Implementation of Real-time Wireless Remote Control System Based on Public Key Infrastructure (PKI를 기반으로 한 실시간 무선 원격제어 시스템의 구현)

  • Existing web-based system management software solutions show some limitations in time and space. Moreover, hey possess such as shortcomings unreliable error message announcements and difficulties with real-time assistance supports and emergency measures. In order to solve these deficiencies, Wireless Remote Control System(W-RCS) was designed and implemented. W-RCS is able to manage and monitor remote systems by using mobile communication devices for instantaneous control. The implementation of W-RCS leads to these security problems as well as solutions to aforementioned issues with existing web-based system management software solutions. Therefore, this paper has focused on the security matters related to W-RCS. The security functions based on public key infrastructure include mobile device user authentication and target system access control. The W-RCS allows real-time user authentication, increases the flexibility of resource administrators and mobile device non, and provides not only uninterrupted services, but also safe mobile office environments.

  • The 4th industrial revolution is progressing by country as a mega trend that leads various technological convergence directions in the social and economic fields from the initial simple manufacturing innovation. The epidemic of infectious diseases such as COVID-19 is shifting digital-centered non-face-to-face business from economic operation, and the use of AI and big data technology for personalized services is essential to spread online. In this paper, we analyze cases focusing on the application of artificial intelligence technology, which is a key technology for the effective implementation of the digital new deal promoted by the government, as well as the major technological characteristics of the 4th industrial revolution and describe the use cases in the field of disaster response. As a disaster response use case, AI assistants suggest appropriate countermeasures according to the status of the reporter in an emergency call. To this end, AI assistants provide speech recognition data-based analysis and disaster classification of converted text for adaptive response.

Lethality-Associated Factors in Deliberate Self-Poisoning

  • Objectives: Deliberate self-poisoning (DSP) is the most common suicide method and can be life-threatening. The purpose of this study was to investigate the factors related to the lethality of DSP and the characteristics of the adolescent group. Methods: A retrospective study was conducted on patients who had visited an academic hospital's regional emergency medical center between 2015 and 2018. The data reviewed through their medical records included sociodemographic factors, clinical variables, and psychiatric treatment. Four groups (Q1-Q4) were categorized by descriptive analysis using the risk-rescue rating scale. Results: A total of 491 patients were enrolled in this study. This study showed that high lethality had statistically significant associations with male sex, older age, admitting suicidal intentions, and the use of herbicides for suicide. Logistic regression analyses showed a significant association between high-lethality and female [odds ratio (OR)=0.50, 95% confidence interval (CI)=0.30-0.81, p=0.01], non-psychiatric drugs (over-the-counter drug: OR=2.49, 95% CI=1.08-5.74, p=0.03; herbicide: OR=8.65, 95% CI=3.91-19.13, p<0.01), and denial of suicide intent (OR=0.28, 95% CI=0.15-0.55, p<0.01). Conclusion: This study showed the clinical factors associated with the high lethality of DSP and suggested that efforts were needed to care for and thoroughly examine patients with DSP.

A case of various clinical aspects associated with cardiotoxicity after glufosinate poisoning (글루포시네이트 중독 후 심장독성의 다양한 임상경과를 보인 1례)

  • Glufosinate-containing herbicides is a non-selective herbicide commonly used worldwide. As the use of them increased gradually since paraquat was banned in 2012, the number of suicides by their ingestion is also increasing continuously. Complications of glufosinate-containing herbicide poisoning include various central nervous system (CNS) toxicities such as convulsions, loss of consciousness, memory impairment, and respiratory depression, which may be accompanied by hemodynamic changes such as bradycardia and hypotension. However, it is very rare that arrhythmias other than bradycardia occurred and Takotsubo cardiomyopathy was combined due to cardiotoxicity. A 71-year-old female patient was transferred to our hospital after ingesting 500 mL of glufosinate-containing herbicide and receiving 5 L of gastric lavage at a local hospital. A few hours later, she presented stuporous mentality, respiratory depression, and convulsions, and was accompanied by hypotension and bradycardia. On the second day of admission, electrocardiogram (ECG) showed bradycardia and QTc prolongation with hemodynamic Instability. Accordingly, we conducted the early treatment with continuous renal replacement therapy (CRRT) and the application of temporary cardiac pacemaker. An echocardiogram demonstrated decreased ejection fraction (EF) and Takotsubo cardiomyopathy on the third day of admission. Then, she was discharged safely with conservative treatment. At the follow-up after 1 year, Takotsubo cardiomyopathy, EF and QTc prolongation were recovered on echocardiogram and ECG. Because cardiac toxicity after glufosinate-containing herbicide poisoning may cause life-threatening consequences, caution is required while treating the patient. Therefore, if electrocardiogram changes are seen in the elderly with a large amount of glufosinate herbicide ingestion, additional cardiac function test through echocardiography should be concerned, and early treatment through CRRT or artificial cardiac pacing should be considered.

