expansion of nursing facilities for elderly and quality management of those facilities are imperative now. Therefore. specific quality management measures or schemes which could be used for the operation of small-scale nursing homes are in urgent need. The purpose of this study was to develop a Quality Management Index(QMI) to guide Nursing Home management. Method: This study developed a QMI for small-scale nursing homes, which focused on quality management in structural dimensions such as environment, staffs, local community network, atmosphere, and quality management in process dimensions such as nursing, communication, resident satisfaction. The QMI developed in this study is based on extensive review of literature and the actual experiences of nursing home operation. It consists of 7 dimensions, 39 components and 148 indexes, which have been verified by three times validity tests of experts group. Then the QMI was used to evaluate the quality management levels of nursing homes in Korea. Result: In sum, after analyzing all nursing homes in Korea operated by nurses, this study found that there were significant differences of quality management level among facilities in several dimensions such as environment, staffs, local community connections, communication. In dimensions such as nursing, atmosphere, resident satisfaction, however, little differences of quality management level among facilities were reported. It is considered that this result has to do with several factors like the size of facilities, operation expenses, operation forms, the disposition of professional human resources, and philosophy and management policy of the owners. As far as generalization is concerned, however, it needs to be noted here that the sample size for this study is not statistically big enough to generalize the results. Conclusion: Having Developed a QMI for small-scale nursing home with 7 dimensions, 39 components and 148 indexes, this study is expected to be used in developing more elaborated quality evaluation tools for nursing homes, and also function as a practical guide of quality management for those who are opening and managing nursing homes. I hope this quality index could lead to further development of a standardized quality management index, and eventually contribute to quality improvement of nursing homes.

시설 거주노인의 자아존중감과 건강상태가 적응에 미치는 영향 (Effects of Self-esteem and Health Status on Adaptation of Elderly Residents in Facilities)

  • Purpose: This study was designed to identify effects of self-esteem and health status on adaptation of elderly residents in facilities. Methods: The sample consisted of 151 elderly residents. The data collected from January to April 2010 were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients and stepwise multiple regression. Using instruments were self-esteem scale (SES) (Rosenberg, 1965), Korean health status measure for the elderly (KoHSME) (Shin et al, 2002), and nursing home adaptation scale (Lee, 2007). Results: The mean scores of self-esteem ($2.90{\pm}0.71$), health status ($2.15{\pm}0.53$) and adaptation ($2.98{\pm}0.44$) of elderly residents in facilities were above the average. Self-esteem was significantly varied according to religion, economic status and living expenses. Health status was significantly different according to age, disease and motivation of getting into the facilities. Adaptation was significantly different according to religion, satisfaction of facilities and decision maker of getting into the facilities. Significant correlations were found between self-esteem, health status and adaptation. Self-esteem and health status were influencing factors of adaptation (22.6%). Conclusion: These findings indicate that perceived self-esteem and health status may be requirements for promoting adaptation of elderly residents in facilities. These results could be utilized in the development of supportive programs for elderly adaptation.
