Background: Survival rates after resection of advanced gastric cancer are extremely poor. An increasing number of patients with gastric carcinomas (GC) are therefore being treated with preoperative chemotherapy. We evaluated 36 month survival rate of GC patients that were treated by adding a neoadjuvant chemoradiotherapy before gastrostomy.Materials and Methods: Patients with stage II or III gastric adenocarcinomas were enrolled. The patients divided into two groups: (A) Neoadjuvant group that received concurrent chemoradiation before surgery (4500cGy of radiation at 180cGy per day plus chemotherapy with cisplatin and 5-fluorouracil, in the first and the end four days of radiotherapy). Resection was attempted 5 to 6 weeks after end of chemoradiotherapy. (B) Adjuvant group that received concurrent chemo-radiation after surgical resection. Results: Two (16.7%) patients out of 12 patients treated with neoadjuvant chemo-radiotherapy and 5 (38.5%) out of 13 in the surgery group survived after 36 months. These rates were not significantly different with per protocol and intention-to-treat analysis. The median survival time of patients in group A and B were 13.4 and 21.6 months, respectively, again not significantly different. Survival was significantly greater in patients with well differentiated adenocarcinoma in group B than in group A (p<0.004). Conclusions: According to this study we suggest surgery then chemoradiotherapy for patients with well differentiated gastric adenocarcinoma rather than other approaches. Additional studies with greater sample size and accurate matching relying on cancer molecular behavior are recommended.
Background: Epidermal growth factor receptor (EGFR) mutations play a vital role in the prognosis of patients with lung adenocarcinoma. Such somatic mutations are more common in women who are non-smokers with adenocarcinoma and are of Asian origin. However, to our knowledge, there are few studies that have focused on men. Materials and Methods: One hundred and eighty-four consecutive patients (90 men and 94 women) of resected lung adenocarcinoma were studied retrospectively. Results: EGFR mutations were positive in 48.9% and negative (wild type) in 51.1%. Overall mutation was significant in women (66.0% vs. 32.2%) compared with men (p<0.001). For overall patients, EGFR mutation status was associated with gender, pStage, pT status, lepidic dominant histologic subtype, pure or mixed ground-glass nodule type on computed tomography and smoking status. However, in men, EGFR mutation status was only associated with lepidic dominant histologic subtype and not the other variables. Interestingly, the Brinkman index of men with mutant EGFR also did not differ from that for the wild type ($680.0{\pm}619.3$ vs. $813.1{\pm}552.1$ p=0.1077). Conclusions: The clinical characteristics of men with lung adenocarcinoma related to EGFR mutation are not always similar to that of overall patients. Especially we failed to find the relationship between EGFR mutations and smoking status in men.
This study was done to explore the effects of dance therapy on pulmonary and cognitive functions in the elderly. The design of this study was a non-equivalent pre-post test experiment. The subjects consisted of elderly persons living in a facility located in Kyoungi-Do. Fifty eight subjects had normal cognition, sensory function and resting blood pressure. They underwent tests of pulmonary and cognitive function as baseline data before dance therapy, and at 6th week and at the end of 12nd week after following dance therapy. Twenty seven elderly persons were assigned to the experimental group and participated with the dance therapy. This therapy was based on the Marian Chace's dance therapy and Korean traditional dance with music. The dance therapy consisted of 50 minutes session, 3 times a week for 12 weeks. One session consisted of warming-up, expression, catharsis, sharing and closing stage. the intensity of the dance therapy was at the 40% of age-adjusted maximum heart rates. Data were analyzed with mean, standard deviation, Chi-square test, unpaired t-test, repeated measures ANOVA, and Bonferroni multiple regression using SAS program. The results were as follows : 1. Pulmonary function(forced expiratory volume at one second and forced vital capacity) of the experimental subjects significantly increased over time more than that of the control subjects. 2. The experimental group had significantly higher score for pulmonary function than the control group at the 12nd week after dance therapy. 3. Cognitive function of the experimental subjects significantly increased over time more than that of the control subjects. 4. The experimental group had significantly higher score for cognitive function than the control group at the 6th week and 12nd week after dance therapy. The findings showed the dance therapy could be effective in improving the pulmonary and cognitive function of the elderly.