Facial injury burden of personal mobility devices: a single-center retrospective analysis

  • Background: Personal mobility devices (PMDs) have become an increasingly popular transport modality globally. With increasing social interest in and demand for PMDs, the number of individuals visiting emergency departments with PMD-related injuries has also increased annually. This study aimed to evaluate injury patterns and treatment costs for patients treated in the department of plastic surgery in a trauma center. Methods: In this retrospective study, data concerning patients with PMD-related injuries from January 2017 to December 2021 were reviewed. The data retrieved included age, sex, alcohol consumption, helmet use, the type of impact, onset of injury, place of first visit, type of injury, admission status, operation status, and treatment cost. Multiple linear regression analysis was performed to determine the effects of various factors on cost. Results: Data were collected from 93 patients. Until 2019, the annual number of PMD-related accidents was less than 10; however, this number increased sharply in 2020. The average cost of hospitalization was USD 7,698 whereas the average cost of non-hospitalization was USD 631. Only fractures had a significant association with total cost in linear regression analysis (p< 0.001). Conclusion: The prevalence of PMD use and related injuries requiring plastic surgery during the study period showed significant health and financial costs both to the patients involved and to society. This cost could be reduced through stricter regulations concerning PMD use, advocating the use of protective gear, and promoting greater awareness of safety measures and of the consequences of PMD-related accidents.

Comparison analysis of rescuer's neck muscle tone and stiffness according to cardiopulmonary resuscitation skill-level

  • The purpose of this study was to identify differences in the muscle tone and stiffness of neck muscles according to levels of cardiopulmonary resuscitation (CPR) skill. The subjects were 30 female students in their 20s who were divided into a skilled group (n=15) and an unskilled group (n=15). According to the results, the skilled group showed statistically significant decreases in the stiffness of the lower cervical muscle and the muscle tone of the upper trapezius on the above hand side of hand grips after CPR (p<.05). The unskilled group exhibited a statistically significant difference in both the muscle tone and stiffness of the upper trapezius muscle on both sides after CPR (p<.05). However, no statistically significant differences were found between the two groups. In addition, the non-skilled group showed statistically significant lower values than the skilled group in the mean compression rate, total number of compressions, accuracy of chest compression, and accuracy of chest relaxation (p<.05). This study confirmed that CPR has a myophysiological effect on the neck muscles of those who perform CPR. However, it found that no differences result from individual CPR skill levels that are an important element for the accuracy of CPR.

Impact of Highway Construction on the Regional Economy: Gangil-Chuncheon Highway (고속도로 건설이 지역경제에 미치는 기여도 분석연구: 강일~춘천 고속도로)

  • Recently, the Ministry of Strategy and Finance announced a plan to reduce the proportion of economic evaluation in non-metropolitan areas and strengthen balanced development evaluation through the reorganization of the preliminary feasibility study system. In addition, the social value of the quality of life, such as job, environment, and safety, which may be affected by the implementation of transportation facility investment projects, was reflected as the main item of policy evaluation. In this study, the ripple effect of the project in terms of social value was reviewed for the Gangil-Chuncheon. The effect of highway opening was investigated by reviewing the feasibility report, post-evaluation report, and statistical indicators. Recently, the Gangil-Chuncheon highway is getting used by around 110,000 people per day. The number of tourists in Chuncheon rapidly has increased from 5 million a year to more than 11 million now. In addition, it was confirmed to produce effects such as population migration, net inflow and land price increase, improvement of living convenience, and expansion of emergency medical care. Although this ripple effect was influenced by various socio-economic factors as well as the opening of the highway, it is clear that it is difficult to occur without the opening of the highway. It is judged that the evaluation of indirect benefits and social values due to the opening of the highway can be quantified through continuous research and data construction. Post-evaluation of construction works, including project efficiency evaluation and ripple effect evaluation, is performed for construction works with a construction cost of more than 50 billion won. In the future, we will continuously improve the evaluation system in order to evaluate the indirect benefits and social values of public investment projects.

