An Indoor Localization Algorithm of UWB and INS Fusion based on Hypothesis Testing

  • With the rapid development of information technology, people's demands on precise indoor positioning are increasing. Wireless sensor network, as the most commonly used indoor positioning sensor, performs a vital part for precise indoor positioning. However, in indoor positioning, obstacles and other uncontrollable factors make the localization precision not very accurate. Ultra-wide band (UWB) can achieve high precision centimeter-level positioning capability. Inertial navigation system (INS), which is a totally independent system of guidance, has high positioning accuracy. The combination of UWB and INS can not only decrease the impact of non-line-of-sight (NLOS) on localization, but also solve the accumulated error problem of inertial navigation system. In the paper, a fused UWB and INS positioning method is presented. The UWB data is firstly clustered using the Fuzzy C-means (FCM). And the Z hypothesis testing is proposed to determine whether there is a NLOS distance on a link where a beacon node is located. If there is, then the beacon node is removed, and conversely used to localize the mobile node using Least Squares localization. When the number of remaining beacon nodes is less than three, a robust extended Kalman filter with M-estimation would be utilized for localizing mobile nodes. The UWB is merged with the INS data by using the extended Kalman filter to acquire the final location estimate. Simulation and experimental results indicate that the proposed method has superior localization precision in comparison with the current algorithms.

비흡연 성인에서 맥압과 폐기능 장애의 상관성 (Association between Pulse Pressure and Impaired Pulmonary Function in Non-Smoking Adults)

  • 선행 연구에서 동맥경화와 폐기능이상과 연관성이 있었다. 맥압은 동맥경화의중요한 예측이지만 아직까지 폐기능이상과 맥압의 연관성은 불분명하다. 따라서, 본 연구는 대한민국 비흡연 성인을 대상으로 맥압과 폐기능장애(예측 강제 폐활량, 예측 강제 날숨량 1초율)의 관련성을 알아보고자 실시하였다. 본 연구는 2013년부터 2015년까지 3년간의 대한민국 국민건강영양조사자료에서 비흡연 성인(6,857명)을 대상으로 실시하였다. 본 연구의 주요 결과는 맥압과 폐기능장애에 대한 관련 변수를 보정한 후의 결과에서, 정상 맥압(pulse pressure, PP≤60 mmHg)에 비하여 고맥압(PP>60 mmHg; 1.337 [95% confidence interval (CI), 1.049~1.703)에서 제한성 폐질환(restrictive pulmonary disease, RPD; the predicted FVC<80.0% with FEV1/FVC≥70.0%)의 교차비(odds ratio, OR)가 유의하게 높았다. 추가적으로, 정상맥압에 비하여 고맥압(1.339 [95% CI, 1.093~1.642])에서 폐쇄성 폐질환(obstructive pulmonary disease, OPD; FEV1/FVC<70.0%)의 교차비가 유의하게 높았다. 결론적으로, 대한민국 비흡연 성인에서 제한성 폐질환 및 폐쇄성 폐질환은 고맥압과 유의한 관계가 있었다.

성인 심폐소생술 술기 점수 비교: 레어달 애니 스킬리포터 대 액타 911 마네킹 (Comparison of adult CPR skill scores: Real-time visual feedback manikin(Resusci Anne SkillReporterTM) vs. Non-feedback manikin(Actar 911 SquadronTM))

  • Purpose: Cardiopulmonary resuscitation (CPR) prevents tissue necrosis of the brain and cardiac muscle in the cardiac arrest patient and requires exact skills in order to increase survival rate. Through comparison of the training effects of feedback manikin and non-feedback manikin, this study present the effective CPR device to CPR instructors. Method: This CPR course for 80 students by using Resusci Anne $SkillReporter^{TM}$ (RASR; Laerdal Medical, Stavanger, Norway) and Actar 911 $Squadron^{TM}$ (A911; Vital Signs, New Jersey, USA) held on December 22, 2009. Thirty seven students and two assistants were placed in one laboratory, there were five RASR manikins which provide the LED performance indicator, not the metronome. Forty two students and two assistants were placed in the other laboratory, there were 20 A911 manikins which don't provide any feedback indicator. Chest compression scores and ventilation compression scores obtaining from two groups were analysed statistically by using independent t-test. Results: Chest compression scores, average depth (mm) was 37.5 in RASR and 41.80 A911 (p=.004), too depth (#) was 2.8 in RASR and 19.4 A911 (p=.005), average number per min (#/min) was 64.4 in RASR and 68.2 A911 (p=.038), wrong hand position (#/min) was 10.9 in RASR and 30.8 A911 (p=.040). Four items that showed better scores in group RASR had statistically significant difference. Ventilation compression scores, percent correct (%) was 40.6 in RASR and 20.6 A911 (p<.001), number correct (#) was 4.7 in RASR and 2.1 A911 (p=.002), too fast (#) was 0.9 in RASR and 2.9 A911 (p=.003), average volume (ml) was 536.5 in RASR and 707.1 A911 (p=.011). Also, three items that showed better scores in group RASR had statistically significant difference. Conclusions: Regarding the positive effect of CPR training feedback, comparison between the real-time visual feedback manikin (RASR) and the non-feedback manikin (A911) showed that RASR had better results than A911 in chest compression except average number per min (it means that we need harder chest manikin) and ventilation. Verification of the training effect in the real world such as CPR outcomes is also necessary. A proper application of manikin in training circumstances and research on retention of CPR skills will be needed.

