항암화학요법과 병행한 한의기반 통합암치료를 통한 전이성 비소세포폐암 환자의 암성 통증 호전 증례보고 (A Case Report on the Improvement of Cancer Pain in a Patient with Metastatic Non-Small Cell Lung Cancer Through Herbal Medicine-based Integrative Cancer Treatment with Chemotherapy)

  • Objective: To demonstrate an improvement in metastatic cancer pain and a decrease in tumor size in a patient with non-small cell lung cancer. Method: A 53-year-old female patient diagnosed with metastatic non-small cell lung cancer in August 2022 underwent integrative cancer treatment (ICT) for two months to decrease the tumor size and improve back pain from bone metastasis. The patient underwent chemotherapy with ICT. Radiologic outcomes were assessed by chest, abdomen, and pelvis computed tomography based on the Response Evaluation Criteria in Solid Tumors (RECIST) protocol. Clinical outcomes were assessed using National Cancer Institute Common Terminology Criteria for Adverse Event (NCI-CTCAE), Eastern Cooperative Oncology Group (ECOG), and a numeric rating scale (NRS). Result: During the two months of treatment, the NRS scores for back pain were improved, and the ECOG score improved from grade 2 to 1. The size and metabolic activity of the primary lung tumor decreased and underwent partial remission based on RECIST. No serious side effects of grade 3 or higher were noted on the NCI-CTCAE test. Conclusion: This case suggests that ICT may have a therapeutic effect for cancer pain and a synergetic effect with chemotherapy for metastatic non-small cell lung cancer.

Successful Treatment of Pleural Effusion in Small Cell Lung Cancer Patient with Gunreyngtang-gagambang

  • Objectives: We report one patient with pleural effusion and effusion-related symptoms in small cell lung cancer (SCLC) successfully treated with Gunreyngtang-gagambang. Methods: Gunreyngtang-gagambang was administered at 30 minutes after mealtime, three times a day, for two months. Except for herbal medicine, the patient did not take any treatment including pharmaceutical or non pharmaceutical for effusion. Result: Two months later, the symptoms and the pleural effusion had disappeared from chest X-ray. Conclusion: Gunreyngtang-gagambang was effective for treatment of malignant pleural effusion due to small cell lung cancer.

비소세포성 폐암의 방사선 치료 (Radiation Therapy in Non-Small Cell LUNG Cancer)

  • From Jan.1984 to Dec. 1986, 90 patients with lung cancer were treated at the Department of Radiation Therapy in Hanyang University Hospital. Histopathologically, 67 cases of them were the squamous cell carcinoma,7 cases were the adenocarcinoma, 4 cases were the large cell undiffe rentiated carcinoma and 12 cases were the small cell carcinoma. Among the 78 patients with non small cell carcinoma, 50 patients had received radiation dosage above 4000 cGy.40 patient had follow up from 17 months to 53 months. The complete response rate was $7.3\%$ and partial response rate was $68.3\%$. Overall survival at 1, 2 and 3 years were $47.5\%,\;23.5\%\;and\;6.3\%$ respectively. None was survived longer than 38 months. Median survival was 12.2 month for 40 patient and 9 month for stage III, M1 group and 9.5 month for stage III, MO group. In M1 patient no survival was seen after 2 years while in M0 patient $23.3\%$ survival was seen.

Pharmacophore Based Screening and Molecular Docking Study of PI3K Inhibitors

  • Non-small cell lung cancer (NSCLC) is the leading cause of cancer-related mortality worldwide. Phosphoinositide 3-kinases (PI3Ks) play important role in Non-Small Cell Lung Cancer. PI3Ks constitute a lipid kinase family which modulates the function of numerous substrates involved in the regulation of cell survival, cell cycle progression and cellular growth. Herein, we describe the ligand based pharmacophore combined with molecular docking studies methods to identify new potent PI3K inhibitors. Several pharmacophore models were generated and validated by Guner-Henry scoring Method. The best models were utilized as 3D pharmacophore query to screen against ZINC database (Chemical and Natural) and the retrieved hits were further validated by fitness score, Lipinski's rule of five. Finally four compounds were found to have good potential and they may act as novel lead compounds for PI3K inhibitor designing.

Immunotherapy for Non-Small Cell Lung Cancer

  • Lung cancer is the leading cause of cancer-related mortality worldwide, and more than 80% of cases are of non-small cell lung cancer. Although chemotherapy and molecularly targeted therapy may provide some benefit, there is a need for newer therapies for the treatment of patients with advanced NSCLC. Immunotherapy aims to augment the recognition of cancer as foreign, to stimulate immune responsiveness, and to relieve the inhibition of the immune response that allows tolerance to tumor survival and growth. Two immunotherapeutic approaches showing promise in NSCLC are immune checkpoint inhibition and cancer vaccination. Although currently immunotherapy does not have an established role in the treatment of NSCLC, these patients should be enrolled in formal clinical trials.

비소세포 폐암의 병기 결정에서 F-18 FDG PET의 역할 (The Role of PET in Staging Non-Small Cell Lung Cancer)

  • Lung cancer has become a leading cause of cancer death in Korea. Accurate staging of non-small cell lung cancer (NSCLC) is essential to the ability to offer a patient the most effective available treatment and the best estimate of prognosis. PET with F-18 fluorodeoxyglucose (FDG) is indicated for the nodal staging of NSCLC and detection of distant metastases. Use of PET for mediastinal staging should not be relied on as a sole staging modality, and positive findings should be confirmed by mediastinoscopy. FDG PET avoids futile surgery by a more accurate selection of patients, especially by the detection of unexpected distant metastases.

