Comparison of Psychotropic Prescriptions between Oncology and Cardiology Inpatients: Result from a Pharmacy Database in a Teaching Hospital in Malaysia

  • Objective: To examine the prescription rates in cancer patients of three common psychotropic drugs: anxiolytic/hypnotic, antidepressant and antipsychotic. Materials and Methods: In this retrospective cohort study, data were extracted from the pharmacy database of University Malaya Medical Center (UMMC) responsible for dispensing records of patients stored in the pharmacy's Medication Management and Use System (Ascribe). We analyzed the use of psychotropics in patients from the oncology ward and cardiology from 2008 to 2012. Odds ratios (ORs) were adjusted for age, gender and ethnicity. Results: A total of 3,345 oncology patients and 8,980 cardiology patients were included. Oncology patients were significantly more often prescribed psychotropic drugs (adjusted OR: anxiolytic/hypnotic=5.55 (CI: 4.64-6.63); antidepressants=6.08 (CI: 4.83-7.64) and antipsychotics=5.41 (CI: 4.17-7.02). Non-Malay female cancer patients were at significantly higher risk of anxiolytic/hypnotic use. Conclusions: Psychotropic drugs prescription is common in cancer patients. Anxiolytic/hypnotic prescription rates are significantly higher in non-Malay female patients in Malaysia.

일반의약품과 전문의약품 의도적 음독 자살 시도자 특성 분석 연구 (A Study on the Characteristics of Intentional Self-Poisoning Patients : Comparison between Non-Prescription and Prescription Drugs)

  • 연구목적 자살시도로 응급실에 내원한 환자들 중 음독이 가장 높은 빈도를 보이고 있다. 본 연구에서는 음독 의약품에 따른 자살 시도자들의 특성을 분석하여 향후 임상 진료 현장에서 도움이 될 제언과 자살 예방 정책을 마련하는데 기초자료를 제공하고자 하였다. 방 법 2011년부터 2019년까지 서울 소재 1개 종합 병원 응급실에 음독 자살 시도 후 내원한 환자 574명의 의무기록을 후향적으로 검토하였다. 의무기록을 통해 인구통계학적 정보 및 자살 과거력, 정신건강의학과 과거력, 정신질환 외 기저질환력, 음독 약물의 종류 및 양, 획득 경로, 단일 혹은 혼합 음독여부 등에 대해서 조사 후 카이제곱 검정 및 독립표본 t-검정을 사용하여 통계 분석하였다. 유의한 결과에 대해서는 위험률 산출을 위해 오즈비를 구하였다. 결 과 일반의약품 음독군이 전문의약품 음독군에 비해 연령은 유의하게 낮았으며 국민건강보험 외 비율은 낮고 배우자가 있는 비율은 유의하게 높았다. 전문의약품 음독군은 일반의약품 음독군에 비해 비자발적 응급실 내원, 정신질환력 및 정신질환 외 기저질환력에서 높은 비율을 보였다. 전문의약품 중 벤조디아제핀 음독군은 비음독군에 비해 본인 처방 비율이 높은 반면 졸피뎀 음독군은 타인처방 비율이 높았다. 벤조디아제핀, 졸피뎀, 항우울제 단독 음독군이 복합 음독군에 비해 유의하게 정신질환력이 없는 비율이 높았다. 결 론 일반의약품 판매 규제에 대한 가이드라인이 필요할 것이며 임상 현장에서 자살 고위험군의 특성을 유념한 면담이 필요하다. 또한 자살 위험성이 높은 환자들에게 전문의약품 처방 시 추적 관찰 기간 및 처방 기간, 포장 방식에 대한 적극적인 고려가 필요할 것으로 보인다.