Preoperative Risk Factors for Pathologic N2 Metastasis in Positron Emission Tomography-Computed Tomography-Diagnosed N0-1 Non-Small Cell Lung Cancer

  • Background: Accurate mediastinal lymph node staging is vital for the optimal therapy and prognostication of patients with lung cancer. This study aimed to determine the preoperative risk factors for pN2 disease, as well as its incidence and long-term outcomes, in patients with clinical N0-1 non-small cell lung cancer. Methods: We retrospectively analyzed patients who were treated surgically for primary non-small cell lung cancer from November 2005 to December 2014. Patients staged as clinical N0-1 via chest computed tomography (CT) and positron emission tomography (PET)-CT were divided into two groups (pN0-1 and pN2) and compared. Results: In a univariate analysis, the significant preoperative risk factors for pN2 included a large tumor size (p=0.083), high maximum standard uptake value on PET (p<0.001), and central location of the tumor (p<0.001). In a multivariate analysis, central location of the tumor (p<0.001) remained a significant preoperative risk factor for pN2 status. The 5-year overall survival rates were 75% and 22.9% in the pN0-1 and pN2 groups, respectively, and 50% and 78.2% in the patients with centrally located and peripherally located tumors, respectively. In a Cox proportional hazard model, central location of the tumor increased the risk of death by 3.4-fold (p<0.001). Conclusion: More invasive procedures should be considered when preoperative risk factors are identified in order to improve the efficacy of diagnostic and therapeutic plans and, consequently, the patient's prognosis.

Impact of Alcohol Consumption on Quality of Life, Depressive Mood and Metabolic Syndrome in Obstructive Lung Disease Patients: Analysis of Data from Korean National Health and Nutrition Examination Survey from 2014 and 2016

  • Background: The objective of this study was to investigate whether alcohol consumption might affect the quality of life (QOL), depressive mood, and metabolic syndrome in patients with obstructive lung disease (OLD). Methods: Data were obtained from the Korean National Health and Nutrition Examination Survey from 2014 and 2016. OLD was defined as spirometry of forced expiratory volume in 1 second/forced vital capacity <0.7 in those aged more than 40 years. QOL was evaluated using the European Quality of Life Questionnaire-5D (EQ-5D) index. Patient Health Questionnaire-9 (PHQ-9) was used to assess the severity of depressive mood. Alcohol consumption was based on a history of alcohol ingestion during the previous month. Results: A total of 984 participants with OLD (695 males, 289 females, age 65.8±9.7 years) were enrolled. The EQ-5D index was significantly higher in alcohol drinkers (n=525) than in non-alcohol drinkers (n=459) (0.94±0.11 vs. 0.91±0.13, p=0.002). PHQ9 scores were considerably lower in alcohol drinkers than in non-alcohol drinkers (2.15±3.57 vs. 2.78±4.13, p=0.013). However, multiple logistic regression analysis showed that alcohol consumption was not associated with EQ-5D index or PHQ-9 score. Body mass index ≥25 kg/m2, triglyceride ≥150 mg/dL, high-density lipoprotein <40 mg/dL in men and <50 mg/dL in women, and blood pressure ≥130/85 mm Hg were significantly more common in alcohol drinkers than in non-alcohol drinkers (all p<0.05). Conclusion: Alcohol consumption did not change the QOL or depressive mood of OLD patients. However, metabolic syndrome-related factors were more common in alcohol drinkers than in non-alcohol drinkers.

