신규간호사의 간호근무환경과 사회적 지지가 현실충격에 미치는 영향 (Effect of Nursing Work Environment and Social Support on Reality Shock Experienced by New Graduate Nurses)

  • Purpose: The purpose of this study was to identify the effects of nursing work environment and social support on the reality shock of new graduate nurses in university hospitals. Methods: The subjects were 153 new graduate nurses with less than 12 months of clinical experience at two university hospitals in the G province, and data were collected from September 15 to October 7, 2020. Data were analyzed using the SPSS/WIN 26.0 program for frequency, average, t-test, ANOVA, Pearson correlation coefficient, multiple regression. Results: Factors influencing reality shock were nursing work environment (β=-.39, p<.001) and social support (β=-.25, p=.002), and gender (female) (β=.20, p=.001), and the explanatory power was 44.8% (F=9.99, p=.002). Conclusion: Our study shows that nursing work environment and social support play an important role in the reality shock of new graduate nurses. It is important to strengthen nursing work environment and social support to alleviate the reality shock of new graduate nurses. The findings of this study may serve as a basis for developing strategies for mitigating the reality shock of new graduate nurses.

혼종모형을 이용한 신규간호사 임상간호교육의 질에 대한 개념분석 (Quality of clinical nursing education for new graduate nurses: A concept analysis with a hybrid model)

  • Purpose: The study aimed to examine the concept and attributes of the quality of clinical nursing education for new graduate nurses. Methods: This study adopted a hybrid model introduced by Schwartz-Barcott and Kim. In the theoretical stage, the meaning and attributes of the quality of clinical nursing education for new graduate nurses were determined by analyzing eight articles. In the fieldwork stage, data were collected using semi-structured interviews with five new graduate nurses and seven experienced nurses. The data were analyzed by qualitative content analysis methods developed by Elo and Kyngӓs. In the final analysis, a final result was arrived at comparing, contrasting, and integrating the attributes of the concepts derived in the theoretical and field-work stages. Results: The quality of clinical nursing education for new graduate nurses was identified as excellence or the standard of education for new graduate nurses that would support them in adapting to clinical settings and transitioning to professional nurses. The attributes of the quality of clinical nursing education were founded to possess three dimensions, six categories, and 18 attributes. The multidimensional attributes of the quality of clinical nursing education for new graduate nurses were confirmed as education resources, design, method, content, evaluation, interaction, and outcome under the three dimensions of input, process, and output. Conclusion: The concept and nature of the quality of clinical nursing education observed in this study can be utilized as a basis for the future development, evaluation, and improvement of clinical nursing education for new graduate nurses in healthcare organizations.

암병동 신규간호사를 위한 행복 셀프코칭 프로그램 개발 및 효과 (Development and Evaluation of the Effect of a Happiness Self-Coaching Program for New Graduate Nurses Working in Cancer Care Unit)

  • Purpose: This study was conducted to examine the effect of a Happiness Self-Coaching program for new graduate nurses working in cancer care units. Methods: A quasi-experimental pretest-posttest, nonequivalent control-group design was applied to conduct the study. Study participants were new graduate nurses employed within 12 months and working as shift employees at G university hospital, located in J city. A total of 21 new graduate nurses participated in the study, 10 in the experimental group, and 11 in the control group. The happiness self-coaching program was conducted weekly for 70 minutes from February 1 to June 6, 2016 for 6 weeks. The experimental group received the Happiness Self-Coaching program through lectures, presentation, group activities, and strength card play. Data were analyzed by using repeated measure ANOVA and paired t-test. Results: The experimental group demonstrated significantly higher happiness (F=6.82, p=.003) and self-efficacy (F=3.38 p=.045) levels compared to the control group. Conclusion: The Happiness Self-Coaching program was effective in enhancing new graduate nurses' happiness and self efficacy. It is recommended that nursing organizations to apply the Happiness Self -Coaching program to facilitate adaption of new graduate nurses in cancer care units.

