A Case Report of a Subarachnoid Hemorrhage Patient whose Paresthesia Improved by Ssanghap-tang (쌍합탕 투약 후 수족부 저림증이 호전된 지주막하출혈 환자 증례보고)

  • ■ Objectives This case is to report the effect of Ssanghap-tang on paresthesia of a subarachnoid hemorrhage patient. ■ Methods A male Korean patient was treated with Ssanghap-tang and acupuncture, moxibustion for total 30 days. We observed limbs numbness, circumferences, other symptoms such as weakness and any adverse event. ■ Results After treatment, numeric rating scale of limb numbness was improved to 5 from 8 at admission without any adverse event. However, we could not find any significant differences on hand circumference and limb weakness. ■ Conclusion This case suggests that Ssanghap-tang and acupuncture, moxibustion therapy could be effective in improving paresthesia of subarachnoid hemorrhage patient.

A Case Report of a Guillain-Barre Syndrome Patient Suspected of Having Acute Transverse Myelitis Treated with Korean Medicine (급성 횡단성 척수염으로 의심된 길랑바레증후군 환자에 대한 한방 치료 증례보고 1례)

  • A 67-year-old female patient diagnosed with acute transverse myelitis visited our hospital in February 2023. Consultation papers indicated that she was diagnosed with acute transverse myelitis and that she complained of quadriparesis and numbness in both hands and feet. However, her spine magnetic resonance imaging (MRI) did not clearly indicate that she suffered from acute transverse myelitis. She was then sent to a high-level hospital and diagnosed with Guillain-Barre Syndrome through electromyography, cerebrospinal fluid test, and spine MRI. On March 2023, she visited our hospital again, still complaining about weakness in both hands and inability to walk. She was treated with Korean medicine, including acupuncture, electroacupuncture, pharmacopuncture, and herbal medicine such as Yanggyeoksanhwatang and Sipimijihwang-tanggami. After three months, she was discharged on cane walking. The manual muscle test (MMT) grade of her upper limbs increased from 3-/3-/3-/3- (shoulder/elbow/wrist/fingers) to 4/4/4/3+, and the MMT grade of her lower limbs increased from 2/2/2/2 (pelvis/knee/ankle/feet) to 4/4/4-/4-. Her numbness in both feet disappeared, and her numbness in both hands decreased from a numeric rating scale of 7 to 2. No adverse events were reported during treatment. This case implies the therapeutic potential of Korean medicine for Guillain-Barre Syndrome, especially for those who had delayed diagnosis and missed the opportunity to get plasma exchange.

A Regression of Miller Fisher Syndrome using Photic Feedback: Possibility of a New Complementary Therapy

  • We present a case of The Miller Fisher Syndrome (MFS), showing a remission during a recently developed noninvasive therapy. Two weeks after an appearance of cough and fever, a 35 years old Japanese male developed diplopia, ataxia and numbness of his fingers and toes. He was diagnosed as MFS, and a fixed dose of prednisolone acetate (60mg/day) was administered for 3 months, but little improvement was observed. In addition to this administration, we tried 20 minutes of Photic Feedback (PFB) treatment daily for 40 days. The PFB system detects brain waves from the subject's forehead, and extracts alpha waves by the band-pass filter with a center frequency set at 10.0Hz. It also simultaneously modulates the augmentation of a red light-emitting diode, corresponding with the amplitudes of the extracted alpha waves. In this treatment, this adjusted photic stimulation was given to the subject's closed eyes, resulting in the effective alpha enhancement by photic driving response. The numbness increased during each of PFB treatment, but the symptoms started to improve gradually after 10 days. Other symptoms disappeared after 40 days. CD20 levels increased with this treatment. This case suggests that the PFB treatment may speed the natural remission of MFS. This treatment may be worth considering in patients who suffer polyneuropathy.

  • This study was performed to investigate the cause, symptom, treatment of OPLL through Western medicine and Dong-Eui-Bo-Kham(東醫寶鑑) Results & conclusins 1. Ossification of the posterior hgament(OPLL) have radiculopathy, myelopathy or both of them such as neck pain, numbness, myatonia 2. Neck pain of OPLL seems to be simular with pain in the neck(頸項痛), neck stiffness(項强), stiffness and pain of head and neck(頭項彈痛). The causes were usually Dampness and Cold, Wind. The treatments were classified according to pathoigenic factor(病因) and muscle along the regular meridian(經筋) 3. Radiculopathy of OPLL seems to be similar with numbness(痺證). The causes were usually, pathogenic Wind, Cold, Dampness. The treatments were classified according to diagnosis of three kinds of BI syndrome(三痺), five kind of Bi synrome(五痺), five jang Bi(五臟痺), six Bu Bi(六腑痺) 4. Myelopathy of OPLL seems to be simular with myatonia(痿證) The cause of myatoma was Lung scorched by Heat(肺熱葉無). And the treatment was purping the south and reinforcing the north(寫南方 補北方). We considered that more study to find various and effective methods oriental medicine for OPLL should be made.

Regrowth of Cervical Intradural Lipoma without Spinal Dysraphism

  • A 49 years old male patient who suffered from deterioration of posterior neck pain, left hand numbness, left lower limb pain and gait disturbance for 3 years visited our outpatient department. He had been diagnosed as non-dysraphic cervical intradural lipoma and operated in August 1990. On the radiologic images, we found the regrowth of non-dysraphic cervical intradural lipoma from C2 to C7 level, which surrounds and compresses the cervical spinal cord. Previous subtotal laminectomy from C2 to C7 and severe cervical lordosis were also found. Appropriate debulking of lipoma mass without duroplasty was successfully done with intraoperative neurophysiological monitoring (IONM). We are following up the patient for 24 months via outpatient department, his neurologic symptoms such as hand numbness, gait disturbance, left lower limb pain and posterior neck pain have improved. We describe a rare case of regrowth of non-dysraphic cervical intradural lipoma.