This research aims to analyze the prep-hop fashion, which is an original look created through the convergence of the hip-hop and preppy look, and identify the design characteristics of the prep-hop fashion. The style mixes the fashion of what was traditionally a lower-class look with the upper class. The design characteristics are as follows: First, the prep-hop fashion expresses slim, casual and sophisticated look with items and silhouettes. The items are categorized into jackets, shirts and pants. The prep-hop jackets provides a casual look to its wearer unlike the jackets of preppy or conservative looks. Slim suits show different meanings and features depending on the changes in the time and culture despite the continued use of the items. Second, the colors of the prep-hop fashion include overall basic colors used in the preppy look and hip-hop fashion. The aesthetic values of the colors express pleasant color images different from the conservative and resistant images expressed in the preppy and hip-hop style before the convergence, respectively. Third, the materials and patterns are expressed in a unique way to fulfill the emotional needs that are considered vital to contemporary fashion. The materials in the prep-hop fashion extend the ranges of usage by using the materials with unexpectedness broken out of general range of usage. While the use of the patterns is one of the methods to demonstrate that the prep-hop fashion is created by the convergence between hip-hop fashion and preppy look, the meaning concludes pleasantness and casualness different from the past. Fourth, the prep-hop fashion is mainly composed of hip-hop fashion accessories, which express the authenticity of the hip-hop fashion to protect the non-mainstream characteristics. The design characteristics of the prep-hop fashion pursue the individuality of the contemporary fashion, which lacks originality due to commercialization. The development of the prep-hop fashion, as a new form that combined contrasting genres and trends, is expected to develop more publicly.

The moving average filter (MAF) is widely utilized to improve the disturbance rejection capability of phase-locked loops (PLLs). This is of vital significance for the grid-integration and stable operation of power electronic converters to electric power systems. However, the open-loop bandwidth is drastically reduced after incorporating a MAF into the PLL structure, which makes the dynamic response sluggish. To overcome this shortcoming, some new techniques have recently been proposed to improve the transient response of MAF-based PLLs. In this paper, a comprehensive performance comparison of advanced MAF-based PLL algorithms is presented. This comparison includes HPLL, MPLC-PLL, QT1-PLL, and DMAF-PLL. Various disturbances, such as grid voltage sag, voltage flicker, harmonics distortion, phase-angle and frequency jumps, DC offsets and noise, are considered to experimentally test the dynamic performances of these PLL algorithms. Finally, an improved positive sequence extraction method for a HPLL under the frequency jumps scenario is presented to compensate for the steady-state error caused by non-frequency adaptive DSC, and a satisfactory performance has been achieved.
Purpose: Diffuse axonal injury (DAI) is clinically defined as a coma of over six hours in a head trauma victim without a focal mass lesion. The emergency physician usually resuscitates and stabilizes a comatose head trauma victim in the emergency Department. After assessment and treatment, the prognosis is very important to both the victim and the physician. The prognosis for DAI is based on Glasgow Coma Scale (GCS) and other imaging data. We investigated the prognostic value of computed tomography (CT) and gradient-echo magnetic resonance imaging (GRI) for head trauma victims with DAI. Methods: Fifty-three(53) head trauma victims of DAI were enrolled in this study from 2007 to 2012. During the study period of six years, data on trauma victims were collected retrospectively. We analyzed the differences in the Glasgow Outcome Scale (GOS) result between the CT and the GRI modalities. Results: We classified the study group by using GOS. Between the good outcome subgroup (GOS scores of 4 and 5) and the poor outcome subgroup (GOS score of 1-3), there were no statistical difference in sex, age, initial vital signs and initial GCS score. The good outcome subgroup had non-hemorrhage on CT(52%), which was correlated with good outcome and a shorter awakening time, while a larger number and a deeper location of hemorrhagic lesions on in GRI were correlated with poor outcome in DAI. Conclusion: We conclude that the existence of hemorrhagic lesions on CT, and the number and location of those lesions on GRI had good prognostic value for head trauma victims with DAI.
Purpose: It is important to begin a transfusion safely and appropriately as soon as possible in a hemorrhagic shock patient. A group $O^+$ unmatched pack red blood cell (universal $O^+$) transfusion may satisfy that requirement. We report our experiences with universal $O^+$ to compare its usefulness for hemorrhagic shock patients with that of a matched pack red blood cell transfusion in the emergency department (ED). Methods: This is a retrospective study. Patients who had systolic blood pressure of less than 90 mmHg or a pulse rate of more than 120 beats per minute in the ED were included, and their medical records were reviewed. The collected data were demographic data, vital signs, blood test results, time to transfusion, the amount of transfusion, complications, and diagnoses. We calculated the emergency transfusion score (ETS) based on the patients' medical records. Results: Two hundred thirty-five patients were included. Forty-eight patients (36 trauma and 12 non-trauma patients) were transfused with a universal $O^+$. These patients had less time to transfusion compared with the cross-matched transfusion groups (35${\pm}$42 versus $170{\pm}187$ minutes, p<0.001). There were no differences in complications between groups (p=0.076). Of the patients who were transfused with universal $O^+$, 94.4% got more than 3 ETS. Conclusion: The universal $O^+$ transfusion, compared with matched pack red blood cell transfusion, should be a useful treatment for ED hemorrhagic shock patient due to its having a shorter time to transfusion without an increase in complications.