MRI 검사 시 음악요법을 동반한 의사소통이 환자의 불안에 미치는 영향 (The Effects of the Communication Accompanied with Music Therapy on the Anxiety of the Patients during the MRI Examination)

  • 본 연구는 MRI 검사 시 음악요법을 동반한 의사소통이 환자의 불안에 미치는 효과를 파악하기 위한 비동등성 대조군 전후 실험연구이다. 연구기간은 2014년 2월 1일부터 2014년 3월 31일이며, 대구광역시에 소재한 C병원에서 MRI 검사를 시행 받은 환자 중 연구에 동의한 60명을 대상으로 분석하였으며, 실험군이 30명, 대조군이 30명이었다. 실험군에게는 MRI 검사 도중 음악요법을 동반은 의사소통을 제공하였고, 대조군에게는 제공하지 않았다. 수집된 자료는 기술통계를 산술하고, 비교검증을 위해 t-test와 ANCOVA를 실시하였다. 연구결과, 음악요법을 동반한 의사소통을 제공받은 실험군과 제공받지 않은 대조군 간의 불안점수는 실험군에서 유의하게 낮았고(p=.001), 활력징후 중에서는 수축기혈압과 이완기혈압 모두 실험군과 대조군 간에 유의한 차이를 보이지 않았으나, 심박수가 실험군에서 유의하게 낮았다(p=.001). 신체적 불편감은 발한, 안면변화, 자세변화, 성음변화 등 4가지 항목 모두 실험군과 대조군 간에 유의한 차이가 없었으며, 정서적 불편감은 어지러움을 제외한 공포감(p=.001)과 긴장감(p=.001)에서 실험군이 유의하게 낮았다, 이상의 연구결과로 볼 때 음악요법을 동반한 의사소통은 MIR검사 시 환자의 불안 감소 및 활력징후 유지, 불편감 감소에 유용한 대체요법이 될 것으로 생각된다.

호흡 측정 기능의 스마트 의류를 위한 기초 디자인 연구 (A Study of the Basic Design for Smart Clothing based on Measurement of the Respiration)

  • 1990년대 이후 웰빙 라이프스타일의 도입 및 인구 고령화 사회의 진입으로 인해 언제 어디서나 지속적으로 측정가능한 생체 신호 센싱 모니터링 시스템에 대한 관심이 높아지고 있다. 이에 따라, 최근 지속적으로 호흡 측정이 가능한 시스템에 대한 연구 및 개발이 활발히 이루어지고 있으나 소비자 수요에 기반한 스마트 의류적용을 위한 호흡 측정 시스템의 연구는 미비하다. 따라서, 본 연구에서는 압전의 원리를 이용하여 호흡시 인체의 체표면이 변화하면서 센서에 가해지는 압력을 전기적 신호로 바꾸어 호흡을 측정하는, 전도성 섬유기반의 전기용량성 섬유 압력 센서(Textile Capacitive Pressure Sensor, TCPS)를 개발하였으며, 이를 실증적 연구를 통해 센서의 유용성을 입증하고자 하였다. 그 결과, Nasal로부터 측정된 호흡율과 TCPS를 통해 측정된 호흡율간의 상관계수는 r=0.9553 (p< 0.0001)로 TCPS의 신호 측정 정확성을 확인할 수 있었으며, 또한, 사용성 및 착용성 평가의 결과 인지적 변화, 착용성, 운동성, 관리의 용이성 및 유용성에서 모두 만족한다 정도의 평가 결과를 가져와 본 연구에서 개발된 TCPS의 착용성 및 의류 적합성을 입증하였다. 마지막으로 TCPS의 의류 적합 위치 평가 결과는 복부 부위에서 신호의 안정감을 갖으며, 착용감도 높은 것으로 나타났다. 최종적으로, 위의 결과들을 종합하여 호흡 측정 기능의 스마트 의류 개발을 위한 기초 디자인을 제시하였다.

민간경비를 활용한 사이버범죄 예방 방안 (Prevention Methods of Cyber-crimes using the Private Security)