신규간호사 교육체계에 대한 실태조사: 프리셉터십 운영을 중심으로 (Survey on the Education System for New Graduate Nurses in Hospitals: Focusing on the Preceptorship)

  • The purpose of this study was to identify the current status of the new graduate nurses' education system, to establish guidelines for the education of new graduate nurses, and to provide dates for the improvement of the education system. Eighty-nine hospitals participated in the survey from July to August 2018. The questionnaires consisted of 24 items including general characteristics, new graduate nurses' education system, operation of nursing education teams, and preceptorship programs. The average duration of education/training for new graduate nurses was 57.3 days, and 26.6%-58.5% of general hospitals, hospitals, and long-term care hospitals had less than 30 days of new graduate nurses education/training, which is shorter than the education/training period of tertiary hospitals. There were new nursing education guidelines in 88.8% of all medical institutions and 58.6% of them had nursing education teams. Most of the personnel in charge of nursing education were not dedicated to education. A total of 87.6% of the all participating medical institutions had preceptorship programs, while 23.1%-33.3% of hospitals and long-term care hospitals did not. The breakdown of preceptorship programs showed 68.4% as "preceptor and preceptee are in charge of one team, and preceptor is teaching preceptee," and 19.7% were "preceptor and preceptee are in charge of each team, and preceptor is teaching preceptee at the same time." These results show that standardization and management of new graduate nurses' education programs is necessary. For the standardization and management of new graduate nurses' education programs, joint efforts of nurses' associations and support from the government should be encouraged.

텍스트네트워크분석을 활용한 신규간호사가 경험하는 현실충격 관련 연구의 지식구조 분석 (Analysis of the Knowledge Structure of Research related to Reality Shock Experienced by New Graduate Nurses using Text Network Analysis)

  • 본 연구의 목적은 신규간호사가 경험하는 현실충격 관련 연구를 텍스트 네트워크 분석을 통해 분석함으로써 신규간호사의 성공적인 임상적응과 이직률 감소에 기여할 수 있는 기초자료를 제공하기 위함이다. 2002년 1월부터 2021년 12월까지 국내외 학술지에 게재된 115편의 논문에서 신규간호사가 경험한 현실충격에 관한 토픽을 추출하였다. 6개의 데이터베이스(국내: DBpia, KISS, RISS / 해외: Web of science, Springer, Scopus)에서 문헌을 검색하였다. 키워드는 문헌의 초록에서 추출되었고 의미론적 형태소를 사용하여 정리되었다. 네트워크분석 및 토픽모델링은 NetMiner 4.5 프로그램을 사용하여 수행되었다. 핵심 키워드는 '신규간호사', '현실충격', '전환', '학생간호사', '경험', '실습', '근무환경', '역할', '돌봄', '교육' 등으로 확인되었다. 최근 신규간호사의 현실충격에 관한 연구에서 잠재적 디리클레 할당(LDA) 기법으로 '이직', '근무환경', '전환 경험'의 세 가지 주요 주제를 추출하였다. 본 연구결과를 바탕으로 신규호사가 경험하는 현실충격을 효과적으로 감소시키고 성공적으로 임상적응을 도울 수 있는 중재 연구의 필요성을 제언한다.

신규간호사의 직장 내 괴롭힘 경험과 회복탄력성이 조직사회화에 미치는 영향 (Influence of Workplace Bullying and Resilience on Organizational Socialization in New Graduate Nurses)

  • Purpose: The purpose of this study was to identify the impact of workplace bullying experience and resilience on the organizational socialization among new graduate nurses. Methods: This is a descriptive correlational study with 148 new graduate nurses working in the hospitals in B city of Korea. The instruments that were used for measuring main variables were the negative acts questionnaire-revised (NAQ-R), the Korean version of the Connor-Davidson resilience scale (CD-RISC), and the measurement tool for new nurses' organizational socialization. Results: The prevalence of workplace bullying was 22.3%, and the subjects reported moderate level of resilience. Significant negative correlations between workplace bullying and resilience, and organizational socialization were found. A significant positive correlation was found between resilience and organizational socialization. As a result of multiple regression analysis, resilience, workplace bullying, and a working in desired unit turned out to be variables that explained new graduate nurses' organizational socialization (adjusted $R^2=.514$, p<.001). Conclusion: The results of this study suggested that new graduate nurses with higher resilience and less experience of workplace bullying would show higher organizational socialization. Further research is needed to identify the regulation effect of resilience in between workplace bullying and organizational socialization.