Associated Factors with Pin-fixing & Pin removal Pain among Patients Undergoing Gamma Knife Radiosurgery (감마나이프 수술 환자의 정위적 틀 고정과 제거 시 통증에 영향을 미치는 요인)

  • Purpose: Gamma knife radiosurgery (GKR) requires frame positioning because the treatment target should be as close as possible to the center of the frame. The purpose of this study was to identify the level of pin-fixing (PFP) and removal pain (PRP), and the associated factors with the pain undergoing GKR. Methods: A total of 116 patients who underwent GKR for their brain tumor were recruited from C University hospital located in H city, J province. The level of pain was measured by the 10 cm VAS. Results: The level of PFP and PRP were 6.36 and 3.26 points, respectively. Step-wise multiple regressions found that the group who have not perceived numbness after applying 5% EMLA cream was the highest associated factor with PFP, following the time from lidocaine injection to pin-fixation, which explained 21% of total variance of the level of PFP. On the other hand, a group who did not perceive numbness after applying 10% lidocaine spray was the highest factor with PRP, among female patients, which explained 27% of total variance of the level of PRP. Conclusion: Both of PFP and PRP of the stereotactic frame were moderate so that nurses should consider diverse strategies to reduce pain among patients undergoing gamma knife radiosurgery.

  • Extracerebral cavernous hemangiomas are rare vascular tumors that are very difficult to remove because of severe intraoperative bleeding. We report a case of 57-year-old male with extracerebral cavernous hemangioma with review of 126 cases in the literature. Patient presented with blurred vision, diplopia, numbness on the left side of his face. Magnetic resonance imaging revealed a well defined mass of $3{\times}4{\times}3cm$ size with heterogenous iso-or hypointensity on T1-weighted image showing strong homogenous contrast enhancement and marked hyperintensity on T2-weighted image. Digital subtraction angiography[DSA] revealed a faint tumor blush by feeders from the left internal carotid artery[ICA] and left external carotid artery[ECA] in the delayed phase. Even with profuse intratumoral bleeding, near total removal was achieved. In addition to preoperative neurologic deficits such as ophthalmoplegia, facial numbness in the V1-2 dermatomes, ptosis appeared postoperatively.

A Case Report of migraine and a case report of restless legs syndrome treated with Osuyu-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 오수유탕(吳茱萸湯)을 투여한 편두통 증례 및 하지불안증후군 증례 각 1례)

  • Objective : This study was intended to evaluate if treatment with Osuyu-Tang following the disease pattern identification diagnostic system based on Shanghanlun provisions (DPIDS) worked analyzing the progress of patients with migraine and restless legs syndrome and to reinterpret the provision of 309 Soyinbing Osuyu-Tang based on the analysis of whether it worked. Methods : Two cases treated with Soyinbing Osuyu-Tang were analyzed using DPIDS. The migraine and restless legs syndrome in both cases were evaluated using the migraine disability assessment (MIDAS) and the Korean versions of the international restless legs scale (K-IRLS), respectively. Results : In the first case, during 45 days of treatment, the MIDAS decreased from 10 days to 3 days. In the second case, during 30 days of treatment, the K-IRLS decreased from 30 to 9. In both cases, Soyinbing was associated with the disease, although the activity was less; and Leng (冷) was associated with numbness in the kneeling position. Conclusions : It was confirmed that treatment with Osuyu-Tang was effective against migraine and restless legs syndrome and that Leng could be associated with numbness in the kneeling position. The etymological analysis of the Shanghanlun characters suggested the possibility of further clarifying the clinical significance and interpretation of Shanghanlun.

A Case Report of Tension Type Headache Patient with Dangkisoo-san and Acupuncture (당귀수산과 침으로 호전된 긴장형 두통 환자 치험 1례)

  • Objectives : The purpose of this study is to report the clinical effects of Dangkisoo-san and acupuncture on a patient with Tension Type headache. Methods : We have given Dangkisoo-san and acupuncture treatment to a patient with Tension Type headache for 8days, and checked changes of symptoms. Main symptoms were numbness and pains in a head and a neck. Results : After treatment, the numbness and pains in a head and a neck were improved considerably. Conclusions : This study shows that Dangkisoo-san and acupuncture are significantly effective in treatment of Tension Type headache.

Symptom Clusters and Quality of Life in Patients on Hemodialysis (혈액투석 환자의 증상 클러스터와 삶의 질)

  • Purpose: The purpose of the study was to investigate symptom clusters experienced by patients on hemodialysis and to identify relations between symptom clusters and quality of life. Methods: Data were collected from 127 patients at 10 local hemodialysis clinics. Symptoms were measured using 10-item physical symptom checklist as well as the Hospital Anxiety depression Scale. Quality of life was measured with the Satisfaction with Life Scale. Data were analyzed using factor analysis, Pearson correlation, and stepwise multiple regression. Results: The most frequently reported symptoms included fatigue, itching, depression, numbness/tingling, and insomnia. Four distinct symptom clusters were identified: cluster 1 was comprised of dry mouth, headache, nausea (gastrointestinal); cluster 2 of decreased appetite, insomnia (basic need); cluster 3 of itching, numbness/tingling (sensory-comfort); and cluster 4 of fatigue, depression (mood-vitality). Among the clusters, the 'basic need' cluster and 'mood-vitality' cluster had a significant negative association with quality of life. The 'mood-vitality' cluster, explained 17.4% of the variance in quality of life. Conclusion: The results of the study indicate that comprehensive symptom assessment provides better symptom management for patients on hemodialysis. Further studies are needed to verify symptom clusters identified in this study.
