Excessive crown height space increases can cause crestal bone loss and screw loosening after prosthesis is placed. Milled bar and implant overdenture can be used as a treatment method for partially edentulous patients who have severe alveolar bone loss and excessive crown height space. Milled bar can provide primary splinting effect and stability between implants. Also, milled bar with additional retention device such as Advanced Dental Device-Treatment Of Choice (ADD-TOC) and magnet can provide additional retention force for implant overdenture. In this case, the patient has a partially edentulous mandible that has severe alveolar bone loss and multiple number of teeth loss after excision due to leiomyosarcoma. Because of the long-term loss of mandibular molars, the opposing teeth were extruded. Maxillary left molars were corrected to the occlusal plane through molar intrusion, and mandibular left molar region were treated with implant overdenture, using milled bar with ADD-TOC and magnet after implant placement. The clinical result was satisfactory on the aspect of esthetic and masticatory function.
To get the evidences for decreasing the discomfort, dissatisfaction, and for improving management of the implant prothesis through assessment implant patients' awareness and discomforts about care of the prostheses. The subjects were 300 implant patients who treated in the 20 dental clinics in Pusan, Ulsan and Daegu. The general characteristics, recognition, maintenance, discomfort and dissatisfaction, and oral hygiene status were collected. Level of the discomfort and dissatisfaction was measured by a 5 Likert scale, and analyzed by the equivalent of 100. The life-span of the implants was overestimated, however, periodic recall check and scaling were underwent. The level of discomfort and dissatisfaction was generally good, but many of them (65.3%) complained the food impaction. As a result of the multiple regression analysis, level of the discomfort and dissatisfaction increased in case of more number of the prosthesis ($\beta$=0.864), however decreased in case of more frequent teeth brushing ($\beta$=0.737), more frequent periodic check ($\beta$=-0.737), longer duration from the implantation ($\beta$=-0.090). It is very important for the implant patients to be recognized that the lack of care may shorten the life-span of the implant prosthesis. Therefore, efforts of the continuous oral hygiene care such as, periodic recall check, scaling, teeth brushing, the correct using of additional oral hygiene product, and education would be more emphasized.
Successful cases of the implant-assisted removable partial denture and implant overdentures are reported in which a few implants are additionally placed to secure the maintenance, support, and stability of the denture when there are a few residual teeth. When the lateral force applied to the tooth abutment and implant surveyed crown is minimized, the horizontal and rotational movement of the denture is significantly reduced which is an effective method that can improve the address in patients who complain of reduced retention and stability of their dentures. In this case, a small number of implants were placed to fabricate an implant-assisted removable partial denture with implant surveyed crown in the maxilla and implant overdenture with Locator® attachment in the mandible to improve the retention, stability, and support of the dentures. The patient was satisfied with both functional and aesthetic aspects after the final dentures were delivered.
Objectives : The oral care during early childhood plays an important role to maintain sound oral health during adulthood. As the number of children's using child care facilities is on the rise recently, the children's oral health awareness and behavior of the educators at nurseries and kindergartens are to be examined. Methods : A self-recording survey was conducted on 194 educators at the nurseries and kindergartens in Chungnam region. Results : The most frequently given snack was milk, 91.8 percent of the facilities had children brush their teeth, 63.4 percent made them apply fluoride and 56.2 percent kept children's toothbrushes in an ultraviolet rays sterilizer. There was a meaningful difference in the awareness of the necessity to educate children about oral health according to the offer of the opportunity to apply fluoride (p<0.05). A meaningful difference was found in the recognition of the necessity to educate children according to their grade (p<0.05) and to train teachers themselves (p<0.05). The most desirable persons in charge of oral health education were dentists in health centers (46.9%) in order. The more experience in teaching they have had (p<0.05), when they're married (50.5%) (p<0.05) and when they're not homeroom teachers (52.6 percent), the more regular checkups they have had (p<0.05). As for the importance of oral health and the results of regular checkups, those who had answered 'very important'(42.4%) showed higher rate of regular checkup (p<0.05) than those who had answered 'important'(23.9%). When teachers have the experience to get trained about oral care (96.3%), the practice frequency of brushing teeth was proved to become higher (p<0.05). Conclusions : The educators for children should recognize the importance of oral health education, educate children to practice oral health care, and the environment and systematic foundation should be established which the educators manage effectively.