울산 지역 소아청소년과 및 이비인후과에서의 항염증제 처방 형태 분석 (Outpatient Prescription Pattern of Anti-inflammatory Drugs by Pediatricians and ENT Physicians in Ulsan City)

  • The prescription sheets for outpatients from July 2008 to June 2009 from 7 community pharmacies in Ulsan City were surveyed for the anti-inflammatory drug (AID) prescription pattern. The AID prescription rate of pediatricians and ENT physicians were 30.0% and 34.8%, respectively. The oral steroidal anti-inflammatory drugs (SAIDs) were prescribed as much as 3.9% by pediatricians and 10.3% by ENT physicians. The chiefly prescribed oral SAID was prednisolone in pediatric clinics and methylprednisolone in ENT clinics. Meanwhile the prescription rate of oral non-steroidal anti-inflammatory drug (NSAID) was 22.5% by pediatricians and 21.4% in ENT physicians. The most favorable NSAIDs were propionate derivatives in both clinics. In case of externally-applied SAIDs, the prescription rate of pediatricians was 3.6% and that of ENT physicians was 2.8%. Among them, nasal spray, inhalant and gargle formulations for upper respiratory infection (URI) treatment occupied 35.8% of externally-applied SAIDs in pediatric clinics and 59.7% in ENT clinics. Further, it was observed that ENT physicians favored much stronger SAIDs in Group III of ATC classification (75.4% of externally-applied SAIDs) than pediatricians (49.2%). In the survey of AID combination rate, pediatric clinics showed much lower rate (1.4% of total AID prescriptions) than ENT clinics (7.5%). Among them, the combination rate of oral SAID and oral NSAID by ENT physicians (52.2% of total AID combinations) was much higher than pediatricians (36.6%), which might be over-prescription of AID agents. In conclusion, the AID prescription rate as well as AID combination rate, especially in SAID prescriptions, was much higher in ENT than pediatric clinics, which implies the higher confidency on AID drugs of ENT physicians even though the severity of patient's symptom could be considered.

국공립병원 의약품 공급중단 현황 및 정보 전달체계 개선연구 (A Study for Identifying Current Drug Shortages of Public Hospitals and Improving Drug Shortage Information Delivery System)

  • A study was performed to identify current drug shortages, assess impact of drug shortages on public hospitals and patients, and investigate needs of pharmacists for a drug shortage list. An e-mail survey was sent to the pharmacists of 13 national public hospitals. Total 61.5% of public hospitals has 10 or fewer drugs a year in short supply. Shortages involved mood drugs, anti-tumor drugs, analgesics, antibiotics and etc. in 2012. Among them 75.0% was prescription drugs and the other 25.0% was non-prescription drugs. 79.2% was domestic products and 20.8% was imported drugs. Only 12.5% was injections. Less than 3 pharmacists usually spent within 3 hours managing one drug shortage. Since a single item for a certain medicine may raise risk of drug shortages, it's needed to consider developing manuals, laying up medicine stocks and holding plural medicines for drug shortages in public hospitals. Main information resources of drug shortages are wholesalers or manufacturers. But the information appeared to be not only inadequate but also too late for appropriate activities. A survey of pharmacists revealed that overall 84.6% of respondents were in need of the drug shortage list. They expected it to be conducted to take proper measures for the drug shortage and to improve patient healthcare outcome and convenience. This study will contribute to improving public health by promoting stable supply of drugs and repairing the information delivery system.

외래약국 복약이행도에 영향을 미치는 요인 연구 - 제3기(2005년) 국민건강영양조사자료를 근거로 - (A Study on the Factors Influencing Medication Compliance At Community Pharmacies - Based on The 3rd (2005) Korea National Health and Nutrition Examination Survey Data -)

  • This study was to identify factors influencing drug compliance based on the subjects' interview regarding community pharmacy utilization for 2 weeks, in 2005 KNHANES. Good compliance was regarded as important factors in improving the effectiveness and minimizing adverse drug reaction, resulting in reducing the medical costs. 83% of total 11,208 pharmacy visits in 7,066 subjects showed good compliance. Good satisfaction for pharmacist's medication counseling (OR=2.23, 95% CI 1.92-2.58), higher out-of-pocket money (OR=1.32, 95% CI 1.14-1.54), and users of prescription drugs than non-prescription (OR=2.21, 95% CI 1.91-2.57) drugs were significant factors for better compliance. Disease of nervous system and mental and behavioral disorders showed lower drug compliance.