  • 2000년대 이후 폭발적으로 증가한 사이버 공간에 대한 활용으로 인해 인간의 삶은 과거와 다르게 큰 발전에 발전을 거듭함과 동시에 사이버 범죄라는 신종 범죄가 증가하게 되는 원인이 되었다. 이러한 사이버 범죄는 과거의 물리적 형태의 범죄와 달리 사이버 공간이라는 특징을 바탕으로 일어나는 범죄로서, 비대면성 익명성 전문성 기술성 반복성 계속성 등을 가지는 특징이 있다. 사이버 범죄 발생추이를 살펴보면, 2003년에 비해 2010년에는 거의 두 배에 가까운 범죄가 발생하고 있다. 주로 일반 피싱 음란물 유포 등과 같은 일반 사이버 범죄가 대부분이었으며, 사이버 범죄를 저지르는 범죄자들의 연령대가 다른 범죄들에 비해 낮은 모습을 보여주었다. 최근 사이버 범죄에 대해서 경찰에서는 '사이버테러 대응 센터'를 설치하여 대응하고 있으나, 예방 및 수사조직의 분산, 수사상의 문제점, 제도상의 문제점 등으로 인해 효과적으로 대처하지 못하고 있다. 따라서 경찰의 문제점을 보완하기 위하여 이 연구에서는 민간경비를 통해 사이버 범죄를 예방하기 위한 방안으로 물리적 경비를 통해 서버룸과 같은 주요시설에 접근을 통제하며 우수한 사이버 전문요원을 육성하는 기본적인 형태의 범죄예방을 비롯하여, 사이버 범죄 컨설팅 및 관련 법령 개정에 관하여 살펴보았다.

손마사지가 허혈성 심질환 환자의 불안에 미치는 효과 (The Effect of Hand Massage Program on Anxiety in Patients with Ischemic Heart Diease)

  • The purpose of this study was to investigate the effect of a hand massage program on anxiety, vital sign and pain in clients with ischemic heart disease. The design utilized for this study was quasi-experimental with a nonequivalent control group non-synchronized design The subjects were fifty-four patients, twenty-eight for the experimental and twenty-six for the control group, who were admitted with ischemic heart diease at a cardiac intensive care unit in K medical center of K university. This study was carried out from May, 1999 to March, 2000. The level of anxiety and pain measured by Visual Analogue Scale, systolic blood pressure, diastolic blood pressure and pulse rate were measured before and after hand massage, the state of Anxiety was measured by the Spielberger (1970) scale at admission and after hand massage for three days. The collected data were processed by using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The scores of VAS anxiety and State of anxiety of the experimental group were lower than those of the control group. 2. The degree of systolic blood pressure, diastolic blood pressure and pulse rate of the experimental group were lower than that of the control group. 3. The score of VAS pain of the experimental group was lower than that of the control group. The results suggested that hand massage can decrease VAS anxiety, State of anxiety, vital signs and VAS pain of patients who were admitted with ischemic heart disease at cardiac intensive care unit Therefore, it is proposed that hand massage is an appropriate nursing intervention to relieve anxiety of the patients who were admitted with ischemic heart disease at a cardiac intensive care unit.

외과중환자를 위한 진통제와 진정제의 투여 양상 (The Administration Patterns of Analgesics and Sedatives for Patients in SICU)

  • Major purposes of this study were to investigate the administration patterns of analgesics and sedatives in SICU and to identify the factors influencing the use of prn analgesics and sedatives by ICU nurses. The sample of this descriptive study was 50 adult patients in SICU and 53 ICU nurses. Patient's medical records were reviewed to investigate names, doses, the routes of administration, the interval of administration, and the type of prescription of sedatives and analgesics administered. Study medications were narcotics, hypnotics, and antipsychotics. To identify the factors influencing the use of prn analgesics and sedatives, 53 ICU nurses checked 9 items, and rank them from first to fifth. The selection of items was based on the previous studies and the experience of the investigator. The results of the study are as follows: 1. The mean age of the subjects was 53 years, 24 patients out of 50 subjects had received mechanical ventilation therapy. Most of the patients received neurosurgeries and abdominal surgeries. 2. For 4 days, 13 total study medications and combination of these were administered to the patients. Commonly prescribed drugs were Ketoprofen and Midazolam. Twenty six to fourty two percent of the patients did not receive any drugs for at least one day during the four days. 3. On the average, the study drugs were administerd 1.4 to 2.6 times per day during 4 days. 4. More than 50 percent of the prescription was as-needed (prn) except those of the POD 3. Fourteen percent of the patients did not have any prescription for sedation and pain control after surgery. 5. Examination of the frequency of sedatives and analgesics bolus administration revealed that a greater number of doses were given during daytime (from 7 am. to 7 pm.) than nighttime (from 7 pm. to 7 am.). The difference was significant at Alpha, .05. 6. First factor that most influenced nurses to administer sedatives and analgesics for intubated patients was the evaluation of patient's vital signs (51%). For non- intubated patients, the factors that nurses considered important were the patients' complaints of pain (64%) and evaluations of patients' vital signs (23%). In conclusion, the results of this study indicate that patients in SICU might not receive enough analgesics and sedatives to feel completely free from pain during the post operational period. Future study should be focused on the evaluation of the adequacy of current practice for pain and anxiety control in terms of the SICU patient's response.
