Next Generation Sequencing (NGS), A Key Tool to open the Personalized Medicine Era

  • Next-Generation Sequencing (NGS) is a term that means post-Sanger sequencing methods with high-throughput sequencing technologies. NGS parallelizes the sequencing process, producing thousands or millions of sequences at once. The latest NGS technologies use even single DNA molecule as a template and measures the DNA sequence directly via measuring electronic signals from the extension or degradation of DNA. NGS is making big impacts on biomedical research, molecular diagnosis and personalized medicine. The hospitals are rapidly adopting the use of NGS to help to patients understand treatment with sequencing data. As NGS equipments are getting smaller and affordable, many hospitals are in the process of setting up NGS platforms. In this review, the progress of NGS technology development and action mechanisms of representative NGS equipments of each generation were discussed. The key technological advances in the commercialized platforms were presented. As NGS platforms are a great concern in the healthcare area, the latest trend in the use of NGS and the prospect of NGS in the future in diagnosis and personalized medicine were also discussed.

Recent Advances in the Clinical Application of Next-Generation Sequencing

  • Next-generation sequencing (NGS) technologies have changed the process of genetic diagnosis from a gene-by-gene approach to syndrome-based diagnostic gene panel sequencing (DPS), diagnostic exome sequencing (DES), and diagnostic genome sequencing (DGS). A priori information on the causative genes that might underlie a genetic condition is a prerequisite for genetic diagnosis before conducting clinical NGS tests. Theoretically, DPS, DES, and DGS do not require any information on specific candidate genes. Therefore, clinical NGS tests sometimes detect disease-related pathogenic variants in genes underlying different conditions from the initial diagnosis. These clinical NGS tests are expensive, but they can be a cost-effective approach for the rapid diagnosis of rare disorders with genetic heterogeneity, such as the glycogen storage disease, familial intrahepatic cholestasis, lysosomal storage disease, and primary immunodeficiency. In addition, DES or DGS may find novel genes that that were previously not linked to human diseases.

Next Generation Sequencing을 통한 미생물 군집 분석의 축산분야 활용 (Application of Next Generation Sequencing to Investigate Microbiome in the Livestock Sector)

  • The objective of this study was to review application of next-generation sequencing (NGS) to investigate microbiome in the livestock sector. Since the 16S rRNA gene is used as a phylogenetic marker, unculturable members of microbiome in nature or managed environments have been investigated using the NGS technique based on 16S rRNA genes. However, few NGS studies have been conducted to investigate microbiome in the livestock sector. The 16S rRNA gene sequences obtained from NGS are classified to microbial taxa against the 16S rRNA gene reference database such as RDP, Greengenes and Silva databases. The sequences also are clustered into species-level OTUs at 97% sequence similarity. Microbiome similarity among treatment groups is visualized using principal coordinates analysis, while microbiome shared among treatment groups is visualized using a venn diagram. The use of the NGS technique will contribute to elucidating roles of microbiome in the livestock sector.

  • 매우 빠른 속도로 발전하고 있는 차세대 염기서열 분석 플랫폼과 최신 생물정보학적 분석도구들로 말미암아, 1,000달러 이하의 가격으로 인간 유전체 염기서열을 해독하고자 하는 궁극적인 목표가 조만간 곧 실현될 수 있을 것 같다. 차세대 염기서열 분석 분야의 급속한 기술적 진전은 NGS 데이터의 분석과 관리를 위한 통계적 방법과 생물정보학적 분석도구들에 대한 수요를 꾸준히 증대시키고 있다. NGS 플랫폼이 상용화되어 쓰이기 시작한 초창기부터, NGS 데이터를 분석하고 해석하거나, 가시화 해주는 다수의 응용프로그램이나 도구들이 개발되어 활용되어 왔다. 그러나, NGS 데이터의 엄청난 범람으로 데이터 저장, 데이터 분석 및 관리 등에 있어서 해결해야 할 많은 문제들이 부각되고 있다. NGS 데이터 분석은 단편서열과 참조서열간의 서열정렬, 염기식별, 다형성 발견, 쌍단편 서열이나 비쌍단편 서열 등을 이용한 어셈블리 작업, 구조변이 발견, 유전체 브라우징 등을 본질적으로 포함한다. 본 논문은 주요 차세대 염기서열 결정기술과 NGS 데이터 분석을 위한 생물정보학적 분석도구들에 대해 개관적으로 소개하고자 한다.

Whole genome sequencing of foot-and-mouth disease virus using benchtop next generation sequencing (NGS) system

  • In countries with FMD vaccination, as in Korea, typical clinical signs do not appear, and even in FMD positive cases, it is difficult to isolate the FMDV or obtain whole genome sequence. To overcome this problem, more rapid and simple NGS system is required to control FMD in Korea. FMDV (O/Boeun/ SKR/2017) RNA was extracted and sequenced using Ion Torrent's bench-top sequencer with amplicon panel with optimized bioinformatics pipelines. The whole genome sequencing of raw data generated data of 1,839,864 (mean read length 283 bp) reads comprising a total of 521,641,058 (≥Q20 475,327,721). Compared with FMDV (GenBank accession No. MG983730), the FMDV sequences in this study showed 99.83% nucleotide identity. Further study is needed to identify these differences. In this study, fast and robust methods for benchtop next generation sequencing (NGS) system was developed for analysis of Foot-and-mouth disease virus (FMDV) whole genome sequences.

