사별노인의 성별에 따른 의존성과 학대 및 우울에 관한 연구 (Dependency, Abuse, and Depression by Gender in Widowed Elderly)

  • Purpose: The purpose of this study was to investigate the effects of dependency and abuse on depression according to gender in widowed elderly. Method: A convenient sample consisted of 246 widowed elderly who were more than 65 years old in four cities. Data was collected using a structured questionnaire from August to September, 2002. A dependency Scale developed by Ahn (1999) was used to measure the level of dependency. Emotional abuse and physical abuse were measured by 10 items for emotional abuse and 7 items for physical abuse selected out of the Conditions Scale of Elder Abuse. The level of depression was measured by the Geriatric Depression Scale (GDS). The SPSS WIN 11.0 version program was used for data analysis. Result: In male widowed elderly, dependency affected depression indirectly through emotional abuse. While in female widowed elderly, dependency affected depression directly and affected emotional abuse indirectly. Conclusion: The study showed that dependency was the most explainable variable on depression in widowed female elderly. Therefore, it dependency should be assessed first in nursing intervention to relieve depression of widowed elderly.

대학생의 자아존중감, 사회적 지지, 인터넷 게임 의존성과 스마트폰 의존성의 관계에 대한 연구 (A Study on the Relationship between Self-Esteem, Social Support, Smartphone Dependency, Internet Game Dependency of College Students)

  • Purpose: The purpose of this study was to investigate the relationship between self-esteem, social support, smartphone dependency and internet game dependency of college students. Methods: This was a descriptive study. The survey participants were 299 college students in M city and I city. The data were collected from June 2 to June 20, 2014 and self-report questionnaires including Self-Esteem Scale, Multidimensional Scale of Perceived Social Support, Smartphone Dependency Sale, Internet Game Dependency Scale. Data were analyzed by descriptive statistics, independent-sample t-test, ANOVA, stepwise multiple regression. Results: Social support & smartphone dependency showed significant differences according to gender. Smartphone dependency was found to have a statistically negative correlation with self-esteem, social support and positive correlation with internet game dependency. Internet game dependency was found to have a statistically negative correlation with self-esteem, social support. Social support was found to have a statistically negative correlation with self-esteem. Stepwise multiple regression analysis revealed that the significant predictors of smartphone dependency were internet game dependency, gender, self-esteem, accounted for 16.6% of the variance. Conclusion: It is necessary for reduction program of college students' smartphone dependency that consider their internet game dependency, gender, self-esteem.

응급실에 내원한 COVID-19 의심환자와 일반환자의 간호의존도 비교 연구 (A Comparative Study on the Nursing Dependency of Suspected COVID-19 Patients and General Patients in the Emergency Department)

  • Purpose: This study was conducted to investigate the nursing needs and workload of nurses according to nursing dependency for effective placement of nursing staff in the emergency department (ED). Methods: In June 2020, 256 adult patients who visited the ED were classified as two groups, suspected COVID-19 patients and general patients. The participants'electronic medical records were analyzed using descriptive statistics, t-test, 𝑥2-test, and Fisher's exact test using the SPSS. Results: The patient dependence score showed a significant difference between the two groups, with an average of 13.99±1.85 for the suspected COVID-19 patient group and 10.58±2.10 for the general patient group (t=12.42, p<.001). There were statistically significant differences in communication (t=3.28, p=.001), mobility (t=3.29, p=.001), nutrition, elimination, and personal care (t=7.34, p<.001) among the six domains of nursing dependency. In the domains of environment, safety, health, and social needs, the dependency score was 3 for all suspected COVID-19 patients and 1 for all general patients. Conclusion: The results of this study confirmed that infection control activities of emergency patients who need isolation affect the patients' nursing dependency on nursing care.

한국형 응급환자 분류도구 및 환자의존도에 따른 환자군별 간호시간과 간호사 배치수준 (Nursing Hours and Nurse Staffing according to Korean Triage Acuity Scale and Patient Dependency)