신규간호사의 삶의 경험 (Lived Experiences of New Graduate Nurses)

  • Purpose: The purpose of this study was to explore the lived experiences of new graduate nurses in personal life and in clinical setting during their first year. Methods: Eleven new graduate nurses were interviewed and the data were analyzed using the Giorgi's phenomenological method. Results: The main results of this research were negative experiences such as poor nursing skills, inability to enjoy personal life, physical exhaustion and health problems, and uncertainty of nursing values. On the other hand, there were some positive aspects on their minds: adjustmental/developmental needs, sense of responsibility, receptive capability, and proficiency with their work. They gradually found themselves being endured and changing to adapt. Conclusion: Transition from students to staff nurses is a very stressful experience for new graduate nurses. It may negatively affect their personal lives as well. However they tried to keep themselves positive to overcome their difficulties. It is necessary for them to be given time, a systematic program, and a supportive environment to adapt.

신규간호사의 실무적응지원 교육 만족도 및 역할 이행 경험에 관한 조사 (New Graduate Nurses' Satisfaction with Transition Programs and Experiences in Role Transition)

  • Purpose: The aim of this study was to investigate the satisfaction of newly graduated nurses with educational programs and their experiences in role transition. Methods: Data were collected from November 1 to December 15, 2018 and 483 new graduate nurses working at 15 tertiary hospitals and 10 general hospitals participated. For data collection, self-report questionnaires including the Casey-Fink Graduate Nurse Experience Survey tool and satisfaction with education were used. Data were analyzed using descriptive statistics, t-test, and one-way analysis of variance. Results: Satisfaction with education ranged from 3.09 to 3.27, and satisfaction with preceptors was 3.45(maximum 4). The skill that new nurses ranked as most difficult during the first 3 months was charting/documentation, and throughout a whole year, the top 4 difficult skills were cardiopulmonary resuscitation/emergency response, ventilator care, end-of-life care, and prioritization/time management. In comfort/confidence, new graduates felt most comfortable with support and least comfortable with patient safety. More than 50 percent of new graduates experienced stress during role transition, and the most frequently experienced stressors were related to job performance and personal life. Levels of satisfaction with education and comfort/confidence differed according to the hospital type and number of preceptors for new nurses. Conclusion: In order to facilitate the transition of new graduate nurses to professional nurse, an extended period of education, systematic and standardized transition programs, and continuous support during the first year of practice are required.

토픽모델링을 활용한 신규간호사 관련 국내 연구동향 분석 (Research trend analysis of Korean new graduate nurses using topic modeling)

  • Purpose: The aim of this study is to analyze the research trends of articles on just graduated Korean nurses during the past 10 years for exploring strategies for clinical adaptation. Methods: The topics of new graduate nurses were extracted from 110 articles that have been published in Korean journals between January 2010 and July 2020. Abstracts were retrieved from 4 databases (DBpia, RISS, KISS and Google scholar). Keywords were extracted from the abstracts and cleaned using semantic morphemes. Network analysis and topic modeling were performed using the NetMiner program. Results: The core keywords included 'education', 'training', 'program', 'skill', 'care', 'performance', and 'satisfaction'. In recent articles on new graduate nurses, three major topics were extracted by Latent Dirichlet Allocation (LDA) techniques: 'turnover', 'adaptation', 'education'. Conclusion: Previous articles focused on exploring the factors related to the adaptation and turnover intentions of new graduate nurses. It is necessary to conduct further research focused on various interventions at the individual, task, and organizational levels to improve the retention of new graduate nurses.

다수준 생존분석을 이용한 신규 간호사 이직 영향요인 (Predictors of Turnover among New Nurses using Multilevel Survival Analysis)

  • Purpose: The purpose of this study was to examine factors influencing new graduate nurse turnover. Methods: This study was carried out as a secondary analysis of data from the 2010 Graduates Occupational Mobility Survey (GOMS). A total of 323 nurses were selected for analysis concerning reasons for turnover. Data were analyzed using descriptive statistics and multilevel survival analysis. Results: About 24.5% of new nurses left their first job within 1 year of starting their jobs. Significant predictors of turnover among new nurse were job status, monthly income, job satisfaction, the number of hospitals in region, and the number of nurses per 100 beds. Conclusion: New graduate nurses are vulnerable to turnover. In order to achieve the best health of the nation, policy approaches and further studies regarding reducing new graduate nurse turnover are needed.