This study was conducted as a basic research to create a sound and vital ecological environment in the city compared to the appearance characteristics of Gap stream(G-stream), which are the main stream of Banseok stream(B-stream), by analyzing the characteristics of waterbirds, including Wild Spot-billed Duck (S-Duck), that live in B-stream built as ecological stream. The waterbird survey was conducted by the line census for 3.2km, Jukdong-bridge to Jamiseon-bridge, from January to August 2018. The analysis of the survey was conducted with Relative species density(RD) of the emerging waterbird species, the Species diversity and Density Per Unit area for 100㎡(DPU). The waterbird survey results of B-stream was conducted 65 times. The five types of water birds that appeared during the survey were Spot-billed Duck(Anas poecilorhyncha), Teal(Anas crecca), Little Egret(Egretta garzetta), Great Egret(Egretta alba), and Grey Heron(Ardea cinerea). As a result, for S-Duck at B-stream, RD was 89.9%, monthly species diversity was simple as 0.3801 in January, 0.5943 in February and 0.3501 in August. The DPU of the S-Duck was 0.165/100㎡ in the B-Stream survey section which was 4.9 times higher than the main stream section, G-stream. The non-freezing zone of the city's small stream is expected to play an important role as a winter stop for wild birds such as S-Duck during the freezing period of the huge stream. For this reason, considering the ecological characteristics of wild waterbirds such as S-Duck when creating ecological stream, a: space and linear selection of waterways which can minimize the impact of natural enemy and increasing the number of walkers, b: management water-friendly plants in the low flow channel, c: arrangement walking-bicycle road will be necessary.

This paper evaluated the optimum Carbon Fiber Reinforced Polymer (CFRP) using a circular concrete bridge pier subjected to dynamic loading. A three-dimensional finite element model was simulated using finite element program, ABAQUS. Concrete Damage Plasticity (CDP) option and plastic properties of the materials were incorporated to model the non-linearity of the structure. The analyses parameters were changed in length-to-height ratio and width-to-span ratio where columns were subjected to dynamic loading. Numerical analysis was conducted, and the seismic performance of the structures were evaluated by analyzing the ductility behavior of the structure. Results showed that the use of CFRP enhances the structural performance of column and revealed that the increase in length-to-height ratio plays vital role of improving the performance of the structure than the change in width-to-span ratio.
BACKGROUND: In recent years, microbial fuel cells (MFCs) have emerged as a promising technology for recovering renewable energy from waste biomass, especially wastewater. In this study, the possibility of bioelectricity generation in two chambered mediator-less microbial fuel cells (MFCs) was successfully demonstrated using fermentable and non-fermentable substrates. METHODS AND RESULTS: Two different electron acceptors have been tested in the cathode chamber for the effects of reducing agent on the power generation in MFCs. The average voltages of $0.26{\pm}0.014$ V and $0.36{\pm}0.02$ V were achieved with acetate using oxygen and potassium ferricyanide as reducing agent, respectively. Similarly, with glucose the average voltages of $0.256{\pm}0.05$ V and $0.340{\pm}0.04$ V were obtained using oxygen and ferricyanide, respectively. Using potassium ferricyanide as the reducing agent, the power output increases by 39 and 43% with acetate and glucose, respectively, as compared to the dissolved oxygen. Slightly higher coulombic efficiency (CE%) was obtained in acetate as compared to MFCs operated with glucose. The maximum power densities of 124 mW/$m^2$ and 204 mW/$m^2$ were obtained using dissolved oxygen and $K_3Fe(CN)_6$, respectively. CONCLUSION(s): This study demonstrates that power generation from the MFCs can be influenced significantly by the different types of catholyte. Relatively higher CE was obtained with $K_3Fe(CN)_6$. Thus, application of $K_3Fe(CN)_6$ as the catholyte can be vital for scaling uppower generation from the MFCs forreal time applications.