Analysis of Safety Management Operations of Fire Risk Factors in Small-Scale Construction Sites (소규모 건설현장 화재 위험요인 안전관리 운영실태 분석)

  • By analyzing the operation status of fire safety management of small construction site workers, deriving problems, and suggesting improvement measures, this study was conducted to present practical basic data for their efficient use in the future, and the following conclusions were drawn. First, it was analyzed that small construction site workers are elderly in the age group of construction workers, have short construction skills, most of the jobs are working in the construction industry, and the employment type is non-regular workers. Second, the fire safety management improvement plan of small construction site workers is systematized, fire safety manager is deployed to manage fire risk, fire escape routes and emergency warning facilities are provided to inform all workers at the construction site. In addition, measures to reduce industrial accidents are needed through realistic evacuation training, fire VR training, and interesting educational programs.

A Case of Successful Treatment of Congenital Syphilis in an Extremely Preterm Baby With Severe Respiratory Distress

  • We report a case of successful treatment of congenital syphilis in an extremely preterm baby. A 1,395 g female infant was born by emergency Caesarean section due to preterm labor and breech presentation at gestational age at 29 weeks and 3 days with an Apgar score of 2 and 4 at 1 minute and 5 minutes, respectively. The mother of the newborn, an illegal immigrant who did not receive any antenatal care, was diagnosed as active syphilis infection by reactive rapid plasma regain (RPR) (titer 1:128) just before the delivery. Upon birth, the newborn presented with various clinical manifestations, including severe respiratory distress syndrome, persistent pulmonary hypertension of the newborn, disseminated intravascular coagulopathy, desquamation and scaling of the whole body, and osteolytic changes of long bone ends. Results of laboratory tests showed signs of early congenital syphilis, including positive syphilis reagin test (12.7 R.U.), reactive with RPR titer of 1:64, and positive for immunoglobulin (Ig) M and IgG fluorescent treponemal antibody absorption test. However, after completion of penicillin G treatment for two weeks, laboratory results dramatically improved, showing a negative syphilis reagin test (0.5 R.U.) and non-reactive in RPR. In conclusion, the incidence of congenital syphilis is prone to be resurgent in South Korea, neonatologists should be fully aware of the clinical features of congenital syphilis because early diagnosis and prompt treatment are essential in order to reduce the social and economic burden due to congenital syphilis.

Transitional care for high-risk elderly patients pre/post discharge by collaboration between general hospital and community pharmacy: a pilot study

  • Background: Medication-related problems (MRPs) frequently occur during the discharge period. Elderly patients, particularly, are at high risk for these problems due to polypharmacy and the use of potentially inappropriate medications. The purpose of this study was to build and implement collaboration between general hospital and community pharmacies to address MRPs among high-risk elderly patients before/after discharge. Methods: This retrospective study was conducted between June and December of 2020. The inclusion criteria were patients with aged ≥65 years; residents of Jeonju; discharged from Jeonbuk National University hospital; either on medication of exceeding 10 medications (or high-risk medications) after hospitalization through the emergency room, or having severe illness. Patients received medication reconciliation and counselling by hospital pharmacists before discharge and home-visit pharmaceutical care as follow-up by community pharmacists after discharge. Results: Twenty-two patients agreed to home-visit pharmaceutical services. Fifteen and 11 patients completed the first and second home-visit pharmaceutical care service, respectively. Forty-two MRPs were identified in 15 patients. The types of high-frequency MRPs were incorrect administration of drug, adverse drug reactions, medication non-compliance, drug-drug interactions, lifestyle modifications, and expired medication disposal. After consultation with the pharmacist, 34 out of 42 MRPs were resolved. Conclusions: Transitional care for high-risk elderly patients before and after discharge was successfully built and implemented through a collaboration between general hospital and community pharmacies. This study suggests that home-visit pharmaceutical services may have positive effects on the safe use of drugs during the transition period; however, additional research is needed to expand on these findings.
