Portable urodynamics monitoring system based on ubiquitous technology (유비쿼터스 기반 휴대용 방광기능 검사 장치)

  • We developed a portable urodynamics monitoring system using personal digital assistance (PDA) and proposed a new method for measuring the abdominal pressure in non invasive mode using surface EMG signals. This system is consisted of a signal conditioning and control module and could measure the vesical, abdominal and detrusor pressure. The result showed a high correlation coefficient between the practical abdominal pressure, using a conventional rectal catheter and the estimated values, by our proposed algorithm (r=0.81). This system should prove a useful tool in the future evaluations of ambulatory urodynamics monitoring study.

Integrated Effect of Non-Invasive Neuromodulation on Bladder Capacity in Traumatic Spinal Cord Injury Patient: Single Case Report

  • Objective: To evaluate the changes in bladder capacity and storage through non-invasive neuromodulation by application of repetitive Trans magnetic stimulation (rTMS) and genital nerve stimulation (GNS) in traumatic spinal cord survivors. Design: A Single Case Study. Method: The Patient was registered in trail with the clinical trial registry of India (CTRI/2022/05/042431). The Patient was interposed with rTMS on lumbar area, from T11-L4 vertebrae with 1 Hz and the intensity was 20% below that elicited local paraspinal muscular contraction for 13 minutes. GNS was placed over dorsum of the penis with the cathode at the base and anode 2 cm distally at 20 Hz, 200 microseconds, Continuous and biphasic current was delivered and amplitude of stimulation necessary to elicit the genito-anal reflex. For assessment, Neurological examination was done for peri-anal sensation (PAS), voluntary anal contraction (VAC) and bulbocavernous reflex (BCR), deep anal pressure (DAP), and American Spinal Injury Association Impairment Scale (ASIA scale). Outcome assessment was done using Urodynamics, Spinal Cord Independence Measure Scale Version-III (SCIM-III), American Spinal Injury Association Impairment Score (ASIA Score), Beck's Depression Inventory Scale (BDI). The baseline evaluation was taken on Day 0 and on Day 30. Results: The pre-and post-data were collected through ASIA score, SCIM-III, BDI and Urodynamics test which showed significant improvement in bladder capacity and storage outcomes in the urodynamics study across the span of 4 weeks. Conclusion: rTMS along with GNS showed improvement in bladder capacity & storage, on sensory-motor score, in functional independence of individual after SCI.

A Non-invasive Measurement of Abdominal Pressure on Ambulatory Urodynamics Study Using Surface Electromyography (휴대용 요역동학 검사 시 근전도 신호를 이용한 복압측정 방법)

  • Conventional rectal catheters which are used for the measurement of abdominal pressure can cause erroneous result affecting detrusor pressure, and the catheter itself is not comfortable to the patients. To reduce these problems, we invented a new method for measuring abdominal pressure in non invasive manner using surface electromyography (EMG) signals of the rectus abdominis muscle. Our results showed that the correlation coefficient and root mean square error (RMSE) between the measured abdominal pressures by the conventional rectal catheters and the estimated values by our proposed algorithm were $0.79{\pm}0.06$ and $0.10{\pm}0.07$, respectively. These findings suggest that the surface EMG of rectus abdominis muscle might be used indirectly for more convenient measurement of abdominal pressure on ambulatory urodynamic study.

Implementation and estimation of the urophonography measurement system for diagnosis of Lower urinary tract symptom (하부요로계 폐색 진단용 요류음 계측 시스템의 구현 및 평가)

  • A sensor and measuring system were implemented to measure both uroflow and urophonography signal during urination for diagnose the lower urinary tract symptom. The implemented system was composed of the uroflow sensor using the load-cell, the urophonography measurement sensor so as stethoscope type, pre-processing part for sensors signal detection, amplifier and filter, system control parts and PC measurement program. A simulator of the lower urinary system that is experimental equipment implemented for evaluate the developed system. The two signals were measured using implemented system and analyzed these signal by means of time domain and frequency domain for extraction of the characteristic parameter which can most effectively reflected by the occlusion of the lower urinary system. Furthermore two signals were measured and analyzed with the subject of 5 healthy adult for clinical application possibility of the implemented system. As a result, the most influence bandwidth of effect according to occlusion of the lower urinary system is $253{\sim}282$ Hz of the urophonography signal.