  • Purpose: The purpose of this study was to analyze nurse staffing according to patients' acuity and dependency by measuring nursing hours. Methods: The study sample included patients who visited the adult emergency departments (EDs) of three tertiary referral hospitals and nurses who worked on shifts for 48 hours from October 24 to 26, 2019. Hourly patient census and nurse staffing were analyzed. Patient acuity was measured using the Korean Triage and Acuity Scale (KTAS), ranging from Level 1 (highest) to Level 5 (lowest). Patient dependency was measured using six items (e.g., clinical attention and communication) and classified into four groups. Nursing activities were observed every 10 minutes and nursing hours per patient and nurse staffing were analyzed according to acuity and dependency. Results: Nurse-to-patient ratio ranged from 1:1.8 to 1:4.2 during the 48 hours of observation. The average work hours of nurses, excluding breaks and meals, was 8.57 hours; 42.5% of which was spent providing direct care. Higher acuity and dependency were associated with higher nursing hours and staffing level. Patients with KTAS Level 1 were provided 74.3 minutes per hour, 5.02 times higher than Level 5 (14.8 minutes). Patients in the highest dependency group were provided 87.4 minutes per hour, 5.75 times higher than the lowest group (15.2 minutes). Newly arrived patients received more nursing hours than continuously stayed patients within the same KTAS Levels. Conclusion: Large variations were found in hourly patient census, acuity, and dependency. Nurse staffing in EDs should be determined based on patient acuity and dependency.

미혼 흡연여성들의 불안, 우울 및 자아존중감이 흡연양 및 니코틴 의존도에 미치는 영향 (The Effects of Anxiety, Depression and Self-Esteem on Smoking and Nicotine Dependency Among Unmarried Woman Smokers)

  • Purpose: The purpose of the study was to investigate the effects of anxiety, depression and self-esteem on the amount of smoking and nicotine dependency among woman smokers. Method: The convenience sample consisted of 121 unmarried woman smokers. The STAI, the CES-D Scale, the Self-Esteem scale and the FTQ were utilized to measure the level of anxiety, depression, self-esteem, and nicotine dependency. The amount of smoking was calculated based on the number of cigarette used. Results: Results indicated that woman smokers were depressed. Nicotine dependency was relatively low. Subjects had smoked a mean of 9.58 cigarettes per day. The level of depression influenced on smoking and nicotine dependency. Self-esteem influenced on the amount of smoking and anxiety influenced on nicotine dependency. Conclusion: The results of this study revealed that the amount of smoking of unmarried woman smokers was very high. Both the amount of smoking and nicotine dependency were affected by levels of depression. Education programs and campaigns are needed to prevent smoking of young adult women.

흡연 남자대학생의 심뇌혈관질환에 대한 예방 지식과 니코틴 의존도 (Knowledge on Cardiovascular Prevention and Nicotine Dependency among Smoking Male College Students)

  • Purpose: This study aimed to identify the level of knowledge on preventing cardiovascular disease (CVD) and nicotine dependency among smoking male college students, and to examine the predicting factors of nicotine dependency. Methods: This study was conducted as cross-sectional descriptive research using structured questionnaires. Data were collected from 411 smoking male college students ($22.5{\pm}2.59$ yr) at two universities located in two cities in Korea from September 2012 to April 2013. Logistic regression analysis was performed to estimate age-adjusted odds ratios of nicotine dependency. Results: The mean value of nicotine dependency was $4.2{\pm}1.96$ and the percentage of participants who had strong nicotine dependency (${\geq}7$) was 10.7%. Logistic regression analysis showed that, when adjustment was made for age, nicotine dependency was predicted by the frequency of alcohol drinking (${\geq}3$ times/week), knowledge on CVD prevention, the age starting drinking, and the total length of smoking. Conclusion: Nicotine dependency was strongly related to the frequency of drinking alcohol and lower knowledge on CVD prevention. Therefore, male college students who have a dependent smoking habit and drink alcohol frequently need to take more interest in health through educational counseling to modify their lifestyle behaviors and to have preventive knowledge related to CVD.

흡연 남자대학생의 스트레스, 금연에 대한 자기효능감 및 흡연유혹이 니코틴 의존도에 미치는 영향 (Influence of Stress, Self-efficacy for Smoking Cessation, Smoking Temptation and Nicotine Dependency in Male College Students who Smoke)

  • Purpose: The purpose of this study was to explore the levels of stress, self-efficacy for smoking cessation, smoking temptation, and nicotine dependency, and to identify factors influencing nicotine dependency among male college students who smoke. Methods: In this study, a cross-sectional survey design was adopted for 283 male college students who smoke in D city, Korea. Data was analyzed for descriptive statistics, Pearson's correlation, and multiple regression using the SPSS 20.0 program. Results: The mean score of stress was $27.00{\pm}6.28$, self-efficacy for smoking cessation was $25.61{\pm}6.71$, smoking temptation was $53.87{\pm}11.02$, and nicotine dependency was $3.63{\pm}1.96$. There was a significant positive correlation between stress and nicotine dependency (r=.58, p<.001) and between smoking temptation and nicotine dependency (r=.59, p<.001). There was a significant negative correlation between self-efficacy for smoking cessation and nicotine dependency (r=-.59, p<.001). The significant factors influencing nicotine dependency were stress (${\beta}=.357$, p<.001), self-efficacy for smoking cessation (${\beta}=.359$, p<.001), and smoking temptation (${\beta}=.297$, p<.001). This model explained 60.3% of variance in nicotine dependency (F=105.59, p<.001). Conclusion: The results suggest that an intervention program is needed to reduce the perception of stress and smoking temptation, and to increase the ability of self-efficacy for smoking cessation among male college students who smoke with the intention to quit smoking.