Precise and accurate measuring of tooth size is essential procedure in orthodontic diagnosis. The purpose of this study was to evaluate the reproducibility and accuracy of tooth size measurements with regard to tooth and investigator. Five investigators measured the size of one side of teeth, from central incisor to first molar, on 20 sets of casts that show a moderate degree of crowding. Mesio-distal width was measured with digital vernier calipers in each tooth and this procedure was repeated three times at two weeks interval. To obtain a standard measurement for each tooth, dental casts were cut into individual tooth, and its width was measured with micrometer. The difference between the measurement from dental cast and the standard measurement was defined as the measurement error. Through various statistical analyses, following results were obtained. 1. The reproducibility did not show significant differences with regard to tooth or investigator. 2. The measurement error showed some difference with regard to tooth and investigator. 3. The magnitude of the measurement error showed increasing tendency from anterior to posterior teeth with maximum value in the first molar. 4. While the measurements obtained on study casts generally showed smaller number compared to standard measurements, the direction of the difference showed variability according to tooth or investigator. 5. The measurement errors did not show significant correlations with the degree of crowding. The results of the present study indicate that the possibility of tooth size measurement error should be taken into consideration when diagnosing an orthodontic case.
Maxillectomy is performed to remove the tumor in the palate, maxillary sinus, buccal mucosa or nasal cavity. The resection range depends on the size and the extent of the tumor and it affects speech production or cause nasal regurgitation during feeding. Obturator can occlude an opening such as an oro-nasal fistula and protect the defect area. Successful reconstrucion of the patient's oral cavity who have gone over the maxillectomy is a difficult task. The condition and number of teeth, the remaining support area, and the extent of the defect area have a great influence on manufacturing the obturator. If these factors are disadvantageous, the prognosis of the prosthesis is uncertain. The final obturator must have a sufficient retention in the patient's oral cavity and must not irritate the surrounding tissue and support area where the resection was performed.In this case, a 55 year old female went through the maxillectomy and the only 3 teeth remained. And the retention of the maxillary prosthesis seems to be poor. So that, we fabricated the closed hollow obturator which has reduced weight compared to the conventional obturator. Consequently the closed hollow obturator can give better sealing and the adaptation.

Purpose: To profile various clinical characteristics of sleep bruxism (SB) patients with idiopathic facial pain (IFP) in the orofacial region. Materials and Methods: We analyzed 28 SB patients among 210 patients with IFP complaints. The profiles were evaluated using patient charts including gender, age, pain duration, pain location, pain intensity and affected areas by pain. Results: SB with IFP occurred more often in females (85.7%) than males (14.3%). The mean age at presentation was 48.9 years. The most common IFP sites of SB patients were the right maxilla (28.6%) and the right mandible (25.0%). The pain complaints occurred mostly in 2 teeth or areas (50.0%), followed by 1 area (28.6%) and then in ≥ 3 teeth or areas (21.4%). The mean pain intensity was 5.9 on a visual analogue scale from 0 to 10. The pain was spontaneous in 20 patients (71.4%), and the mean pain duration was 24.4 months. Conclusion: Identification of clinical characteristics of SB patients with IFP could be useful in the diagnosis of various IFP patients and beneficial in decreasing unnecessary care to reduce IFP. Further studies with larger number of subjects and extended duration are required for more systemized diagnostic methods and development of future treatment guidelines.
A number of the remains of animals were unearthed from Anup pond in Kyungzu during the repair work in March, 1975. To determine the species of animal origin and the ages and sexes of them, some judgement and morphological examination of the animal remains were conducted and the results obtained with the bone remains believed to be of horses are summarized as follow. 1. The total of the remains of horses excavated were 28 teeth and 9 pieces of bones. The bones were classified into on each of skull, humerus, radius, the first phalanx, femur and tibia, the remaining 3 pieces being cervical vertebrae. 2. The cranial bone was relatively in fair preservation and, on the whole, the gross appearance was quite similar to that of current Korean native horses. With the defacement of the teeth, the horse remains could be of a mare with the age less than 10 years. 3. The size of the appendicular skeletons were a bit smaller than that of Korean native horse at present. 4. The results obtained in the present study strongly indicated that the horse remains excavated from the An up pond could be of at least 2 or more horses.