  • 본 연구는 콩과 식물인 비수리의 유용유전자를 NGS (Next Generation Sequencing)와 분자생물학의 융합적인 연구를 통해 분리하고 가능성을 알아보고자 시행하였다. 비수리는 자원식물이지만 많은 유용물질을 가지고 있다. 특히 항당뇨 기능을 하는 D-pinitol을 많이 함유하고 있는데 아직까지 비수리에서 D-piniol의 생합성에 관련된 유전자가 분리 되지 않았다. 비수리에 비생물학적 스트레스(가뭄)를 처리하고 처리하지 않은 대조군과 같이 total RNA를 추출한 후에 library를 만들어 NGS를 실시하였다. 이를 통해 D-pinitol 생합성에 관련된 유전자들을 분리하여 in silico 상에서 염기서열을 확인하였다. 이를 뒷받침하기 위해 Blast 프로그램을 사용하여 D-pinitol 생함성에 관여하는 ononitol epimerase를 확인하였고 in vitro 상에서도 RT-PCR을 통해 유전자 발현이 증가됨을 확인함으로써 융합적 연구를 통해 유전자를 찾고 분리하여 발현양상을 확인하였다.

  • In the present study, fast and robust methods for the next generation sequencing (NGS) were developed for analysis of PRRSV full genome sequences, which is a positive sensed RNA virus with a high degree of genetic variability among isolates. Two strains of PRRSVs (VR2332 and VR2332-R) which have been maintained in our laboratory were used to validate our methods and to compare with the sequence registered in GenBank (GenBank accession no. EF536003). The results suggested that both of strains had 100% coverage with the reference; the VR2332 had the coverage depth from minimum 3 to maximum 23,012, for the VR2332-R from minimum 3 to maximum 41,348, and 22,712 as an average depth. Genomic data produced from the massive sequencing capacities of the NGS have enabled the study of PRRSV at an unprecedented rate and details. Unlike conventional sequence methods which require the knowledge of conserved regions, the NGS allows de novo assembly of the full viral genomes. Therefore, our results suggested that these methods using the NGS massively facilitate the generation of more full genome PRRSV sequences locally as well as nationally in regard of saving time and cost.

Next generation sequencing-based salivary biomarkers in oral squamous cell carcinoma

  • Selection of potential disease-specific biomarkers from saliva or epithelial tissues through next generation sequencing (NGS)-based protein studies has recently become possible. The early diagnosis of oral squamous cell carcinoma (OSCC) has been difficult, if not impossible, until now due to the lack of an effective OSCC biomarker and efficient molecular validation method. The aim of this study was to summarize the advances in the application of NGS in cancer research and to propose potential proteomic and genomic saliva biomarkers for NGS-based study in OSCC screening and diagnosis programs. We have reviewed four categories including definitions and use of NGS, salivary biomarkers and OSCC, current biomarkers using the NGS-based technique, and potential salivary biomarker candidates in OSCC using NGS.

식품 미생물 균총 연구를 위한 최신 마이크로바이옴 분석 기술 (Recent next-generation sequencing and bioinformatic analysis methods for food microbiome research)

  • Rapid development of next-generation sequencing (NGS) technology is available to study microbes in genomic level. This NGS has been widely used in DNA/RNA sequencing for genome sequencing, metagenomics, and transcriptomics. The food microbiology area could be categorized into three groups. Food microbes including probiotics and food-borne pathogens are studied in genomic level using NGS for microbial genomics. While food fermentation or food spoilage are more complicated, their genomic study needs to be done with metagenomics using NGS for compositional analysis. Furthermore, because microbial response in food environments are also important to understand their roles in food fermentation or spoilage, pattern analysis of RNA expression in the specific food microbe is conducted using RNA-Seq. These microbial genomics, metagenomics, and transcriptomics for food fermentation and spoilage would extend our knowledge on effective utilization of fermenting bacteria for health promotion as well as efficient control of food-borne pathogens for food safety.

Normative Issues in Next Generation Sequencing Gene Testing

  • Despite the commercialization of Next generation sequencing (NGS) gene testing, only a few studies have addressed the various ethical and legal problems associated with NGS testing in Korea Here, we reviewed the normative issues that emerged at each stage of the wet analysis and bioinformatics analysis of NGS gene testing. In particular, it was in mind to apply various international guidelines and the principles of bioethics to actual clinical practice. Considering the characteristics of NGS testing, wet analysis of additional testing can be justified if presumptive consent is recognized. Furthermore, the medical relationship between diseases needs to be established and it should be clear that the patient would have given consent if the patient had been aware of the correlation between genes. At the stage of bioinformatics analysis, the question of unsolicited findings arises. In case of unsolicited and relevant findings, according to American College of Medical Genetics and Genomics (ACMG), a recognized relationship between genes and diseases needs to be established. In case of unsolicited and not-relevant findings, it is almost impossible to determine whether knowing or not knowing the findings is more beneficial to the patient. However, it seems to be certain that the psychological harm an individual may suffer from such information is likely to be greater if the disease is severe and if there is no cure. The list of genes for which the ACMG guidelines impose reporting obligations is a good reference for judgment.
