열성전염병(熱性傳染病)에 대한 오당의 상한론처방(傷寒論處方) 활용법(活用法) 연구(硏究) (A study on the application of Sanghallon prescription for febrile disease by Wu-Tang)

  • Before the advent of febrile disease theory, people had used Sanghallon's theory to cure febril disease. Therefore, Wu-Tang both suggested new prescription and used the former prescription of Sanghallon(傷寒論) in curing febrile disease. However, he didn't use the original prescription of Sanghallon and modified the quantity and ingredients properly. Through this process, the fault of Sanghallon was supplemented and the method of curing febrile disease was advanced. To research about this, it will be much easier to understand prescription of Sanghallon and even the treatments and views of Wu-Tang about febrile disease. In this study, I researched the way Wu-Tang applied prescription of Sanghallon, focusing on Decoction for Purgation, White Tiger Decoction, Decoction for Restoring Pulse which was used by Wu-Tang in various ways and applied in treatment of febrile disease.

Optogenetic neuromodulation with gamma oscillation as a new strategy for Alzheimer disease: a narrative review

  • The amyloid hypothesis has been considered a major explanation of the pathogenesis of Alzheimer disease. However, failure of phase III clinical trials with anti-amyloid-beta monoclonal antibodies reveals the need for other therapeutic approaches to treat Alzheimer disease. Compared to its relatively short history, optogenetics has developed considerably. The expression of microbial opsins in cells using genetic engineering allows specific control of cell signals or molecules. The application of optogenetics to Alzheimer disease research or clinical approaches is increasing. When applied with gamma entrainment, optogenetic neuromodulation can improve Alzheimer disease symptoms. Although safety problems exist with optogenetics such as the use of viral vectors, this technique has great potential for use in Alzheimer disease. In this paper, we review the historical applications of optogenetic neuromodulation with gamma entrainment to investigate the mechanisms involved in Alzheimer disease and potential therapeutic strategies.

  • Myotonic muscular dystrophy is a disease characterized by progressive muscle weakness with myotonia and multiorgan involvement. Two subtypes have been recognized; each subtype is caused by nucleotide repeat expansion. So far, there has been no cure for myotonic muscular dystrophy. In this article, we introduce ongoing clinical trials for new drugs to modify disease course by correcting genetic derangement or its downstream in myotonic dystrophy type 1.

  • 많은 연구들이 심혈관 질환의 병인과 병리적 기전에 있어 사회심리학적, 행동학적 위험요인의 역할을 조사해 왔다. A형 행동 유형과 심장 질환의 이환 및 사망과의 관련성을 확인한 여러 전향적 연구들을 바탕으로, A형 행동 유형은 관상동맥질환의 독립적인 위험요인으로 밝혀졌다. 또한 최근 제안된 새로운 성격유형인 D형 인격유형 역시 심혈관 질환의 이환율과사망률의 증가에 관여함이 보고되고 있다. 그러나 국내에서는 아직 이들과 관련된 연구가 부족한 실정이다. 따라서 저자는 심혈관 질환과의 관련성에 대해 널리 연구, 보고되고 있는 A형 행동유형과 D형 인격유형에 초점을 맞추면서 지금까지의 연구 문헌을 정리 검토하여 향후 연구에 도움을 주고자 하였다.

Peri-implant disease: what we know and what we need to know

  • Peri-implant disease is a serious problem that plagues today's dentistry, both in terms of therapy and epidemiology. With the expansion of the practice of implantology and an increasing number of implants placed annually, the frequency of peri-implant disease has greatly expanded. Its clinical manifestations, in the absence of a globally established classification, are peri-implant mucositis and peri-implantitis, the counterparts of gingivitis and periodontitis, respectively. However, many doubts remain about its features. Official diagnostic criteria, globally recognized by the dental community, have not yet been introduced. The latest studies using metagenomic methods are casting doubt on the assumption of microbial equivalence between periodontal and peri-implant crevices. Research on most of the features of peri-implant disease remains at an early stage; moreover, there is not a commonly accepted treatment for it. In any case, although the evidence so far collected is limited, we need to be aware of the current state of the science regarding this topic to better understand and ultimately prevent this disease.

Magnetic Resonance-Guided Focused Ultrasound : Current Status and Future Perspectives in Thermal Ablation and Blood-Brain Barrier Opening

  • Magnetic resonance-guided focused ultrasound (MRgFUS) is an emerging new technology with considerable potential to treat various neurological diseases. With refinement of ultrasound transducer technology and integration with magnetic resonance imaging guidance, transcranial sonication of precise cerebral targets has become a therapeutic option. Intensity is a key determinant of ultrasound effects. High-intensity focused ultrasound can produce targeted lesions via thermal ablation of tissue. MRgFUS-mediated stereotactic ablation is non-invasive, incision-free, and confers immediate therapeutic effects. Since the US Food and Drug Administration approval of MRgFUS in 2016 for unilateral thalamotomy in medication-refractory essential tremor, studies on novel indications such as Parkinson's disease, psychiatric disease, and brain tumors are underway. MRgFUS is also used in the context of blood-brain barrier (BBB) opening at low intensities, in combination with intravenously-administered microbubbles. Preclinical studies show that MRgFUS-mediated BBB opening safely enhances the delivery of targeted chemotherapeutic agents to the brain and improves tumor control as well as survival. In addition, BBB opening has been shown to activate the innate immune system in animal models of Alzheimer's disease. Amyloid plaque clearance and promotion of neurogenesis in these studies suggest that MRgFUS-mediated BBB opening may be a new paradigm for neurodegenerative disease treatment in the future. Here, we review the current status of preclinical and clinical trials of MRgFUS-mediated thermal ablation and BBB opening, described their mechanisms of action, and discuss future prospects.

