Serum Osmolality and Its Association with Health-Related Factors and Biochemical Parameters in the Elderly (노인의 혈청 삼투압과 건강관련 요인, 생화학적 변수들과의 상관성)

  • This study was designed to determine serum osmolality and to investigate the association among serum osmolality, health-related factors and biochemical indices. Two hundred thirty seven elderly(86 male, 151 female : mean age 73.8) residing in the chungbuk area participated. Sociodemographic data and self-perceived health status were obtained by interview, and biochemical parameters were measured. The mean serum osmolaity was 301.2$\pm$10.1 mOsm/kg(range 240.8~328.9 mOsm/kg) and serum osmolality of the females(302.4 mOsm/kg) was significantly(p<0.05) higher than that of the males(299.0 mOsm/kg). Only 11.0% of the elderly were within the accepted normal range(280~295 mOsm/kg) and the majority of the subjects were in a hyperosmolar state(60.8% : 296~307 mOsm/kg). It was observed that serum osmolality was reduced with increasing age but not statistically significant. There was no significant difference in serum osmolality according to marital status. Serum osmolality was significantly lower(p<0.05) in the elderly with higher self-perceived health scores than the elderly with lower self-perceived health scores, however no significant difference was observed between the presence or absence of disease. Serum osmolality tended to be higher(p=0.06) in the elderly with difficulty in mobility than the elderly who have no problem in mobility. Serum osmolality tended to be higher(p=0.06) among drug users compared to that of non-users. There was a significant association between serum osmolality and serum albumin in females but not in males. Significant associations were shown between serum osmolality and serum transferrin in both sexes. However, there was no significant correlation between serum osmolality and hemoglobin or hematocrit except with hematocrit in females only. The results of this study indicated that the elderly were dehydrated and hydration state seems to influence health status and, the elderly should be encouraged to drink an adequate amount of fluids. Also, the hydration state is required to be assessed and considered for the interpretation of biochemical parameters.

Prevalence of Types of Cancers in the Elderly Covered by Insurance of the Islamic Republic of Iran Broadcasting Company in 2015 - Comparison with Younger Groups

  • Presently, the world population of the elderly is growing. By improving health hygiene and welfare indicators, mortality and birth rates decrease and life expectancy increases, making the present century the century of elderly. Aging is one of the main risk factors for development of cancer, which itself is the second cause of death in old people. This study was conducted to assess the prevalence of cancer in the elderly covered by the Islamic Republic of Iran Broadcasting (IRIB) insurance program and to obtain suitable programs for cancer screening and early detection, increase patient survival, improve elderly care and to reclaim the cost of treatment in comparison to the national and international statistics. This is a cross-sectional study conducted on all elderly patients diagnosed with malignancy based on their pathology reports. In this study, of the total 75,500 patients covered by IRIB insurance, 17.2% belonged to the elderly group, males accounting for 53.3%. The most common cancers in old men were prostatic cancer (61.3%), colon cancer (10.3%) cancer of the hematologic system, bladder cancer (9.6%), lung cancer (9.1%), thyroid cancer (3.9%) and brain tumors (1.3%). In the elderly women, the most common cancers were breast cancer (80.1%), colon cancer (5.1%), thyroid cancers (4.4%), bladder and hematologic system malignancies (3.6), lung cancer (2.9%) and brain tumors (0.7%). In addition, the prevalence of cancer was almost the same as national and international statistics. With the exception of non-melanoma skin cancer no difference was shown in prevalence of cancer between IRIB elderly patients and the other groups of cancer patients in Iran.

A Study on the Analysis and Improvement of the Welfare Ordinance for the Elderly in Gwangju and Jeonnam

  • The purpose of this study is to examine the current status of the elderly welfare ordinances currently enacted and operated by local governments, analyze the actual conditions and problems of the contents, and suggest the direction of enactment and revision of the elderly welfare ordinances for actively responding to the demand for various welfare services for the elderly. To this end, the scope of the study was specified by ordinances in Gwangju Metropolitan City, five autonomous districts, Jeollanam-do, and 22 cities and counties, and the criteria for content analysis of the elderly welfare ordinance of these local governments were selected and compared and analyzed in the four major areas of elderly welfare: income and jobs, care, health, leisure activities, and social participation. According to the analysis results, the direction of enacting the ordinance is to secure financial resources for the elderly welfare budget, develop differentiated welfare policies for the elderly that reflect the characteristics of each local government, expand ordinances related to infectious diseases, enact ordinances for medical support for non-infectious diseases, and enact customized ordinances for the elderly living alone. Finally, the limitations on the research subject and analysis scope of this study and future research directions were presented.

A Study of Cognitive Function and Depression of the Elderly in a Institution (수용시설 노인들의 인지기능과 우울에 관한 연구)

  • Purpose : To estimate the prevalence of and identity the associates of cognitive function and depression among people over 70 years of age, and to examine the relationship between cognitive impairment and depression, Methods : The elderly in Pusan area were studied on the Korea version Mini Mental Scale Examination instrument(MMSE-K), Short form of Geriatic Depression Scale(SGDS). Results : The prevalence of cognitive impairment($MMSE{\leq}24$) was 64.4% and that of depression($GDS{\geq}8$) was 40.7% Lower education, no religion and no work experience were associated with cognitive impairment. Depression in demented group was 68.2%, in suspect dementia group was 43.7% and in non-demented group was 9.5%. Conclusion : The prevalence of cognitive impairment and depression in elderly was high and depression was in association with cognitive impairment.