충북지역 노인들의 약물복용 및 영양상태 - I. 질병 및 약물복용실태- (Drug Consumption and Nutritional Status of the Elderly in Chung-Buk Area - I. Diseades and Drug Consumption-)

  • Three hundred sixty-two(male 131, female 231) elderly aged over 65 in Chungb- uk area were interviewed to determine the disease states and drug usage patterns. The prebalence of disease was 78% and women reported more chronic diseases(83%) than men(71%). Elderly who live with spouse and have an occupation have a lower rate of disease. Average number of diseases of the elderly was $1.8\pm{1.1}$, and women$(2.1\pm{1.3)}$ have significantly higher average number of diseases than that of men$(1.4\pm{0.7)}$. Also the elderly in urban areas$(2.1\pm{1.4)}$ have significantly higher number of diseases than that of the elderly in rural areas$(1.6\pm{0.9)}$. Arthritis, hypertension, cardiovascular and gastric diseases were the most frequently listed chronic diseases in order for both men and women. Anemia and fracture of bone were relatively higher in women than in men. Particularly, the arthritis of the urban elderly have a rate of 1.5 times higher than that of the rural elderly. Fifty-two percent of the elderly were currently using drugs ; among drug users 71.2% used prescription drugs and 20.5% used nonprescription drugs. The average number taken per person was 2.1$\pm$1.4 and there was no sex or age difference. However, the elderly in rural areas $(2.7\pm{1.7)}$ consumed a significantly higher number of drugs than those in urban areas$(1.7\pm{0.7)}$. The average number of prescripti- on drugs taken was 2.0$\pm$1.4 while the average of nonprescription drugs taken was $(1.3\pm{0.6)}$. Analgesics and antihypertensive drugs were most commonly used. Vitamin and analgesics were the most frequently used self-prescribed drugs. It was noted that potential adverse drug interaction by concominant drug consumption for arthritis and antihypensive drug, abuse of digestants and antiacid without treatment of the underlying disease, and misuse of quick-acting bowel medications were problematic for the elderly. In addition drugs used for the elderly have some adverse effect on the digestive system. The types and composition of drugs used by the elderly were identified and presented. Medication compliance was poor and 13.5% reported adverse reactions such as edema, heartburn, nausea, and difficulty with eating. Seventeen percent of the elderly obtained drugs arranged by those other than medical staff. Also, even among those elderly who obtained drugs prescribed by a doctor, 69.1% of subjects had not receive instruction about potential adverse reactions. These results suggest that nutritional problems related to drug usage might exist and so dietitians, either individually or as members of health teams, need to have a better understanding of drug-nutrient interaction and closer supervision, and drug information/education service should therefore be provided to prevent or minimize adverse drug reaction in elderly users of medication.

종합병원의 외래환자 처방전에 대한 약물상호작용 검토 (Drug Interaction Review of Prescriptions for Outpatients at General Hospital)

  • To investigate drug interaction, 23,536 prescriptions published for 1 year were investigated with 'Drug Inter­action Fact 2002'. Dispensing records and a database file written in a local general hospital in South Korea were used as a sample. The number of total cases of drug interaction was 3,238 ($13.76\%$) out of 23,536 prescriptions. The incidence of drug interaction in each prescription the children, the adults, and the elderly were $1.33\%,\;10.97\%,\;25.50\%$, respectively. The incidences of drug interaction per each prescription were $22.03\%,\;20.52\%,\;0.51\%,\;and\;0.36\%$ in neurosurgery, internal med­icine, pediatrics, and orthopedics, respectively. In neurosurgery and internal medicine, risk-high drugs of drug interaction such as antihypertensive drugs, diuretics, and cimetidine were used very often in elderly. In this paper, several suggestions to reduce drug interaction were postulated with regard to the usage of analgesics, non-steroidal antiinflammatory drugs, and antibiotics.

