DEVELOPMENTAL AND CLINICAL CHARACTERISTICS IN CHILDREN WITH NOCTURNAL ENURESIS : RESULTS OF A MULTICENTER STUDY (야뇨증 환아들의 심리사회적 특성에 대한 다기관 연구 : 발달학적 및 임상적 특성을 중심으로)

  • Objectives : The aim of this study was to examine the developmental and clinical characteristics associated with nocturnal enuresis in Korean children. Methods : Three hundred eighteen children with nocturnal enuresis, together with their parents, completed a parent questionnaire consisting of a Child Behavior Checklist(CBCL). Data related to the prenatal, perinatal and developmental history, past and current medical history were collected. Ninety-three normal students were selected as the control group. Results : The nocturnal enuresis group attained diurnal and nocturnal urinary control significantly later than those in the normal control group.49.2% of the children with nocturnal enuresis had the family history of nocturnal enuresis. Daytime incontinence was present in 41.3% of the nocturnal enuresis group. The percentages of sleep-related disturbances were significantly higher in the nocturnal enuresis group when compared to the normal control group.42.6% of the children with nocturnal enuresis experienced pharmacotherapy, and 0.4% experienced enuresis alarms. Conclusion : The results of this study suggest that children with nocturnal enuresis in Korea have a high genetic load and a possibility of developmental delay, which supports the neurodevelopmental point of view with regard to the etiology of nocturnal enuresis. The physicians in Korea prefer pharmacological interventions to alarm interventions in treating Korean children with nocturnal enuresis.

Characteristics of Nocturnal Boundary Layer Observed in Kyungpook Province (경북지역에서 관측된 야간 대기경계층의 특성)

  • Characgcteristics of nocturnal boundary layer(NBL) were analyzed by the upper-air observations data using with the airsonde and pilot balloons from 1994 to 1999 in Kyungpook province. The automate weather boundary layer can become stably stratified when the surface is cooler than the air. Stable nocturnal boundary layer height were estimated from the top of surface stable layer where the vertical gradient of temperature and mixing ratio tend to zero or negative. The depth of the stable nocturnal boundary layer depended largely on the thermal effect rather than the wind effect at nighttime. The NBL was more developed on the land than on the coastal region. The stability index (bulk Richardson number) showed that the NBL was stable when the wind was weak and the vertical gradient of the temperature was strong. The heat budget in the NBL was studied by considering the effect of the radiative and the cooled by both the longwave radiative flux and the divergence of the heat flux, while NBL under the cloudy sky the longwave radiative flux played a role of the warming. It was noted that the heat was not conserved in both cases. To complete the heat budget in the NBL the warming/cooling by advection and subsidence must be considered.

  • Even though the origin and nature of nocturnal paroxysmal dystonia (NPD) remains unclear, it has been considered as a manifestation of the nocturnal frontal lobe epilepsy. We report a 17-year-old man with abnormal stereotyped movement during sleep. Video-EEG monitoring, ictal SPECT and night polysomnography did not show any evidence of epilepsy. However, the partial response to large dose of carbamazepine and the scoring according to the frontal lobe epilepsy and parasomnias (FLEP) scale suggest his events could be classified as epilepsy. Therefore we think the FLEP scale might be a useful tool for differential diagnosis in a patient presenting NPD.

  • Objectives The purpose of this study is to report the nocturnal enuresis in children using Sensitiv $Imago^{TM}$. Methods Twenty nocturnal enuresis children were tested using Sensitiv $Imago^{TM}$. Results 10 cases showed the lowest score at filter $#5^*$ and 7 cases showed the lowest score at filter $6^*$ in {Express Monitoring} of {Review of System Disorders of Homeostasis}. Conclusions Further study with more samples is necessary to establish the accuracy of Sensitiv $Imago^{TM}$.