  • Purpose: The purpose of this study was to analyze the effects of a quit smoking program using the Web and short message service on exhaled carbon monoxide, self-efficacy, and depression according to nicotine dependency level in undergraduate students. Methods: In this study a non-equivalent control group pretest-posttest design was applied. The participants included 90 students (52 in the low nicotine dependency group and 38 in the high nicotine dependency group) who succeeded in quitting smoking. Data were collected on 3 occasions, that is, before the program, immediately after the program, and 3 weeks after the program. Collected data were analyzed using independent t-test, repeated measure ANOVA, and paired t-test with SPSS 20.0. Results: Exhaled carbon monoxide was higher in the high nicotine dependency group than in the low nicotine dependency group. Self-efficacy significantly increased 3 weeks after the program in the low nicotine dependency group and significantly increased immediately after the program in the high nicotine dependency group. Depression significantly decreased 3 weeks after the program in the low nicotine dependency group. Conclusion: Self-efficacy may be enhanced when it is dealt with during an early phase of the quit smoking program for the high nicotine dependency group. Long-term intervention and persistent intervention are needed with regard to depression during a quit smoking program.

한국형 환자분류체계의 단축형 개발과 간호요구 유형 분류 (Shortening of Korean Patient Classification System-1 and Classification of Nursing Care Needs)

  • Purpose: The purpose of the study was to shorten the KPCS-1 (Korean Patient Classification System-1) for predicting nursing care need level and to explore whether the patients can be clustered by their acuity and dependency. Methods: The participants were inpatients in two surgical wards and two internal medicine wards at a teritory hospital during 14 days investigations. The KPCS-1 was evaluated once a day for all inpatients and 2,082 cases of data from a total of 411 patients were analyzed. Results: The items were reducted from 50 items to 26 items by partial least squares analysis and expert review. Through factor analysis, it was confirmed that hygiene, diet, elimination, and exercise were categorized as dependence factors. Patients were clustered with low acuity/low dependency (average score: 7.68±2.81/1.05±1.33), high acuity/low dependency (average score: 17.20±4.15/1.94±2.40), medium acuity/high dependency (average score: 13.56±5.30/9.66±2.64) through cluster analysis. The total score of the three groups for their nursing care needs was 8.73±3.36, 19.14±5.74, and 23.24±6.31 in order, and the results showed a statistically significant difference (F=1712.12, p<.001). Conclusion: The shortening of the KPCS-1 and the new criteria for categorizing patients according to acuity and dependence will increase clinical utility and be useful for manpower assignment criteria in detail.

금연프로그램이 성인 흡연자의 금연 및 흡연행위 변화에 미치는 영향 (Effect of a Smoking Cessation Program on the Stop-smoking Rate and Nicotine Dependency for the Adult Smoker)

  • Purpose: This study was to compare the stop-smoking rate and nicotine dependency for an adult smoker after implementing the smoking cessation program. Method: Fourty-six adult smokers participated. Self report questionnaire and telephone interview were conducted at the pretest, 6 months later, 1 year later, 2 years later, 3 years later. Result: 1. Stop-smoking rate of smoking cessation program participants was 43.5% 6 months later, 35.6% 1 year later, 24.4% 2 years later, 45.5% 3 years later. 2. The mean of nicotine dependency for continuing smokers was significantly decreased 6 months later(paired-t=4.719, p=.000). 3. The mean of nicotine dependency for continuing smokers was significantly decreased 1 year later(pired-t=4.296=.000). 4. The mean of nicotine dependency for continuing smokers was significantly decreased 2 years later(paired-t=2.778. p=.000). 5. The mean of nicotine dependency for continuing smokers was significantly decreased 3 years later(paired-t=2.795. p=.000). Conclusion: The smoking cessation program was effective to induce cessation in the adult smoker, and to decrease nicotine dependency in the continuing adult smoker.