The purpose of this study was to evaluate the effects of bleaching agent through the dentinal tubules of cervical area in the intracoronal bleaching of pulpless teeth on cutured fibroblast cells. Extracted human incisors were enlarged to # 40 K-file and obturated with gutta-perella and AH 26 sealer. The gutta-percha was removed to 2mm below the cementoenamel junction of the root The teeth were divided into 3 experimental and control groups. Experimental groups; Experimental group 1: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity. Experimental group 2: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after placement of ZOE cement to cementoenamel junction. Experimental group 3: Temporary inlay wax filld with 30% $H_2O_2$ in pulp cavity after application of Copalite to cementoenamel junction. Control group: Temporary inlay wax filled without 30% $H_2O_2$ in pulp cavity under the same condition at each experimental group. Each tooth was immersed in well of multidish cultured fibroblast cell for 48 hours. The cellular multiplication and cell viability were calculated at the interval of 1, 3, 5. 7 hours and the morphological changes in well were observed and their photographs were taken with inverted microscope. The obtained results were as follows : CD The cellurar multiplicaton and cell viability decreased in all experimental groups at 1 hour after experiment and the morphology of fibroblast cell was changed from star shape to round (2) The cell viability was lowered to 34 % in experemental group 1, 44 % in experimental group 2, and 38 % in experemental group 3 at 3 hours after experiment (3) The cell multiplication was decreased to 54% in experemental group 1. 47% in experimental group 2, and 40% in experemental group 3 at 7 hours after experiment. (4) The decrease of cell number and morphological changes of fibroblast cell were remarkable in experimental group 1, group 3 and 2 in order. These results suggest that the fibroblast cells receive severe damage by 30% $H_2O_2$ solution leaked through the dentinal tubules and the dentinal tubules are able to be obturated better by ZOE cement than by Copalite.
Recently the development of rotary instrument makes it possible that in root canal treatment operator saves much more time, maintans original curved canal shape and easily prepares continuous tapered root canal. The purpose of this experiment was to examine the smoothness of the internal surface of prepared root canal and the effectiveness of debridement in prepared root canal by SEM for the comparison of hand and Ni-Ti rotary instrument. 25 extracted human teeth were access opened and # 10 K-type file was introduced into canal until it was appeared at the apical foramen. The working length was established by subtracting 0.5mm from this measurement. Group 1. The root canal preparation was done to # 30 with working length and then step-back until # 45 with K-Flexofile (Maillefer, Swiss). Group 2. Root canal preparation was done by Naviflex Ni-Ti file (Brasseler, USA) as the same technique with group 1. Group 3. Canal was prepared by Profile .04 (Maillefer, Swiss) taper until #30. Group 4. With use of Quantec (Tycom, USA) root canal was prepared from file number 1 to 8. In group 1 and 2, the root canal irrigant was NaOCl and the other groups, NaOCl and RC-prep (Premine Dental Products, USA) was used. The prepared teeth were notched with high-speed bur as bucco-lingual direction and fractured with chisel and mallet, then examined with SEM. Group 1 showed smooth internal surface. There were scratches mainly to the axial direction. Group 2 showed similar characteristics to those in group 1. Group 3 showed more smoother and linear cutting surface with bised scratches. Group 4 has the almost same characteristics group 3 and there was no difference in the file design. Ni-Ti rotary root canal instrument prepare the dentinal wall more smoother than hand instrument. The effectiveness of debridement was not fully affected by file design. The isthmus area and accessory canals of the root canal system were not prepared in any group. According to the result, hand and rotary type instrumentation techniques were effective in removal of major amount of tissue from root canal but it was not complete. In the direction of cutting movement there was difference between them.