  • The increasing elderly population and elderly household are very serious problem in our time. They are weaker physically and mentally than they were in the past. There is hardly a house with floor and facility design for them. So the purpose of this study is to find the need of the elderly for the living condition in Daejeon city. Their needs of interior are invested by questionnaire for elderly over 65 years old. Remarkably, they are not unsatisfied the present condition in spite that it is not agreeable in fact. The informations, for example the equipments and remodeling, to make their life more conveniently are not noticed to them fully. And they point up the emergency bell, the handiness of controlling windows, getting rid of the difference of thresholds, non-slip materials, grab bars around the a chamber pot and the expediency of controlling a tap as important things to select a house. First of all the sensitive considerations to make their living comfortable by designers are needed absolutely and the publicity work to notice the beneficial inform for them is needed.

Effects of Calcium/Vitamin D Intake and Taekkyeon Exercise on the Elderly's Frailty (칼슘/비타민D 섭취와 택견운동이 노인 허약에 미치는 효과)

  • Purpose: The purpose of this study is to understand the effect of taking calcium/vitamin D supplements and performing Taekkyeon exercises on the frailty of the elderly targeting a vulnerable senior social group. Methods: A total of 124 people aged 65 and older consisting of 60 persons in the experimental group, and 64 persons in the control group participated in this study, and this study is a non-equivalent control group pre/post-test design quasi-experimental study. This study analyzed the homogeneity between the experimental group and the control group using a chi-square test and t-test while analyzing the difference in the degree of frailty and bone density with the practice of Taekkyeon and a calcium agent using t-test and GEE. Results: The study results showed that the frailty and bone density of the experimental group significantly improved. Conclusion: Through this study, it was confirmed that calcium/vitamin D intake and Taekkyeon exercises were an effective intervention which strengthened the elderly's physical condition.

Structural Characteristics and Feasibility of Per Diem Payment System for Elderly Dementia Inpatients (치매노인환자 입원진료비의 구조적 특성과 일당수가제화의 타당성)

  • This study was conducted to analyze the medical charges of the elderly dementia inpatients, to identity their characteristics, and there by to evaluate feasibility of the per diem payment system for the patients. Data on medical charges of the patients were collected from the National Federation of Medical Insurance and sample hospitals from October through December 1997. The data were analysed in order to find the characteristics and test hypotheses postulated. The results are summarized as follows; Firstly, there was no difference statistically in between disease groups and between the dementia inpatients belonging to each disease group. Secondly, the amount of the non-insurance medical charges of the elderly dementia inpatients is considerably high compared to the insurance medical charges paid by the patient, which implies that some measures are to be prepared by the Government. Finally, medical charges per inpatient day of the dementia patients are not different statiscally by sex, by age group, and by disease group. This result supports the feasibility of the per diem payment system for the elderly dementia inpatients.

  • This study analyzed the perception types of women over the age of 60 in regards to attitude and the effect of wearing brassiere. In addition, it was also to provide the basic data demand for the development of a brassiere for elderly women. Q methodology was used for this study. Analyze the data using the QUANL pc program. Type 1 was the nipple cover non-preferred type. Type 1 thought brassiere has uplifting effect, but is uncomfortable and only worn when going out. Type 2 was a function-oriented preference type. They wore a brassiere for beauty and psychological satisfaction. The brassiere push-up the breast and enhanced volume. Type 3 was the pursuit of vibration reducing and beauty type. The brassiere reduced breast vibration and kept armpits comfortable. The brassiere was worn for beauty. In the future, the actual development of brassieres for elderly women should be investigated for wearing sensation and satisfaction.

Relationship between smoking history and periodontal disease among the elderly in Korea

  • Objectives: The objective of this study was to investigate the relationship between smoking history and periodontal disease among the elderly in South Korea. Methods: The study subjects comprised 2,703 elderly people who underwent oral health examination as part of the 7th South Korea National Health and Nutrition Examination Survey (KNHANES; 2016-2018). Data were analyzed using frequency analysis, Rao-Scott chi-square test, t-test, and binary logistic regression analysis. Results: A complex sample logistic regression analysis showed that the odds for periodontal disease development were higher in past smokers (odds ratio [OR]=1.461; 95% CI=1.070-1.994) and current smokers (OR=1.601; 95% CI=1.011-2.536) than in lifetime non-smokers. Conclusions: Smokers must actively participate in smoking cessation programs and interventions starting from middle age.

Association between smoking behavior and denture wear in the elderly aged 65 years and older in South Korea: The 7th Korea National Health and Nutrition Examination Survey (우리나라 65세 이상 노인의 흡연과 의치장착 관련성: 제7기 국민건강영양조사를 바탕으로)

  • Objectives: This study was conducted to analyze the relationship between smoking and denture wear in the elderly individuals aged 65 years and older, and a total of 3,112 people were included. Methods: A complex sample chi-square test was performed for denture wear according to general, smoking, and smoking-related characteristics. Factors related to denture wear were assessed using multiple logistic regression analysis. Results: Among non-smokers, present smokers were 4.192 times more likely to wear dentures, and former smokers were 2.195 times more likely to wear dentures. The average number of daily past smokers was 0.564 times less likely to wear dentures if they smoked 15 cigarettes or less per day. Conclusions: Smoking among the elderly and wearing dentures are related, and it is necessary to develop and utilize a smoking cessation education program considers socioeconomic characteristics.