소아청소년에서 편두통 치료 약물 처방 현황 (Medication Prescribing Status in Pediatric Patients with Migraine)

  • Background: Migraine is one of the leading causes of poor quality of life and disability, and migraine incidences in pediatrics are increasing. Proper medication is important for the preventive and acute treatment of migraine. This study aimed to identify the current status of prescribed medication in pediatric patients with migraine. Methods: We used data from a sample of pediatric patients from the Health Insurance Review and Assessment Service (HIRA-PPS-2018) and analyzed the status of prescription drugs and frequency of visits to medical institutions with migraine diagnoses in pediatric patients. Results: A total of 12,228 pediatric patients diagnosed with migraine during 2018 were analyzed. Among these patients, 7,170 (58.64%) were girls and 9,510 (77.77%) were adolescents. Additionally, 9,157 patients (74.89%) received acute treatment, and 592 patients (4.84%) received combination therapy with analgesics and triptans. Acetaminophen for acute treatment and flunarizine for preventive treatment were the most commonly prescribed. In most children and adolescents, acute treatment drugs were prescribed for less than 14 days. Conclusion: Analgesics, such as acetaminophen or non-steroidal anti-inflammatory drugs, were prescribed frequently for acute treatment in pediatric patients with migraine. The drug prescription duration was within the recommended range, indicating a low risk of overdose. For preventive treatment, clinically studied medication for pediatric patients with migraine was used.

처방의약품과 일반의약품의 복약지도 시간에 영향을 주는 약사 요인 (The Factors Related to Patient Consultation Time for Prescription and Non-Prescription Medications in Community Pharmacies)

  • The effects of characteristics of community pharmacists on consultation time for prescription (RX) and nonprescription drugs (nRX) were investigated. A crosssectional descriptive design was established with a self-administered anonymous mail survey. Response rate was 52%. Significant pharmacists' factors related to having less than 3 minutes consultation for RX were one's dispensing duties exceeding 50%, low satisfaction with one's own level of consultation, working at the pharmacies nearby clinics, or shorter duration of consultation time for nRX. Consultation promoting conditions should be built up for the provision of sufficient consultation.

관절염 증상의 모의 환자를 이용한 약사의 처방 행태 연구 (The Study of Prescription Behaviors of Practicing Pharmacists with Simulated Patients of Arthritis)

  • Objectives : In Korea, pharmacists can dispense medicines without doctor's prescription. This causes the high proportion of pharmaceutical expenditures. The study shows the prescribing behaviors of practicing pharmacists with the simulated patient of arthritis. We select the arthritis as a subject of simulation, because the arthritis is one of the major health problems and the abuse of cortico-steroids is usual in treatment of arthritis patients. Methods : Twenty drug stores among the 320 drug stores in a district, Seoul, Korea were randomly selected. One of the researchers visited the drug stores and received the medicines from the pharmacists after explaining standardized scenario of arthritis. The simulated patient recorded the practice behaviors of pharmacists. Results: The mean number of prescribed drugs are four and half. Among the twenty pharmacists, the nineteen prescribed non-steroidal anti-inflammatory drugs and the seven(35%) prescribed the cortico-steroids. The antacids were prescribed by the fourteen(70%) pharmacists. The five(25%) pharmacists only recommended the simulated patients to visit the medical doctors, and the three(15%) performed physical examination to the simulated patients. The three pharmacists(15%) asked the past history of the drug adverse effects and no pharmacist explained the adverse effects of prescribed medicines. Conclusions : The research shows that the cortico-steroids are frequently prescribed and the pharmacists commonly do not give the explanations of the prescribed medicines to the arthritis patients.