A revaluation of algal diseases in Korean Pyropia (Porphyra) sea farms and their economic impact

  • As with land crops, cultivated algae are affected by various diseases ranging from large outbreaks of a disease to chronic epiphytes, which may downgrade the value of the final product. The recent development of intensive and dense mariculture practices has enabled some new diseases to spread much faster than before. A new disease is reported almost every year, and the impact of diseases is expected to increase with environmental change, such as global warming. We observed the incidence of diseases in two Pyropia sea farms in Korea from 2011 to 2014, and estimated the economic loss caused by each disease. Serious damage is caused by the oomycete pathogens, Pythium porphyrae and Olpidiopsis spp., which decreased the productivity of the Pyropia sea farms. In Seocheon sea farms, an outbreak of Olpidiopsis spp. disease resulted in approximately US $1.6 million in loss, representing approximately 24.5% of total sales during the 2012-2013 season. The damage caused by green-spot disease was almost as serious as oomycete diseases. An outbreak of green-spot disease in the Seocheon sea farms resulted in approximately US $1.1 million in loss, representing 10.7% of total sales in the 2013-2014 season in this area. However, the causative agent of green-spot disease is still not confirmed. "Diatom felt" is regarded as a minor nuisance that does not cause serious damage in Pyropia; however, our case study showed that the economic loss caused by "diatom felt" might be as serious as that of oomycete diseases. Bacteria and cyanobacteria are indigenous members of epiphytic microbial community on Pyropia blades, but can become opportunistic pathogens under suitable environmental conditions, especially when Pyropia suffers from other diseases. A regular acid wash of the Pyropia cultivation nets is the most common treatment for all of the above mentioned diseases, and represents approximately 30% of the total cost in Pyropia sea farming. However, the acid wash is ineffective for some diseases, especially for Olpidiopsis and bacterial diseases.

식중독 발생지수 개발 (Developing the Index of Foodborne Disease Occurrence)

  • 외식산업의 발달과 함께 학교와 기업 등의 단체 급식이 증가하고 있는 오늘날, 관련분야에서는 식중독 예방에 대해 많은 관심을 갖고 연구하고 있다. 우리나라에서는 온도에 따른 세균의 증식속도를 이용하여 식중독지수를 개발하여 식중독 위험에 대한 정보를 제공하고 있는데 그 정보가 식중독 발생상황과는 차이가 있음이 지적되고 있다. 본 연구에서는 최근 3년간($2004{\sim}2006$년)의 일별 식중독 발생과 기상자료를 이용하여 일 최고기온, 습도, 월효과를 설명변수로 하고, 식중독 발생건수를 반응변수로 하는 로그선형모형(Log Linear Model)을 이용하여 식중독 발생의 위험을 예보할 수 있는 사고발생지수를 개발하였다. 개발된 지수와 기존지수를 비교한 결과 개발된 지수가 식중독 발생 상황을 반영함에 있어서 더 나은 설명력이 있음이 나타났다.

  • 신약 재창출은 현재 사용되는 약물의 새로운 용도를 발견하는 방법이다. 텍스트 마이닝은 정형화되지 않은 문서로부터 의미 있는 지식을 획득하는 과정을 의미한다. 본 논문에서는 약물-유전자와 유전자-질병에서 동시에 측정된 유전자 출현 빈도의 비율을 고려하여 새로운 약물-질병 관계를 추론하는 방법을 제안한다. 생물학적 문헌으로부터 약물-유전자와 유전자-질병의 동시출현 빈도를 측정하고 각 약물과 질병에 대하여 유전자의 출현 비율을 계산한다. 약물-질병 관계의 가중치는 동시에 측정된 유전자 출현 비율의 평균을 이용하여 계산되고 이를 이용하여 각 질병의 분류 정확도를 측정한다. 약물-질병 관계를 추론하는 것에서 동시출현 빈도를 문장 단위로 측정하고 여러 관계를 고려하는 방법이 기존 방법보다 더 정확히 식별해내는 것을 보였다.

동일 질환에 대한 상병분류기호의 의료기관별 변이에 관한 연구 (Individual Variations in the Code of the International Classification of Disease for Similar Outpatient Conditions among General Practitioners)

  • The code of the International Classification of Disease(ICD) is seriously questioned on its effectiveness in identifing an independent disease entity from similar conditions at general practitioner's offices. This study has attempted to show individual coding variations in ICD for similar ambulatory care conditions. It has been assumed that a following outpatient visit is regarded as the sane kind of visit owing to the same disease if a visit to the different source of care would be mad within an interval of less than two days. The 'D' health insurance association was selected for this analysis. The 'D' association had 153,298 members and made claims of 642,605 outpatient care in 1990. Out of the total outpatient claims, 8.6%(55,102 claims) were counted as the same disease which could meet the above assumption. Percent of conditions classified as the 10 leading causes of frequent visits which were matched accurately to the subsequent ICD diagnostic code found to be 15.8% on the average. The URI was noted for the highest concurrence rate of 20.4%. This proportion was even decreased to 11.6% on the case of chronic disease. Despite the fact that the assumption underlying the definition of the above same disease is rather rough and inappropriate, this study reveals that the code of ICD currently in use has weaknesses in seperating a certain independent disease from similar conditions at the outpatient setting. Thus, efforts need to be elaborated to meet the need of a new system of classification for conditions and diseases encountering at ambulatory care.