기관지표면상피의 미세침윤성 편평세포암과 동반된 혼합성 소세포암종 -1례 보고- (Combined Small Cell Carcinoma Associated with Microinvasive Squamous Cell Carcinoma of Bronchial Surface Epithelium - A case report -)

  • 폐의 편평상피암종에서 기관지표면상피의 화생에 대해서는 많은 이들에 의해서 밝혀진 바 있지만 그 외의 다른 종류의 폐암과 연관된 기관지 표면상피의 변화에 대해서는 그 예가 많지 않고 소세포폐암종에 대해서는 더욱 그러하다. 한편 소세포암종의 조직학적 분류와그아형에 대해서 지난수십년 간많은 관심이 모아져 왔다. 이 종량의 아형을분류하는기본적인 근거는 그 형태학적 판정 기준에 있다. 1967년 WHO폐암종 분류법이 제긍된 이후 3여러 기관에서 SCLC의 임상경과나 치료에 대한 반응, 그 아형을 연구하였다. 1988년 IASLC에서는 소세포암종을 그 형태학적 구분으로 세가지 아형 : Small cell carcinoma (2) Mixed small cel1/1arge ceil carcinoma (3) Combined small cell carcinoma로 분류하고 예후와 치료에 대한 반응의 차이에 대해서 언급하였다. 최근 저자들은 68세 남자환자에서 소세포암종에서 국소적으로 편평양분화를 보이는 세포가 함께 혼합되어 있는 주종괴와 분리되 어서 존재하는 기관지 상피세포에 편평상피암종이 동반된 예를 전폐 절제 술을 하고 조직 검사상 경험하였기에 문헌고찰과 함께 보고하는 바이다.

항암플러스 투여로 호전된 진행성 비소세포성 폐암 연속환자증례 (Case Series of Advanced Non-small Cell Lung Cancer Patients Treated with Hang-Am-Plus)

  • Objective : To investigate the therapeutic effects of Hang-Am-Plus (HAP) on advanced non-small cell lung cancer (NSCLC) patients. Method : We prescribed HAP three times a day (3,000-6,000 mg/ day) during the treatment period (8 - 24 months). Computed tomography (CT) was performed to evaluate the therapeutic efficacy. Results : Four patients who were diagnosed with NSCLC were recommended chemotherap y, but refused it because of old age, side effects, or treatment failure, and instead sought oriental medicinal therapy. They were treated around 12 months. The patients showed stable disease (SD) state for 5 months, 3 months, 19 months and 3 months, respectively. Conclusion : These four cases may give us the possibility that HAP offers potential benefits for non-small cell lung cancer patients.

N2 병기 비소세포 폐암의 수술후 방사선치료 (Postoperative Radiation Therapy in Resected N2 Stage Non-Small Cell Lung Cancer)

  • A total of forty patients with resected N2 stage non-small cell lung cancer treated with postoperative adjuvant radiation therapy between Jan. 1975 and Dec. 1990 at the Department of Radiation Oncology, Yonsei University College of Medicine, Yonsei Cancer Center were retrospectively analysed to evaluate whether postoperative radiation therapy improves survival. Patterns of failure and prognostic factors affecting survival were also analysed. The 5 year overall and disease free survival rate were $26.3\%,\;27.3\%$ and median survival 23.5 months. The 5 year survival rates by T-stage were $T1\;66.7\%,\;T2\;25.6\%\;and\;T3\;12.5\%.$ Loco-regional failure rate was $14.3\%$ and distant metastasis rate was $42.9\%$ and both $2.9\%.$ Statistically significant factor affecting distant failure rate was number of postitive lymph nodes(>=4). This retrospective study suggests that postoperative radiation therapy in resected N2 stage non-small cell lung cancer can reduce loco-regional recurrence and may improve survival rate as compared with other studies which were treated by surgery alone. Further study of systemic control is also needed due to high rate of distant metastasis.

한의기반 통합암치료로 비소세포폐암 환자의 항암화학요법 후유증 개선 2례 (Korean Medicine-based Integrative Oncology Therapies on adverse effects of Chemotherapy in Non-Small Cell Lung Cancer : 2 Cases)

  • Objective: To report the improvements with Korean medicine-based integrative cancer therapies on adverse effects of adjuvant chemotherapy in non-small cell lung cancer patients. Method: There were two patients complained cough, rhinorrhea, numbness, general weakness, nausea and dyspepsia after chemotherapy. They got treated centered on Korean medicine including herbal medicine, acupuncture, electro-acupuncture, pharmacopuncture, moxibustion, hand and foot bath. They were also treated Western immunotherapies like Thymosin at regular intervals. The symptoms were measured by the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 15 for Palliative Care(EORTC QLQ C-15 PAL) and their subjective assessments. Results: Their chief complaints were relieved and their quality of life scores was improved even though they have been receiving chemotherapy continuously. Conclusion: These cases revealed a possibility that Korean medicine-based integrative cancer therapies could improve some symptoms after chemotherapy in non-small cell lung cancer.