  • Nocturnal panic involves sudden awakening from sleep in a state of panic characterized by various somatic sensation of sympathetic arousal and intense fear. Many(18-71%) of the spontaneous panic attacks tend to occur from a sleeping state unrelated to the situational and cognitive context. Nocturnal panickers experienced daytime panics and general somatic sensation more frequently than other panickers. Despite frequent distressing symptoms, these patients tend to exhibit little social or occupational impairment and minimal agoraphobia and have a high lifetime incidence of major depression and a good response to tricyclic antidepressants. Sleep panic attacks arise from non-REM sleep, late stage 2 or early stage 3. The pathophysiology and the similarity of nocturnal panic to sleep apnea, dream-induced anxiety attacks, night terrors, sleep paralysis, and temporal lobe epilepsy are discussed.

  • Nocturnal enuresis is a heterogeneous disorder with various underlying pathophysiological mechanisms and causes a mismatch between the nocturnal bladder capacity and the amount of urine produced during sleep at night. It is associated with a simultaneous failure of conscious arousal in response to the sensation of bladder fullness. Generally, a complete history and physical examination, with a specific focus on the genitourinary, gastrointestinal, and neurologic systems, is sufficient to evaluate a patient with enuresis. The therapeutic focus is directed toward a differential approach based on the underlying mechanism and toward combination therapies such as alarm devices and desmopressin as well as anticholinergic agents and desmopressin. Children with increased nocturnal urine production usually have a good response to desmopressin therapy. Patients with a small bladder generally show a poor response to desmopressin treatment, but they would benefit more from combination therapy with enuretic alarm, urotherapy, and antimuscarinic agents in addition to desmopressin. Different types of bladder dysfunction, which result in a small nocturnal bladder capacity, probably contribute significantly to the pathogenesis of nocturnal enuresis, particularly in those with treatment failure and refractory symptoms. Because different clinical subgroups may show different responses to treatment, it is necessary to distinguish these subgroups before a decision on the specific treatment protocol can be made.

  • Objectives : This study aimed to investigate acupuncture trend of childhood nocturnal enuresis in current Traditional Chinese Medical Journals. Methods : During the period from January 2000 to August 2004, we searched all Traditional Chinese Medical Journals in oriental medical library in dongguk university. We selected 28 literatures on acupuncture treatment for nocturnal enuresis in children Results : 28 studies are included and classified by diagnosis criteria and outcome criteria. And reclassified into two large groups, case report and comparison study. Totally 2324 cases, complete cures are 1701, significant improvements 2239, complete cure rate 73.2%, total improvement rate 97.3% 33 acupuncture points are used and main meridians are bladder, conception vessel and governor vessel. Conclusion : There remains a debate about diagnosis and outcome criteria. But Acupuncture is very effective and safe in the treatment of enuretic children.

The Impacts of Built Environmental Features on the Land Surface Temperatures for the Heat Wave Seasons in Gwangju, South Korea (도시화에 따른 건조환경이 하절기 광주시 외부공간의 열환경에 미치는 영향에 대한 연구)

  • This study aims to examine the impacts of built environmental features on the nocturnal and diurnal temperatures during the heat wave season in Gwangju, Korea. Built environmental measures are summarized at micro-scale level, such as 50 meters and 100 meters from temperature monitoring spots. Regressing the built environment on nocturnal and diurnal temperatures, we estimate how the artificial constructs contribute to temperature either day and night times. We found that impervious surface ratio is positively and negatively associated with nocturnal and diurnal temperatures, respectively. Buildings and structures tend to construct high thermal mass and absorb heat during day time and emit it for the night time. This property contributes to the nocturnal temperature model. On the other hand, urban areas with more vertical structure tend to block sun radiation more than rural, and it is more likely to find the negative relationship between impervious surface ratio and the diurnal temperatures.

  • Cough is not only serves to maintain normal function of respiratory tract, but also many diseases may lead to this symptom. In Korean medicine, Zhudanxi(朱丹溪), was the first to classify cough according to the different stages of time. In this research, I investigated the stage of Nocturnal Cough(夜嗽) through 26 oriental medical books. As a result, deficiency of Yin(陰虛) was the most common causes of Nocturnal Cough and indigestion(食積) is the next. Liuweidihuang-tang(六味地黃湯) prescribed as the treatment for Yin Xu(陰虛) and Xeibaisan-jiawei(瀉白散加味) prescribed as the treatment for indigestion(食積).
